        Angle's class I 교합의 한국인 성인 코에 두부방사선 계측학적 연구

        The purpose of this study was to establish a set of standard values for the form of the nose and its relationship to other craniofacial structures in young korean adults in angle's class I occlusion. The subjects were 40 (20 male, 20 female) Korean dental students in angle's class I occlusion. Lateral cephalometric radiographs were used to determine the form of the nose and its positon relative to other craniofacial structures. The results were obtained as followed. 1. There was a significance between the registered male and female measurements in nasal height(P<0.05). (Male: 58.23mm Female : 54.62mm) 2. There was a significance between the registered male and female measurements in nasal length(P<0.05). (Male : 54.18mm Female : 49.38mm) 3. The vertical distances from the tip of the nose in Korean adults were significantly greater in the male(P<0.05) and were smaller in both sex in comparison with caucasian.

        골형성단백질(BMP)의 골유도 실험에서 맥동전자기장의 효과에 관한 연구

        Urist demonstrated that a specific protein purified from bone matrix had osteogenesis, and named it Bone Morphogenetic Protein (BMP). The author employed Urist method to obtain crude BMP, and then implanted it into the mandibular defects of rats. A demineralized bone matrix was obtained from a femur of bovine bone. It was separated and purified by repeated solubilization and precipitation of the protein, and molecular weight of its BMP was evaluated by means of SDS-polyacrylamide gel electrophoresis. BMP was implanted as inlay graft on defect area of rat mandibulae in the experimental groups, and a collagen without BMP was implanted as the same in the preliminary experimental groups, then each group was correlated whether pulsating electromagnetic fields was exposed or not and aperiosteum was preserved or not. Thee histological studies of two groups were employed to observe the osteoinduction process and BMP-induced bone growth on the effect of pulsating electromagnetic fields 1, 2, 3 and 4 weeks after implantation respectively. The results were as follows : 1. The molecular weight of bovine BMP was about 19,100. 2. BMP had a excellent bone forming activity as a implant materials to the repair on the bone defects. 3. The exposure of pulsating electromagnetic fields influenced to the cell differentiation and proliferation since initial stage of ossification as independent from existence of periosteum and BMP, and the exposure of pulsating electromagnetic fields combined with BMP activated the osteoinduction process more. 4. In case of implantation of BMP, preservation with a periosteum showed quicker repair in early ossification stage, but there was no difference between defects and preservation with periosteum after 4 weeks.

        Immediate implant placement has become an acceptable treatment in dentistry and oral maxillofacial surgery for reducing the treatment period and utilizing extraction socket. From the viewpoint of the healing of extraction socket and periodontology during implantation, integrity of the soft tissue and hard tissue after loss of tooth should be considered. The advantages of the immediate implantation after extraction is prevention of natural buccal or labial alveolar bone loss and is available to healing of extraction socket and osteogenesis. But osteointegration is affected to the defect of soft tissue and alveolar bone after extraction. And healing condition of extraction socket is related to implant-prosthesis treatment and oral hygiene.

