淪落女性에 있어서 페니실린 耐性淋菌檢査成績

윤락여성들에 있어서 PPND양성률을 알아보기 위하여 대구직할시 중구 성병진료소에 등록된 785명의 여성들을 대상으로 1984년 1월 1일부터 12월 31일까지 1년간 실시한 정기검진 결과를 분석하였다 가검물은 질경 직시하에서 무균면봉으로 자궁경부 입구 내벽을 가볍게 swab 하여 채취한 다음 T. M. 배지에 바른 뒤 배양하였으며 임질 및 PPNG는 Gram염색, oxidase반응검사 및 PPNG검사에 의해 동정하였다. 검진회수를 person-week로 계산하여 보았더니 1년간 13,702회이었다. 그 중 정상인 경우가 4,399회로(32.1%) 나타났으며, 임질이 1,973회(14.4%) 그리고 비임균성 감염이 7,251회(53.5%) 이었다. 정기검진 결과에서 임질은 14.4%에서 15.0%로 나타났으며, 이들 중 배양등으로 확진된 임질 1,973건중 PPNG양성인 적으로 판명된 경우는 162건(11.1%)으로 나타났다. 따라서 person-week로 산출한 경우 윤락여성에 있어서 전체검사 회수중 PPNG양성률은 1.2%이었다. 1년동안 이동이 거의 없었던 윤락여성들 중 95.3%가 1회이상 임질에 감염되었다. 이상의 결과로 보아 윤락여성을 통한 임질과 PPNG의 확산유려가 적지 않으므로 적절한 조치가 요망된다. To assess the prevalence of PPNG in a prostitute population, 785 women registered at Jung-ku public V.D. clinic were weekly examined by smear and culture during the period from JAnuary 1 to December 31, 1984 as a regular check. The cultures and smears were made by sterile swab from the endocervix under the speculum examination. The specimens were immediately plated on T. M. media and cultured in 10% CO_2 candle jar. Positive cultures, were identified by Gram stain oxidase reaction and PPNG test. The number is by person-week which was counted at public health center visit during the study was 13,702. cases, (32.1%) were normal, (14.4%) were gonorrhea, and 53.5% were non-gonococcal infection. The routine screen yields 14.4% to 15.0% positive culture for gonorrhea in a prostitute population. The prevalence of PPNG identified by person-week among gonorrhea was 11.1%. And the prevalence of PPNG by person-week in the total visits was 1.2%.

한국 여성의 정상 요도 내압 측정

정상 다산 여성 20례에 요류 역학 검사를 실시하여 내국인의 정상 요도 내압치를 보고한다. In the female, the data of normal urethral pressure profile (UPP) is very important to evaluate the patient with urinary symptoms, especially with stress incontinence. So, author have made urodynamic study in twenty healthy multiparous women to obtain the normal data of UPP in Korean woman from June, 1988 to July, 1989. Results were as followings. 1.In empty bladder, 1) Urethral length was 3.95 cm (SD=0.4 cm). 2) Mean functional urethral length was 3.35 cm (SD= 1.37cm). 3) Mean maximum urethral pressure was 85.90 cm H2O (SD=22.84 cm H2O). 4) Mean maximum urethral closure pressure was 79.45 cm H2O (SD=23.20cm H2O). 5) The percentage of pressure transmission from abdomen to urethra was 84.86% (SD=7.43%). 2.In filled bladder with 200 ml of normal saline, 1) Urethral length was 3.64 cm (SD=0.57 cm). 2) Mean functional urethral length was 3.01cm (SD=0.50cm). 3) Mean maximum urethral pressure was 90.32cm H2O (SD=20.20 cm H2O). 4) Mean maximum urethral closure pressure was 80.95 cm H2O (SD=20.90 cm H2O). 5) The percentage of pressure transmission from abdomen to urethra was 86.30% (SD=5.56%).

전립선질환에 있어 경직장 초음파검사의 진단적 가치

최근 노년층의 증가에 따라 전립선비대증 및 전립선암 환자의 발생과 사망률이 남성에서 차지하는 비중이 높아져 가고 있으며, 경직장 초음파검사법의 개발에 의해 여러가지 질환으로 초래되는 전립선내부의 변화 및 경계의 관찰이 가능하게 되어 전립선환자의 수술방법의 선택과 전립선암의 조기발견 및 정확한 병기분류에 미치는 초음파검사의 정확도를 알아보고자 하였다. Seventy two prostatic disease patients underwent transrectal ultasonography on inpatient basis at the Department of Urology, Inje University, Paik Hospital, Pusan, between January 1990 to December 1991. The results were as follows : 1.The average values of maximum anteroposterior, superior-inferior and transverse diameter obtained from ultrasonograms were 3.53±1.01cm, 3.60±0.91cm, 4.28±0.95cm in B.P.H., 3.42± 1.19cm, 3.55±0.50cm, 4.50±1.19cm in prostatic cancer. 2.In patients treated with transurethral resection of the prostate the mean estimated weight was 24.58gm, while the actual weight of resected mass was 8.27gm. In patients treated with open prostatectomy the mean estimated weight was 72.24gm, while the mean actual weight of resected mass was 62.07gm. 3.In a comparision of sonography and X-ray for the detection of prostatic calculi, the diagnostic accuracy of sonography (70.8%) was higher than X-ray (37.5%) 4.Diagnostic accuracy of transrectal ultrasonography was 90.9% in B.P.H. and 52.9% in prostatic cancer. In conclusion the transrectal ultrasonography was useful for diagnosis of prostatic disease, but the accuracy seemed to depend on experience of physician.

요도협착환자의 내시경하 내요도절개술의 치료성적

비뇨기과 영역에서 오래된 난제중 하나인 요도협착을 내시경하 내요도절개술을 이용하여 완전 및 불환전 요도협착환자에서 성공률 88.5%의 우수한 성적을 얻었다. Under the direct endoscopic manipulation, 27 cases with urethral strictures were managed at our department from January, 1987 to December, 1990. Satisfactory results were obtained in 88.5 per cent of the patients. Patients with strictures caused by trauma and infection had unsatisfactory results, while the duration of catheterization did not affect the results. Endoscopic internal urethrotomy was done on the 4 cases of complete obstruction of the urethra, all of them remained well, continent and free of voiding symptom. Complications were minimal without serious problems. These result implied that the endoscopic internal urethrotomy is a valuable method as the management of urethral stricture before deciding to perform urethroplasty due to high satisfactory rate and many adventages.
