        Development of a Quality of Life Scale For Children on Chronic Peritoneal Dialysis

        목 적 : 본 연구에서는 만성 복막투석 아동용 삶의 질 척도(Quality of Life Scale for Children on Chronic Peritoneal Dialysis: QOLCPD)를 개발하였다. 방 법 : QOLCPD문항 개발을 위한 자료수집을 위해서 서울대학교 어린이병원에서 복막투석 중인 30명의 만성 신부전 환아가 본 연구에 참여하였다. 타 임상 집단으로 28명의 소아정신과 장애 아동과 32명의 소아정형외과 아동이 연구에 포함되었고, 정상 통제집단으로는 초등학교에 재학중인 47명의 아동이 참여하였다. 연구대상 아동들의 연령범위는 7세-16세였다. 56개의 예비문항으로 이루어진 QOLCPD와 한국판 아동용 우울척도(CDI)를 모든 아동들에게 개별적으로 실시하였다. 결 과 : 수집된 자료에 대해 주축 요인분석을 실시하여 문항-전체 상관이 낮고 요인부하량이 .2이하인 16문항을 제외시켜서 총 40문항으로 이루어진 최종 QOLCPD를 개발하였다. QOLCPD의 신뢰도 계수인 Chronbach's $\alpha$는 .87로 양호한 수준이었다. 복막투석 집단과 소아정신과 아동 집단은 정상 집단에 비해 더 낮은 삶의 질 총점을 보였다. 특히 두 집단은 삶의 질 영역 중 신체적 기능과 학업 기능에서 정상집단에 비해 낮은 점수를 보였다. 복막투석 집단의 CDI 점수는 경한 우울 수준에 해당되었다. 결 론: 자기보고형 만성 복막투석 아동용 삶의 질 척도는 임상적 유용성이 있는 것으로 나타났다. Purpose : The aim of this study was to develop a quality of life scale for children on chronic peritoneal dialysis(QOL-CPD). Methods : Thirty children on chronic PD at Seoul National University Children's Hospital participated. A healthy control group included 47 elementary school children. Other patients groups are 32 children from the department of pediatric orthopedics and 28 children from the department of child psychiatry. The age range of all children was 7 to 16 years. Preliminary items of the QOL-CPD were developed and administered along with the Korean version of the Children's Depression Inventory(CDI) to all children. Results : The final QOL-CPD was constructed by excluding those items with a factor loading of less than .20, and the principal axis factor analysis was performed again. The QOLCPD demonstrated a good internal consistency with a value of .87. The dialysis and childpsychiatric groups showed significantly lower QOL scores compared to the healthy control group. In addition, the dialysis and child-psychiatric groups showed greater difficulties on physical and academic functions. For the CDI, the PD group showed a mild level of depression. Conclusion : The results of this study demonstrate the clinical utility of a newly developed self-report QOL scale specific for children on chronic PD.

        소아정신과 장애에서 주의력의 문제

        This paper first reviewed the definition, criteria, and neurological theories concerning the etiology of AD/HD, and the empirical studies dealing with the comorbidity of AD/HD with other psychiatric disorders. Secondly, results of studies using various neuropsychological tests for assessing the cognitive and behavioral problems in AD/HD children were examined, which suggest the possibility that dysfunction may exist in neural pathways involving many areas of the brain in AD/HD. However, because most of neuropsychological test used in Korea for ADHD children had been developed abroad, further study involving AD/HD, normal control, and other psychiatric control groups is needed to obtain developmental norms for interpreting the results, and to make more accurate diagnosis, and to clarify comorbidity of AD/HD with other disorders.

        소아정신과 장애 아동의 신경심리학적 평가

        Present study reviewed various standardized neuropsychological assessment methods for children that are widely used in Korean child-psychiatric clinic settings to evaluate neurological problems, especially soft neurological signs that could not be identified by neurological techniques like CT, MRI. The characteristics of those neuropsychological test responses in children with psychiatric disorders that neurological factors are thought to play more important role than psychological factors in their etiology were examined. It is more important and required to establish the developmental norms for interpreting the results of neuropsychological tests and for identifying the signs of brain damage in case of children than adults. There are many difficulties in diagnosing brain dysfunction and conducting research for neurological problems in psychiatric disordered children due to lack of the standardized Korean version of neuropsyhological test for children, Therefore, several issues on developing the Korean version of neuropsyhological tests for children were discussed.

        학령전기 아동 대상의 스마트기기를 사용한 전산화 주의력 평가과제의 개발 및 타당도 검증 연구

        Objectives:The aim of this study was to develop computerized attention tasks using smart devices for preschool aged children for assessment of auditory attention, visual attention, and attention shifting abilities. We then evaluated their construct and concurrent validity of them. Methods:Sixty-seven 5- to 7-year-old children attending kindergarten were recruited. Newly developed computerized attention tasks and existing standardized attention tests such as Advanced Test of Attention (ATA) and Children’s Color Trails Test-2 (CCTT-2) were successively administered. To examine the concurrent validity of these tasks, correlation coefficients between the participants’ scores on the newly developed tasks and the scores on well-known measures were calculated for assessment of each component construct. We also examined the construct validity of the tasks using the developmental trend of the auditory attention, visual attention, and attention shifting abilities with age. Results:Significant correlations were observed between the scores of computerized attention tasks using smart devices and corresponding scores of ATA-auditory task, visual task, and CCTT-2. And there were significant linear increasing trends of correct scores with age from 5 to 7 years. Conclusion:This study provides promising evidence for the utility of computerized attention tasks using smart devices for preschool-aged children.

