        급성 족관절 염좌에 대한 침구병행 치료와 침 단독 치료 효과의 비교 연구

        Objectives :The purpose of this study is to compare with the effect of acupuncture treatment with or without indirect moxibustion treatment for acute ankle sprain. Methods :This study was carried out on the 48 patients who had been treated for acute ankle sprain from June 1st to November 30th 2009 in the Dept. of Oriental Rehabilitation, Pohang Oriental Medical Hospital, Daegu Hanny University. The patients were divided into 3 groups ; Group A took near acupuncture point needling, Group B took remote acupuncture point needling, Group C took remote acupuncture point needling with indirect moxibustion. 3 Groups had been co-treated with rest, ice, compression and elevation(RICE) therapy, physical treatment and administered Ojeok-san(Wuji-san) extract granule. To evaluating the efficiency of each treatment, Ankle-Hindfoot Scale(AHS) and Visual Analogue Scale(VAS) were applied before treatment and after 3rd treatment. Results :In results, the AHS scores were increased and the VAS scores were decreased after 3rd treatment in all group. Also, AHS scores and the VAS scores of intergroup were showed no significant change. Conclusions :In this study, there is no significant effect between only acupuncture treatment and moxibustion co-treatment for acute ankle sprain.

        침 자극이 고지방식이로 유발된 비만 백서에 미치는 영향

        Objectives The aim of this study was to investigate the effects of acupuncture on weight loss, food intake, lipid metabolism, adipogenesis. Methods Four week-old C57BL/6J mice acclimatized to the laboratory environment for 1 week were allocated into four groups of 6~8 mice each: normal diet group, high fat diet group, high fat diet group and treated with acupuncture at HT7, high fat diet group and treated with acupuncture at ST36 for 90days. High fat diet group was used to control group. Results 1) Body weight, food intake of the ST36, HT7 groups decreased compared with those of control group. ST36 group was more effective with body weight decrease, and HT7 group was more effective with food intake decrease. 2) NEFA, TG, TC, ALT, AST of the ST36, HT7 groups decreased compared with those of control group. ST36 group was more effective. 3) The expression of SREP-1c, SREBP-2 of the ST36 group decreased compared with those of control group. These decreased rates were statistically significant. SREBP-2 of the HT7 group decreased significantly compared to the control group. 4) LXR, FAS, SCD of the ST36 group decreased significantly compared with control group. 5) p-ACC, HMGCR of the ST36 group decreased significantly compared with those of control group. HMGCR of the HT7 group decreased significantly compared with control group. Conclusions These results suggest that ST36, HT7 acupuncture may be used prevent or treat the obesity induced by high fat diet. (J Korean Med Rehab 2014;24(3):1-9)

      • 족저근막염에 대한 임상증례보고

        Objectives : The object of this study is to report the effectiveness of Korean medicine treatment and to urge in-depth research on plantar fasciitis. Methods : Four patients, diagnosed as plantar fasciitis, were treated by Korean medicine interventions; acupuncture at K13, BL60, BL40, indirect moxibustion at local region, oral intake herbal medicine and Korean physical therapy on plantar fascia. Visual analog scale(VAS) was adpoted as a method of measuring the effect of treatment on plantar pain. Results : As a result, the plantar pain decreased and VAS score was declined at three cases. However, at one case, the symptom was not changed and VAS score was same. Conclusions : Korean rehabilitative complex therapy can be effectively used for plantar fasciitis. Further studies are needed to set up a rehabilitation protocol with Korean medicine interventions on this disease.

