        보철물 제작 후 부정확한 발음을 가진 환자에서 체계적인 진단 및 치료과정을 통한 보철 수복 증례

        Recently, there are cases where anterior esthetic prostheses are fabricated for better esthetics, but biologic, mechanical factors could be overlooked, too focusing on esthetic factor. This leads to changes in neutral zone, dentition, position of tongue and lips, occlusion and anterior guidance causing inaccurate pronunciation. Therefore, consideration of systematic diagnosis and treatment procedure are required. In this case, prosthesis was refabricated through a systematic diagnosis and treatment procedure using four factor (acoustic analysis, esthetic analysis, occlusion, neutral zone) for the patient who complained of inaccurate pronunciation and esthetics of the fixed prosthesis fabricated 10 years ago. Thus, by promoting functional, esthetic recovery, this case report demonstrates satisfying results to both the patient and dentist. 최근 들어 치아의 심미적 개선을 위해 전치부 심미 보철물을 제작하는 치료를 진행하는 경우가 있는데, 이 때 심미적인 요소에 치중하여 생물학적, 기계적 요소를 간과하기 쉽다. 이로 인해 중립대 공간 및 치열에 변화가 발생하고, 혀와 구순의 위치가 변화하고, 교합 및 전방유도 변화로 인해 부정확한 발음문제가 유발될 수 있어 이에 대한 체계적인 진단 및 치료과정에 대한 고려가 필요하다고 사료된다. 본 증례에서는 10년 전 제작한 고정성 보철물로 인해 부정확한 발음 및 비심미성으로 불편감을 호소하는 환자에서 음향학적 분석, 심미적 분석, 교합, 중립대 라는 4가지 요소를 이용하여 체계적인 진단 및 치료과정을 통하여 보철물을 재수복하였고, 이에 기능적, 심미적 회복을 도모하여 환자와 술자 모두에게 만족스러운 결과를 얻었기에 보고하는 바이다.

        임플란트 식립 환자의 $PostPlant^{TM}$ Calcium 투여 효과에 대한 임상연구

        연구목적: 본 연구에서는 건강기능식품인 $PostPlant^{TM}$ Calcium을 임플란트 식립 환자에게 투여하고, 투여하지 않은 군과 비교하여 그 유효성과 안정성을 평가하여 투여 후 골밀도의 변화 및 골유착 기간의 단축 여부를 평가하고자 하였다. 연구 재료 및 방법 임플란트 식립 후에Post$PostPlant^{TM}$ Calcium을 투여한 실험군은 18명의 환자를 대상으로 25개의 임플란트를 식립하였고, 투여하지 않은 대조군에서는 7명의 환자에게 9개의 임플란트를 식립하여 관찰하였다. 일차수술 후, 이차수술 후, 6개월 후에 $Osstell^{TM}$ Mentor, $Periotest^{(R)}$를 이용하여 임플란트 안정성을 비교 분석하였고, 일차수술후, 1, 3, 6개월 후에 방사선 사진 촬영을 시행하여 임플란트 주위 골밀도를 측정하여 비교 분석하였다. 결과:1. $Osstell^{TM}$ Mentor의해 측정된 ISQ 값은 측정 결과 시간이 경과함에 따라 실험군, 대조군 모두 수치가 높아지는 경향을 보였다. 실험군의 측정값이 대조군에 비해 더 높은 수치를 보였으며, 식립 6개월 후에 실험군과 대조군 간에 통계적으로 유의한 차이가 관찰되었다 (P< .05); 2. $Periotest^{(R)}$ 측정 결과 시간이 경과함에 따라 실험군, 대조군 모두 수치가 낮아지는 경향을 보였으며, 실험군이 대조군에 비해 더 낮은 수치를 나타내었으나 각 군간에 통계적인 유의성은 없었다. 3. 골밀도 측정 결과 시간이 경과함에 따라 실험군, 대조군 모두 골밀도 수치가 증가하는 경향을 보였으며, 실험군이 대조군에 비해 더 높은 수치를 나타내었으나 각 군간에 통계적인 유의성은 없었다. 결론:임상적으로 임플란트 식립 후 환자가 $PostPlant^{TM}$ Calcium을 장기간 복용하는 것이 임플란트 안정성을 높이고, 골유착을 성공적으로 이루는데 도움을 줄 수 있다고 사료된다. Purpose: The aim of this study was to evaluate the changes in bone density and reduction of time consumed for osteogenesis by $PostPlant^{TM}$ Calcium and to find out efficacy of $PostPlant^{TM}$ Calcium by comparing the group prescribed with $PostPlant^{TM}$ Calcium with the group without $PostPlant^{TM}$ Calcium prescription. Material and methods: The experimental group of 18 patients with 25 dental implant placement and the control group of 7 patients with 9 dental implant placement were randomly selected from the patients who visited prosthetic department of Dankook University Dental Hospital since July, 2006 (IRB Number ; 20060710). The experimental group was instructed to take $PostPlant^{TM}$ Calcium for 6 months after the implant surgery while the control group was instructed not to. Both experimental and control group were assigned for measurement using $Osstell^{TM}$ Mentor and $Periotest^{(R)}$ and radiographic examination was performed using specifically manufactured Aluminum Step Wedge. The results were compared and analyzed. Results: 1. According to the $Osstell^{TM}$ Mentor measurement, both the experimental and control group showed increase in values as time elapses and the experimental group showed significantly higher rate of increase (P < .05). 2. According to the $Periotest^{(R)}$ measurement, both the experimental and control group showed decrease in values as time elapses. In addition, greater decrease can be seen in the experimental group but no statistical significance was found. 3. By examining the radiographic images, both the experimental and control group showed tendency of increase in bone density. In addition, greater increase can be seen in the experimental group but no statistical significance was found. Conclusion: Clinically, taking $PostPlant^{TM}$ Calcium medicine for a long period of time after implant placement is expected for a better prognosis.

