        우리나라 권역 장애인구강진료센터 이용자 만족요인 비교

        Objectives: The purpose of this study was to identify the factors of satisfaction with regional oral health centers for people with disabilities in Korea. Methods: A survey was conducted with 784 patients who visited regional oral health care centers in Korea. A self-developed questionnaire (29 items) was adapted with a consent form and IRB (Institutional Review Board). All collected data were analyzed for statistics using SPSS version 25.0. Results: The study participants were 452 (57.7%) male patients, 207 (26.4%) aged 20-29 years old, and mostly had physical, developmental, and mental disabilities. The patients were introduced by their acquaintances, and the major reason for visit was specific dental care for the disabled. Most patients used their cars and took half to one hour to commute. The common factor affecting the comprehensive satisfaction was the facility satisfaction. Conclusions: A more comprehensive and standardized questionnaire should be developed for the annual evaluation of all centers. This study presents the basic data that can aid to activate the operation of the four oral health care centers for the disabled, which are scheduled to be implemented in the future, as well as 10 currently operated centers.

        대구 지역 일부 중고등학교 학생의 구강상태와 구강건강관련 삶의 질의 관련성

        이 연구는 대구 지역 일부 청소년을 대상으로 구강건강 상태와 구강건강 관련 삶의 질에 대해 조사하였으며, 청소년의 구강건강 관련 삶의 질에 미치는 요인에 대해 분석하였다. 연구 대상자 중 주관적 구강건강 상태에 대해서 나쁘다고 생각하는 청소년은 24명(20.2%), 보통은 67명(56.3%), 좋다고 생각하는 청소년은 28명(23.5%)이었고, 1년 내 구강 내 통증을 느낀 적이 있다고 응답한 청소년은 72명(60.5%), ‘없다’는 47명(39.5%)이었다. 구강건강 관련 삶의 질의 점수는 최하 14∼70점까지로 연구 대상자의 평균 총 점수는 64.76 (±6.71)점으로, 점수가 높을수록 구강 문제로 인한 삶의 질의 영향이 낮은 것이다. 청소년의 구강건강 관련 삶의 질에 영향을 미치는 요인은 1년 내 구강 내 통증 경험이 있을수록 구강건강 관련 삶의 질이 낮은 것으로 나타났으며, 주관적 구강건강 상태, 학년 순으로 영향이 있는 것으로 나타났다. 일생 동안 신체적, 정신적 변화가 가장 많은 청소년 시기의 삶의 질은 매우 중요하다. 이러한 삶의 질에 구강건강이 미치는 영향에 대해 조사한 연구로 매우 의미 있는 연구이며, 청소년의 구강건강 불편 증상을 최소화하기 위한 노력이 필요할 것으로 생각된다. The purpose of this study was to evaluate the association between oral health status and oral health-related quality of life (OHRQoL) by using the Oral Health Impact Profile-14 (OHIP-14) among adolescents in a metropolis of Korea. A total of 119 students in 1st grade in middle school and 1st grade in high school participated in this cross-sectional study. Oral examination was conducted by two trained dentists to collect in for mation about Decayed, Missing, and Filled Teeth (DMFT) and Community Periodontal Index (CPI). OHRQoL was evaluated by short-form of OHIP-14. Wilcoxon correlation, Kruskal-Wallis test and multiple regression analyses were performed to assess the association between oral health status and OHRQoL. All data was analyzed by SPSS 22.0 and the significance level was set at 0.05. The average of OHIP-14 score in adolescents having no dental caries (17.88±5.13) was higher than those having more than one DMFT index (19.75±7.16). The average of OHIP-14 score in adolescents having bleeding observed (20.21±8.01) was higher than those having more than calculus detected (18.66±5.65). The level of OHRQoL was higher in the participants perceiving better oral health condition compared to those perceiving poorer condition (p<0.05). The result of this study could provide the information that OHRQoL from adolescents positively associated with self-assessed oral health status than oral health with DMFT and CPI. Further studies are needed to well-designed follow-up studies.

        대도시지역 일부 초등학생의 치과용 아말감 충전치아와 요중 수은 농도의 관련성

        Dental amalgam is an alloy composed of a mixture of approximately equal parts of elemental liquid mercury and an alloy powder. Amalgam has been the most popular and effective restorative material used in dentistry. Despite the long history and popularity of dental amalgam as a restorative material, there have been periodic concerns regarding the potential adverse health effects arising from exposure to mercury in amalgam. Since children are more at risk for mercury toxicity, we aimed to assess the association between dental amalgam filling and urinary mercury concentration in children. 581 of elementary school children in grades 1st4th were conveniently recruited from two schools located in Daegu city, Korea. To obtain dental caries experience states, oral examination were conducted using the full term for DFS index, number of amalgam filling surfaces and the type of filling materials. A questionnaire was used to collect information about general characteristics and the frequencies of tooth brushing, gum chewing and fish/seafood consumption. The statistical analysis was done using the SPSS 18.0 program. The mean urinary mercury concentration in children having more surfaces was highest. As a results Urinary mercury concentration of children who have 79 teeth of amalgam filling and more than 10 is higher than without amalgam filling. The number of amalgam filling surface is closely related with urinary mercury concentration.