        부자사심탕(附子瀉心湯)이 산화적 손상, 염증 및 골관절염 병태모델에 미치는 영향

        Objectives This study was performed to investigate the effects of Bujasasim-tang ethanol extract (BST) on oxidative stress, inflammation and osteoarthritic rat model. Methods To ensure safety of BST, heavy metal levels were measured and cytotoxicity test was done. In vitro, To evaluate antioxidative effects of BST, total phenolic contents, 1,1-diphenyl-2-picryl-hydrazyl (DPPH), 2,2``-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) scavenging activity, reactive oxygen species (ROS) levels were measured. Also, to evaluate anti-inflammatory effects of BST treated group, total nitric oxide (NO) and pro-inflammatory cytokines (IL-1β, IL-6, TNF-α) levels were measured in lipopolysaccharide (LPS)-stimulated RAW 264.7 cells. In vivo, We injected MIA 50 μl (60 mg/ml) into knee joints of rats to induce osteoarthritis. Rats were divided into total 3 groups (normal, control, BST treated group, each n=7). Normal group was not treated at all without inducing osteoarthritis and taken normal diet. Control group was induced osteoarthritis by MIA and taken with 2 ml of distilled water once a day for 4 weeks. BST treated group was induced osteoarthritis by MIA and taken BST 2 ml (200 mg/kg/mouse) once a day for 4 weeks. We evaluated dynamic weight bearing with the Incapacitance Test Meter. At the end of experiment, the rats were sacrificed to observe the functions of liver and kidney, changes of WBC, neutrophil, lymphocyte, monocyte levels in blood, to evaluate the levels of pro-inflammatory cytokines, tissue inhibitor of metallopreteinases-1 (TIMP-1), matrix metalloproteinase-9 (MMP-9), prostaglandin E2 (PGE2), leukotriene B4 (LTB4) within serum. We observed change of articular structures by Hematoxylin & Eosin (H&E), safranin-O staining method and measured amount of cartilage by micro CT-arthrography. Statistical analysis was done by unpaired student``s t-test with significance level at p<0.05 in SPSS 11.0 for windows. Results 1. Safety of the BST was identified. 2. AST, ALT, BUN, creatinine levels of BST treated group were within normal limit. In vitro, 1. DPPH and ABTS free radical scavenging activities of BST showed dose-dependent increase. 2. ROS production were significantly decreased. 3. Total nitric oxide (NO) and IL-1β production were decreased. 4. IL-6 and TNF-α production were significantly decreased. In vivo, 1. Weight bearing ability was significantly increased. 2. WBC, neutrophil, lymphocyte, monocyte levels in blood were decreased. 3. IL-1β and TNF-α levels in serum were significantly decreased. and the IL-6 level was decreased. 4. TIMP-1, MMP-9, LTB4, PGE2 levels in serum were significantly decreased. 5. Cartilage volume of BST treated group was significantly increased. Also changes of cartilage, synovial membrane, fibrous tissue were suppressed. 16 J Korean Med Rehab 2015;25(2):15-35 Conclusions The results obtained in this study Bujasasim-tang have effects of antioxidative, anti-inflammatory, relieve pain and protection of cartilage. Therefore we expect that Bujasasim-tang is effective treatment for osteoarthritis. (J Korean Med Rehab 2015;25(2):15-35)

        건설업 기초안전보건교육의 실태분석 및 개선에 관한 연구

        With regard to the basic safety and health education in construction industry issued on January, 26, 2012 by Occupation Safety And Health Acts, this study conducted a survey with self-administered questions for workers in the construction field in order to comprehend the problems since introduction of basic safety and health education. By analyzing the effectiveness of the safety and health activities, this study sought to reduce the trials and errors as well as to secure the improvement toward the early settlement of the basic safety and health education in construction industry. As a result of the question investigation, the survey respondents in the field sites who finished the basic safety and health education showed positive attitudes toward the improvement of safety awareness and the prevention of accidents. They also needed constant implementation of the education. At the same time, the government should improve the education program with strong policies to secure the effectiveness of the basic safety and health education in construction industry.

        지백지황탕(知栢地黃湯)의 골관절염 Rat 모델에 대한 항산화, 항염증 효과

        Objectives This study intended to evaluate antioxidative, anti-inflammatory effects of Jibaekjihwang-tang on monosodium iodoacetate (MIA)-induced osteoarthritic rat model and investigate the potential mechanism. Methods Jibaekjihwang-tang (100 or 200 ㎎/㎏ body weight) was orally administered once daily for 2 weeks days from day 7 after intra-articular MIA injection. And blood analysis, the histologic examinations were performed. Moreover, protein expressions related to anti-oxidant and cartilage degradation and anti-inflammatory cytokines were measured by western blot analysis in cartilaginous tissue. Results Jibaekjihwang-tang reduced serum inflammatory cytokines such as tumor necosis factors-α and interleukin-6. Furthermore, the increase of anti-oxidant enzymes reversed the oxidative stress caused by MIA. Meanwhile, Jibaekjihwang-tang suppressed MIA-induced inflammation and cartilage degradation in cartilaginous tissue. Conclusions Jibaekjihwang-tang alleviated MIA-induced inflammation. Jibaekjihwang-tang was associated with a protective effect on cartilage and by reducing inflammation and cartilage degradation. These findings provide new approaches for understanding osteoarthritis therapy. (J Korean Med Rehabil 2020;30(2):1-18)

        실비산의 항산화 및 지방축적 억제 효과

        Objectives The purpose of this study was to investigate the effects of Silbi-san on the antioxidant and fat accumulation inhibition and to analyze the anti-obesity effect by analyzing the changes in serum lipid composition in obese mice. Methods We compared contents of phytochemicals like total polyphenols and total flavonoid and antioxidant activities such as 2,2-dipheny-1-picrylhydrazyl and 2.2’-azino- bis (3-ethylbenzothiazoline-6-sulphonic acid) radical scavenging activity. After Silbi-san in 3T3-L1 cells in vitro and mouse adipose tissue ex vivo, we quantified intracellular triglyceride accumulation and lipolysis. Moreover, the anti-obesity activity though inhibiting pancreatic lipase were analyzed. In 3T3-L1 cells, morphological changes showed that control cells had many lipid while cells treated with Silbi-san had less lipid accumulation. 30% EtOH Silbi-san treatment also suppressed the fat absorption by inhibiting the activity of pancreatic lipase and led to high lipolysis through promoting glycerol release. The experimental group was divided into four groups: Normal group fed normal feed, Control group fed 60% high fat diet (HFD) and distilled water, drug group fed 60% high fat diet and 200 mg/kg of Silbi-san water extract, drug group fed 60% HFD and 200 mg/kg of Silbi-san 30% ethanol extract. Results Serum total cholesterol content and serum low density lipoprotein-cholesterol content were significantly decreased in the Silbi-san extract group compared to the control group, serum high density lipoprotein-cholesterol content was significantly increased in Silbi-san extract group. Conclusions In this study, the antioxidant and fat accumulation inhibitory effects of Silbi-san were confirmed. (J Korean Med Rehabil 2020;30(4):41-53)

