뇌졸중 환자 평가를 위한 Stroke Rehabilitation Assessment of Movement의 측정자간 신뢰도

The aim of this study was performed to determine the inter-rater reliability of the Stroke Rehabilitation Assessment of Movement (STREAM) translated in Korean. Ths was a new clinical measurement tool for evaluating the recovery of voluntary movement and basic mobility following stroke. A direct-observation reliability study was conducted on 20 patients who had strokes and were in a rehabilitation setting. Subjects were assessed by two physical therapists. The reliability of the STREAM scores was demonstrated by weighted kappa statistics for inter-rater agreement on scores for individual items ranged from .83 to 1.0, intraclass correlation coefficients for total score was .99, and for subscale scores was ranged from .~ to .99. The internal consistency of the STREAM scores was demonstrated by Cronbach alphas of greater than .99 on the subscales and overall. These high levels of reliability support the use of the STREAM translated in Korean instrument for the measurement of motor recovery following stroke.

본 연구는 무릎뼈의 높이를 측정하기 위한 방법으로 도수측정의 검사자간, 검사자내의 신뢰도와 도수측정과방사선 측정과의 타당도를 알아보기 위하여 실시되었다. 도수측정방법으로 이전에 사용하던 줄자 대신 측정의 정확도를 높이기 위한 디지털 버니어 캘리퍼를 이용하여 15명을 대상으로 양쪽의 30개의 무릎관절을 측정하였다. 연구결과 검사자간 신뢰도[ICC(3,1)]는 .88, 검사자내 신뢰도[ICC(3,1)]는 .86으로 높은 상관계수를 나타내었다. 무릎뼈의 길이를 도수측정 할 때 나타나는 편향을 줄이기 위해서 무릎뼈의 길이에 10%를 줄여 보정한 결과, 방사선 측정과 도수측정간의 유의한 차이는 없었으며(p>. 05), Pearson의 상관계수는 .69로 양호한 상관관계를 보였다. 따라서 임상에서 무릎뼈 높이를 도수측정 할 때 디지털 버니어 캘리퍼를 이용하여 측정한 무릎뼈 길이를 10%를 줄여서 보정한다면 무릎관절의 높이를 객관적으로 정확한 측정을 할 수 있을 것이라 사료된다. This study was to determine the reliability and validity of manual measurements of patellar height to standard radiographic measurements in 30 knees of 15 subjects. Patellar height was measured using manual and radiographic methods. The manual measurements were performed by two examiners using digital vernier calipers with the subject sitting and the knees in 30° of flexion. The radiographic measurements were performed in the same position. The reliability of the manual measurements was assessed by means of intraclass correlation coefficients [ICC(3,1)], and the validity was investigated using the Pearson`s product-moment correlation coefficient and an independent t-test. The intra- and inter-rater reliabilities of the manual measurement of patellar height were excellent (ICC=.86 and .88 respectively). The validity of patellar height measured manually compared to the radiographic method was good (Pearson`s r=.69). In conclusion, the manual method is an objective, qualitative measurement of patella height.
줄자 및 사진촬영 측정을 이용한 어깨뼈 벌림 측정에 대한 신뢰도 연구

본 연구는 어깨뼈 안쪽 모서리부터 등뼈의 가시돌기까지의 거리를 측정하기 위한 방법으로 줄자를 이용한 측정과 사진촬영 후 분석을 이용한 측정간의 측정자간, 측정자내 신뢰도를 알아보기 위하여 실시되었다. 20명의 대학생을 대상으로 양쪽의 40개의 어깨뼈의 위치를 측정하였다. 줄자를 이용한 측정의 측정자내 신뢰도와 측정자간 신뢰도가 각각 ICC(3,1)=. 77, ICC (3,1)=. 74로 나타났고, 사진촬영 후 분석을 이용한 측정의 측정자내 신뢰도와 측정자간 신뢰도가 각각 ICC(3,1)=. 76, ICC (3,1)=. 76으로 우수한 상관계수를 나타내었다. 사진을 이용한 재평가 및 자료의 보관이 용이한 사진촬영후 분석을 이용한 측정방법도 어깨뼈의 안쪽 모서리부터 등뼈의 가시돌기까지의 거리를 객관적으로 정확한 측정을 할 수 있을 것이라 여겨진다. The purpose of this study was to determine the reliability of distance measurements from the medial border of the scapula to the spinous process of the thoracic vertebrae when obtained using tape and photography measurements. The study included 20 healthy subjects (10 males, 10 females); for a total of 40 scapula measurements. The distance measurements made using tape or photography methods were conducted with the subject in a standing position. Repeated tape and photography measurements were conducted in two sessions on different days by two examiners to assess the inter-rater and intra-rater reliability of the two methods. The reliability of scapular distance measured using tape and photography methods was tested using intraclass correlation coefficients (ICC(3,1)) and the standard error of measurement (SEM). The inter-rater reliability of the tape measurement method was excellent (ICC=.77) and the intra-rater reliability was fair to good (ICC=.74). The inter-rater and intra-rater reliabilities of the photography measurement method were excellent (ICC=.76 and .76 respectively). Our results suggest that photography measurement is an objective and qualitative measurement tool for scapular distance measurements.
엉덩정강근막띠의 길이 평가를 위한 세 가지 측정방법들의 신뢰도 비교

The purpose of this study was to compare the intra-rater and inter-rater reliability of three methods for measuring iliotibial tract length in 40 knees of 20 subjects. The measurements were performed by twoexaminers and one assistant. The length of the iliotibial tract was measured using three methods: the first used a modified Ober test with an inclinometer, the second used a modified Ober test with a marker, and the third used active adduction test with a marker in the supine position. Each examiner used the three methods in two sessions. For the first method, the inclinometer value was recorded manually. For the second and third methods, Image J analysis software was used to analyze the photographs taken. The intra-rater and inter-rater reliability of the measured value was assessed by means of intra class correlation coefficients [ICC (3,1)] and the standard error of measurement. The intra-rater reliabilities of the three methods were excellent (ICC=.97, .95, and .94, respectively), and the inter rater reliabilities of the first and second methods were fair to good (ICC=.64 and .65, respectively). The inter rater reliability of the third method was excellent (ICC=.88). Our results suggest that the active adduction test is an objective and qualitative measurement method for measuring iliotibial tract length. We recommend that the active adduction test be used for measuring iliotibial tract length in the clinical setting.
