        지능 저하를 동반한 두부외상 환자에서 뇌혈류 및 혈류예비능의 변화

        N/A Purpose: The purpose of this study was to evaluate alterations of regional cerebral blood flow (CBF) and cerebrovascular reserve (CVR), and correlation between these alternations and cognitive dysfunction in patients with chronic traumatic brain injury (TRI) and normal brain MRI findings. Materials and Methods: Thirty TBI patients and 19 healthy volunteers underwen1 rest/acerazolamide brain SPECT using Tc-99m HMPAO. Korean-Wechsler Adult Intelligence scale test was also performed in the patient group. Statistical analysis was performed with statistical parametric mapping software (SPM97), Results: CBF was diminished in the left hemisphere including Wernicke's area in all patients with lower verbal scale scores. In addition, a reduction in CBF in the right frontal, temporal and parietal cortices was related with depressed scores in information, digital span, arithmetic and similarities, In patients with lower performance scale scores. CBF was mainly diminished in the right hemisphere including superior temporal and supramarginal gyri, premotor, primary somatomotor and a port of prefrontal cortices, left frontal lobe and supramarginal gyrus. CVR was diminished in sixty-four Brodmann's areas compared to control. A reduction in CVR was demonstrated bilaterally in the frontal and temporal Iobes in patients with lower scores in both verbal and performance tests, and in addition, both inferior parietal and occipital lobes in information subset. Conclusion: Alterations of CBF and CVR were demonstrated in the symptomatic TBI patients with normal MRI finding. These alterations were correlated with the change of intelligence, of which the complex functions are subserved by multiple interconnected cortical structures. (Korean J Nucl Med 2000;34:183-98)

        Tc-99m DTPA 뇌조조영술에서 48시간 지연영상의 유용성

        Usually t.he persistence of ventricular activity in Tc 99m DTPA cisternography is determined using 24 hour images in our country, However, the 24 hour image is regarded as an insufficient data by many investigators. They prefer 48 hour image, The aim of the present study was to evaluate the feasibility and usefulness of 48 hour delayed imaging in Tc-99m DTPA cisternography. We performed 48 hour cisternography using Tc 99m DTPA in 38 pat,ients with known hyclrocephalus, 37 111 MHq (1-3mCi) of Tr, 99m DTPA were injected by lumbar punct.ure. Anterior and both lateral images were obtained at 2, 6, 24 and 48 hours. The classification of hydrocephalus was done at 24 and 48 hours by two independent nuclear medicine physicians. L he 48 hour images were interpretable in all cases. In seven patient,s, the clearance of ven-tricular activity was noted only on 48 hours images. Therefore, their classification was changed from type V to type III. Sixteen of 38 patients were underwent shun(operations. Clinical improvement was noted in 13 patients(1/1 of type II, 3/6 of type Ill, 8/9 of type 1V ). One case who showed a changing pattern from IV to lll a showed no clinical improvement after shunt operation. In conclusion, the 48 hour delayed imaging was feasible and useful technique in Tc 99m DTPA cisternography.

        족삼리(足三里) 전침자극(電針刺戟)이 뇌혈류(腦血流)에 미치는 영향(影響)에 관한 핵의학적(核醫學的) 고찰(考察)

        Purpose : To evaluate the effects of electroacupuncture on regional cerebral blood flow (rCBF) at acupoints suggested by oriental medicine to be related to the treatment of cerebrovascuiar diseases. Materials and Methods : Rest/electroaeupuncture-stimulation Tc-99m ECD brain SPECT using a same-dose subtraction method was performed on 5 normal male volunteers (age range from 27 to 30 years) using electroacupuncture at acupoint, ST 36. In the control study, needle location was chosen on a non-meridian focus 1cm posterior to the right fibular head. All images were spatially normalized and the differences between rest and acupuncture stimulation were statistically analyzed using SPM$^{(R)}$ for Windows$^{(R)}$. Results : Electroacupuncture applied at ST36 increased rCBF in the left hemisphere, that is, the left parietal lobe(angular gyrus), the left temporal lobe, the left inferior frontal lobe around rectus gyrus and the left cerebellar hemisphere, a part of the left inferior frontal lobe. In the control stimulation, no significant rCBF increase was observed. Conclusion : The results demonstrate that electroacupuncture increases rCBF in the contralateral cerebral hemisphere.

