This study is to grasp selection factors and habits of information searching of customers of ophthalmic service and to verify the differences in them and to investigate how they affect in selecting medical institute by demographic sociological characters, selection factors by classification and habits of information searching, how many times they used and the type of medical treatment. The result of analysis of importance of selection factors of medical institute, it showed that doctors' career were evaluated high by classification and it showed in order of university hospital, hospital, clinic in facilities and equipment and in order of university hospital, clinic, hospital in distance transportation Analysis of importance of selection factors by sex distinction, it showed that doctors' career were high for both male and female and according to the result of analysis of selection factors by an age, doctors' career variable was measured high and it showed in order of facilities, equipment, distance and convenient transportation. The result of analysis by the form of medical treatment, doctors' career were measured high in all diseases. Facilities and equipment were measured high in case of a corrective operation of eyesight and distance transportation variable showed high in simple eye diseases. According to the result of analysis of habits of searching information by utility frequency, one's own experience in the past(direct visits) was the highest over all and it showed in order of introduction of other ophthalmic department in case of people who go to the institutes many times.

사회적 혼합아파트에 대한 거주자 인식 연구

In Korea, apartment complexes are built according to the social strata for which they are intended to house, thus the buildings tend to separate society both physically and even visually. Because of the sense of social exclusion this has caused the government has conceived a plan to develop a "social-mix" apartment complex master plan. Perhaps the foremost example of this type of plan is the Seoul Eunpyeong New Town. This study examines how resident's perception of the social mix plan has evolved, and also attempts to better understand the effectiveness of the government's attempt at social integration. The result shows that the perception of social-mix housing has improved after residents moved into the Eunpyeong New Town and that people responded positively to the concept of actually mixing residents socially. From the result, we can see that the visually unexposed environmental elements of rental housing and socials exclusion which was felt in the past has been reduced to a certain extent. However, residents living in solid-lot apartments have a negative reaction to social mixing especially when they are in the same building. Therefore, to achieve better social integration in a socially-mixed apartment complex, we need now to change the perception toward socially mixed housing among the residents living in solid-lot apartment buildings.

다한증 치료로 B형 보툴리눔 독소 피하주사 후 발생한 일과성 조절장애 1예

Purpose: To report a case of temporary accommodation difficulties after botulinum toxin injections for the treatment of palmar and plantar hyperhidrosis. Case summary: A 27-year-old woman presented with difficulties performing near work that had begun 1 week prior. She underwent injections of botulinum toxin type B (Myobloc?) for the treatment of both palmar and plantar hyperhidrosis 3 weeks earlier. The patient had subdermal injections of 3000 MU (mouse unit) of Myobloc? on each palm and sole. On manifest refraction, the patient was emmetropic. Near point of accommodation was 32 cm in the right eye and 25 cm in the left eye, near point of convergence was 10 cm, and the monocular accommodative power measured with minus lenses was 1.5 diopter (D) in the right eye and 2.0 D in the left eye. After 3 weeks, accommodation difficulties were completely resolved and hyperhidrosis symptoms did not recur. Conclusions: Ophthalmologic adverse effects related to the autonomic nervous system such as accommodation difficulties may occur with injections of botulinum toxin type B for the treatment of hyperhidrosis. These symptoms should be explained carefully to the patient before the botulinum toxin injection procedure.

단안 시신경염으로 발현한 눈톡소플라즈마증 1예

Purpose: To report an atypical case of ocular toxoplasmosis presenting as isolated unilateral papillitis. Case summary: A 53-year-old female presented with visual difficulty in her right eye that had begun 1 week previously. Best corrected visual acuity was 0.8 in the right eye and 1.0 in the left eye. On fundoscopic examination, optic disc swelling and focal edema with hard exudates in the nasal parapapillary retina were found in her right eye. Fluorescein angiography revealed marked leakage of dye from the swollen optic disc. There was no evidence of vasculitis and chorioretinitis. Pupillary light reflex and color vision were normal. Visual field (VF) showed generalized reduction in the right eye, and was normal in the left eye. Optical coherence tomography (OCT) revealed right optic disc swelling. Serology was positive for toxoplasma IgM and IgG. The patient was treated with oral steroids and antitoxoplasma antibiotics. Two months later, visual acuity was 1.0 in the right eye. There was no disc swelling on fundoscopy or OCT and VF was normal. Conclusions: Ocular toxoplasmosis can present atypically as isolated papillitis without chorioretinitis and mimic idiopathic optic neuritis. A thorough serologic examination for toxoplasmosis along with proper treatment should be performed.