        우리나라 치아우식증 고위험군 12세 아동의 구강특성과 구강건강관련행동 위험요인

        Objectives: The aim of this study was to examine the characteristics of the high caries risk group among 12-year-old children inKorea. Methods: Information on oral health status and interview data were collected from 6,253 children (3,309 male and 2,944 female children) aged 12 years who participated in the Korea National Oral Health Survey in 2010. The subjects were sorted according to the decayed missing filled tooth (DMFT) data. The highest one-third was selected as the significant caries (SiC) group (N=2,184), and the others were classified as the middle group (N=4,466) and the caries-free group (N=7,067). This study also useddemographic variables such as gender and area of residence. The variables for the oral health behavior were the number of dental sealants on the first molar, self-perceived oral health, average frequency of tooth brushing per day, and the mean frequency of daily snack consumption. The variables for the oral health status were the mean number of DMFTs and the SiC index. The associations between the high caries risk group and the demographic and social variables, oral health status, and oral health-related consciousness and behavior were evaluated by logistic regression analysis. Results: A significant proportion of the high caries risk group participants had no dental sealant on the first molar (46.4%) and lived in rural areas (38.1%, P<0.05). The determinants of the high-risk group were whether the first molar was sealed (``0``, OR=52.67) and the self-perceived oral health (``fair``, OR=1.43, ``poor``, OR=2.09). Conclusions: The characteristics of the high caries risk group among 12-year-old Korean children were the absence of dental sealant on the first molar, poor self-perceived oral health, gender, and region.

        일부 성인의 구강건강 지식이 구강건강 증진 행위에 미치는 영향

        Objectives: This study aimed to investigate oral health-related knowledge in adults and to determine the effect of oral health knowledge on correct brushing and use of dental care products. Methods: The subjects of this study were 552 adults who visited dental clinic, and a self-administered questionnaire was administered. Statistical analysis was performed using the t-test, ANOVA, and multiple regression using the SPSS statistics program (ver. 23.0, IBM Co., Chicago, IL, USA). Results: The oral health knowledge level, correct brushing, and dental care product usage scores were found to be positively correlated (r=0.015, 0.016), and both correlation coefficients were statistically significant (p<0.05). Oral health knowledge was found to have a significant effect on correct brushing among oral health promotion behaviors (β=0.116, p=0.006), but it was not found to have a significant effect on the use of dental care products (β=0.053, p=0.214). Conclusions: Oral health promotion behavior is closely related to oral health knowledge, and it is thought that oral health promotion behavior can be improved by raising oral health knowledge levels.

        일부 지역사회 노인의 치주염 심도에 따른 타액 내 치주염 원인균 분포와 연관성 분석

        Objectives: Periodontitis-causing microorganisms and their virulence factors can provoke periodontal destruction in the host. This study was aimed at evaluating the distribution of periodontal disease and its relationship with 11 periodontal disease-causing bacteria in the elderly. Methods: Individuals aged 60 years or above were recruited after obtaining informed consent. The clinical attachment loss was measured at studied sites to determine the severity of periodontitis. Further, the stimulated salivary samples were collected and analyzed with real-time polymerase chain reaction to detect 11 strains of periodontitis-causing bacteria. Results: The severity of periodontal disease was proportional to the amount of periodontal diseasecausing bacteria. Porphyromonas gingivalis in the red complex increased from 6.60±5.50 in stage 1 to 5.36±5.39 in stage 2 and 7.19±5.56 in stage 3 (P =0.003). Tannerella forsythia increased from 6.54±4.60 in stage 1 to 7.44±4.56 in stage 2 and 8.49±4.70 in stage 3 (P =0.007). Conclusions: The presence of complex bacterial groups and their number of strains were high in participants with severe periodontitis. Controlling periodontitis-related bacteria is important for periodontal health in the elderly.

