Lupus enteritis is a rare, severe complication of systemic lupus erythematosus (SLE), needing prompt diagnosis and proper management. However, SLE rarely presents as lupus enteritis at the time of initial diagnosis. Thus, delayed diagnosis and misdiagnosis are common. We report a case of a 25-year-old woman with lupus panenteritis. The patient had multiple hospitalizations for abdominal pain, nausea, and diarrhea, initially without any other symptoms suggestive of SLE, but was later observed to have malar rash and oral ulcers. Laboratory investigations were compatible with SLE, including positive antinuclear antibody (1:320) with speckled pattern. CT revealed diffuse hypodense submucosal thickening of the stomach, the entire small bowel, colon, appendix, and rectum. Treatment with high-dose corticosteroids followed by maintenance therapy with mycophenolate mofetil, hydroxychloroquine, and azathioprine resulted in clinical improvement. Diagnosis of lupus enteritis requires a high index of suspicion given the low incidence and nonspecific clinical findings.

알레르기접촉피부염 환자의 첩포검사에서(T.R.U.E. TEST^(R)) 판독일에 따른 결과 고찰

Background: The patch test is widely used for diagnosis of allergic contact dermatitis. However, nearly half of positive reactions can be observed only on day 2 or day 4 and it is difficult to interpret these reactions. Objective: The purpose of this study is to assess the frequency of transient and delayed reactions in TRUE-test and detect common antigens that provoke these reactions. Methods: A total of 311 patients with allergic contact dermatitis were evaluated by TRUE-test between Jan 2007 and December 2011. Records of patch test results of day 2 and day 4 were reviewed and analyzed. Results: A total 311 cases of T.R.U.E. TEST^(R) records (male 79, female 232) were analyzed. Persistent reactions were observed in 80.1% patients tested, transient reactions were observed in 18.3%, and delayed reaction in 5%. Frequent allergens which showed transient reactions were cobalt dichloride (2.9%), nickel sulfate (2.2%), thiomersal (1.9%), and carba mix (1.6%), in order of frequency. Allergens which showed delayed reactions were nickel sulfate (0.3%), fragrance mix (0.3%), p-tert-butylphenol formaldehyde resin (0.43). Conclusion: Our results showed a relatively high frequency of transient reaction in T.R.U.E. TEST^(R). This suggests that additional reading at day 4 in the patch test would be of value. Objectives : The aim of this study is to compare the improvement of Low back pain (LBP) depending on female inpatient's Bone mineral density (BMD) after menopause. Method : We evaluated 10 cases of normal and 25 cases of osteopenia and osteoporosis based on Bone mineral density test among the female inpatients with LBP who were after menopause, admitted at Jaseng Hospital of Oriental Medicine from January 2008 to August 2008. Result : Depending on the analysis of pathogenic cause, most were without specific reason and except that, overwork and lifting heavy weight were the most common reason. Using improvement-analysis based on treating period, the normal group showed higher decrease of Numerical Rating Scale (NRS) than osteopenia and osteoporosis group. Conclusion : Statically, among the female inpatients with LBP, who were after menopause, had no significant differences between the normal group and osteopenia and osteoporosis group's improvement rate, depending on treating period.

Washboard nail is a dystrophy that presents as a series of ridges that extends across the nail. It is often accompanied by a central depression that extends from the cuticle to the free edge. Washboard nail is often a result of frequently repeated trauma, but sometimes predisposing factors cannot be found. When habitual tic is the cause of the lesions, restraining the habit is the most important treatment. Here, we report a case of a 6-year-old male with washboard nails on both thumbnails and was treated with topical 0.1% tacrolimus ointment. Significant improvement was observed after 4 months of application. This case suggests that topical tacrolimus can be considered as a primary therapy in the treatment of washboard nails in the absence of trauma. (Korean J Dermatol 2012;50(12):1054∼1056)

Falcotentorial meningiomas are rare. The infratentorial supracerebellar and the occipital transtentorial approaches are frequently used to reach the tumor. A 56-year-old female was admitted to the hospital because of headache. Magnetic resonance imaging (MRI) of her brain showed a 35×38×20 mm-sized tumor, suspected as a meningioma, with heterogeneous signal intensity in the pineal region. The patient was operated using the occipital bi-transtentorial/transfalcine approach. The observed tumor was a fibrotic mass with calcification and was not pulled to one side after internal decompression. It was totally removed through the bilateral approach. Immediately after surgery, however, the patient complained of total blindness. Following steroid therapy, the patient showed remarkable improvement. The occipital bi-transtentorial/transfalcine approach provides a well-exposed operation field and an optimized visibility of anatomical structures. However, the potential visual cortex injury should be considered due to undue, prolonged retraction.

