Dermal melanocytosis is histologically characterized by the presence of ectopic melanocytes in the dermis. Mongolian spots, nevus of Ota, nevus of Ito and blue nevus are the most common types and these are usually present at birth or in early childhood. However, it has been reported that several types of dermal melanocytosis could appear in adult life. We report here on an unusual case of acquired dermal melanocytosis occurring in a 51 year-old female. She had bilaterall brownish or slate-bluish pigmented patches on the face and the posterior auricular, supraclavicular, scapular and back areas. The skin biopsy specimen taken from the upper back revealed scattered, darkly pigmented, spindle-shaped cells and dendritic cells containing abundant golden brown pigment in the dermis. Exogenous factors, including environmental substances and drugs, are known to induce scleroderma-like reactions. Various scleroderma-like reactions induced by anti-cancer drugs have recently been reported. This is the first report that doxifluridine (Didox), an oral prodrug of the antineoplastic agent 5-fluorouracil (5-FU), induced a localized sclerderma-like reaction. A 51-year-old woman was referred to our clinic with multiple pearly, shiny patches on both her breasts, her pelvis and her back. After surgical excision and radiation therapy due to her right breast cancer, she took Didox for 7 months. A skin biopsy specimen revealed that the dermal collagen thickening extended even to the subcutaneous tissue. The routine laboratory tests were within the normal ranges and the tests for antinuclear antibody (ANA), anti SS-A antibody, anti SS-B antibody and anti U1-RNP antibody were all negative. After discontinuation of Didox, the lesions gradually improved. Based on these finding, we diagnosed this case as a localized scleroderma-like reaction induced by doxifluridine and we should pay attention to detect this adverse effect of the long term use of doxifluridine. Allopurinol (4-hydroxypyrazolo-[3,4-d]pyrimidine) is an effective and widely used xanthine oxidase inhibitor administered in the treatment of hyperuricemic states such as gout. Allopurinol-induced DRESS (Drug rash with eosinophilia and systemic symptoms) syndrome is characterized by hematologic abnormalities, especially eosinophilia and mononucleosis-like atypical lymphocytosis, skin rash, fever, lymph node enlargement and single or multiple organ involvement, which starts within 8 weeks after the initiation of therapy. We report three cases of allopurinol-induced DRESS syndrome who developed erythematous skin eruption six weeks, nine weeks and seven weeks, respectively, after allopurinol therapy. The clinical, laboratory and histologic findings of these patients were compatible with allopurinol-induced DRESS syndrome. (Korean J Dermatol 2007;45(7):714∼719)

Background: Calcium plays a role in the proliferation and differentiation of keratinocytes. In a normal situation, the calcium concentration forms a gradient across the epidermal layers. Calcium is sparse in the basal layer and spinous layer. Skin organ culture is a useful model for conducting research on various aspects of skin biology. Skin organ culture systems are used for defining factors that affect homeostasis when elucidating the modulatory effects of biologic response modifiers, drugs and physical agents on the skin and also when studying complex aspects of cutaneous biology in normal and diseased skin. Objective: In this study, we investigated the effects of extracellular calcium on the epidermis in a skin organ culture. Methods: We compared the skin organ culture patterns under various culture conditions (calcium 0.1, 0.7, 1.4 and 2.0 mM). Results: H&E staining showed different phenotypes according to the calcium concentration and IHC also showed different phenotyes compared to that of keratin 10, involucrin, filaggrin, loricrin and PCNA. Conclusion: As a result, we concluded that the calcium gradient is also an important factor in skin organ culture to maintain the vivo-like environment and the appropriate calcium concentration is 1.4 mM. The amniotic band syndrome is a collection of fetal malformations associated with fibrous band that appears to entangle or entrap various fetal part, in utero, leading to deformation, malformation or disruption. Its pathogenesis has been debated in the literature for many years. The associated anomalies vary from minor limb anomalies to major craniofacial defects and visceral defects. We experienced a case of limb anomalies due to amniotic band and present the findings with a brief review of literature. (Korean J Dermatol 2008;46(6):815∼817)
단기 고용량 스테로이드 정맥주입요법과 Narrowband UVB로 빠른 호전을 보인 백반증 2예

