Flurbiprofen- and flurbiprofen axetil-loaded microemulsions composed of soybean oil, poloxamer 407, glycerine and water were prepared by generator-type homgenizer and ultrasonic probe system. The particle size of microemulsions was measured by the dynamic light scattering method. The pharmacokinetics and organ distribution of flurbiprofen were investigated after intravenous injection of flurbiprofen solution, flurbiprofen-loaded microemulsion and flurbiprofen axetil-loaded microemulsions equivalent to 10mg/kg of flurbiprofen to rats. Blood samples were collected from the anterior ciliary artery of rats for 24hr, and flurbiprofen in plasma and organs was analyzed by HPLC. Stable microemulsions were prepared. Even though there is a little change in droplet size just after the preparation, no creaming and no separation were occured during the storage period for 6 months at 4, 21, 37 and 45oC. Pharmacokinetic parameters and organ distribution of flurbiprofen after intravenous injection of flurbiprofen- and flurbiprofen axetil-loaded microemulsions emulsified with poloxamer 407 were not significantly different from those of commercial lipid microemulsion emulsified with lecithin. Therefore, it is concluded that flurbiprofen- and flurbiprofen axetil-loaded microemulsion prepared with poloxamer 407 could be used as a parenteral formulation.
교차소뇌해리현상 : SPECT와 MRI 소견의 비교와 임상징후

Objectives : The purpose of our study was to compare findings of brain SPECT representing crossed cerebellar diaschisis(CCD) with brain MRI, to evaluate relation between CCD and location of lesions on MRI and to elucidate clinically apparent cerebellar sign in patients with CCD. Methods : The study population was 20 patients representing CCD on SPECT. Percentage differences(${\triangle}%$) of activity on each cerebellar hemisphere were obtained from ipsilateral and contralateral cerebellum[${\triangle}%cbll=(IL-CL)/IL{\times}100$] and from cerebrum [${\triangle}%cbr=(CL-IL)/CL{\times}100$]. From MR studies, the percentage differences of signal intensity were also calculated as the same method. We compared the degree of percentage differences with location of cerebral lesions and with clinical cerebellar signs of the patients. Results : Among those representing CCD, the parietal lesions were the most common. There was significant correlation of the percentage differences in cerebellum between SPECT($18.8{\pm}7.22$) and MRI($4.4{\pm}3.38$) (p<0.05) and in cerebrum between SPECT($28.7{\pm}15.35$) and MRI($42.8{\pm}10.94$) (p<0.05). Cerebellar signs were observed in 3 of the 20 patients. However, there was no statistically significance between degree of percentage differences of each cerebellar hemisphere on SPECT and clinical cerebellar sign(p>0.05). Conclusion : Using the percentage differences in the cerebellum, the CCD evaluation can be easily done. On MRI, the signal changes of cerebellum were not as definite as SPECT. Despite of our assumption, there was no significant correlation between clinical cerebellar signs and CCD on SPECT.

In order to ameliorate disadvantages of buccal ointments and mucoadhesive tablets used for the treatment of aphthosis, a thin mucoadhesive patch containing triamcinolone acetonide was designed and evaluated for the pharmaceutical properties. The adhesive gel layer consisting of Noveon AA-1, hydroxypropylcellulose-M and ethylcellulose N 100, and the protective gel layer of ethylcellulose N 100, Eudragit RSPO and castor oil have been formulated and various properties such as viscosity of drug gel layer, thickness, in vitro adhesion time, adhesive strength, surface pH, content uniformity and drug release are tested. The mean viscosity of drug-containing gel layer was found to increase with increasing amount of Noveon OAA-1 or hydroxypropylcellulose-M. The optimum formulation showed the thickness of 171 mcm, surface pH of 4.6, in vitro adhesion time of 8 hours and adhesive strength of 272.7g/sheet. The drug content of each patch was relatively homogeneous with the value of 273+/-6.77g. Drug release study showed that compared to mucoadhesive tablet, the patch showed a faster drug release. Drug release was delayed by hydroxypropylcellulose-M, but not by ethylcellulose N 100. The patches prepared were nonirritant and the muco adhesion was better than the commercial product (AftachR) on the market. Based on these results, this mucoadhesive patch is expected to be an effective dosage form for the treatment of aphthosis.

To design and evaluate the double-layered mucoadhesive film preparation for the treatment of aphthous stomatitis, the film consisting of an adhesive layer containing triamcinolone acetonide and impermeable protective layer was prepared by a solvent-casting evaporation technique and subsequent drying. The polymers, Polycarbophil®, Carbopol®, CMC and gelatin, swell appoximately 2 times its volume in 30 minutes when added to water. In vitro adhesive time was longer than 8hrs and adhesive force was 177-482g/㎤. The diffusion rate of triamcinolone acetonide from film was faster than that of commercial tablet but slower than that of the film. the effect of plasticizer on the protective layer showed that the more amount of plasticizer, the greater elongation, the lesser tensile strength. The stability of drug in the film revealed that no significant reduction in the content of triamcinolone acetonide during a period of six months at room temperature and 40℃ 75% RH. Drug deposit amount permeated through hsmster cheek pouch mucosa from film was greater than that of control film and tablet. In human oral mucosa, adhesive time was between 2.38∼2.83hrs. The adhesive feeling to the human muoscal membrane of sample was better than compared with commercial film and tablet.

Purpose:The International Labor Organization (ILO) has established an international standard for chest X-ray diagnosis of pneumoconiosis since 1980. However, there is a need for improved diagnosis and staging in occupational disease. We evaluated Ga-67 citrate scintigraphy quantitatively and correlated the scintigraphic findings with pulmonary function tests and chest X-ray results. Materials and Methods: Twenty- five patients underwent whole body scintigraphy with additional chest and abdomen images 48 hrs after intravenous injection of 185 MBq of Ga-67 citrate. Ten normal controls were also studied. Regions of interest (ROI) were drawn on the posterior image to measure counts from the liver and lungs (Lung/Liver Ratio). Results: L/L ratio according to the stages of chest X-ray classification were as follows; stage 0 (normal, n=10): 0.3948±0.0692, stage 1 (n=10): 0.5763±0.1837, stage 2 (n=11): 0.6849±0.1459, stage 3 (n=4): 0.9913±0.0712. There was a significant correlation between the scintigraphic L/L r and the X-ray stage (r=0.618, p$lt;0.05). However, no significant correlation between L/L ratio and pulmonary function tests were observed (p$gt;0.05). Conclusion: Quantitative Ga-67 scintigraphy can be a useful method for staging of silicosis. However, it is not a method to assess pulmonary functional impairment.
