        서혜부에 발생한 골화성 지방종

        서혜부에서 골조직과 분리되어 독립적으로 발생한 골화성 지방종은 아주 드물다. 저자들은 좌측 서혜부에서 어른 주먹 크기의 단단하고, 움직임이 없는 무통성 종괴를 주소로 내원한 환자를 종양 변연 절제술로 치료하였으며, 병리조직 소견으로 골화성 지방종으로 확진되었다. 골 조직을 동반한 많은 지방종의 아형들이 문헌에 보고되고 있으나 저자들의 증례처럼 골화성 지방종이 서혜부에서 발견된 문헌 보고는 거의 없는 실정이다. 이에 저자들은 골조직과 분리되어 발생한 골화성 지방종의 치료 시에 석회화 병변을 동반하는 다른 종양과 감별 진단함이 중요하기에 영상의학적 소견 및 조직병리학적 소견과 연관시켜 문헌 고찰과 함께 보고하고자 한다. It is extremely rare to observe ossifying lipoma that developes separately from bone tissues in the groin. A patient with an adult fist-sized, firm, non-movable and painless mass in the left groin area, had been treated with marginal excision, which turned out to be ossifying lipoma. Although many different variants of lipoma with bone tissue have been reported, a case like this has never previously been reported. It is important to distinguish ossifying lipoma, from tumors with calcific lesions. We report its uniqueness in radiologic and pathologic ways, with specific findings of ossifying lipoma.

        정부출연연구기관에 있어서 보상시스템에 대한 인식이 조직시민행동에 미치는 영향

        본 연구의 목적은 1966년 설립 이후 과학기술계 고학력 연구원이 집단적으로 근무하고 있는 과학기술계 정부출연연구기관에서 종사자들의 보상시스템에 대한 인식이 조직시민행동에 어떠한 영향을 미치는 지를 실증적으로 분석하는 것이다. 다른 민간 연구기관과는 다르게 외부로부터 상대적으로 독립된 환경에서 연구원이 중심이 되어 운영되는 정부출연(연)의 독특한 상황에서 최근 확대되고 있는 성과주의 보상시스템에 대한 인식에 따른 종사자의 조직행동은 전체 사회분위기에 의해 영향을 받을 수도 있지만 정부출연(연) 나름대로 특수한 형태로 나타날 수도 있다. 분석 결과 출연(연) 종사자는 보상 자체의 결과나 차등보다는 성과와 보상이 제대로 연계되어 있는지, 그리고 본인이 보상과 성과를 분배하는 의사결정과정에 얼마나 참여하고 발언할 수 있는지 하는 절차나 과정을 중요시 하는 것으로 나타났다. 이는 향후 출연(연) 보상시스템을 성과와 보상이 연계되도록 설계하는 것과 보상 과정에서의 체계적인 정보 전달 및 참여가 중요함을 시사해 주고 있다. The study aims at gaining the effect of the perception of performance-compensation relationship, variability of compensation, and organizational justice to the organizational citizenship behavior. A questionnaire survey method was adopted for this study. A total of 182 individuals from nine Government-Supported Research Institutes responded to the questionnaire survey. The results of this study were summarized as follows. First, the perception of performance-compensation relationship positively influence on the organizational citizenship behavior. Second, the perception of procedural justice also positively effect on organizational citizenship behavior but the perception of distributive justice was not effective to organizational citizenship. Third, it was found that variability of compensation was not effective to the dependent variables. Conclusively, to improve the organizational effectiveness in the Government-Supported Research Institutes, it is necessary to connect the Compensation System with performance, confirm the procedural justice, and provide accurate and enough information of Compensation System for employees.

