연구목적: 본 연구의 목적은 노인장기요양보험제도 내에서 방문재활서비스 도입과 활성화를 위한 근거자료를 마련하는데 목적이 있다. 연구방법: 문헌연구를 통해 한국의 노인장기요양보험, 방문재활 서비스의 정의와 문제점, 선진국의 방문재활 사례를 검토하였고, 이를 바탕으로 하여 방문재활 서비스의 도입과 활성화의 근거를 마련하기 위해 연구하였다. 연구결과: 2009년 10월 노인장기요양보험 서비스 이용현황은 79.3%가 장기요양급여를 이용하였다. 2008년 요양 시설은 171%증가, 재가기관 현황은 263%증가 되었다. 재가서비스 중 방문요양과 주간 보호가 가장 많았다. 2010년 복지용구에 사용된 금액은 전년대비 129억원이 증가되었다. 하지만 신체적 능력 향상 및 회복을 촉진하여 일상생활수행능력의 극대화를 돕는 방문재활서비스(작업치료, 물리치료)는 재가급여 항목에 포함되어있지 않다. 결론: 높아져가는 노인 인구와 만성 질환자를 위해 방문 재활서비스가 필요하다. 일상생활수행능력의 극대화를 위해 재활인력 및 서비스 종류 증대 복지용구 사용에 관한 교육, 그리고 장기요양보험 내 등급외자에게도 서비스를 제공하여야 한다. 이를 위해 선진국의 방문재활 사례를 충분히 검토해야 할 것이다. Objective: This study aims to provide the ground for introduction and vitalization of home rehabilitation service in public long-term care insurance Methods: We reviewed that Korea public long-term care insurance, definition and problem of home-based rehabilitation and a case of home-based rehabilitation in advanced nations through a study of literature, which is a basis that offers introduction and vitalization of home-based rehabilitation. Results: Long-term Care Insurance for the Aged presents Long-term Care Insurance is used 79.3% at Sep. 2010. Respectively, Nursing facility and home-based service facility are increased at 171/%, 263% in 2008 when compared in 2007. Home-in visitation care and daycare is very common among Home-care service. The money that is used for welcare product is increased when compared previous year, that is One thousand Two hundred ninty million wons. However, home-based rehabilitation service(Occupational and physical therapy) that facilitates physical capability and recovery and helps maximize Activity Daily Life ability is not included in Home-in pay item. Conclusion: Home rehabilitation service is necessary for patients of chronic disease and a increasing number of older-aged people. It is also provided to not-included people in-Long-term Care Insurance for the Aged for education of welcare product use, rehabilitation staff and a kind of service increases to maximize activity daily life ability, And for this, we should review thoroughly a case of Home rehabilitation service in developed country.

