Tuberculosis of the cystic duct lymph node is very rare. Only four cases have been reported in the literature. This paper presents the case of a young male patient with a tuberculous cystic duct lymph node and chronic cholecystitis, who was diagnosed with cystic duct stones and a gall bladder polyp preoperatively. 랜덤포레스트와 적외선 분광법을 이용한 팽나무속 (Celtis) 수종 식별 및 분광학적 특성 중요도 분석

본 연구는 잠재적 활용 가치가 높은 팽나무속 수종의 분류군 프로파일링을 위한 데이터를 확보하기 위해 수행되었으며, 본 논문에서는 적외선 분광법과 기계 학습 기법을 이용한 목재 식별 과정을 다루었다. 한반도에 자생하는 팽나무속 5 수종과 푸조나무속 1 수종으로 구성된 삼과 6 수종으로부터 4000-400 cm<SUP>-1</SUP> 영역의 적외선 스펙트럼을 획득하여 데이터세트를 구축하였다. 수종 식별을 위해 랜덤포레스트, 서포트 벡터 머신, 인공신경망 모델을 수립하였으며 랜덤포레스트의 평균 불순도 감소에 기반한 스펙트럼 특성 중요도를 산출하였다. 조사된 수종들의 분광학적 특성은 매우 유사하여 원본 적외선 스펙트럼으로 학습된 모델의 식별 정확도는 0.533-0.733 수준이었으나 2차 미분으로 전처리된 스펙트럼을 학습한 모델의 성능은 0.800-0.867로 상당히 개선되었다. 랜덤포레스트에서 1800-700cm<SUP>-1</SUP> 스펙트럼 영역이 특성 중요도가 높게 산출되었다. 해당 영역의 스펙트럼 데이터로 선택 학습된 모든 모델의 정확도가 0.867-0.933 수준으로 향상되어 실제로 수종 식별에 대한 중요도가 높은 영역임이 확인되었다. This study aims to accumulate data for taxonomic profiling of Celtis species with high potential usefulness. This paper describes a process for wood identification using machine learning techniques with infrared spectroscopy. A spectral dataset was built by acquiring infrared spectra in the 4000-400 cm<SUP>−1</SUP> region from six species of the Cannabaceae family. Random forest, support vector machine, and artificial neural network models were established for wood identification. In addition, the mean decrease impurity-based feature importance was produced from the random forest model. Because the spectral characteristics of the six species were very similar, the identification accuracies of the models trained with the original infrared spectra were low as 0.533-0.733. Data preprocessing using the Savitzky-Golay algorithm resulted in improved accuracy to 0.800-0.867. Random forest suggested that the feature importance of the 1800-700 cm<SUP>−1</SUP> region was relatively high. The identification performance of all models trained with spectral data of the 1800-700 cm-1 region was improved to 0.867-0.933, proving that the selected region has high importance for the identification of the Celtis species.

Purpose: This article compares the results of various methods of inguinal hernia repair in adults. Methods: We retrospectively reviewed the medical records of 473 patients over 20 years of age who received an inguinal hernia repair procedure between January 2000 and June 2007. We analyzed the patients’ clinical factors and outcomes, dividing them into five groups according to method of repair: Bassini group, Lichtenstein group, mesh plug group, Prolene Hernia System (PHS) group, and laparoscopic group. Results: The mean number of used analgesics was significantly smaller in the PHS group than in other groups (P<0.05, PHS group: 0.46, Bassini group: 1.55, Lichtenstein group: 1.20, mesh plug group: 0.82, laparoscopic group: 1.44). The mean operation time was significantly shorter in the PHS group than in the other groups (P<0.05, PHS group: 45.18 min, Bassini group: 59.85 min, Lichtenstein group: 68.20 min, mesh plug group: 66.73 min, laparoscopic group: 83.33 min). Finally, the mean postoperative hospital staywas significantly shorter in the PHS group than in the other groups (P<0.05, PHS group: 2.93 day, Bassini group: 4.96 day, Lichtenstein group: 4.17 day, mesh plug group: 4.01 day, laparoscopic group: 5.11 day). Out of five groups, there was onecase of postoperative recurrence in the Bassini group. Conclusion: Mesh PHS operation is a more preferred method of inguinal hernia repair in adults because it offers a lower postoperative pain, shorter operation time, shorter postoperative hospital stay.

Extramedullary plasmacytomas are uncommon malignant neoplasms that can occur in any organ. They arise most frequently from the upper aerodigestive tract. Lymph nodes are exceedingly rare sites of extramedullary plasmacytomas. Most extramedullary plasmacytomas of the lymph node arise in the cervical lymph nodes. Here, we report a case of an extramedullary plasmacytoma of the lymph node arising from a cervical lymph node. A 43-year-old male patient was admitted to our hospital and presented with a non-tender mass on the left side of his neck, which had been growing slowly for 1 month. The mass was excised. The pathology showed diffuse infiltration of immature plasma cells that were encapsulated by a layer of lymphoid cells, indicating an extramedullary plasmacytoma. He was treated with local radiation of the left cervical area. As of March 2009, he is doing well and shows no further evidence of the disease. (Korean J Med 2013; 84:751-754)
