        류숙희,안덕선,이원택,박전한,정현수,박무석,양은배,Ryue, Sook-Hee,Ahn, Duk Sun,Lee, Won Taek,Park, Jeon Han,Jung, Hyun Su,Park, Mu Seuk,Yang, Eun Bae 연세대학교 의과대학 2009 의학교육논단 Vol.11 No.2

Medical Students' competencies depend on the medical school curriculum. Basic medical education, in particular, is an important starting point for further medical competency development. We aimed to identify the most important areas of reform in the basic medical education curriculum of Yonsei Medical School. To accomplish this, we sought case studies of different medical schools and discussion points for quality improvement methods. A qualitative comparison method saturated through the systematic discussions on the emerging thematic approaches to determine the current directions in medical school curriculum reform. The discussions, which involved 7 experts, spanned 8 months and were based on a literature review, with focus on the 7 selected case studies. From the discussions, we concluded that in order to improve basic medical education curriculum, the following measures need to be carried out. First, an outcome-based curriculum is to be designed. The expected outcome is to be deliberately and succinctly defined and should be expressed as teaching and learning objectives. Second, the core subjects and elective subjects are to be classified on the basis of the aim, content, and passage level of the subjects. Hence, the core curriculum must be treated as a standard part of medical knowledge, and the elective curriculum must be richer and more in-depth. Third, universities should institutionalize regular evaluation of their departments. Appropriate and just evaluations should be made, and feedback given to the school's administrative department. Fourth, the departmental and administrative management of the basic medical education curriculum should be harmonized with each other. Finally, teaching and learning resources are to be increased and diversified and made available to professors and students for basic medical education.

First, the ABMEK needs to propel legal personality of organization and should install independent executive office. Second, the ABMEK should establish the alteration procedure of accreditation standards and develop the accreditation standards of the second cycle that take into account international flowing of medical education. Third, the ABMEK must decide forms and scope to investigate medical college present situation. Finally, to propel development tasks effectively, it needs to get the recognition of Ministry of Education and Human Development.

Purpose: This study was performed to evaluate the educational status of medical residents and to find factors that hinder resident education in Korea. Methods: A survey questionnaire on educational status of residents in internal medicine was developed, and sent to three groups: (1) specialists in internal medicine who are involved in resident training, (2) specialists in internal medicine who are not involved in resident training, and (3) residents who are under training in internal medicine. Results: The surveys revealed that the education to medical residents is mainly focused on delivery of knowledge of specialty. Insufficient or unsatisfactory items in the residents education are those of skills, attitude, communication skill, leadership, and practical matters concerning medical practice such as insurance. Most respondents replied that medical residents were employed doctors in hospitals rather than trainees to become qualified medical specialists. Conclusion: Residents are supposed to work in hospitals as trainees to become qualified specialists rather than as employed doctors. However, resident education in Korea is hindered by such factors as lack of teaching resources, heavy workload of educating specialists, heavy duty for residents, and lack of teaching skills of resident training staffs.

Tissue expansion has been extremely valuable in the treatment of scalp defect. Recently, tissue expansion of the hair-bearing scalp provides an excellent surgical option for correcting male pattern baldness. During the period March, 1992 to February, 1996, 22 patients, including 5 patients of male pattern baldness, were treated for alopecia of the scalp using tissue expansion in the department of plastic surgery, Korea university hospital. All cases were successfully reconstructed without major complications. Especially, in 5 patients of male pattern baldness, we used the expanded BAT(bilateral advancement transposition) flap of TAT(triple advancement transposition) flap that introduced by Anderson. We have experienced that tissue expansion for alopecia is simple, safe, and reliable method and provides excellent aesthetic results. In conclusion, tissue expansion have proved to be a valuable technique in the treatment of alopecia of scalp and male pattern baldness.

Currently, Various kinds of techniques for hair restoration surgery including hair transplantation, scalp reduction, and scalp flaps are performed in our clinic. Among these, hair transplantation is the basic ant the most popular procedure, which can be performed not only by itself but also in conjunction with other procedures. 123 consecutives cases of hair transplantations were performed from Jan. 1995 to Feb. 1996 for male pattern baldness, traumatic alopecia, and female pattern baldness under patient base. The authors introduced a new classification for male pattern baldness, which are the type M, O, C, U, M-O, and C-O after alphabet to make simple and easy for clinical application. The ancillary procedures were scalp reduction, preauricular flap, and scalp expansion. An ellipsis of hair bearing scalp taken from the occipital area was Sliced into slit-, mega-, mini-, and micro-grafts. The survival rate of the grafts was over 90% with minimal complications. This hair transplantation technique enabled us to achieve a good density and more natural looking hair with avoidance of cobble stoning and apparent scar.