웹크롤링과 텍스트마이닝을 활용한 국군간호사관학교와 간호장교 인식도 조사

국군간호사관학교(국간사)는 한 때 폐교될 위기에 처한 적이 있다. 기업과 마찬가지로 군대내 부대나 조직도 정체성을 스스로 진단하고 그 필요성을 입증해 나가지 않으면 그 존폐를 장담할 수 없을지 모른다. 국간사의 폐교 위기 전후 국간사와 간호장교의 역할에 대해 군 내부적으로 진단하는 연구가 있었지만 군 외부에서 바라보는 시각에 대한 연구가 없었기 때문에 정확한 진단을 하는 것이 제한되었다. 실제로 군 외부에서 국민들이 바라보는 시각에 대한 분석은 군의 신뢰도를 분석하는데 밀접한 관련이 있고 이러한 군의 신뢰도는 군의 무형적인 전투력인 사기와 연관되고 전투나 전쟁의 승패에 큰 영향을 미칠 수 있다. 본 연구는 국민들이 국간사와 간호장교를 대상으로 온라인상에서 2011년부터 2020년까지 개진한 방대한 양의 데이터를 웹크롤링을 통해 수집하고 텍스트마이닝 기법으로 인식도를 분석하였다. 분석시 사용한 K-Means 클러스터링과 Doc2Vec 기법은 의견으로 제시된 단어를 벡터화하고 주변 단어들과의 관련성을 유지하면서 군집 형성을 수행하는 기법을 적용함으로서 군집별 의미론적 분석의 신뢰성을 보장하고 있다. 본 연구 기법은 국간사와 간호장교를 군 외부에서 바라보는 인식을 텍스트마이닝의 토픽모델링 기법을 적용한 분석이라는 의의를 가지며 연구 결과 군 외부 인식이 긍정적임을 확인할 수 있다. The Korean Armed Forces Nursing Academy (KAFNA) once faced the possibility of being shut down. Just like enterprises, The survival of military units or organizations within the military cannot be guaranteed if they do not diagnose their identity and prove its necessity. On the other hand, an accurate diagnosis is limited because there has been no research on the roles of KAFNA and nursing officers from an external perspective before and after the crisis of the KAFNA closure. Indeed, analyzing external perspectives is closely related to analyzing the credibility of the military, and such credibility can have a significant effect on the outcome of battles or wars. This study used web-crawling and text-mining techniques to analyze a vast amount of data on KAFNA and nursing officers posted online from 2011 to 2020. The K-Means clustering and Doc2Vec techniques used in the analysis vectorize the words presented as opinions and perform clustering while maintaining the relevance with surrounding words, ensuring the reliability of semantic analysis by cluster. This research technique is the first text-mining technique to analyze the perception of KAFNA and nursing officers from an external perspective, and the study results show that the external perception is positive.

Background: Mutation in the gene encoding filaggrin (FLG) is a major predisposing factor for atopic dermatitis (AD), in association with distinct features such as increased allergic sensitization, higher severity, and frequent skin infections. Genetic diversity in FLG mutations exists across ethnicities. Objective: This study aimed to investigate the clinical features of AD according to the presence of FLG mutation in Korean individuals. Methods: We performed reverse blot hybridization assay to detect FLG mutation in Korean patients with AD. Classifying subjects into AD with or without FLG mutation, clinical features of AD and patch test results were compared between the two groups. Results: Among a total of 281 subjects, 39 (13.9%) were found to have FLG mutation. AD with FLG mutation was associated with higher risk of impetigo and eczema herpeticum but lower risk of prurigo nodularis. In the patch test, there was no difference in positive reactions of major contact allergens between the groups. Conclusion: In Korean patients with AD, FLG mutation was associated with more frequent skin infections but not with personal or family history of atopic diseases, allergic sensitization, contact allergy, and protracted course. It is important to consider other skin-barrier-related genes, such as KLK7 and SPINK5, and immune response-related genes in conjunction. (Korean J Dermatol 2019;57(7):363∼370)