에이디 긴장성 동공의 임상양상

Purpose: To report the clinical features of Adie’s tonic pupil. Methods: The medical records of 22 patients who had been diagnosed with Adie’s tonic pupil from February 1998 to February 2009, were retrospectively reviewed. On March 2010, a cross-sectional examination was performed in 16 patients (19 eyes) who underwent a follow-up of more than 1 year. Measurements included pupil size in room light, bright light and in darkness; near point of accommodation; presence of segmental iris palsy; light-near dissociation; denervation supersensitivity; corneal sensitivity; and deep tendon reflex (DTR). Results: Among the patients studied, 16 were women in Adie’s tonic pupils. Only 3 of patients had bilateral involvement. The mean age of onset was 38.3 years. The mean size of Adie’s tonic pupils was 2.3 mm larger than the fellow eyes. Segmental iris palsy was detected in 93.8% of the patients. Denervation supersensitivity was observed in all patients. Light-near dissociation was present in 88.2% and over 90% of the patients had decreased DTR in the biceps, triceps, knee and ankle jerk. Conclusions: This cross-sectional study showed Adie’s tonic pupil tended to become miotic and recover accommodation power over the years.

김남천의 『사랑의 水族館』에 나타난 '현대'의 성격과 '현대청년'

Aquarium of Love written by Nam-cheon Kim is a novel that was published serially from 1939 to 1940 in Chosun Ilbo. It features the characteristics of modern times that isolates the main character. The author also delineated them in his previous work. While Gwang-joon Kim, brother of Gwang-ho Kim, who ended up in dying of acute tuberculosis after abandoning his old-style wife and cohabiting together with a barmaid was .1 prototype of previous generation that implied the background of the times when the ideology of the past socialists collapsed, Gwang-ho Kim, the main character of this work, represents "the characteristics of modern times". Nam-cheon Kim already showed his interest in the way of the world and public morals through the theories of complaint literature and view literature. Accordingly, his delineation of the characters including Gwang-ho Kim and Gyeong'-hee Lee is just a complaint about and view of the then social reality, and the features of their living were the epitome of the social reality of the times. In this work, Nam-cheon Kim explores the love, belief, the way of the world, and public morals of young people who represent "the characteristics of the times" and are represented by Gwang-ho Kim and Gyeong-hee Lee. Outwardly, however, the story progresses with a focus on the plot of conflicts and reconciliation that arise in the course of love and marriage of Gwan-ho Kim and Gyeong-hee Lee. Furthermore, since the real time from 1939 to 1940 and the internal time of the story coincide, the work fully reproduces the sense of reality of the times. The reason why Nam-cheon Kim positively delineates the new senses of "modernized young people" is that vogue is closely associated with productive relationship and it is a means to sensitively view the public morals of 'modern times'.

Plasma arc 광원을 이용한 과산화수소의 치아미백 효과

The purpose of this study was to compare of the effectiveness of tooth bleaching with different hydrogen peroxide concentrations and light activated plasma arc on extracted premolar tooth The 100 specimens were stained using the CEYLON black tea. Among of them 60 specimens were selected by lightness values (*L) and were classified into five groups (n=12): Group Ⅰ, Distilled water; Group Ⅱ, 5% hydrogen peroxide gel ; GroupⅢ, 5% hydrogen peroxide+plasma arc; Group Ⅳ, 15% hydrogen peroxide; GroupⅤ, 15% hydrogen peroxide +plasma arc. Specimens were bleached until 4 times (60 min). The color change (ΔE*) of specimens was measured after stained and each bleaching time by Shade Eye-NCC. The samples were analyzed qualitatively by evaluating morphological changes in the SEM images, microhardness and quantitatively by using roughness parameters (Ra). The acquired data was analyzed by paired t-test and one-way ANOVA followed by Tukey s multiple test for variable at a 0.05 significant level. The color changes (ΔE*) significantly increased after bleaching in all groups (p<0.05). Microhardness (VHN) significant decreased after bleaching in all groups AFM and SME images exhibited that the surfaces of specimens became roughed after bleaching. Especially, Group of hydrogen peroxide with plasma arc showed more rougher than hydrogen peroxide without plasma arc in SEM. This study suggested that hydrogen peroxide with plasma arc may be better tooth whitening effects than only hydrogen peroxide.

