생애전환 초기 노인의 성별에 따른 낙상경험 및 내재적 요인 비교

연구목적: 본 연구는 생애전환기를 맞이한 초기노인을 대상으로 낙상경험과 낙상의 내재적 요인을 성별에 따라 비교하였다. 연구방법: 연구대상자는 2008년 1월부터 12월까지 국민건강보험공단에서 실시한 66세 생애전환기 건강진단검사를 받은 수검자 중 자료가 불충분한 것을 제외한 255,505명을 대상으로 2차분석한 서술적 단면연구이다. 연구결과: 생애전환기를 맞이한 66세 초기노인의 낙상경험은 전체 10.6%로 남자 8.5%, 여자 12.4%로 여성노인이 남성노인에 비해 낙상비율이 높았다. 남녀에서 낙상위험에 영향을 미치는 요인은 배뇨장애 6.2-6.6배, 우울 1.5-1.8배, 보행장애 1.3-1.5배, 및 실명 1.3-1.4배 순서로 나타났다. 연구결론: 비록 내적 낙상위험요인이 다양할지라도 효과적인 낙상예방을 위해서 배뇨장애가 중요한 내재적 예측요인이며, 이는 초기노인의 낙상에 영향을 줄 수 있다. PURPOSE: This study compared the elderly falls and the intrinsic fall risk factors according to gender. METHODS: This descriptive cross-sectional study was a secondary analysis of the data collected for medical checkups for the Transitional Periods of Life supported from the National Health Insurance. The subjects examined were 255,505 people aged 66 years in Korea between January and December, 2008. RESULTS: The elderly people aged 66 when in the transitional periods of life experienced 10.6% of their first falls. The first falls of women (12.4%) was greater than that of men (8.5%) in the elderly. The risk factors for falls included dysuria with an odds ratio of 6.2 to 6.6, depression with an odds ratio of 1.5 to 1.8, gait disturbance with an odds ratio of 1.3 to 1.5, and blindness with an odds ratio of 1.3 to 1.4 in both elderly women and men. CONCLUSIONS: Effective fall prevention should focus on dysuria because it is a more important predictor of falls, even though many intrinsic fall risk factors can affect falls in elderly people.

Purpose: The study was done to compare the status of dispatch of public officials in charge of health care in response to COVID-19 according to location of primary health care posts and the practitioners’ knowledge, attitude and support related to COVID-19. Methods: Among the 193 practitioners surveyed, 175 people in Chungcheong-do, Jeolla-do and Gyeongsang-do were selected for final analysis according to work status and differences in knowledge, attitude and support. Data were analyzed using x² and one-way ANOVA, followed by a Scheffé-test. Results: There was a statistically significant difference by region in the number of days and places for dispatch teams to respond to COVID-19. In particular, in the case of support, the overall score was low, but Gyeongsang-do, which had the longest number of dispatch days due the occurrence of a large-scale confirmation of patients, showed higher support compared to other regions. Knowledge of COVID-19 was high, but there were no regional differences. Conclusion: Regarding performance of COVID-19 response tasks, the support level was low, indicating that psychological support and appropriate compensation from the local government are required. In addition, it is necessary to prepare a system for the provision of alternative services to prevent a work gap in health clinics.

Background: Initial treatment of acute upper respiratory infection (AURI) should not include antibiotics because most AURIs are caused by virus. However, the prescription rate of antibiotics in Korea is higher than in any other countries. Inappropriate use of antibiotics in Korea accelerated the emergence of antibiotics resistance and increased the social and economic burden. The objective of this study was to investigate the factors related to antibiotics use for the AURI among children-adolescents and adults. Methods: This study analyzed the Health Insurance Review and Assessment Service-National Patient Sample data which was nationally representative sampling stratified by sex and age. Results: The influencing factors of antibiotics use for AURI are gender, age, types of medical security, primary disease, existence of concomitant disease, treatment seasons, first visit or revisit, indicated specialty, types of medical institution, and location of medical institution. Conclusion: The results showed health policy makers are required to place more efforts to resolve inappropriate antibiotics use. Especially they need to establish a health policy to reduce the gap between areas and specialties and recommend standardized clinical guidelines according to the subgroup code of AURI and the age group of patients.
다요인적 프로그램이 농촌 지역 여성노인의 요실금, 자아존중감 및 자기효능감에 미치는 효과

Purpose: The purpose of this study was to examine the effect of a multifactorial program for urinary incontinence, self-esteem and self-efficacy in older women. Methods: A one-group pretest-posttest with no control group was used. Nineteen women completed weekly 90-min group sessions for eight weeks. The group received a multi-factorial program comprised of Kegel exercise, education and psychosocial programs over 8 weeks. Data on urinary incontinence, self-esteem and self-efficacy were collected twice: before the intervention and 8 weeks after the intervention. Data were analyzed using paired t-test by non-parametric statistics. Results: Total scores for the International Consultation on Incontinence Questionnaire decreased significantly after the intervention (pretest 7.95±5.48, posttest 5.15±4.72, p<.05). The scores for self-esteem and self-efficacy improved significantly. Conclusion: These results indicate that a multifactorial program for older women is feasible to prevent urinary incontinence in women living in the community.