무작위 배정된 운동 중재법이 노인의 균형에 미치는 영향에 대한 체계적 고찰

연구목적 본 연구에서는 체계적 고찰을 통하여 고령자의 균형 감각 무작위 배정된 운동 중재법 중 효과적인 방법을 확인하고자 한다. 연구 결과를 통해 고령자의 균형 감각의 치료적용에 대한 구체적이고 유용한 자료를 제공하고자 하며, 추후 고령자 균형을 위한 중재 적용 및 중재법 개발에 대한 기초자료를 제시하고자 한다. 연구방법 2000년 1월부터 2017년 4월까지 DBpia, KISS를 통해서 학위 논문 및 학회지에 게재된 논문을 검색하였고 주요 검색용어로는 고령자, 균형, 운동중재의 단어를 사용하였다. 연구결과 연구에 사용 된 연구는 총 11편이었다. 각 연구에서의 적은 n수로 내적 타당도는 2 평가를 받았다. 연구대상자는 총 11편의 논문 중 노인대상으로 한 연구가 5편과 여성 노인을 대상으로 한 연구가 5편이었으며, 나머지 1편은 장기노인요양 1등급 노인을 대상으로 적용되었다. 치료효과 측면에서는 운동프로그램이 10편에서 가설과 일치한 유의미한 효과를 얻은 것으로 확인되었다. 나머지 1편인 Son, & Park(2016)의 연구에서는 유의미한 효과는 얻지 못했으나, 동적·정적 평형능력의 향상이 있었다고 보고하였다. 고찰 본 연구에서는 운동 중재 프로그램이 균형 기능 뿐만 아니라, 우울증 감소, 근력 향상, 자세조절 능력 향상 등 다양한 효과가 있다는 결과를 보였다. 이러한 연구 결과를 바탕으로 추후의 연구에서는 운동 중재를 통한 세부적인 균형기능에 대한 효과성 검증 및 균형 수준별로 적용 가능한 운동중재 개발에 대한 보다 체계적인 연구가 진행되어야 할 것이다. Objective : The purpose of this study was to present the effect of randomized control exercise intervention for balance function on elderly. The results of this study will provide concrete and useful data on the application of the therapy of exercise, and will provide basic data on the development and intervention, analysis of the exercise intervention for further study. Method : This study was searched form papers published from Jan, 2000 to Apr, 2017 using DBpia, KISS based. key words using on the search were ‘Elderly’, ‘Balance’, ‘Exercise intervention’ and ‘Randomized control’. Results : Thirteen studies were included in this review. In all studies, internal validity of research evaluation was 2 in oder to small number of study groups. Among the 13 subjects, five were for elderly and five were for elderly women. the other research evaluation is long-term care patients(Grade 1). effects of exercise balance program had a meaningful effect consistent with the hypothesis in 10 subjects. The other study, Son, & Park (2016), reported that although there was no significant effect, there was an improvement in dynamic and static equilibrium capacity. Conclusion : In this study, it was shown that exercise intervention have various effects such as decreased depression,, improvement of muscle strength, posture control ability, flexibility, grip strength as well as balance function. Based on the results of this study, further study should be conducted to verify the effectiveness of detailed balance function through exercise intervention and to analyze more systematically applicable board game according to cognitive level.

Purpose : The purpose of this study was to provide some basic information about current status of occupational therapy in Korean University Hospital NICUs. Method : This study was conducted by mail and by analyzing 61 survey questionnaires sent out pediatric occupational therapist in 39 university hospitals. Result : 23 people of the current pediatric occupational therapists worked in NICU under many constraints. Some reasons for not providing occupational therapy services in the NICU were lack of pediatric occupational therapists(55.3%), funding difficulties(42.1%) and problem in the hospital system(42.1%). Conclusion : The results of this study can be used as a basis to increase the provision of occupational therapy in the NICU. Further research is needed to improve the role of occupational therapy in the NICU.

산불 발생 시, 조기 발견 여부는 피해 규모의 정도를 좌우하는 중요한 요소이다. 본 논문은 산불 초기 발견과 위치 및 규모를 효과적으로 파악하기 위하여 PAN/TILT 동작이 가능한 단일 카메라로부터 파노라믹 영상을 구성하여 효율적으로 산불을 모니터링 할 수 있는 시스템을 제안한다. 입력된 RGB 이미지를 YCrCb 이미지로 색상 변환한 후, 차영상을 추출하여 연기의 움직임 변화를 감지함으로써 산불 후보 영역을 추출한다. 산불 후보 영역은 히스토그램 분석을 통한 불꽃 검출로 화재 여부를 판단하도록 한다. 또한 SURF와 영상 접합을 이용한 파노라믹 이미지를 지원한다. 이는 다음과 같은 장점들을 가진다. 첫째, 한 대의 카메라와 한 대의 모니터를 사용하므로 경제적인 시스템 구성이 가능하다. 둘째, 파노라믹 영상을 통해 넓은 뷰를 한 번에 확인함으로써 실재감 있는 규모 확인 및 조기발견이 가능하다. 셋째, 구축된 파노라믹 영상을 저장함으로써 데이터의 양을 줄일 수 있다. In case of wild fire, early detection of wild fire is the most important factor in minimizing the damages. In this paper, we suggest an effective system that detects wild fire using a panoramic image from a single camera with PAN/TILT head. This enables the system to detect the size and the location of the fire in the early stages. After converting RGB image input to color YCrCb image, the differential image is used to detect changes in movement of the smoke to determine the regions which may be prone to forest fire. Histogram analysis of fire flame is used to determine the possibility of fire in the predetermined regions. In addition, image matching and SURF were used to create the panoramic image. There are many advantages in this system. First of all, it is very economical because this system needs only a single camera and a monitor. Second, it shows the live image of wide view through panoramic image. Third, this system can reduce the quantity of saved data by storing panoramic images.