양안 시신경염으로 발현한 눈매독 1예

<b>Purpose:</b> To report a case of ocular syphilis presenting as bilateral optic neuritis. <b>Case summary:</b> A 25-year-old man presented with visual difficulty in both eyes that had begun 2 weeks previously. Best corrected visual acuity was 0.7 in the right eye and 0.1 in the left eye. Anterior segment was normal. On fundoscopic examination, optic disc swelling was found in both eyes. Color vision was abnormal in the left eye. The visual field (VF) showed peripheral constriction in the right eye and generalized reduction in the left eye. Fluorescein angiography revealed early, bilateral leakage of dye from the optic disc with multiple hot spots in the chorioretinal level. Serology was positive for fluorescent treponemal antibody absorbance (FTA-ABS) IgM, IgG and rapid plasma reagin test. A test of the cerebrospinal fluid was positive for venereal diseases research laboratory and FTA-ABS IgG. A diagnosis of syphilitic optic neuritis was made and the patient was treated with antibiotics. Six months later, visual acuity was 1.0 in the right eye and 0.8 in the left eye. There was no disc swelling on both fundoscopy and optical coherence tomographs. VF also improved after treatment, with the restoration of central vision. <b>Conclusions:</b> Syphilis can present as optic neuritis. 양자폭포레이저에서 활성영역의 모의실험을 위하여 Runge-Kutta 방법과 shooting 방법을 이용하여 슈뢰딩거 방정식의 해를 구하였다. 활성영역의 두께 변화에 대하여 발진파장, 포논공명 에너지, 분극행렬요소 (dipole matrix element) 등의 특성변화를 관찰하였고, 이로부터 양자폭포레이저를 위한 에피성장에서 허용될 수 있는 최소한의 두께 정밀도를 제안하였다. For the simulation of active region in the quantum cascade lasers (QCL), we solved Schrodinger equation utilizing Runge-Kutta method and Shotting method. Wavelength, phonon resonant energy, and dipole matrix element were simulated with the variation of active region thickness. As a result of such simulation, it was suggested the tolerance range of epi-layer thickness error when 3-quantum well QCL structures are grown.

소아에서 발생한 슬관절의 결핵성 관절염

Tuberculosis still ranks as one of the three most important infectious diseases in the world in terms of morbidity and mortality. In Korea, an increased incidence of tuberculosis has been observed in recent years. With the resurge of tuberculosis, extrapulmonary tuberculosis has increased, too. From this point of view, tuberculous arthritis affecting knee is rare in all forms of tuberculosis, but we can consider tuberculous arthritis in patients with osteomyelitis. We report the case of a 14-month-old male child who presented with fever and swelling on the left knee joint. Histologic examination of knee joint fluid showed consistent with tuberculosis. Ziehl-Neelsen stain of joint fluid was positive for acid-fast bacilli(AFB). The outcome was favorable after treatment with anti-tuberculosis medication. Because clinical signs and symptoms of musculoskeletal tuberculosis in children is more indolent, we can misdiagnose or delay diagnosis. The diagnosis of tuberculous arthritis can be elusive, necessitating a high index of suspicion. 국내에서는 아직도 소아연령에서 결핵 감염이 적지 않게 존재하기 때문에 폐외 결핵 환자가 발생될 위험도 지속적으로 존재한다. 이런 측면에서 폐외 결핵의 하나인 골관절 결핵은 매우 드물기는 하나, 관절염을 주소로 내원한 환자의 원인으로 고려해 볼만하다. 나이 어린 소아에서는 증상과 징후가 비특이적이기 때문에 오진이나 진단 지연이 우려되므로, 항생제 치료에 잘 반응하지 않는 관절염 환자에서는 결핵성 골관절염에 대한 진단과 치료를 시도하는 것이 중요하다.