        경막하 지방종으로 오인될 수 있는 외상성 요천추부 아급성 경막하 출혈

        외상성 요천추부 경막하 혈종은 해부학 및 병리학적인 원인으로 인하여 경막외 혈종에 비해 빈도가 극히 드문 것으로 알려져 있다. 외상의 시기가 불분명한 경우 시간 경과에 따른 혈종의 신호 강도가 두개강내 혈종과 척추강 내 혈종에서 일치하지 않는 경우가 많기 때문에 감별진단에 있어서 많은 혼돈을 주게 된다. 이에 지방억제 자기공명영상 및 컴퓨터단층촬영 등의 시도들이 행해지고 있다. 외상 시기를 정확히 알 수 없는 경막하 혈종을 자기공명영상과 컴퓨터단층촬영을 이용하여 경막하 지방종과 감별진단하여 조기에 수술적 감압술 시행 후, 좋은 예후를 보인 증례를 문헌 고찰과 함께 보고하고자 한다. Traumatic lumbosacral spinal subdural hematoma due to anatomical and pathological causes is rare, compared to epidural hematoma. If the time of trauma cannot be determined, intracranial and intraspinal signal intensity according to lapse of time are not coincident, resulting in confusion in terms of differentiation. Fat suppression magnetic resonance image (MRI) and computed tomography (CT) are utilized for differentiation. The intention of this study is to report on a case where spinal subdural hematoma of unknown time of occurrence is differentiated from subdural lipoma by taking advantage of fat suppression MRI and CT in order to perform an early surgical decompression with auxiliary review of literature demonstrating good prognosis of the procedure.

      • 수술 전 감별진단이 어려웠던 소뇌 중심부 종양:2예

        The exact preoperative diagnosis in the cerebellar midline tumor is crucial to design the operative plan such as the extent of tumor resection, preservation of critical structures, and need to be shunt. We experienced 2 cases, which were suspected as usual radiologic diagnoses but were revealed as rare entities in the pathology. A 15-year-old child presented with sudden headache while he was studying. The features of radiologic images suggested the hematoma caused by the underlying lesion. Medulloblastoma, cavernous hemangioma, and ependymoma were suspected. After operation, pathology revealed it to be hemorrhagic pilocytic astrocytoma. A 40-year-old woman suffered from lasting headache and ataxia. In the radiologic finding, it was a huge cystic mass with a mural nodule. We took into consideration cerebellar tumor such as dermoid cyst, hemangioblatoma, or cystic astrocytoma. The pathologic diagnosis was the mature teratoma. These rare entities of pathology occurred in the cerebellar midline region should be considered in the progress of diagnosis and we will review it with pertinent literatures.

        소아에서 발생한 연필에 의한 관통 뇌손상

        Pencils are common instruments for children to use and play with. This report describes an unusual penetrating orbitocranial injury in a 5-year-old girl who was struck in her facewith a pencil. She was holding it at a desk, and her friend pushed her back. The pencil penetrated the left lower eyelid and went deep into the right frontal lobe through the base of the skull. It was removed at the emergency room, after which brain CT was performed to detect the development of an intracranial hematoma. No complications occurred after conservative management with antibiotics and an antiepileptic drug. Pencils can be hazardous to children, and a penetrating head injury with a pencil may be managed without cranial surgery. (J Korean Soc Traumatol 2012;25:28-31)

        Cosmetic problems after the orbitopterional craniotomy are big concerns caused by the injury to the temporalis muscle and more destructive resection of the orbit. This report describes the techniques to dissect the physiologic plane of the temporalis muscle and fascia and to preserve the contour of orbit. Subfascial dissection of the temporalis muscle for the scalp reflection was applied to preserve the frontal branch of the facial nerve. The temporalis muscle was detached from the temporal fossa with anterior to posterior and proximal to distal manner. The muscle was not incised vertically or cauterized. A usual pterional craniotomy was performed and then an orbital craniotomy was followed. The passing drill (#8TA11, Midas Rex) was used to cut the orbital rim. The first cut was made on the lateral to the supraorbital notch. The second cut was proximal to the frontozygomatic suture. Following, the orbital roof was thinned to 3 to 4 cm posteriorly with cutting drills. The drilling was connected to the cutting edges of the orbital rim and the superior orbital fissure. After dural closure, the bone flaps were fixed with a Neuroclip??. This technique has been used for the 21 adult patients (11 male and 10 female patients) to clip anterior communicating artery aneurysms. The thickness of the temporalis muscle was measured at 3 points before surgery and in 3 months after surgery. As a result, it provided a wide basal exposure for clipping aneurysms. There were no injuries to the frontal branch of facial nerve. The most vulnerable area of periorbital injuries was just distal to frontozygomatic suture. All the patients suffered from the periorbital swelling after operation, which was subsided around 5 days. The bulk of the temporalis muscle was not significantly reduced and the reduction of the muscle thickness was less than 10%. The rigid fixation of the orbital and pterional bone flaps could be achieved. Although orbitopterional craniotomy requires extensive works, cosmetic results may be optimal if the physiologic dissection of the temporalis muscle and fascia and appropriate resection and fixation of orbital roof were performed.