        동종이식골의 탈회정도가 이소성 골형성유도에 미치는 영향

        동종 이식골 내의 탈회 정도에 따른 잔존 칼슘치가 이소성 골형성능에 미치는 영향을 파악하고자 Sprague-dowley계 백서에서 채취한 경골 및 대퇴골의 골간부를 0.5cm 크기로 절단하여 부착 연조직은 제거한 다음, 초음파 세척장치를 이용하여 $60^{\circ}C$ 0,6N HCl용액으로 5분, 10분, 15분, 20분, 25분, 30분, 35분, 40분간 각각 처리하여 탈회 동종 이식골을 준비하였다. 이때 탈회시간에 따른 시편의 무게를 측정하였고, 각 탈회용액으로부터 1cc를 취한 다음 Sigma사의 진단용 칼슘치 측정kit를 이용하여 spectrophotometer로 600nm 파장하에서 칼슘치를 측정하였다. 그리고 탈회 정도에 따른 이식골편의 골형성 유도능을 확인하기 위하여 24마리의 Sprague-dowley계 백서를 탈회시간별로 8군으로 나누어 배부에 0.5cm 크기로 4군데의 피하낭을 형성한후 각각의 처리된 탈회 이식골편을 이식하였다. 매식된 동종 이식골편들을 술후 1, 2, 3주째 채취하여 통법에 따라 H&E염색 표본을 제작하여 광학현미경으로 관찰하였다. 이상의 실험에서 다음과 같은 결과를 얻었다. 1. 탈회 30분까지의 용출되는 칼슘 농도 변화는 평균 15.91mg/ml로, 탈회 20분 이후에 평균 99.65%의 탈회정도를 보였다. 2. 동종골 무게 변화량은 탈회시작 25분까지 뚜렷한 무게변화를 보였으나, 그 이후에는 변화의 정도가 미약하였다. 3. 탈회된 이식 통종골에의한 이소성 골형성 유도능 비교에서 20분에서 30분간 탈회된 군에서 가장 양호하였고 그 외의 군에서는 그 정도가 열등하였다. 이상의 결과는 탈회된 동종골에 있어 골형성 유도능을 극대화하기 위해서는 골기질내 무기성분의 완전한 탈회가 선행되어야 하며, 탈회골 기질의 변성이 최소화되어야 함을 시사한다. This study has been performed to evaluate the relationship between the remained mineral components in a decalcified bone matrix and an ectopic bone formation efficiency. The freezed rat diaphyseal cortical bones measuring 0.5cm in length were demineralized in heated 0.6N HCl at $60^{\circ}C$ for 5, 10, 15, 20, 25, 30, 35, 40 minutes, respectively, using a controlled heat ultrasonic cleaner. Each 1cc of decalcifying solution taken during decalcification procedure was used to calculate calcium content using calcium dignostics kit under 600nm of spectrophotomer. After decalcification, each specimen was also weighed. Then each prepared specimen was implanted into the dorsal pouch of 24 Sprague-Dawley rats divided into 8 groups by time course. The implants were harvested at 1, 2, and 3 weeks and prepared for routine H-E stain specimens to evaluate osteogenic activity. The results are as follows: 1. There was statistical significant difference in change of calcium concentration up to demineralization of 30 minutes and each allogenic bones decalcifed up to 20 minutes revealed 99.65% of decalcification in average. 2. There was statistical significant difference in change of weight in demineralized allogenic bone up to 20 minutes treatment but, no significant change was noted after that time. 3. The histologic analysis revealed active ectopic bone formation in the implanted allografts demineralized for 20, 25, 30 minutes, respectively. However, the other groups of allografts showed relatively poor osteoinductive activity. These findings suggest that complete decalcification with a minimized degeneration of collagen matrix is necessary to induce maximal osteogenesis by decalcified bone allograft.

        상악 측절치 근관치료 중 수산화칼슘 호제근충제 과충전으로 인하여 발생한 신경손상의 치험례

        Calcium hydroxide root canal filing paste (vitapex) is widely used as canal filling paste for infected canal. However, chemical burn is possible because of the high alkali base of calcium hydroxide. A 57-year old woman was admitted to our clinic for consistent dull pain and paresthesia in the left upper lip, zygoma and buccal cheek area, which developed during an endodontic treatment of the left lateral incisor. Radiographic finding showed radiopaque material, which exits from the left incisor root apex, and was within the left canine and first premolar buccal soft tissue. The overfilled Vitapex extended to the soft tissue was surgically curetted. The result of the surgical curettage was favorable. Though slight hypoesthesia on the upper lip was still remained, paresthesia on zygomatic and buccal cheek area was completely recovered. As far as we know, this is the first case report of infraorbital nerve damage from overfilled Vitapex material.

        치성각화낭의 외과적 소파술 후 하치조 동맥에 발생한 가성 동맥류: 증례보고

        Pseudoaneurysms are rare complications of orgthognathic surgery, trauma or other surgical procedures in the head and neck regions. Surgical approach is a routine procedure of bleeding control. However, bleeding control using angiography and embolization can also be an excellent method. We experienced pseudoaneurysm of the inferior alveolar artery occurring after surgical curettage of cystic lesion. The 33-year-old man who underwent surgical curettage of keratocystic odontogenic tumor was presented with severe bleeding in the right mandible ramal inner surface 14 days after the surgical curettage. After hemostasis with vaseline gauze packing and pressure, bleeding temporarily stopped. However, bleeding started after 14 days and soon it has been continued. Finally, the patient was diagnosed as pseudoaneurysm of the inferior alveolar artery from the enhanced-computed tomography images, and angiographic embolization was performed successfully. We report a rare case of pseudoaneurysm of the inferior alveolar artery after benign tumor curettage and review of the previewer's literature.

