Restaging TURB : 언제 어떻게 하는 것이 좋은가?

Approximately 70% to 75% of patients with bladder cancer initially present at a low stage (stage 1), a category that includes carcinoma in situ (CIS, 1-10% alone as primary), tumors confined to the urothelial mucosa (Ta, 70%-80%), and those that invade only the underlying lamina propria (T1, 20%). The prognosis for patients with non-muscle invasive bladder cancer (NMIBC) is generally good, with approximately 80% to 90% of patients alive at 5 years. In contrast, muscle-invasive bladder cancer, which represents about 25% of cases, has a significantly lower relative 5-year survival rate of 17% to 66% depending on tumor stage. Thus, adequate TUR is critical not only to ensure accurate staging and guide future management options, but also to remove all tumors from the bladder. However, understaging rates of up to 40% for NMIBC have been reported based on radical cystectomy data. Moreover, absence of muscularis propria in the specimen leads to a significantly higher rate of understaging (50-78%). In these cases, restaging transurethral resection (TUR) should be performed. In addition, patients with high-grade (HG) Ta and HG T1 tumors, regardless of presence of muscle, are also strongly encouraged to undergo a restaging TUR. Restaging resection should be performed 2 to 6 weeks following initial TUR. Deep biopsies in the base and periphery of the old resection site should be performed. The goal of restaging TUR is threefold: to improve staging accuracy, resect any residual tumor, and potentially to improve the response to intravesical treatment.
혼합농축콩단백을 첨가한 대체 소시지의 이화학적 특성에 관한 연구

본 연구는 우리나라에서 재배되는 대두, 녹두, 적두 및 완두 4종 콩 원물을 한 번에 추출하여 만든 혼합농축콩단백을 이용하여 대체 소시지를 개발하여 물리화학적 특성을 닭 소시지와 비교하였다. 경제성과 영양학적 특성을 고려하여 4종 콩 원물의 최적 혼합 비율을 설정하여 혼합농축콩단백을 추출하였을 때 혼합 농축단백 100 g을 기준으로 한국인 성인 남성 기준의 필수 아미노산 하루 평균 필요량이 충족되었다. 혼합농축콩단백을 이용하여 대체 소시지, 부분적 대체 소시지 및 닭 소시지의 물리화학적 분석 결과, 대체 소시지가 닭 소시지에 비해 요리 손실량, 튀김 손실량 및 유화성이 유의적으로 낮게 나타났으며(p<0.05), 닭 소시지 보다 물리화학적 품질이 우수하였다. 개발된 소시지의 물성적 특성은 대체 소시지가 닭 소시지에 비해 경도, 응집성, 검성, 씹힘성 및 탄성이 유의하게 높았다(p<0.05). 개발된 소시지의 색도 특성은 대체 소시지의 적색도(a) 및 황색도(b)가 닭 소시지에 비해 유의하게 높았고(p<0.05), 명도(L)는 닭 소시지가 대체 소시지에 비해 유의하게 높았으며(p<0.05), 부분적 대체 소시지는 대체 소시지와 닭 소시지의 사이 특성을 보였다. 따라서 본 연구를 종합해 볼 때, 혼합농축콩단백 소재가 영양학적인 면이 강화되었음을 확인하여 대체육 식품 소재로써의 활용 가능할 것으로 판단된다. 또한, 육류가 조금 들어간 부분적 대체 소시지는 육제품의 섭취를 줄이고 싶은 소비자를 대상으로 하는 대체육 산업에 잠재적인 가능성을 보였으며, 본 연구에서 개발된 대체 소시지는 식품 개발에 기초자료로서 활용 가능성을 보였다. The objective of this study was to optimize the mixing ratio of mixed bean protein concentrate (MBPC) and to improve the quality of sausage analogues. Soybean (Glycine max MERR), mung bean (Phaseolus radiatus L.), red bean [Vigna angularis (Wild.)], and pea (Pisum sativum L.) were mixed and processed to produce a MBPC, which was used to make a sausage analogue. The protein, moisture, and carbohydrate content were significantly (p<0.05) different among the samples. A significant (p<0.05) improvement was observed in textural properties (hardness, gumminess, and chewiness), cooking loss, frying loss, and emulsion stability of the sausage analogue. This study suggested the possibility of attaining high-quality sausage analogues and partial sausage analogues using MBPC, which could serve as a potential ingredient in meat analogues.
산양유 단백질 분해물/키토올리고당 나노 전달체 제조 및 물리화학적 특성연구

