Purpose: To investigate the efficacy of a subconjunctival injection of αvβ5 integrin antibody on corneal angiogenesis induced by chemical epithelial denudation in a rabbit eye model. Methods: One week after debridement by heptanol, rabbits were treated with a subconjunctival injection of αvβ5 integrin antibody or control immunoglobulin G weekly for 2 weeks. Rabbits that did not receive injection after debridement served as the untreated group. The percentage of neovascularized corneal area was calculated by biomicroscopy, and the sectioned area and number of new vessels were calculated by histological examinations. Results: At 7 days after the first injection, αvβ5 integrin antibody-treated eyes had 9.5% (P=0.02) and 6.8% (P=0.03) less neovascularized corneal area than vector-treated eyes and untreated eyes, respectively. At 7 days after the second injection, αvβ5 integrin antibody-treated eyes had 21.1% (P=0.02) and 18.3% (P=0.02) less neovascularized corneal area than vector-treated eyes and untreated eyes, respectively. Light microscopic examination showed a smaller neovascularized corneal area and a reduced number of new vessels in the αv β5 integrin antibody-treated eyes compared to the control eyes. Conclusions: Subconjunctival injection of αvβ5 integrin antibody effectively reduces experimental corneal neovascularization induced by chemical injury, and could be used as a corneal angiogenesis inhibitor in the future.

Purpose: To investigate the efficacy of fibrin glue for conjunctival autotransplantation in pterygium. Methods: Forty eyes of 32 patients who underwent conjunctival autotransplantation for primary pterygium with a minimum follow-up period of 6 months were included. Postopertive pain, foreign body sensation, tearing, subconjunctival hemorrhage, surgery time, complication, and recurrence were evaluated prospectively in 20 eyes of 15 patients undergoing surgery with fibrin glue, and in 20 eyes of 17 patients undergoing surgery with sutures. Results: One day postoperative, the scores of pain (P<0.01), foreign body sensation (P<0.01), tearing (P<0.01), and subconjunctival hemorrhage (P=0.01) were significantly lower in the fibrin glue group than in the suture group. One week postoperative, the scores of foreign body sensation (P=0.01) and tearing (P=0.01) were significantly lower in the fibrin glue group than in the suture group. Mean surgery time in the fibrin glue group (18±8 minutes) was significantly shorter than in the sutures group (32±11 minutes) (P<0.01). There was no significant difference in the recurrence rates of the two groups. Conclusions: Use of fibrin glue for conjunctival autotransplantation in pterygium can reduce postoperative discomfort and required surgical time.

Purpose: To evaluate the clinical effect of photodynamic therapy (PDT) with verteporfin (Visudyne?, Novartis Ophthalmics AG, Hettingen, Switzerland) in the treatment of corneal neovascularization. Methods: We performed photodynamic therapy on one eye with corneal neovascularization associated with lipid keratopathy, and on another eye with corneal neovascularization after corneal perforation. Verteporfin was administrated intravenously for 10 minutes at a dose of 6 mg/m2 of body surface area. Fifteen minutes after the start of administration, a 689 nm nonthermal laser light (Opal photoactivator, Coherent Inc., USA) was delivered directly over the neovascular network for 83 seconds with a spot size of 4000 μm. The patients were followed up at 3 days, 1 week, 1 month, and 6 months after treatment. Results: In both cases, new vessels were successfully occluded after the photodynamic therapy. No recanalization of new vessels was observed at last follow-up and no complications were observed. Conclusions: PDT with verteporfin is considered to be a safe and effective procedure to regress corneal neovascularization.

Purpose: To evaluate the long-term visual outcome after surgery for congenital unilateral cataract in children. Methods: We retrospectively analyzed age, associated ocular disease, preoperative and postoperative visual acuity, type of cataract, operation method, and final visual outcome in 34 children, 12 years or younger, that had undergone congenital unilateral cataract surgery with a minimum follow-up period of one year. Results: The mean age was 4.9 years and the mean follow-up duration was 26.5 months. Final visual acuity was less than 0.1 in 41.2%, better than 0.2 in 58.8%, better than 0.5 in 32.4%, and better than 0.8 in 20.6%. In the group whose preoperative visual acuity was less than 0.1, 27.2% of the eyes had a final visual acuity better than 0.5. This outcome is statistically significant compared with the 71.4% in the group whose preoperative visual acuity was better than 0.2 (p=0.02). In comparing the portion of patients whose final visual acuity was better than 0.5, only 8.3% were in the age group younger than 2 years at surgery; this is significantly lower than the 40.0% of patients in the age group between 3 to 6 years and the 50.0% of patients older than 7 years (p=0.03) whose final visual acuity was better than 0.5. Conclusions: Because long-term outcome of surgery for congenital unilateral cataract is not always satisfactory, it is necessary to determine the proper surgical time according to preoperative visual disturbance or severity of cataract, and to also do optical correction and aggressive treatment for amblyopia postoperatively.

Purpose: To report a case of candida infection after Medpor<sup>® orbital implantation. Methods: A 51-year-old man was admitted complaining of painful swelling and discharge about 3 years after orbital Medpor<sup>® implantation. External examination revealed signs of infection and exposure of the implant. Medpor<sup>® was surgically removed. In laboratory studies, white colony was detected in blood agar, potato dextrose agar, and CHROMagar. Automatic identification with yeast standard test revealed Candida parapsilosis. Results: Six months after surgery, the wound had healed well. In addition, cosmesis was acceptable with the artificial eye, except mild superior sulcus deformity. Conclusions: Although rare, fungal infection of the implant should be suspected when there are the long standing signs of infection and exposure after orbital Medpor<sup>® implantation.

Purpose: To evaluate the clinical effect of AlloDerm? graft on the management of exposed hydroxyapatite orbital implants. Methods: Seven eyes of 7 patients with large exposure of hydroxyapatite orbital implants were included. After the spicules of hydroxyapatite implants were smoothened with a drill, AlloDerm? was applied to the exposed site and sutured with 8-0 Vicryl. One or two layers of amniotic membrane were transplanted on the AlloDerm? graft. Results: All patients underwent the drilling, and the mean intervals from drilling to exposure was 73.4±46.6 months. The mean follow-up period was 11.17±2.0 months (range, 9-14 months). Complete conjunctival epithelization occurred in 3 eyes, and partial conjunctival epithelization in 3, including one eye which underwent reoperation with AlloDerm?. In one eye which had been associated with previous infection, hydroxyapatite was removed due to progressive exposure and inflammation of the implant. Conclusions: AlloDerm? graft is useful in the management of exposed hydroxyapatite implant.