        The effects of music combined with heartbeat sounds played through stuffed animals on emotional regulation ability in children : 봉제 인형을 통해 제공한 심장박동 소리 결합 음악이 유아의 정서조절 능력 발달에 미치는 영향

        본 연구에서는 심장박동 소리를 결합한 음악을 제작하여 봉제 인형 속에 넣은 스피커를 통해 4주간 아동에게 들려준 후, 이러한 음악이 아동의 정서조절 능력, 부모-자녀 애착, 부모의 양육 스트레스에 미치는 효과를 검증하 였다. 연구대상자는 유치원에 다니는 만 5-6세의 아동 63명으로(남아 33명, 여아 30명), 훈련 집단(31명)과 대기 집단(32명)에 무작위 배정되었다. 훈련 집단 아동에게는 작은 무선 스피커와 이를 넣을 수 있는 부드러운 봉제 인형을 제공하였고, 이를 이용하여 심장박동 소리가 결합된 음악 4곡(동요 2곡과 느린 연주 음악 2곡)을 4주간 매일 듣도록 하였다. 훈련 전과 후에는 모든 아동의 부모를 대상으로 자기보고식 설문 평가를 실시하였다. 주요 평가도구로는 정서조절 체크리스트, 부모 애착 안정성 척도, 부모 양육 스트레스 검사-축약형이 사용되었다. 그 결과, 훈련 집단에서 아동의 적응적 정서조절 능력, 부모-자녀 애착(상호 신뢰감), 긍정적 부모-자녀 상호작용 점 수의 유의미한 향상이 나타났다. 이에 부모와 함께 심장박동 소리를 결합한 음악을 듣는 활동이 학령전기 아동의 정서조절 능력과 부모-자녀 상호작용 개선하였다는 본 연구의 결과가 시사하는 바에 대해 논의하였다. This study investigated the effects of music combined with heartbeat sounds played through stuffed animals for 4 weeks on children’s emotional-regulation ability, parent-child attachment, and parenting stress. We randomly allocated 63 healthy 5- to 6-year-olds (33 boys and 30 girls) to a music training (n = 31) or a waiting-list control (n = 32) group. Each training-group subject was given a soft stuffed animal with an inbuilt speaker. For 4 weeks, subjects listened to four types of music combined with heartbeat sounds (two children’s songs and two instrumental pieces) daily. Pre- and post-training assessments included ratings from parent reports. The outcome measures were Emotion Regulation Checklist, Inventory of Parents and Peer Attachment, and Parenting Stress Index–Short Form. Results indicated an improvement in adaptive emotion regulation in children, parent-child attachment, and positive parent-child interactions in the training group. We discussed the implications of the use of music combined with heartbeat sounds on the improvement of children’s emotional regulation and parent-child relationship.

        학령기 아동용 스마트기기를 사용한 기억력 평가과제의 개발 및 타당도 검증 연구

        Objectives: The aim of this study was to develop auditory-verbal and visual-spatial memory tasks using smart devices for children aged 8 to 10 years and examine their validity. Methods: One-hundred and fourteen school-aged children were recruited through internet advertising. We developed memory tasks assessing auditory-verbal memory, visual-spatial memory, and working memory, and then examined their construct validity by examining the developmental trend of the children’s mean scores with age. In order to examine the concurrent validity of the tasks, we conducted correlation analyses between the children’s scores on the newly developed auditory-verbal, visual-spatial memory and working memory tasks and their scores on well-known standardized tests of memory and working memory, including the auditory-verbal memory subtests of the Korean Luria-Nebraska Neuropsychological Battery for Children, Korean Rey-Osterrieth Complex Figure Test, digit span and arithmetic subtest of Korean Educational Development Institute Wechsler Intelligence Scale for Children Revised, and Corsi block test. Results: The memory and working memory scores measured by the newly developed tasks tended to increase with age. Further, there were significant correlations between the scores of the four cognitive tasks and the corresponding scores of the standardized assessment tools. Conclusion: This study revealed promising evidence for the validity of the memory tasks using smart devices, suggesting their utility for school-aged children in research and clinical settings.

        스마트 토이를 활용한 아동용 인지능력 증진 프로그램의 예비 효과 연구

        Objectives: This study was to examine the effectiveness of computerized cognitive ability enhancement program (CCAEP) using Smart-toy. The CCAEP using Smart-toy which can interact with children via bluetooth is a kids-friendly and convenient method for improving children’s cognitive abilities by increasing their motivation for performing the program. We developed the CCAEP which designed to train auditory-verbal memory, visual-spatial memory, auditory-verbal working memory, and visual-spatial working memory. Methods: Eighteen children aged 8 to 10 participated in CCAEP individual training composed of 8 sessions of 40 minutes each for 4 weeks. The effect of the training was measured with Smart Toyweb’s cognitive assessment tasks (smart device based assessment) as well as traditional neuropsychological tests before and after the training. Results: Children showed significant improvement in auditory-verbal memory, visual-spatial memory, auditory-verbal working memory and visual-spatial working memory abilities after the training. Conclusion: This study demonstrated promising results suggesting the effectiveness of CCAEP using Smart-Toy in clinical settings as well as school and home situations. Further controlled study with larger sample size including various clinical groups is needed to confirm the present results.