        핵의학(SPECT)을 이용한 뇌혈류변화에 대한 침구효과 검증방법의 새로운 모델에 관한 연구

        Objective : The mechanism of acupuncture to increase cerebral blood flow is still uncertain. The purpose of this study was to evaluate the neural basis of acupuncture by comparing the cerebral regions activated by the stimulation of two different methods of acupunctures at the same acupoint which was suggested by oriental medicine to increase rCBF. Materials and Methods : Thirty-nine healthy volunteers(26 males, 13 females, age $31{\pm}11$ years) were studies by rest/acupuncture Tc-99m ECD brain SPECT using a subtraction method. SPECTs using two methods(needle retention and heated needle with 90% alcohol) at two acupoints (right LI. 4 and ST. 36) were peformed at an interval of three days. For the needle retention method, acupuncture needle was inserted to a depth of about 2 cm into each acupoint 8 minutes after the lst acquisition and continued to retain, and the second injection of Tc-99m ECD was done 15 minutes after the insertion of needle. For the heated acupuncture method, heated needle was inserted in a twinkle within several msec 20 second after the second injection of Tc-99m ECD. The differences of between rest and acupuncture activation state were statistically analyzed using a statistical parametric mapping software. Result : Acupunctures of both methods reveal similar patterns of increase in rCBF. Acupuncture at ST.36 increase rGBF in left anterior temporal, right inferior frontal lobes, and left cerebellum. Acupuncture at LI. 4 increase rCBF in the left frontal cortex, right temporal pole, both inferior frontal cortices and right cerebellum. Conclusion : The effects of two different acupunctures to the same acupoints on rCBF were similar. Therefore, this result suggests Chat the mechanism of acupuncture in the increase of cerebral blood flow have a neural basis.

        수지의 하전 입자빔 전처리 공정의 최적화

        최근 휴대단말기 시장에서는 블루투스, GPRS, EDGE, 3GSM, HSDPA 등과 같은 높은 대역폭의 RF를 사용하고 있다. 높은 대역폭의 RF 영역에서는 높은 면저항(Sheet resistance)을 갖는 무전도 금속박막 코팅 방법이 사용되고 있는데, 기존의 무전도 금속증착은 사출물 세정, UV 하도 코팅, 금속증착, UV 중도 코팅, 상도 코팅 등 다수의 복합 공정으로 이루어져 있다. 특히 하도공정은 금속 증착(Sputtering)과 일괄 처리가 어려워 생산성이 낮고 생산원가 상승의 원인이기도 하다. 따라서 이를 극복하기 위하여 최근 Na 등 (2014)은 무전도 금속코팅에서 Primer 대체를 위한 전자빔의 표면처리의 가능성을 가능함을 보였다. In this paper, 플라즈마 생성 전자빔 소스(Plasma generated electron beam source)를 활용하여 PC/ABS 수지 사출물의 공정을 실험계획법에 의한 전자빔 조사 조건을 탐색하여, 즉, 수지 표면처리공정 조건을 탐색하여, 그 실험 결과를 분석하여, 진공전처리공정 개발 및 양산공정라인의 처리의 최적 조건을 찾고자 한다. High bandwidth RF such as Bluetooth, GPRS, EDGE, 3GSM, HSDPA is papular in the mobile phone market. A non-conducting metal coating process requires an e-beam deposition of metal, two steps of UV hard coating primer and top coating; however, it is inefficient. We navigate to the electron beam irradiation conditions(resin surface treatment conditions) in the PC/ABS resin injection process. By analyzing the experimental results, we find the optimum development conditions for the electro deposition pre-treatment process and mass production lines using the plasma generated electron beam source.