      • 골격성 III급 부정교합자의 편악(하악)수술후 연조직 변화의 평가

        The purpose of this study was to evaluate the amount and interrelationship of the soft and hard tissue change after mandibular setback surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patient (12 male and 13 female) who had severe anteropostrior skeletal discepancy. These patient had received presurgical orthodontic treatment and surgical treatment which is bilateral sagittal split ramus osteotomy. The presurgical and postsurgical lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The result were as follows: 1. After mandibular bilateral sagittal split ramus osteotomy, lower facial soft. tissue horizontal posterior changes were high significance value. but vertical soft tissue changes were low significance value. 2. After mandibular bilateral sagittal split ramus osteotomy, relative upper lip protrusion increased(p<0.01) and relative lower lip protrusion decreased(p<0.01) and lower facial soft tissue thickness increased(p<0.01). 본 연구는 골격성 III 급 부정교합 환자에서 하악골 후방이동수술을 시행하였을 때 경, 연조직 변화를 관찰하고 수술전후의 경, 연조직 변화의 상관성을 산출하여 교정-악교정 수술 복합 치료 계획의 수립과 결과의 예측에 이용하고자 하였다. 영남의료원 치과에 내원하여 상하악골의 수직적인 부조화는 경미하고 전후방적인 부조화가 심한 골격성 III급 부정교합으로 진단되어 수술전 교정치료를 받고 1989년 10월부터 1997년 1월 중에 하악골 시상분할 골절단술로 하악골의 후방이동을 시행한 성인 환자 25명(남자 12명, 여자 13명)을 대상으로 수술전후 측모두부방사선사진을 계측, 분석하여 다음의 결과를 얻었다. 1. 하악골의 시상분할 골절단술로 하악골의 후방이동에 따른 하안면부의 연조직의 수평적인 후방 이동은 상당한 유의성을 가지고 나타났으나 수직적인 변화는 거의 없었다. 2. 하악골의 시상분할 골절단술로 하악골의 후방이동을 시행한 경우 상순의 상대적인 돌출도는 증가하고 (p<0.01) 하순의 상대적인 돌출도는 감소하였으며(p<0.01) 하악골 전방부의 연조직의 두께는 증가하였다(99% 유의수준).

        구개열 환자의 치조열 골이식의 평가

        Purpose: The purpose of this study is to evaluate the results of alveolar bone grafting in patients with various types of cleft lip and to compare the success rates according to the lateral incisors and canines. Methods: The postoperative radiographs of 20 patient with a cleft lip and alveolar process alone (CLAP), complete unilateral cleft lip and palate (UCLP) and complete bilateral cleft lip and palate (BCLP) were retrospectively analyzed. The alveolar bone height was classified according to ${\AA}$byholm (1981) and Bergland (1986) and we evaluated the dentition at the time of surgery and the existence of a lateral incisor and impacted canines. Results: 16 (80%) of the 20 patients were assigned to Type I & II and they were considered successful. In the UCLP group, the success rate was significantly better than that of the UCLP and BCLP groups. The success rate was significantly better than when the cleft was grafted with the existence of a lateral incisor and before the eruption of the canines. Conclusion: The severity of the deformity influenced the success rate. The timing of the operation was a critical variable that affected the outcome in patients with cleft lip and palate.

        관혈적 정복술로 치료한 하악골 골절에서의 합병증

        Purpose: The aim of this study was to evaluate the complications of open reduction and internal fixation of mandibular fracture with miniplates. Patients and Methods: A total of 134 patients who presented with 196 fractures were analyzed retrospectively. Complications were evaluated for factors such as age, the site of fracture, the severity of fracture, delayed operation, preoperative wound contamination, the site and disposition of teeth in the fracture line and midfacial fractures. Statistical analysis was used to compare complications to risk factors. Results: Of the 134 patients, 20 patients had some form of postoperative complications and complication rate was 14.9%. These complications included infection, plate fracture, malocclusion, wound dehiscence, nerve injury and nonunion. There was a significant correlation between complication rate and the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth(p<0.05). Conclusion: The occurrence of postoperative complications in the treatment of mandibular fractures was related to the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth.