      • 양측 상지 소력감으로 수핵성형술 받은 환자의 한방치료: 증례보고

        This study was to investigate the effectiveness of Korean medicine treatment on a patient who had nucleoplasty for both upper extremity weakness. The patient was treated with acupuncture, pharmacupuncture, cupping and moxa. We evaluated patient's progress using visual analog scale (VAS), neck disability index (NDI), EuroQol 5-dimension 5-level (EQ-5D-5L), active range of motion AROM) and Both grip strength. VAS and AROM are same after treatment. NDI decreased from 7 to 5. EQ-5D-5L increased from 0.596 to 0.763. Right grip strength decreased 27.6 kg to 25.8 kg and left grip strength increased 9.6 kg to 21.3 kg. As a result, clinical improvements were found in a patient. In conclusion, this study shows that Korean medicine treatment can be considered as effective treatment for both upper extremity weakness.

      • 일개 한방병원에서 치료받은 산업재해로 인한 척추 골절 환자의 특징에 대한 보고

        Objectives : The purpose of this study is analysis patients who get fractured vertebral at occupational injuries in a Korea Medical hospital. Methods : We analyzed the medical charts of 20 patients in a local Korean Medical hospital. The patients had been treated from January 1, 2016 to April 30, 2022. Results : By gender, males accounted for 70.0%, while females 30.0%. By age, over 60 accounted for 50%. Most causes of accident was fall down 50.0%. Patients visited secondary medical institution before treated in Korean Medical Hospital 100%. All of patients get acupuncture, electric acupuncture, moxa. By herbal medicine, Ojeog-san is the most Conclusions : In order to overcome the limitations of the scope of accurate examination and compensation for industrial accidents, research on various diseases of industrial accidents will be conducted in the future, and institutional improvement is needed.

        Therapeutic Effects of Binsosan(檳蘇散) on Adjuvant-induced Rheumatoid Rats

        실험목적 : 빈소산은 11가지 생약으로 구성된 복합 한약 처방으로 관절염을 포함한 다양한 염증성 질환의 치료제로 사용되어 왔으나, 관절염에 대한 직접적인 효력평가는 찾아 보기 힘들다. 따라서 본 실험에서는 빈소산 추출물이 Freund's complete adjuvant (FCA)로 유발된 rat 류마티스성 관절염에 미치는 치료 효과를 dexamethasone (15mg/kg, 복강 투여) 의 효과와 비교 평가하였다. 실험방법 : 류마티스성 관절염은 FCA (10mg in 1ml paraffin oil 0.1ml/rats)를 좌측 후지에 피내 투여하여 유발하였다. 실험동물은 Wistar 랫트를 사용하였고, FCA 투여 14일 후 유사한 무릎관절 둘레를 나타내는 류마티스성 관절염 유발 rat와 정상 rat 및 실험군을 그룹당 9마리씩 나누었다. 실험동물은 100 또는 200mg/kg의 빈소산 추출물을 FCA 투여 14일 후부터 14일간 경구 투여하였으며, dexamethasone은 15mg/kg 농도로 복강 투여한 다음, 희생하여, 체중, 연골내 collagen 함량 및 chondroitin sulphate, heparin sulphate 및 hyaluronic acid와 같은 뼈내 glycosaminoglycan 함량의 변화를 각각 관찰하였다. 실험결과는 항염 효과가 이미 입증되어 있는 dexamethasone 15mg/kg 복강 투여군과 비교하였다. 결과 : FCA 투여는 현저한 체중, 연골내 collagen 함량 및 chondroitin sulphate, heparin sulphate 및 hyaluronic acid와 같은 뼈내 glycosaminoglycan 함량의 감소와 함께 유발 관절 둘레 및 조직내 prostaglandin $E_2$의 증가와 같은 전형적인 류마티스성 염증을 초래하였으나, 이러한 류마티스성 관절염 소견은 dexamethasone 및 모든 용량의 빈소산 추출물 투여에 의해 현저히 억제되었으며, 특히 빈소산 투여군에서는 투여 용량 의존적인 감소가 인정되었다. 결론 : 이상에서 빈소산 추출물은 투여 용량 의존적인 prostaglandin $E_2$ 억제를 매개하여 FCA 유발 류마티스성 관절염에 대한 치료 효과를 나타내는 것으로 관찰되었다. 따라서 새로운 관절염에 대한 치료제로서 개발 가능성이 있을 것으로 판단된다. 한편 빈소산 추출물은 주로 prostaglandin $E_2$ 억제작용에 의해 항염 효과를 나타내는 것으로 관찰되었으나, 금후 다른 작용기전에 대한 연구와 빈소산의 구성성분 중 유효 성분 규명을 위한 실험이 수행되어야 할 것으로 판단된다.
