Hepatocellular carcinoma is one of the most prevalent malignancies and frequent causes of death worldwide. Treatment options of hepatocellular carcinoma consist of locoregional therapy, surgical resection, liver transplantation, and systemic therapy. Assessment of tumor response is required in patients receiving locoregional and systemic therapy. The Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 is widely used tumor response evaluation criteria. However, the RECIST does not reflect the extent of tumor necrosis after some locoregional therapies and molecular targeted agents. The Modified RECIST (mRECIST), which has the concept of viable tumor, was introduced in order to overcome this problem. The mRECIST were developed on the basis of RECIST version 1.1 and only tumoral tissue showing contrast uptake in arterial phase of dynamic radiologic imaging techniques was measured to assess tumor response. Recently, immune checkpoint inhibitors have emerged as a promising therapeutic modality for the treatment of hepatocellular carcinoma. To identify tumor response after immunotherapy, immune RECIST (iRECIST) has been proposed as consensusbased criteria. After achieving complete response after curative treatment, optimal surveillance was needed to detect recurrence. Individualized surveillance schedule should be considered, taking into consideration the risk factors of the patient and the risk associated with the treatment modalities. Background and Objective : Laryngeal Papillomatosis (LP) is the most common benign neoplasm of the larynx, but it tend to recur and it makes eradicating difficult. Meticulous $CO_2$ laser excision has been the most effective treatment to date. This article analyzes the clinical feature and therapeutic results of 42 LP patients who were undergone $CO_2$ laser excision. Methods : Forty two patients with recurrent LP were treated with $CO_2$ laser. And their medical records were reviewed retrospectively. Demographics, chief complaints at onset, initial distribution of papillomas, number of operations performed on each patient, and current results were evaluated. Results : Male in their twenties and forties are dominant in number in patient number. Most common site was anterior one thirds (69%) of glottis area (86%). LP recurred in 17 cases (40%), and in 4 cases, the lesion extended over the original margin. Patients were undergone surgery $1.62{\pm}0.87$ times, $2.53{\pm}0.72$ in recurred cases. Mean relapsing time was 6 momths (from 1momth to 8years). Ant. laryngeal web occurred in 2 cases (4.8%) and 1 case was combined with squamous cell carcinoma. Conclusion : Meticulously performed $CO_2$ laser excision can achieve significant voice and airway improvement and clinical cures. The $CO_2$ laser through microdirect laryngoscopy allows more precise and bloodless removal of papillomas.

Duodenal varices are a serious complication of portal hypertension. Bleeding from duodenal varices is rare, but when bleeding does occur, it is massive and can be fatal. Unfortunately, the optimal therapeutic modality for duodenal variceal bleeding is unclear. This paper presents a patient with duodenal variceal bleeding that was managed successfully using percutaneous trans-splenic variceal obliteration (PTVO). A 56-year-old man with a history of alcoholic cirrhosis presented with a 6-day history of melena. Emergency esophagogastroduodenoscopy revealed a large, bluish mass with a nipple sign in the second portion of the duodenum. Coil embolization of the duodenal varix was performed via a trans-splenic approach (i.e., PTVO). The patient no longer complained of melena after treatment. The duodenal varix was no longer visible at the follow-up esophagogastroduodenoscopy performed three months after PTVO. The use of PTVO might be a viable option for the treatment of duodenal variceal bleeding. (Korean J Gastroenterol 2020;76:331-336)
