        In surgical treatment of the malignant melanoma, radical resection and a sentinel lymph node biopsy are essential procedures to eradicate the tumor and to minimize the risk of local recurrence. For the reconstruction of skin defect after tumor resection, a skin graft has been generally performed procedure. However, if tendon or bone is exposed after tumor resection, simple skin graft is not enough for the coverage of the defect and additional procedure is mandatory. In this study, we reviewed the clinical results of 16 patients, who had been diagnosed with malignant melanoma of the limb and underwent surgical resection and reconstruction of the defect with various methods. The sentinel lymph node dissection was performed in all patients combined with radical excision of the tumor. In 11 cases of positive sentinel lymph nodes, further elective lymph node dissections were performed. The mean tumor invasion depth was 4.54 mm (2~10 mm), and AJCC stage II was most common (9 cases). The pedicled flaps were performed in 10 cases for the reconstruction of defects. All flaps were successfully survived without significant complications until the last follow up. In conclusion, a pedicled flap coverage is very useful reconstruction strategy for the soft tissue defects after radical resection of malignant melanoma.

      • 신생아 주관절 화농성 관절염의 초음파 이용의 제한: 증례 보고

        신생아 주관절에 발생한 화농성 관절염은 드물지만 관절에 심각한 손상을 입힐 수 있어 발견과 치료에 매우 주의를 요하는 질환이다. 하지만 신생아들은 급성 감염의 증후나 증상이 잘 나타나지 않아 조기 진단이 어려운 경우가 많다. 이러한 경우 관절 천자는 관절 감염을 확인할 수 있는 매우 중요한 검사 방법중의 하나이다. 많은 임상 의사들이 그 결과를 중요하게 생각함에도 불구하고 저자들은 신생아에서 주관절 내 농양이 완전히 고형화되어 관절천자에서 음성 소견을 보임을 경험하였다. 초음파 검사는 주관절 내에 일반적인 관절 삼출액과는 다른 신호 강도의 관절 팽대 소견을 보여주어 빠른 수술적인 치료를 결정하는데 주요한 지침이 되었다. 모호한 증상과 비특이적인 검사 소견을 보여 치료 결정이 어려운 신생아들에게 초음파 검사는 빠르고 쉽게 시행할 수 있는 좋은 검사 도구이기는 하지만 비특이적인 경우 반드시 MRI 등의 추가적인 검사를 시행하여야 할 것으로 생각된다. Septic arthritis of the elbow is a rare, but serious disorder in neonates. However, neonates often present with few systemic signs other than irritability or poor feeding. When the clinical findings are minimal or absent, joint aspiration gives us helpful information in making a diagnosis. Although many clinicians rely on its result, we experienced a case of early clotted joint effusion, which resulted negative joint aspiration in a neonate with septic elbow arthritis. Ultrasonography helped us to make an early decision to perform surgical intervention as showing different echogenicity of intracapsular portion from generally presented in septic effusion. Presenting case shows that ultrasonography can be an excellent second step examination of neonatal elbow presenting atypical clinical findings of septic arthritis.

      • 감염 창상 치료에 있어 유리 피부 및 생골 피부편 이식

        Purpose : We analyse retrospectively the clinical result of consecutive free flap and osteocutaneous flap transfer in the chronic osteomyelitis, nonunion combined with infection and soft tissue defect with infection. Materials and Methods : From December 1989 to Jun 2003, free flap and osteocutaneous flap transfer was performed in 225 patients with osteomyelitis or infected non-union. 44 cases of these patients had revealed antibitotics resistant organism in wound culture, and these 44 cases were investigated in the mechanism of the injury, recurrence of infection, radiographic union, follow-up clincal results, and postoperative complications. Results : Among the 44 cases, consecutive procedures of osteocutaneous flap transfers(26 cases) and free flaps(18 cases) were performed. Causative organisms were MRSA(20 cases), Pseudomonas aeruginosa(18 cases), acinetobacter(2 cases), and so forth. Initial bony union was obtained in the average 6.3 months. Recurrence of infection in free flap and osteocutaneous free flap were occurred in 3 and 4 cases respectively. Eventually, all the cases attained successful subsidence of the inflammation. Conclusion : Free flap and osteocutaneousflap transfer have provided the greatest improvement of surgical results in infected non-union, chronic osteomyelitis and soft tissue defect with infection. Further clincal studies maybe required to minimize failure rate.

      • 선택적 iNOS 억제에 의한 골격근 재관류 손상의 감소

        The purpose of this study is to determine the effects of selective inducible nitric oxide synthase (iNOS) inhibitor N-[3-aminomethyl]benzyl]acetamidine (l400W) on the reperfused cremaster muscle. The extracellular superoxide dismutase knockout ($EC-SOD^{-/-}$) mice was used to make the experimental window for ischemia-reperfusion injury. The muscle was exposed to 4.5 h of ischemia followed by 90 min of reperfusion and the mice received either 3 mg/kg of 1400W or the same amount of phosphate buffered saline (PBS) subcutaneously at 10 min before the start of reperfusion. The results showed that 1400W treatment markedly improved the recovery of the vessel diameter and blood flow in the reperfused cremaster muscle compared to that of PBS group. Histological examination showed reduced edema in the interstitium and muscle fiber, and reduced nitrotyrosine formation (a marker of total peroxinitrite ($ONOO^-$) in 1400W-treated muscle compared to PBS. Our results suggest that iNOS and $ONOO^-$ products are involved in skeletal muscle I/R injury. Reduced I/R injury by using selective inhibition of iNOS is perhaps via limiting cytotoxic $ONOO^-$ generation, a reaction product of nitric oxide (NO) and superoxide anion ($O_2^-$). Thus, inhibition of iNOS appears to be a good treatment strategy in reducing clinical I/R injury.