        장애인구강진료센터 내원환자들의 재내원 의향 요인

        Objectives: The aim of this study is to identify the factors related to re-visit intentions of patients who had visited the 10 oral health care centers for the disabled currently operating in Korea. Methods: A self-developed questionnaire (29 items) was distributed among 1,000 randomly selected patients and their guardians who had visited one of the 10 oral health care centers for the disabled, with a consent. Consequently, 874 questionnaires were collected and analysed for statistics using SPSS 25.0. Results: The patients traveled a long distance to visit the oral health care centers because of their specialization for the disabled. With the level of satisfaction with dentist services, dental hygienist services, and facilities increasing, the re-visit intention proportionally rose. Conclusions: This study presents basic data that provides information on the operation of 10 oral health care centers for the disabled. It is imperative to develop a more structured questionnaire, and conduct an annual survey of the disabled visiting the centers.

        β-Gluconsan Calcium이 구강암 세포의 성장에 미치는 영향

        In recent years, there has been a global trend toward the importance of natural extracts for the prevention and treatment of human diseases. β-glucan is known to have anti-inflammatory activity, anti-cancer, and improvement of immune system. Polycan is purified β-glucan from Aureobasidum pullulans SM-2001. The anti-cancer effects of β-gluconsan calcium, polycan and calcium gluconate complex, were evaluated in human oral cancer YD-10B cells. YD-10B cells were cultured in the presence of 0, 0.5, 0.75, 1 mg/ml β-gluconsan calcium for 48 hours. MTT assay, cell counting, and observation of cell morphology were conducted. The number of cells decreased and cell morphology changed in the 0.5 mg/ml of β-gluconsan calcium. Almost all cells were dead in the 0.75 and 1 mg/ml. MTT assay showed a dose-dependent reduction in cell proliferation (p<0.05). These results indicate that β-gluconsan calcium exhibiting anti-cancer effects in YD-10B cells through changes in cell morphology and cell death.

        치과 내원환자의 추천지수에 영향을 미치는 서비스 요인

        Objectives: Many people rely on recommendations from family, relatives, and other information sources in order to select the appropriate dental clinic for treatment. The object of this study was to find out the relationship between quality of dental service and Korean Net Promoter Score (KNPS). Methods: A total of 520 patients were selected through consecutive sampling from four dental clinics in Daegu city. Informed consent of all selected patients was obtained. The patients were required to complete a questionnaire that comprised of three categories of queries related to provision of satisfactory dental service, relation quality, and personal data. Sub-categories of questions included physical and environmental factors, human service factors, patients` satisfaction with related quality and KNPS. Data was collected by interviewing all individuals on a personal basis over a period of 3 months. Statistical analysis was performed using studental t-test, ANOVA and multiple regression analysis using the SPSS 20.0 software (IBM Co., Armonk, NY, USA). Results: Among the various socio-economic factors studied, income was most significantly related to KNPS while satisfaction of relation quality was the largest factor affecting the KNPS. Factors such as satisfactory physical environment, quality of human service, relation quality and income accounted for 52.4% of KNPS. In short, the three major factors that affected KNPS were patient compliance, professionalism of the attending dentist and response of hygienist to patient. Other minor factors influencing KNPS included trust upon the attending dentist, aesthetics of clinicstiinterior, and physical convenience. Conclusions: It is highly recommended that dental clinics should make strenuous efforts to increase the level of patient compliance, professionalism, and response towards patients, which can be achieved by improving the quality of service and through positive marketing of the hospital.

        구강건강정보 이해능력이 인지된 구강건강상태에 미치는 영향

        Objectives: This study was conducted to analyze the effect that oral health literacy of adults has on perceived oral health state and to identify the factors that affect perceived oral health status. Methods: A self-administered questionnaire survey was distributed to 554 adults who lived in the Daegu or Gyeongbuk areas and visited dental institutions located in the Daegu area. For adults who were 60 years old or older, direct interviews were conducted. Statistical analysis was performed with t-tests, analysis of variance, and multiple regression using SPSS. Results: Between oral health literacy and perceived oral health status, a significant positive correlation was revealed (r=0.142, P<0.01). A significant positive correlation was revealed between oral health literacy and knowledge (r=0.397, P<0.01). A significant but weakly positive correlation was revealed between oral health knowledge and perceived oral health status (r=0.086, P<0.05). To identify factors affecting perceived oral health status, multiple regression analysis was conducted, and the result showed that oral health literacy was a statistically significant factor (b=0.105, P<0.05). Among general characteristics, age was a statistically significant negatively affecting factor, as it showed a negative relationship with perceived oral health status (β=―0.140, P=0.001, adjusted R<sup>2</sup>=0.049). Conclusions: When these study results are taken into consideration, oral health literacy is a factor affecting perceived oral health status, and as such, oral health literacy of the research targets should be taken into consideration when implementing oral health education to promote oral health.
