      • $^{99m}Tc$ 표지화합물을 사용한 심근 관류 SPECT 검사에서 심장과 타 장기와의 중첩 방지를 위한 자세 변화의 유용성

        게이트 심근 관류 SPECT는 심근관류를 평가하고 관상동맥질환을 진단하는 비침습적인 방법으로 매우 가치가 있는 검사이다. $^{99m}Tc$를 이용한 표지화합물들은 특성상 간과 담낭에 많이 농축이 되며 장으로 배설이 된다. 본 연구에서는 소화기관을 통해 배출되는 방사성의약품의 일부가 장기 내에 남아 심장의 음영을 침범하는 경우 자세의 변화를 통해 영상의 획득 방법을 변형하여 효과적으로 분리 관찰할 수 있도록 하는데 그 목적을 두었다. 2011년 1월부터 8월 사이에 심장과 심혈관계 질환이 의심되거나 이에 따른 시술 및 수술을 받은 20명의 환자들을 대상으로 시행하였다. 대상 군에 대하여 $^{99m}Tc$-sestamibi 또는 $^{99m}Tc$-tetrofosmin 370 MBq (10 mCi)을 정맥 주사하였다. 주사 30분 후에 지방식을 하도록 하였으며, 약 1시간 후에 검사를 시행하였다. 기본적인 검사는 앙와위 자세에서 양팔을 머리 위로 거상한 자세로 하였다. 휴식기 검사가 종료되면 약 3~4시간 후에 아데노신에 의한 약물부하를 시행하였다. 부하는 6분간 실시 하였으며, 아데노신이 주입되기 시작한지 4분째에 다시 한번 방사성의약품을 주사하였다. 부하기 때의 방사성 의약품은 1,110 MBq (30 mCi)을 주사 하였으며, 지방식 및 검사 시간은 휴식기 때와 동일하게 하였다. 휴식기와 부하기 검사에서 심장과 장기의 중첩이 발생한 경우에는 곧바로 앙와위 자세에서 고정 용구를 사용하여 상체를 거상시켜 환자 자세를 변화시키고 그 이외의 다른 조건들은 동일하게 하여 영상을 다시 한번 획득하였다. 기존의 앙와위 자세에서는 횡격막 이하 장기의 일부가 심장과 중첩이 일어났으나 고정 용구를 사용한 환자 자세의 변화로 심장과 중첩되었던 타 장기들의 대부분이 신체의 아래쪽으로 내려가 심장과 충분히 분리되는 모습을 육안적으로 관찰할 수 있었다. 장기의 일부가 심장에 중첩된 경우에 비해 분리된 경우의 재구성 영상에서 각각 전체 평균의 차이는 확장기말 용적이 2.75 mL 증가, 수축기말 용적은 3.16 mL 감소, 심박출 계수는 3.58% 증가하였고, 결손 부위의 면적은 부하기에서 $3.58cm^2$ 감소, 휴식기에서 $3.92cm^2$ 감소하였다. 또한 각각의 수치들을 토대로 시행한 대응표본 $t$-검정에서는 $p$>0.05로 유의한 차이는 없었다. 본 연구에서는 심근 관류 SPECT에서 심장과 타 장기와의 중첩된 부분을 고정 용구를 사용함으로써 효과적으로 분리시켜 영상화할 수 있었다. 이와 같이 지연 검사가 필요한 경우 적절한 고정 용구를 사용하게 된다면 보다 더 효율적인 검사가 이루어질 수 있을 것이라 사료된다. Purpose: The present study has an objective of effectively separating and making observations on a portion of radiopharmaceutical excreted via digestive organ to remain in the organ and invade a heart shadow. Materials and methods: When heart shadow is blocked by the organ in tests during a resting phase and a loaded phase, additional images were obtained using immobilization device. The immobilization devices were used to tilt the upper body forward from supine position. Results: In the reconstructed image for the separated case, as compared with the case where a part of organ is overlapped with heart, in terms of an overall mean value for each parameter, the end-diastolic volume increased by 2.75 mL, the end-systolic volume decreased by 3.16 mL, the left ventricle cardiac coefficient increased by 3.58%, and the area of defect region decreased by 3.58 and 3.92 cm for loading and resting phase, respectively. Conclusions: In the present study with myocardial perfusion SPECT, overlapped areas of heart and other organs could be effectively separated and visualization by the use of an immobilization device.