        The Effect of Nitric Oxide Donor, SNAC on Motor Functional Recovery of Reperfused Peripheral Nerve

        목적: 산화질소 공여물질인 S-nitroso-N-acetylcysteine (SNAC)의 투여가 말초신경 허혈 재관류 손상 후 운동기능의 회복에 미치는 영향을 알아 보았다. 대상 및 방법: 78마리의 쥐를 SNAC 투여군, methylprednisolone (MP) 투여군, PBS 투여군으로 분류하였다. 1 ㎝의 좌골 신경에 2시간 동안 허혈을 유도한 후 재관류 시켰으며, 재관류 후 다양한 시점에 walking tract 검사(SFI), 근 수축 검사 및 근육의 무게를 측정하여 운동 능력의 회복정도를 비교하였다. 결과: SNAC 투여군은 MP, PBS 투여군에 비해 월등히 우수한 좌골신경지수의 회복 및 근 수축력의 회복을 보였다. SNAC 투여군은 재관류 7일에서 28일 사이 좌골신경지수의 조기 회복을 보였으며, 재관류 11일 이후부터 장 족지 신전근 수축력의 조기 회복 및 근육 무게의 회복을 보인 반면, MP, PBS 투여군은 21일 까지도 근 수축력의 감소 및 근 위축을 보였다. 조직검사상 SNAC 투여군은 축색의 퇴화가 심하지 않고, 조기 재생이 관찰되었다. 결론: 산화질소의 투여가 말초신경의 재관류 손상 시 조기 운동기능의 회복에 유효함을 보여주었으며 신경계의 허혈, 재관류 손상 시 치료제로서 사용될 수 있는 가능성을 보여준다. Purpose: To investigate the effects of nitric oxide (NO) donor, S-nitroso-N-acetylcysteine (SNAC) on the motor functional recovery of a reperfused rat sciatic nerve. Materials and Methods: Seventy-eight rats were divided into the following 3 groups: SNAC, methylprednisolone (MP), and phosphate buffered saline (PBS)-treated groups. A 1 ㎝ segment of the sciatic nerve underwent 2 hours of ischemia followed by 90 minutes of reperfusion. The results were evaluated using a walking track test, muscle contractile test and an examination of the muscle weight and histology at various reperfusion periods. Results: There was a significant improvement in the sciatic functional index (SFI) and contractile force of the SNAC-treated group compared with the MP and PBS groups. The SNAC group showed earlier improvement in the SFI measurement between days 7-28. The contractile force and muscle weight of the extensor digitorum longus muscle began to be restored earlier in the SNAC group after day 11, while the others showed progressive atrophy until day 21. The histology examination revealed less severe degeneration and earlier regeneration of axons in the SNAC-treated rats than the other groups. Conclusion: The supplementation of NO is effective in promoting the motor functional recovery of a reperfused peripheral nerve and has the potential to replace or augment steroids as a therapeutic agent in the treatment of nervous system ischemia/reperfusion injuries.

      • TPHC방법에 의한 152Sm핵 준위의 수명측정

        Measurements of the electromagnetic lifetime of 2g+ staate in Sm-152 have been made by the TPHC method. Single gamma spectrum of Eu-152 taken with a Ge(Li) detector is analyzed by using DEC PDP-11/23 computer. From the results the energies and relative intensities were obtained. The partial lifetimes of the principal transitions were computed from the previous results. The expressed their results in terms of the reduced transition probabilities (B (E2) and B(M1) of the principal electromagnetic transitions. The present results are compared with calculatins based on the Interacting Boson Approzimation (I.B.A.) model. The values yidlded results in good agreement with I.B.A. model.

      • ¹²5Te 붕괴시 K각 이중이온화에 관한 연구

        Double ionization of the K-shell accompaning K-shell internal conversion of the 0.035 MeV transition of ???Te has been studied by recording coincidences between Kα hyper satellite x-rays and Kα satellite x-rays emitted when double vacancies are filled. The ???I source material was used for this study. The recorded coincidence spectrum, using two Ge(Li) detectors and TAC, was analyzed. And then we obtained the measured coincidence unmber N(Kα?? Kα??) and the total number N(Kα) of K x-rays. Using these values, the probability per K-shell internal conversion that a double vacancy is formed, Pkk(IC), was found to be 0.51×10??. We recognized that this value is almost the same as the result from fitting thend of experimental values for various elements by computer. However there is some difference between our result and the corresponding theoretical value.

      • 154Gd 핵 준위의 수명측정과 B(E2) 계산

        B(E2) and B(M1) from lifetimes of first several excited states of ??Gd have been calculated. And lifetime of 123.08 keV transition from the lowest excited state was measured by delayed coincidence method using TPHC. The lifetime of the transition was 1.176 ns. Furthermore lifetime of the other transitions, 248.11keV, 346.71keV, 557.58keV, 1004.76keV were obtained from other experimental papers. With these values, B(E2) and B(M1) were evaluated by using the Unified Model made by Bohr-Nottelson and compared them with the theoretical values calculated based on I.B.A (Interacting Boson Approximation) model. Excellent agreement was found in the case of B(E) for 123.08keV in 0.26% error.
