        HA/TCP 골이식재상에 이식된 지방유래 줄기세포의 골모세포로의 분화 및 골형성에 대한 연구

        Aim of the study: An alternative source of adult stem cells that could be obtained in large quantities, under local anesthesia, with minimal discomfort would be advantageous. Adipose tissue could be processed to obtain a fibroblast-like population of cells or adipose tissue-derived stromal cells (ATSCs). This study was performed to confirm the availability of ATSCs in bone tissue engineering. Materials amp; Methods: In this study, adipose tissue-derived mesenchymal stem cell was extracted from the liposuctioned abdominal fat of 24-old human and cultivated, and the stem cell surface markers of CD 105 and SCF-R were confirmed by immunofluorescent staining. The proliferation of bone marrow mesenchymal stem cell and ATSCs were compared, and evaluated the osteogenic differentiation of ATSCs in a specific osteogenic induction medium. Osteogenic differentiation was assessed by von Kossa and alkaline phosphatase staining. Expression of osteocyte specific BMP-2, ALP, Cbfa-1, Osteopontin and osteocalcin were confirmed by RT-PCR. With differentiation of ATSCs, calcium concentration was assayed, and osteocalcin was evaluated by ELISA (Enzyme-linked immunosorbant assay). The bone formation by 5-week implantation of HA/TCP block loaded with bone marrow mesenchymal stem cells and ATSCs in the subcutaneous pocket of nude mouse was evaluated by histologic analysis. Results: ATSCs incubated in the osteogenic medium were stained positively for von Kossa and alkaline phosphatase staining. Expression of osteocyte specific genes was also detected. ATSCs could be easily identified through fluorescence microscopy, and bone formation in vivo was confirmed by using ATSC-loaded HA/TCP scaffold. Conclusions: The present results show that ATSCs have an ability to differentiate into osteoblasts and formed bone in vitro and in vivo. So ATSCs may be an ideal source for further experiments on stem cell biology and bone tissue engineering.

        Fibrin glue is composed of fibrinogen and thrombin and used in various regions for multiple use. Basic principle is that thrombin converts fibrinogen to fibrin in the presence of $Ca^{2+}$. The structure of fibrin is loose at the beginning, but after about 5 minutes a tight structure is formed under the influence of factor VIII which changes fibrin monomer into fibrin polymer. Fibrin glue is used for tissue adhesive, suture, local hemostasis, wound healing, closure of subdural space. Fibrin adhesive has been used in oral and maxillofacial surgery for hemostasis after tooth extraction in patients with coagulation disorders, skin graft fixation, reattachment of periodontal flaps, in combination with autogenous bone chips to fill the bony cavities following cyst removal, and for securing the hydroxyapatite granules for maxillary alveolar ridge augmentation. This study was designed for researching influence of fibrin glue during healing phase after making artificial bone defect.

        HSP27 EXPRESSION IN OSTEOBLAST BY THERMAL STRESS

        Aim of the study: Thermal stress is a central determinant of osseous surgical outcomes. Interestingly, the temperatures measured during endosseous surgeries coincide with the temperatures that elicit the heat shock response of mammalian cells. The heat shock response is a coordinated biochemical response that helps to protect cells from stresses of various forms. Several protective proteins, termed heat shock proteins (hsp) are produced as part of this response. To begin to understand the role of the stress response of osteoblasts during surgical manipulation of bone, the heat shock protein response was evaluated in osteoblastic cells. Materials & methods: With primary cell culture studies and ROS 17/2.8 osteoblastic cells transfected with hsp27 encoding vectors culture studies, the thermal stress response of mammalian osteoblastic cells was evaluated by immunohistochemistry and western blot analysis. Results: Immunocytochemistry indicated that hsp27 was present in unstressed osteoblastic cells, but not fibroblastic cells. Primarily cultured osteoblasts and fibroblasts expressed the major hsp in response to thermal stress, however, the small Mr hsp, hsp27 was shown to be a constitutive product only in osteoblasts. Creation of stable transformed osteoblastic cells expressing abundant hsp27 protein was used to demonstrate that hsp27 confers stress resistance to osteoblastic cells. Conclusions: The demonstrable presence and function of hsp27 in cultured bones and cells implicates this protein as a determinant of osteoblastic cell fate in vivo.