Systemic corticosteroid arrests the progression of vitiligo and leads to repigmentation, but it may produce side effects. It has been reported that the maximum effect for corticosteroid can be achieved without major side effects when it is used at high doses over a short period of time. Recently, narrowband UVB has been used to treat vitiligo. However, there have been no reports on the effectiveness for a combined treatment with narrowband UVB and systemic corticosteroid. We encountered 2 cases of vitiligo patients who had rapid and effective repigmentation after combination therapy with high dose methylprednisolone and narrowband UVB. Intravenous high dose (25 mg/kg) of methylprednisolone for 3 days was followed by narrowband UVB once or twice weekly. After 2 months, rapid improvements were seen in both patients with >75% repigmentation. Combination treatment with high dose methylprednisolone therapy and narrowband UVB may be an effective therapeutic option for vitiligo. (Korean J Dermatol 2008;46(12):1619∼1622)

Recently, use of Ti has been expanded as material of artificial joint and dental implant due to superior property of bio-suitability. Especially, dental implant requires surface evaluation as it have an important effect on surface structure. It is very important thing to evaluate performance of implant with optical surface measuring method on such a surface state. Surface topography is very useful in surface quality evaluation. It includes the evaluation of surface area, lateral area and surface roughness. In this study surface evaluations were carried out with two kinds of implants, machined and sandblasted after machining. Surface evaluation method using optical three-dimensional surface roughness measuring method is introduced.
CNT강화 알루미늄 나노복합재의 자동차용 부품 적용성 연구

CNT(carbon nanotubes) 강화 알루미늄 나노복합재는 우수한 기계적 특성으로 자동차용 차세대 경량재료로 주목을 받고 있으나 소재 제조 과정에 있어 CNT의 균일 분산 확보가 어렵고 대량제조 공정 확립이 어려워 자동차 부품으로의 적용이 어렵다. 그러나 점차 CNT 생산이 대량화 되고 있고 복합재로서의 특성이 개선되고 있다. 따라서 본 연구는 CNT강화 알루미늄 나노복합재의 현 수준을 확인하고 자동차 부품 관점에서의 적용가능성을 검토하고자 하였다. 평가에 사용된 소재는 20L급 High energy milling기에서 알루미늄 분말과 CNT를 혼합한후, 소결 및 압출하여 봉상(ø80)으로 제조되었다. 소재 관점에서 기계적 특성 및 열적 특성을 분석하였으며, 부품적용성 관점에서는 현재 자동차 부품에 사용되는 소재와 그 소재가 사용되는 각 부품의 주요 요구 특성을 상대비교하였다. 고강성과 성형성이 요구되는 부품에 사용되는 상용소재(A390) 및 SiC/Al 복합재와 성형성 비교평가를 진행하였으며, 탄성계수를 측정하였다. 피로 내구 및 경량화가 요구되는 메인베어링캡 양산소재와의 내구성비교 평가를 실시하였다. 또한 고온 안정성이 요구되는 피스톤용 내열 소재와 열팽창계수 및 열화에 따른 강도저하를 비교 평가하였다. 구배가 큰 금형을 설계하여 단조 가공 후, 성형성(외관 crack 및 성형압 측정)을 비교하였으며, 내구성 평가를 위해 실제 부품인 H社 소형 엔진블록의 메인베어링 캡을 적용하여 일축 단품 피로 시험을진행하였다. 이를 통해 우수한 소재 특성을 확인하였고, 자동차 구조용 부품으로 적용이 가능함을 확인하였다. Various characteristics(thermal expansion, microstructure, etc.) and mechanical properties of CNTaluminum nano composites manufactured by volume production system were evaluated. Also, formability and durability were evaluated for potential applications in automotive parts, via compared with high-elasticity material (A390) and the current commercial product. As a result, this composite has excellent mechanical properties and formability, therefore, to verity its potential for application as light and high strength materials in automobile part.
