Purpose: In most cases, the main cause of chest or back pain during playing golf is rib fractures. This kind of rib fractures can be easily overlooked in plain radiographs. The purpose of this article is to clarify the value of the ultrasound in detecting rib fractures. Materials and Methods: We compared the findings of chest-X rays (anteroposterior, left or right lateral, both oblique view) with those of the ultrasound in 56 patients between the ages of 20th & 60th who were diagnosed rib fractures. Their chief complaints were chest or back pain which developed during playing golf. The results were analyzed statistically. Results: Among 56 cases of rib fractures, 34 cases were diagnosed with initial X-rays. 51 cases were diagnosed with ultrasound. The sensitivity of radiograph and ultrasound were 60.71% & 91.07%, respectively. Dignosis was ultrasound with statical significance whereas radiography did not(p=0.0004). Conclusion: It was concluded that ultrasound is a valuable tool in the diagnosis of the rib fractures which were undetected with X-rays during playing golf. 목적: 골프 운동시 발생한 흉배부 통증의 원인은 늑골골절인 경우가 많다. 이런 늑골 골절은 일반 방사선 검사상 놓치기 쉬운데 이러한 놓치기 쉬운 늑골 골절에 대한 진단에 초음파 검사의 유용성을 확인하는데 있다. 대상 및 방법: 골프 운동 중 발생한 흉부 및 배부통증을 주소로 내원하여 늑골 골절로 진단된 20 대에서 60대까지의 남녀 환자 56명을 대상으로 시행한 일반 흉부 방사선 검사(전후, 측면, 좌우측 사선 촬영)의 결과와 초음파 결과를 비교 통계적 분석을 시행하였다. 결과: 56례의 늑골 골절로 진단된 환자 중 일반 흉부 방사선 검사상 골절로 진단된 경우는 34례였고, 초음파 검사상 골절로 진단된 경우는 51례였다. 흉부 방사선 검사의 민감도는 60.71%, 초음파 검사의 민감도는 91.07% 를 보였으며, 카이제곱 검정상 초음파 검사의 결과가 일반 방사선 검사에 비해 통계적인 유의한 차이가 있었다(P=0.0004). 초음파 검사가 늑골 골절을 진단하는 데 매우 민감한 검사임을 확인할 수 있었다. 결론: 골프운동 중 발생한 흉부 및 배부 통증 환자에 이학적 검사 상 늑골 골절이 의심되는 경우 일반 흉부 방사선 검사상 골절 소견이 관찰되지 않을 시, 초음파 검사가 진단 및 치료에 도움을 줄 수 있다.

야구 선수 주관절의 통증과 골 변화에 대한 유병률 조사

목적: 소년부터 성인까지의 야구 선수들의 주관절 손상의 빈도와 발생 양상 및 추이를 알아보고자 하였다. 대상 및 방법: 2004년 1월부터 12월까지 초등학교, 중학교, 고등학교 그리고 프로 야구 선수 각각 80명, 총 320명을 대상으로 설문 조사 및 양측 주관절의 전 후면과 측면 단순 방사선 촬영을 시행하였다. 결과: 모든 선수 군에서 주관절의 통증 부위와 골 변화가 내측에서 가장 많았으며 특히 내측 상과 골단 비후(Hypertrophy of medial epicondyle)가 많았다. 선수의 연령이 증가할수록 주관절의 방사선학적 변화가 증가하였으며 초등학교 선수 65%, 중학교 선수 82%, 고등학교 선수 85%, 프로 선수 88%에서 주관절의 골 변화가 나타났다. 결론: 야구 선수들의 주관절 골 변화는 연령이 증가할수록 많았으며 과거에 비해 증가하였다. Purpose: To evaluate the aspect of occurrence and transition of the elbow injuries in baseball players from childhood to adult. Materials and Methods: We had a survey and took the simple X-ray from the anteroposterior view and the lateral view of both elbows for subjects of total 320 baseball players. - each of 80 players in elementary school, middle school, high school, and professional baseball players between January and December in 2004. Results: The region of pain and the bony changes in elbow were the most frequent in medial side from all the groups of players. Especially, hypertrophy of medial epicondyle were the most numerous. As baseball players got older, the radiological changes of elbow were increased. The bony changes were found out 52 cases (65%) of the elementary school players, 66 cases (82%) of the middle school players, 68 cases (85%) of the high school players and 70 cases (88%) of the professional players out of the total 320 baseball players. Conclusion: The bony changes of the elbow in baseball players got frequent as they were getting older and increased in comparison with past.