        악골의 낭종에 관한 임상적 연구

        The purpose of this study is to find the histopathological pattern of cysts. We reviewed the hospital chart, out-patient chart, roentgenogram, histopathologic report and operation report of 152 patients who had been diagnosed as cyst and treated at the department of Oral and Maxillofacial Surgery of Korea university hospital between Jan. 1, 1995 and Dec. 31, 1998. And then we studied clinically with regard to pathological classification, age and sex distribution, anatomical distribution and so on. The results were as follows: 1. In pathologic classification, radicular cyst (97cases, 64%), dentigerous cyst (35cases, 23%), odontogenic keratocyst (8cases, 5.3%) were dominant among cases of cyst. 2. The pattern of age distribution in cases of radicular cyst, dentigerous cyst and odontogenic keratocyst was similar to that found in previous studies. The peak incidence was in the second decade (27%) and third decade (29%) in overall cases. 3. The male-to-female ratio was 1.9:1. 4. Radicular cyst occured most frequently in the maxillary incisor teeth area, dentigerous cyst in mandibular wisdom teeth area, and odontogenic keratocyst in mandibular molar area.

        골석화증에 대한 문헌고찰

        Osteopetrosis는 드물게 볼 수 있는 질환으로 일명, Albers-Schonberg disease 혹은 Marble bone disease 라고도 하며 그 주요한 특징은 전반적인 골경화증, 임파선의 증대, 간비 증대, 범혈구 감소증, 다발성 골절과 실명 등이다. 이 질환은 X-선상에서 독특한 소견을 보이며 임상적으로 악성형과 양성형으로 나눌 수 있다. Dominant form은 양성형으로써 다양하게 표현되어 단독 X-선상에서 이상을 나타내는 것부터 골절과 뇌신경을 침범하기까지 다양하게 나타나며, Recessive form은 악성형으로 자궁내에서 진단되어지고 빈혈이 심하며 골수양화생이 있으며, 감염 특히 하악의 골수염으로 대개는 첫 1 년 이내에 사망 한다고 보고되어 있다. 이에 저자들은 개인치과에서 상악 좌측 제 2대구치 발거후 골수염이 발생한 31세 남자 골석화증 환자를 치험하였기에 문헌고찰과 함께 보고하는 바이다. Osteopetrosis (Albers-Schonberg disease) is a rare disease characterized by generaized sclerosis of bones, hepatoslenomegaly, pancytopenia, multiple fracture, blindness. The disease shows characteristic radiographic feature and two rather well - defined patterns have been recognized, We experienced a case of osteopetrosis in a 31 year old man, who has had pus discharge and fetid odor after extraction of upper maxillary molar. We made a brief review of literature.

        하악 제 3대구치의 치근 만곡도와 매복정도의 관계 분석

        Purpose : We examined the relationships between the root curvatures of the mandibular third molars and their locations on the mandible, so that it might be contributed to elucidate the origin of the formation and development of the Korean mandibular third molar and help many clinicians to treat the mandibular third molars. Materials and Methods : Panoramic radiographs of 237 patients who had been treated at Korea University Ansan Hospital from March, 2000 to April, 2001 and had bilateral mandibular third molars were examined and analyzed. Results : There was no statistically significant difference between men and women (p<0.05). But there were significantly fewer subjects with bilateral curved roots in bilateral third molars regardless of their status (p<0.01) and were statistically significant differences between the groups with bilateral curved roots in the mandibular third molars with same status and with unilateral curved roots in the mandibular third molars with different status and between the groups with unilateral curved roots in the mandibular third molars with same status and with bilateral curved roots in the mandibular third molars with different status(p<0.05). Subjects with bilateral curved roots in the mandibular third molars with bilateral partial impacted status and with bilateral curved roots in the mandibular third molars with bilateral complete eruption status were significantly fewer (p<0.01) and subjects with the mandibular third molars with partial impacted and complete impacted status and with the mandibular third molars with complete erupted and partial impacted status had significantly fewer bilateral curved roots (p<0.05). Conclusions : When we postulate that it is genetic and environmental factors that affect the root formation, it seems that environmental factors have more effects on the formation of the mandibular third molars than on that of the mandibular first and second molars