본 연구에서는 산양유 단백질 분해물과 키토올리고당을 이용하여 약 138~225 nm 크기를 지니는 구형의 나노 전달체를 성공적으로 제조하였다. 제조된 나노 전달체는 소수성 건강기능성 물질인 DHA와 레스베라트롤을 각각 ~22 mg/100 mL와 ~4.5 mg/100 mL 씩 포집가능하며, 제조 공정요인(예, TPP 농도, 산양유 단백질 분해물 농도, 키토올리고당 농도) 조절을 통해 입자 크기, 표면전하등과 같은 나노 전달체의 물리화학적 특성과 소수성 건강기능성 물질의 포집 효율을 조절할 수 있음을 알 수 있었다. 또한 산양유 단백질 분해물/키토올리고당 나노 전달체는 동결건조를 통해 분말화가 가능하기 때문에 식품 적용성이 뛰어나다. 결론적으로 본 연구에서 food-grade 물질인 산양유 단백질 분해물과 키토올리고당을 이용하여 제조한 나노 전달체는 저지방 및 무지방 유식품에 적용 가능한 무지방 기반(non-fat based)의 소수성 건강기능성 물질 전달체로 이용될 수 있을 것으로 기대된다. The aims of this study were to manufacture a hydrolyzed goat milk protein (HGMP)/chitosan ologisaccharide (CSO) nano-delivery system (NDS) and to investigate the effects of production variables, such as sodium tripolyphosphate (TPP), HGMP, and CSO concentration levels, on the formation and physicochemical properties of the NDS. An HGMP/CSO NDS was produced using the ionic gelation method at pH 5.5. Transmission electron microscopy and a particle size analyzer were used to determine the morphological and physicochemical properties of NDSs, respectively. The size of the HGMP/CSO NDS decreased from 225 to 138 nm as HGMP and CSO concentration levels decreased. The NDS had a positive surface charge, with a zeta-potential value of +23 mV. The encapsulation efficiency (EE) of docosahexaenoic acid was enhanced as the HGMP concentration level increased. Additionally, increasing the concentration level of CSO resulted in an increase in the EE of resveratrol. The HGMP/CSO NDS exhibited good physical stability during freeze-drying. Thus, our findings showed that the HGMP/CSO NDS was successfully manufactured and that HGMP and CSO concentration levels were key factors affecting the physicochemical properties of the NDS.

This study was carried out to investigate the various roles and effects of Morinda citrifolia L.(noni) on the human body and to utilize the findings obtained for future disease related research on food and nutrition. Morinda citrifolia L. has been used in folk medicine by Polynesians for over 2,000 years and is reported to have a broad range of therapeutic effects. Noni is an alkaloid system such as proxeronine, scopoletin, octanoic acid, terpenoids, alkaloids, anthraquinones, nordamnacanthal, morindone, rubiadin, b-sitosterol, flavone glycosides, linoleic acid, acubin, L-asperuloside, caproic acid, caprylic acid, ursolic acid, rutin. Noni's side effects have been reported to affect anti-inflammatory, analgesics, LDL antioxidant, anti-tumor, antifungal, antibiotic, antiviral, antiparasitic and immunosuppressive effects. In particular, noni's efficacy is considered to be important for the prevention of diseases by inhibiting active oxygen, which is a direct cause of oxidative stress, through various metabolites through 'xeronine system'. Noni's functions and effects that have been examined in this study include anti-inflammation, pain relief, antioxidant, anticancer, hepatocyte protection among others. Therefore, noni's extracts are considered highly useful for diverse scientific and nutritional health functional foods.
음주 여부에 따른 남자 대학생의 건강상태와 식습관에 관한 연구

The purpose of this study was to investigate the health status and food habits of male college students in Seoul according to drinking habits. In order to identify risk factors, health surveys were conducted from October 2013 to December 2013. Average height, weight, and BMI of subjects were $175.47{\pm}5.27cm$, $67.93{\pm}9.28kg$, and $22.04{\pm}2.67kg/m^2$, respectively. Weight and BMI were higher in the non-drinking groups compared to the drinking groups (p<0.05). Frequency of drinking was 2~3 times per month, and 52.4% of subjects started drinking when starting university. The motivation to start drinking was cited as "From necessity". Reason for drinking was "Social relations". The favorite kind of drink was beer. Rate of taking supplements was 24.4% of the subjects. Vitamin preparations were highest in drinking groups and non-drinking groups (p<0.05). The reasons for skipping meal were "too busy" in the drinking groups, and "oversleep" in the non-drinking groups (p<0.05). The preference for snacking in drinking groups were milk, milk products, and fast food, with the highest being listed first. The non-drinking groups preferred carbonated drinks and ice cream. The drinking groups need more education about their drinking habits. Drinking groups also need an increased intake of vegetables and fruits.
