The incidence of the patellar fracture have been increasing due to traffic and industrial accident. Joint stiffness and traumatic arthritis are common complications because of intra-articular fracture. And so we should pay attention to its treatment. The options of treatment vary from closed treatment to total patellectomy. We analysed 37 cases of patellar fractures from Jan. 1, 1988 to Dec. 31, 1991. The results were as follows: 1. Most fractures were occured in man among 2nd, 3rd and 4th decades. 2. The most common causes were traffic accident and slip down injury and frequent configuration of fracture was the comminuted. 3. 12 cases were treated consevertively and 25 cases by open reduction and internal fixation. Most of them which were treated by open reduction and internal fixation had excellent range of motion of knee joint and less complications. 4 cases which were treated by Modified tension band wiring and circumferential wiring had short immobilization periods and bone union. So we think we have better result in comparison with other treatment.

From March 1991 to August 1995, twenty-eight cases of intraarticular comminuted distal radius fractures were treated by percutaneous pinning with closed reduction under fluoroscopy. From all cases, twenty two fractures(79%) were healed satisfactorily with no special problems. But in cases of fractures showing severe comminution or osteoporosis, the result was not good. We conclude that percutaneous pin fixation is an excellent technique for an unstable fractures in combination with external immobilization, provided the distal radius is not severly comminuted or osteoporotic.