        ■ Objectives The purpose of this case report is to show an effect of korean medicine for patient with bilateral abducens nerve palsy. ■ Methods This patient was 58 year-old man who was diagnosed with bilateral abducens nerve palsy. The patient had Diabetes Mellitus, hypertension, benign prostatic hyperplasia and dyslipidemia as underlying diseases. The patient had symptoms of limited eye movement to lateral, diplopia, dizziness and mild headache. He was treated by korean medicine, containing electroacupuncture, pharmacoacupuncture, moxibustion and herb medicines during 13 days of hospitalization and following outpatient department. His eye movement was evaluated by the method of Scott and kraft and discomfort from his symptoms was scored by the Numerous Rating Scale. And other general conditions were measured. ■ Results After that period, patient's abduction deficit was improved and subjective discomfort from his symptoms was getting better. His other conditions were also better, compared with his admission. ■ Conclusion This study suggests that Korean medicine could have a therapeutic effect for bilateral abducens nerve palsy. So, It could help to relieve patient's symptoms and make general conditions better.

        兩義供辰丹을 倂用한 한방치료가 重症度의 통증 환자에 미치는 진통 효과 : 후향적 연구

        Objectives: The aim of this study was to investigate the analgesic effect of Yang Eui Gongjin-dan (YEGJD), a traditional Korean medicine, on severe pain or numbness, including acute and chronic non-cancer pain. Methods: Records of patients visiting the Department of Traditional Korean Internal Medicine, Stroke and Neurological Disorders Center, from March 2011 to January 2015, were searched. Their chief complaint was severe numbness or pain (NRS≥7). We compared the NRS of pain pre- and post-YEGJD treatment in 34 patients; and analyzed changes in pain severity pre- and post-YEGJD treatment in 17 chronic non-cancer pain patients. We also investigated other accompanying symptoms, such as insomnia, anorexia, dyspepsia, fatigue, coldness, and excessive sweating, related to the deficiency state (虛症) in traditional Korean medicine. Results: The average pain NRS for the 34 patients significantly decreased from 8.04±1.08 to 3.75±2.54 after YEGJD treatment. The average pain NRS score in chronic non-cancer pain patients also significantly declined from 7.91±0.97 to 3.29±2.29. Conclusions: The traditional Korean medicine Yang Eui Gongjin-dan has an analgesic effect and is useful for treating severe pain or numbness. We recommend a randomized controlled trial using objective assessment scales and a large patient sample to further test the effects of Yang Eui Gongjin-dan.

        It is known that the fluidized bed incinerator, which is the subject of analysis, shows excellent performance in heat and mass transfer due to excellent mixing and contact performance between fluidized sand and fuel, and also shows relatively good combustion characteristics thanks to good mixing and long residence time for low-grade fuels. have. In this study, air flow analysis is performed to understand the characteristics of co-firing of sludge, waste oil and solid waste in the fluidized bed incinerator, flow characteristics of flue gas, and discharge characteristics of pollutants.The fluidized bed incinerator subject to analysis is a facility that incinerates factory waste and general household waste together with sludge, with a processing capacity of 32 tons/day. to be. In addition, the operation method was designed for continuous operation for 24 hours. As a result, it can be seen that the lower combustion air and the introduced secondary air are changed to a strong turbulence and swirl flow form and exit through the outlet while rotating inside the freeboard layer. The homogeneous one-way flow form before reaching the secondary air nozzle has very high diffusivity with the high-speed jet flow of the nozzle.

        하이브리드 태양광 LED 가로등 시스템 연구

        In line with the rapid economic growth of many countries, fossil fuel energy sources are also rapidly depleting. Therefore, the price is also rising rapidly, so it is necessary to develop new and renewable energy sources such as hydropower, geothermal power, nuclear power, wind power and solar energy to replace fossil fuel energy in the future. In this study, development of rotating concentrator module system, development of rotating module control control system, development of lamp and charge control controller, configuration and prototype production of rotating concentrating solar LED street light system, efficiency of rotating concentrating solar LED street light, and power production. The research was conducted in the order of evaluation of comprehensive performance tests such as consumption and consumption. As a result, the developed high-efficiency rotation-concentrating hybrid solar LED street light module system has a 50% higher light-gathering efficiency than existing products by tracing sunlight by self-developing a rotation-collecting module on existing solar LED street lamps according to the characteristics of Korea's topography. and the power generation was improved by more than 40%.