        Offshore heave compensator is a structure that bears dynamic load (environmental load) such as ocean wave and wind, and influenced by vibration during operation. However, While the natural frequency of the structure is close to the dynamic load frequency, it can load to resonance phenomena. This study is composed of 3 model (upper position, middle position, lower position) depend on positions of crown block to vibrate characteristic of offshore heave compensator. Modal analysis and harmonic analysis fulfill vibration characteristic evaluation of finite element model by ANSYS Workbench V13, to judge stability of the structure.

        시야 결손이 있는 뇌하수체샘종 환자에서 수술 후 시야 변화

        Purpose: To evaluate visual field (VF) changes in patients with pituitary adenoma after surgical treatment. Methods: The present study retrospectively evaluated 96 eyes of 48 patients with pituitary adenoma who received surgical tumor removal between July 2001 and February 2010. Preoperative and postoperative clinical data including age, tumor volume, logMAR BCVA, surgical technique (transsphenoidal surgery and transcranial surgery), static perimetry scores (mean deviation [MD], pattern standard deviation [PSD], and visual field defect [VFD] scores) were reviewed. Results: The MD (15.79%, p = 0.001) and PSD (3.98%, p = 0.003) improved postoperatively (mean postoperative follow- up period 1.85 months). Transsphenoidal surgery for tumor removal showed significant MD (26.99%, p = 0.000) and PSD (12.92%, p = 0.003) improvements. A multivariate regression analysis of the transsphenoidal surgery patient group revealed that the preoperative MD was related to the postoperative MD (Pearson = 0.762, p = 0.000), but negatively correlated to the amount of postoperative improvement in MD score (Pearson = -0.231, p = 0.046). Transcranial surgery did not significantly improve the MD (p = 0.419), PSD (p = 0.562), VFD score (p = 0.135), or logMAR BCVA (p = 0.708). Conclusions: Visual filed defects in patients with pituitary adenoma improved after neurosurgical treatment. Better postoperative visual field outcomes were achieved in patients who had smaller preoperative visual field defects. Transsphenoidal surgery significantly improved the visual field defects and visual acuity in patients with non-functioning pituitary adenoma, compared to the transcranial surgery patients.         경추간판탈출증 환자에서 요추천자후에 발생한 급성 양하지마비 - 증례보고 -

        경추간판탈출증 환자에서 척수조술을 위해 요추천자를 시행한 후에 갑자기 양하지마비가 발생한 1례를 보고 한다. 척추종괴환자에서 요추천자후 합병증으로 나타나는 운동마비는 요추천자후 1~4일째에 서서히 오는 경우가 일반적이며, 수시간내에 급성으로 하지마비를 나타내는 경우는 매우 드물어 현재까지 6례 정도만 보고되고 있을뿐이며 그 원인질환은 대부분 척수종양이다. 경추간판탈출증 환자에서 척수조술을 위해 요추천자를 시행한 후에 갑자기 양하지마비가 발생한 임상보고례는 현재까지 없다. 이러한 합병증을 막기위해서는 척수장애증상을 보이는 경추간판탈출증 환자에게 척수조술대신에 자기공명촬을 시행함이 현명하리라 본다. 이러한 합병증이 올 수 있는 기전과 예방방법에 대해 고찰하다. The incidence of paraplegia following drain of cerebrospinal fluid(CSF) by lumbar puncure below a spinal block is rare, and most of them occurred in spinal tumor. We report a case of acute paraplegia following lumbar puncture for computed tomography myelography(CTM) in a 42-year-old man who sustained a cervical disc herniation. Four hours after lumbar puncture for CTM, sudden paraplegia was developed. After emergent anterior cervical discectomy and fusion with cervical plating, the patient recovered completely. To the authors' knowledge, this is the first case of spinal shock complicating lumbar puncture for routine myelography in a patient with cervical disc herniation. The prompt recognition of this unusual complication of lumbar puncture may lead to good clinical outcome. Instead of CTM requiring lumbar puncture, MRI should be considered as the initial diagnostic procedure in a patient of cervical disc herniation associated with myelopathy. We discuss the possible mechanisms of acute paraplegia following lumbar puncture with literature review.