        Sturge-Weber syndrome은 드물게 볼 수 있는 선천성질환으로 일명 encephalotrigeminal angiomatosis, 또는 Sturge-Weber syndrome 이라 하며 삼차신경의 분지를 따라 분포하는 안면 혈관종, 안면 혈관종과 같은 부위의 intracerebral calcification 경련의 특징을 가진다. 이에 저자들은 Sturge-Weber syndrome 3예를 치험하였기에 문헌고찰과 함께 보고하는 바이다. Sturge-Weber Syndrome is a congenital disorder and characterized by facial hemangioma following one or more divisions of the trigeminal nerve, epilepsy, mental retardation, contralateral hemiplegia, occlular involvement, gingival involvement. A 34 year old Korean man, a 25 year old Korean woman and a 48 year old Korean woman were found to have red - purple colored pigmentation on the hemifacial area and upper oral mucosal area

        생선뼈로부터의 BIOCERAMICS(천연 APATITE)에 관한 연구 I. 재교학적 성상에 관한 연구

        Comparing with easily customed synthetic Hydroxylapatite and human bone, several assays on the chemical composition and physical properties of natural Hydroxylapatite which was abstracted from fishbone (Pollac). The results were as follows: 1. The chemical composition of natural Hydroxylapatite was very similar to that of human bone. 2. Through transmission electron microscopic findings, crystalloid configuration of natural Hydroxylapatite was recognizable. 3. Through scattered electron microscopic findings, the presence of micropores in the particles of natural Hydroxylapatite and three-dimensional passage of the pores could be seen. 4. Physically, natural Hydroxylapatite possessed higher ratio of surface area and porosity, lower specific gravity and smaller dimension of micropores compared with synthetic Hydroxylapatite. 5. The same diffraction image on X-ray diffraction analysis showed that natural Hydroxylapatite and synthetic Hydroxylapatite were same material. 6. In X-ray microanalysis the concentration of clacium and phosphate showed extremely high peak, and in Raman spectrum the band width of phosphate was thought to be related with the regularity of crystalloid structure of natural Hydroxylapatite, and the fact that X-ray diffraction was prominent was supposed to have meaningful relationships with these results. 7. In infrared absorption spectrum, some proteins which was supposed to have osteoconductivity could be seen in the powder of human scattered bone or teeth, however, in any kind of Hydroxylapatite was not discovered at all.

        생선뼈로부터의 BIOCERAMICS(천연 APATITE)에 관한 연구

        Clinical evaluation was performed for 15 cases of odontogenic Cyst patients who were operated from october, 1988 to July, 1990 via natural Hydroxylapatite graft with or without autogenous iliac bone marrow following enucleation of cyst sac and exodontia or apicoectomy of adjacent and causative teeth. 1. Among 15 cases, men were 8(53.3%) and women 7(46.7%), and cysts in upper jaw were 9 cases (60.0%) and 6 cases (40.0%) in lower jaw., 2. In order to enhance the osteogenetic activity, mixture of natural Hydroxylapatite and autogenous iliac bone marrow was grafted for 3 large cystic cavities including 1 case of maxillary cyst and 2 cases of mandibular cysts. 3. Operated wound of 3 cases (20.0%) was infected as postoperative complications, but the prognosis was relatively fair by way of routine dressing technique. Among the infected cases, all of them occured at mandibular lesions and 1 of them was grafted with the mixture of natural Hydroxylapatite and autogenous iliac bone marrow. 4. During postoperative follow-up clinical evidence of resorption of natural Hydroxylapatite was not recognizable, however some cases of dispersed Hydroxylapatite was found in X-ray findings. 5. Infected Hydroxylapatite granules and bone chips were taken out spontaneously by saline irrigation, but the lower margin of graft maintains continuity well throughout the dressing period and denuded wound was fairly healed with ingrowth of soft tissue. 6. Apicoectomized tooth adjacent to denuded wound was sacrificed during healing process, because the distal wall of alveolus of the adjacent tooth was infected and resorbed. 7. The natural Hydroxylapatite(HAP-B) is supposed to be able to be used as a bone substitute without difficulties compared with synthetic Hydroxylapatite in Oral & Maxillofacial surgery with same indication of synthetic Hydroxylapatite.

        호산구 증가증을 수반하는 ANGIOLYMPHOID HYPERPLASIA (KIMURA S DISEASE)의 치험예

        A patient suffered from palpable masses at the region of submandibular gland and oral mucosa on the premolars in both side was operated and the tumors were removed surgically. 1. The progress of the disease was chronic, painless, long durated and not accompanied with any significant systemic symptoms, but the onset period was obscure. 2. An interesting observation of complete blood count was eosinophilia (13%), and the pathologist confirmed the postoperative diagnosis as angiolymphoid hyperplasia with eosinophilia (Kimura s disease).
