알코올의존 환자에서의 중독평가지수에 대한 신뢰도 및 타당도 연구

이 연구의 목적은 알코올의존의 체계적인 진단 및 평가를 위하여 ASI 한국어판의 타당도와 신뢰도를 측정한 것이며 그 결과는 다음과 같았다. 1) ASI의 6가지 영역은 정신의학적인 상태영역과 가족/사회관계 영역 사이의 유의한 상관관계를 제외하면 상호 독립적이었다. 2) 평가자간에는 6가지 영역 모두에서 유의한 신뢰도가 있었다. 3) 복합점수는 신체상태, 알코올복용 가족/사회관계 그리고 정신의학적 영역에서 만족할 만한 항목 일치성을 보였으며 6가지 영역 모두에서 각 영역의 복합점수와 심각성 지수 사이에 유의한 상관관계를 보여주었다 . 4) 각 영역의 주요문항들은 두 영역을 제외하고는 속하는 영역의 지수와 가장 높은 유의한 상관 관계를 보였다. 5) 다중회귀분석에서 각 영역의 심각성 지수의 변량은 일치된 영역의 가장 중요한 4가지 문항에 의해서 신체상태영역은 83%, 직업/경제 상태영역은 45%, 알코올복용영역은 43%, 법적 상태영역은 55%,가족/대인관계 영역은 53%, 그리고 정신의학적 상태영역은 84%가 설명되었다. 6) 정신의학적인 상태영역은 MMPI, MAST, SCI-90-R과 유의한 상관관계가 있었다. 결론적으로 ASI 한국어판은 일부분 문항을 제외하고는 비교적 만족할 만한 신뢰도와 타당도를 보여 주었으며 알코올의존 환자의 치료와 연구에 유용하게 사용될 수 있음을 시사한다. Objective : The purpose of this study was too evaluate the validity and reliability of Korean torsion of Addiction Severity Index for the systematic diagnosis and evaluation of alcoholism. Methods : The authors selected 65 alcoholic patients with the criteria of DSM-VI. Results : The results were as fo11ows : 1) The exception of the significant correlation between psychiatric and the family/socialproblem area, the 6 sections of ASI were mutually independent. 2) There was significant inter-rater reliability in all sections of ASI. 3) In medical, alcohol use, family / social and psychiatric section, the calculation of composite score showed reasonable level of item consistency. In all sections, there was significant correlation between composite scores and severity ratings of corresponding section. 4) The important items of each section were most highly correlated with their corresponding severity ratings except two sections. 5) In the multiple regression analysis, the amount of variance accounted for by the most important 4 items were 83% in medical section,45% in employment/support,43% in alcohol use section,55% in legal section,53% in family / social section and 84% in psychiatric section. 6) Severity rating of psychiatric section was significantly correlated with the scores of MMPI, MAST and SCL-90-R. Conclusion : The result suggests that Korean version of the ASI showed relatively reasonable reliability and validity and it can be used in treatment and study of alcoholic patients.