Acute pulmonary infarction by tumoral thromboemboli is an extremely rare fatal complication as the first clinical manifestation of hepatocellular carcinoma (HCC) patient with tumoral thrombi in the inferior vena cava. The treatment method has not been established and shown to very poor prognosis despite of trying various modalities such as anticoagulation, radiotherapy and thromboembolectomy. Here, we describe a 74-year-old man who was diagnosed with HCC that presented as pulmonary thromboembolism and subsequent pulmonary infarction as the first manifestation. A clinicll observation on 62 cases of the pelvic bone fractures treated in the 77th Army Hospital from Jan. to Dec. 1967 is presented here. The results were as follows: 1. The pelvic bone fractures were 6.6% in all the fracture patient who were treated in this hospital through last year. 2. The cause of the pelvic bone fractures was all automo비Ie accident, Exclude 2 cases of explosive injury. 3. 51. 6% of the pelvic bone fractures were complicated by other bone fractures and soft tissue injuries. 4. The pubic rami fractures were the most common fractures of the pelvic bone, and bilateral pubic rami fractures were more common than unilateral pubic rami fractures. 5. There were the most complication in bilateral pubic rami fractures, rupture of the bladder and urethral injuries were 60.7% in bilateral pubic rami fractures. 6. Cases in inferior pubic rami fractures were complicated by urethral injuries.

Three cases of acute fatty liver of pregnancy were observed, in a 24-7year-old second gravida with twin pregnancy, in a 22 year old second gravida with a male fetus, and in a 22 year primigravida with twin pregnancy. In all three cases the symptoms had begun with nausea, vomiting and epigastralgia in their last trimester or immediately after delivery. All three patients had expired with rather jaundice and hepatic failure syndrome. On histological examination of liver, the characteristic microvesicular fatty infiltration in hepatic cell cytoplasma were observed in all three cases, and no marked hepatic cellular necrosis and inflammatory reaction, neither the disarranged hepatic cellular cords were noticeable, except one cases. In the last case (case 3 ), there was massive hepatic cell necrosis and distortion of hepatic cell cord were noticed, in addition to the typical microvesicular fatty infiltration of the hepatic cell. In this patient HBs antigen in the peripheral blood was positive and the transaminase values were moderately elevated in comparison with the other two cases. These findings suggested that fulminating hepatic failure was occurred simultaneously with fatty liver of pregnancy.

Clinical analysis of fracture of femral shaft in 73 cases treated at the 17 Army Hospital during the period from May 1965 to July 1968 is presented. 1) All the patient were males and in the 3rd decade of life. 2) Sites of fracture were 28 in midshaft, 23 in lower one-third, 22 in upper one-third, and 29 cases: had open fracture. 3) 32 cses(43. 8%) were injured by traffic accident which is the most common cause in this series. 4) Conservative treatmet was given in 30 cases, plate fixation in 15 cases, and Kuentscher type of I. M. nailing in 28 cases. 5) Criteria evaluating the results were; knee joint motion, length & alignment of the femur, and pain during weight bearing. 6) Severely comminuted fracture tends to deserve conservative measure only and sites of fracture contraindicated for I. M. nailing were best treated by plate or screw fixation. 7) A comparison has been made between the functional results treated by conservative method and open reduction (plate & screw fixation and I. M. Kuentscher nailing). Excellent or good results were obtained, by conservative (30 cases), plate and screw fixation. (15cases), and I. M. Kuentscher nailing(28cases), in 46.7%, 60%, and 60.7% respectively. Functional results were slightly better in I. M. nailing than the other two methods. 8) Time of complete healing by solid bone union was; 6.5 months in (30 cases of) conservative treatment, 6 months in (10 cases of) plate and screw fixation, and 5. 5 months in (22 cases of) I. M. Kuentscher nailing. 9) Postoperative infection rate,severe enough to interfere healing) was 6.6% (only one case) of plate group and 3.6%(only one case) of I.M. nailing group.

Uric acid transport across the basolateral membrane of renal proximal tubules was studied in rabbit kidney cortical slices. Uric acid uptake was greater under O<sub>2</sub> atmosphere compared to under N<sub>2</sub> atmosphere, and was increased with Na<sup>2+</sup>} concentration in incubation medium. Uric acid inhibited PAH uptake but not TEA uptake and did trans-stimulated PAH efflux. PAH also inhibited uric acid uptake. Uric acid uptake was inhibited by harmaline, ouabin, SITS, DIDS and pyrazinoic acid. The inhibition of PAH uptake by these inhibitors also was reasonably comparable to that of uric acid uptake. These results suggest that uric acid was transported across the basolateral membrane of renal tubule by a carrier-mediated process which was by a common transport system with PAH in rabbit.