      • Effective topical treatment for preventing acne scar

        Acne scars result from delayed or inadequate treatment and healing of inflammatory acne lesions. Early, effective, and sustained treatment of inflammatory acne lesions can reduce the risk of atrophic acne scarring which is known to be related to dermal tissue loss. Topical retinoids can stimulate dermal fibroblasts to increase the production of procollagen in photoaged skin and topical tretinoin has a protective effect against ultraviolet radiation-induced loss of procollagen by antagonizing transcription factor AP-1, thereby blunting the increase in MMP synthesis. Adapalene 0.1 % and 0.3 % gel formulations have been shown to treat actinic keratoses and solar lentigines, and improve the effects of photodamage (fine and coarse wrinkles, mottled hyperpigmentation, and sallow complexion). As both photodamaged skin and atrophic acne scars share the feature of dermal matrix loss, adapalene 0.3 % may potentially exert a beneficial effect in the treatment of atrophic acne scars, similar to its effect on photoaging.

        심하게 흡수된 치조제를 가진 환자에서 CAD/CAM을 이용한 임플란트 고정성 보철치료

        Loss of dentition can lead to not only compromised esthetics and functions of the patient, but also alveolar bone resorption. Bone grafting with prosthetic reconstruction of the gingiva can be selected for the treatment, and it provides many benefits as prosthetic gingival reconstruction does not require a complicated surgical process and is available within a short period of time, with stable clinical results. However, conventional porcelain fused to metal prosthesis has certain limits due to its size, and deformation after several firing procedures. In this clinical report, the author would like to introduce a patient with severe alveolar resorption who was treated with gingiva-shaped zirconia/titanium CAD/CAM implant fixed prosthesis for esthetic and functional rehabilitation. Clinical reports Clinical report 1, 2 : A case of loss of anterior dentition with atrophied alveolar bone. Implant retained zirconia bridge applied with Procera implant bridge system to simulate the gingiva. Upper structure was fabricated with zirconia all ceramic crown. Clinical report 3, 4 : A case of atrophied maxillary alveolus was reconstructed with fixed implant prosthesis, a CAD/CAM designed titanium structure covered with resin on its surface. Anterior dentition was reconstructed with zirconia crown. Conclusion and clinical uses All patients were satisfied with the outcome, and maintained good oral hygiene. Zirconia/titanium implant fixed prosthesis fabricated by CAD/CAM system was highly accurate and showed adequate histological response. No critical failure was seen on the implant fixture and abutment overall. Sites of severe alveolar bone loss can be rehabilitated by implant fixed prosthesis with CAD/CAM system. This type of prosthesis can offer artificial gingival structure and can give more satisfying esthetics and functions, and as a result the patients were able to accept the outcome more fondly, which makes us less than hard to think that it can be a more convenient treatment for the practitioners.