        치매(痴呆)에 대한 임상 한의학 연구 경향 분석 : 지난 10년간의 국내 논문을 중심으로

        Objective : In this article, we overviewed clinical Korean Medicine studies on dementia by analyzing papers published in Korea from 2010 to 2019. Methods : We explored research articles on dementia from 5 Korean research databases using the keyword 'dementia' and 'Korean Medicine'. We included original articles and reviews of clinical Korean Medicine and excluded single case study and experiment article of animal model. Then we analyzed them according to the published year, journal type, and research topics. Results & Conclusions : Research topics were classified into 'research trend', 'treatment', 'prevention', 'evaluation', 'medical expenses', 'collaborative practice' and 'others'. The largest number of articles were published in 2011(24.4%). 'Journal of Oriental Neuropsychiatry' was the journal with the most published articles(46.3%). In research topics, the analyses of research trend accounted for the largest portion(39.0%) and the articles about treatment for dementia or evaluation index in Korean medicine had the second largest proportion(14.6%). We hope that various researches for therapeutic effects, mechanisms and preventive effects of Korean Medicine in dementia would be conducted for future study.

        This paper explored that medial consumers` trusts in both hospitals and doctors are developed by and during investigating hospital websites and that these trusts directly build the consumers` purchase intentions of the hospitals` offline medical services. 208 questionnaires based on the websites of two virtual hospitals, captured on survey brochures, were collected and analyzed. This research indicated information quality of hospital websites affected more on developing consumers` trusts than service quality.

        In this paper, we proposed a haptic navigation system which used the tactile data for the user guides of the mobile robot to the reference point via tele-operation in unknown blind environment. This navigation system can enable a mobile robot to avoid obstacles and move to the reference point, according to the direction provided by the device guides through a haptic device consisting of a vibration motor in a blind environment. There are a great deal of obstacles in real environments, and so mobile robots can avoid obstacles by recognizing the exact position of each obstacle through the superposition of an ultrasonic sensor. The navigation system determines the direction of obstacle avoidance through an avoidance algorithm that uses virtual impedance, and lets users know the position of obstacles and the direction of the avoidance through the haptic device consisting of 5 vibration motors. By letting users know intuitionally, it lets the mobile robot precisely reach the reference point in unknown blind environment. This haptic device can implement a haptic navigation system through the tactile sensor data.

        In this paper, the IR and compass sensors for the underwater system were used. The walls of the water tank have been recognized and avoided treating the walls as obstacles by the bio-mimetic underwater robot. This paper is consists of two parts: 1.The hardware part for the IR and compass sensors and 2.The software part for obstacle avoidance algorithm while the bio-mimetic robot is swimming with the obstacle recognition. Firstly, the hardware part controls through the RS-485 communications between a microcontroller and the bio-mimetic underwater robot. The software part is simulated for obstacle recognition and collision avoidance based upon the data from IR and compass sensors. Actually, the bio-mimetic underwater robot recognizes where is the obstacle as well as where is the bio-mimetic robot itself while it is moving in the water. While the underwater robot is moving at a constant speed recognizing the wall of water tank as an obstacle, an obstacle avoidance algorithm is applied for the wall following swimming based upon the IR and compass sensor data. As the results of this research, it is concluded that the bio-mimetic underwater robot can follow the wall of the water tank efficiently, while it is avoiding collision to the wall.

        In this paper, a new haptic device is proposed for the teleoperation, which can recognize the invisible environment of a mobile robot. With this new device, it is possible for the user to identify the location of an obstacle and to avoid it. The haptic device has been attached on the top of a joystick so that the user can remotely control the mobile robot to avoid the obstacles which are recognized by the ultrasonic sensors. Also, the invisible environment is recognized more accurately overlapping the data from the ultrasonic sensors. There are five vibration motors in the haptic device to indicate the direction of the obstacle. So the direction of the obstacle can be recognized by the vibration at the finger on each vibration motor. For various situations and surrounding environments, experiments are performed using fuzzy controller and overlapping ultrasonic sensors. The results demonstrate the effectiveness of the proposed haptic joystick.