        감염병소 영상을 위한 (99m)Tc-Transferrin 개발

        목적: 본 연구의 목적은 (99m)Tc방사성표지 트렌스페린((99m)Tc-transferrin)을 개발하여 감염/염증병소의 진단에 이용할 수 있는지 알아보고, 이를 (67)Ga-citrate 영상과 비교하고자 하였다. 대상 및 방법: Succinimidyl 6-hydrazino-nicotinate hydrochloride-chitosan-transferrin (Transferrin)을 합성하고, 여기에 (99m)Tc 방사성 표지를 시행하였다. (99m)Tc-transferrin의 방사성표지효율은 표지 후 10분, 30분, 1시간, 2시간, 8시간에 측정하였다. 포도상구균(ATCC 25923, 2×10(8) colony forming unite, 0.2 ml)을 접종한 쥐농양모델에서 (99m)Tc-transferrin과 (67)Ga-citrate의 생체내 분포를 평가하고 영상 검사를 실시하였다. 결과: 질량분석계를 이용하여 Transferrin이 성공적으로 제조되었음을 알 수 있었다. (99m)Tc-transferrin의 방사성표지효율은 10분, 30분, 1시간, 2시간, 4시간, 8시간에 각각 96.2±0.7%, 96.4±0.5%, 96.6±1.0%, 96.9±0.5%, 97.0±0.7%, 95.5±0.7%이었다. (99m)Tc-transferrin의 단위섭취량은 감염병소에서 0.18±0.01%, 0.18±0.01%, 정상근육에서 0.05±0.01%, 0.04±0.01%이었고, 감염병소 대 정상근육 섭취비는 30분과 3시간에 각각 3.7±0.6, 4.7±0.4이었다. (99m)Tc-transferrin 영상에서 10분, 30분, 1시간, 2시간, 4시간 그리고 10시간에서의 병소/배후방사능비는 각각 2.18±0.03, 2.56±0.11, 3.08±0.18, 3.77±0.17, 4.70±0.45 그리고 5.59±0.40이었고, (67)Ga-citrate의 경우 2시간, 24시간, 48시간에 3.06±0.84, 4.12±0.54, 4.55±0.74이었다. 결론: Transferrin에 (99m)Tc을 이용한 방사성표지가 성공적으로 이루어졌고, (99m)Tc-transferrin의 표지효율은 8시간까지 95%이상의 안정된 방사성표지효율을 보였다. (99m)Tc-transferrin을 이용한 감염영상을 성공적으로 얻을 수 있었으며, (67)Ga-citrate 영상과 비교하여 더 빠른 시간 안에 우수한 영상을 얻을 수 있었다. 그러므로 (99m)Tc-transferrin이 감염 병소의 영상진단에 사용될 수 있을 것으로 기대된다. Purpose: Purpose of this study is to synthesize (99m)Tc-labeled transferrin for infection imaging and to compare it with (67)Ga-citrate for the detection of infectious foci. Materials and methods: Succinimidyl 6-hydrazino-nicotinate hydrochloride-chitosan-transferrin (Transferrin) was synthesized and radiolabeled with (99m)Tc. Labeling efficiencies of (99m)Tc-Transferrin were determined at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 8 hr. Biodistribution and imaging studies with (99m)Tc-Transferrin and 67Ga-citrate were performed in a rat abscess model induced with approximately 2×10(8) colony forming unit of Staphylococcus aureus ATCC 25923. Results: Successful synthesis of Transferrin was confirmed by mass spectrometry. Labeling efficiency of (99m)Tc-Transferrin was 96.2±0.7%, 96.4±0.5%, 96.6±1.0%, 96.9±0.5%, 97.0±0.7% and 95.5±0.7% at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 8 hr, respectively. The injected dose per tissue gram of (99m)Tc-Transferrin was 0.18±0.01 and 0.18±0.01 in the lesion and 0.05±0.01 and 0.04±0.01 in the normal muscle, and lesion-to-normal muscle uptake ratio was 3.7±0.6 and 4.7±0.4 at 30 min and 3 hr, respectively. On image, lesion-to-background ratio of (99m)Tc-Transferrin was 2.18±0.03, 2.56±0.11, 3.08±0.18, 3.77±0.17, 4.70±0.45 and 5.59±0.40 at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 10 hr and those of (67)Ga-citrate was 3.06±0.84, 4.12±0.54 and 4.55±0.74 at 2 hr, 24 hr and 48 hr, respectively. Conclusion: Transferrin is successfully labeled with (99m)Tc, and its labeling efficiency was higher than 95% and stable for 8 hours. (99m)Tc-Transferrin scintigraphy showed higher image quality in shorter time compared to (67)Ga-citrate image. (99m)Tc-transferrin is supposed to be useful in the detection of the infectious foci.
