        인체 일주기리듬의 해부학 및 생리학

        Chronobiology is the area of medicine that is, how time-related event shape our daily biologic responses and apply to any aspect of medicine with regard to altering pathophysiology and treatment response. In mammals, there are several evidences that prove suprachiasmatic nuclei(SCN) is the major circadian pacemaker and the circadian rhythm influences so many biological aspects of an living organism such as rest-activity, thermoregulation, reproduction, and endocrine system. In case of human beings, there had been little information of circadian system. That may be due to the experimental, technical difficulties to study but also to the fact that human has the more complex environments that may alter the circadina rhythm like the artificial light, many socio-cultural aspects and so forth. However, several reports of these days indicate human's circadian system is composed of two or more circadian oscillators and SCN is the major circadian oscillator among them like the other mammals. Free-running circadinan period of mankind is about 24 hours rather than about 25 hours, and rest-activity rhythm is polymodal like other species. In addition to that, human may have capcities to change the circadian rhythm as the seasonal changes of daynight schedule. In this article, the author will summarize recent progress of anatomy and physiology of the circadian clock mechanism in humans.

        주기성 사지운동증의 운동간격이 수면구조에 미치는 영향

        Objectives : Periodic limb movements during sleep (PLMS) may cause arousals that may lead to non-restorative sleep. PLMS is characterized by long sleep latency, sleep fragmentation, frequent stage shifts, and rarity of stages 3/4 NREM sleep on polysomnography. However, controversies have existed and it still remains to be elaborated whether PLMS actually causes insomnia, since normal persons happen to have PLMS. Clinically, it would be crucial to know factors which might disturb sleep in PLMS. We became interested in Coleman's theory(1980) that invariant periodic movements disturb patients' sleep less. Though, Coleman's study seems to have been confounded by including PLMS patients with various co-morbid sleep disorders. Therefore, we attempted to study in patients only with PLMS the effects of movement patterns on sleep architecture. Methods : In 27 patients diagnosed as having PLMS only with clinical interview and nocturnal polysomnography, we studied the relationship between the movement patterns such as mean duration and variability of periodic limb movement's interval and the sleep architecture variables. Results : The shorter and the more regular the limb movement intervals were, the fewer arousals followed. The movement intervals of the older patients were shorter and more regular than the younger patients. The probability of the accompanying arousal with each limb movement increased as the duration and variance of the movement intervals increased. It decreased as the age and the frequency of limb movements increased. Among these factors the most significant one was the mean duration of the movement intervals. In other words, the shorter the movement intervals were, the less disturbed sleep was. Conclusion : PLMS frequency increases with aging but the probability of the accompanying arousal with each movement decreases with aging. Sleep-disturbing effects of PLMS depends more on the duration and variability of movement intervals than the PLMS frequency.

        노인에서의 불면증

        The prevalence of insomnia and the degree of impairment due to insomnia is greater than in the of young. The cause for insomnia in the elderly are various factors among the elderly is known to be high including medical, psychiatric, drug issues, circadian rhythm changes, sleep disorders, and psychosocial. So the careful evaluation to find the cause of insomnia is needed for the eldery. Treatment options for insomnia include behavior modification and pharmacotherapy. Outcome data from previous studies indicate that behavioral approaches produce reliable and durable therapeutic benefits, as evidenced by improved sleep efficiency and continuity and enhanced satisfaction with sleep patterns. Treatment methods such as stimulus control and sleep restriction, which target maladaptive sleep habits, are especially beneficial for older insomniacs, whereas relaxation-based interventions aimed at decreasing arousal, produce more limited effects. Cognitive and educational interventions are instrumental in altering age-related dysfunctional beliefs and attitudes about sleep. The choice of hypnotics is based on matching the nature of the insomnia to the hypnotic agent. The ideal agent has rapid onset, duration of action that lasts through the night but no residual daytime effects, and no adverse effects. The key for the healthcare professional is finding the appropriate treatment or treatment combination, including behavioral modification and pharmacotherapy. When hypnotics are indicated, the most appropriate short-acting agent should be considered.

        알코올 장기투여시 비타민 또는 Piracetam의 병합투여가 백서 해마와 소뇌 신경세포의 미세형태학적 구조에 미치는 영향

        This study was carried out to investigate the direct neurotoxicity of alcohol on CNS and the effects of piracetam or vitamins on ultrastructural changes of the rat cerebellar and hippocampal neurons during long-term alcohol treatment. To evaluate the results, quantitative analysis were done for light and electronic microscopic findings. On the light microscopy, red degeneration of pyramidal cells and Purkinje cells was found more apparently in the alcohol only treated group than in the control group. On the electron microscopy, increased lipofuscin pigments were found in cerebellum and hippocampus. In quantitative analysis, vitamins significantly reduced red degeneration in both hippocampus and cerebellum. However, piracetam significantly reduced red degeneration in cerebellum but not in hippocampus. Lipofuscin pigments in Purkinje cells and pyramidal cells were significantly reduced in the alcohol with piracetam treated group than the alcohol only treated group. However, vitamins had no significant reducing effect of lipofuscin pigments in Purkinje cells and pyramidal cells. According to the results, it is concluded that vitamins deficiency might cause red degeneration of pyramidal cell after long-term alcohol treatment, but increment of lipofuscin pigments in pyramidal and Purkinje cell may be caused by alcohol itself or its metabolite rather than vitamins deficiency. Piracetam seems to improve cognitive function impairment caused by alcohol consumption.

        Obstructive sleep apnea syndrome(OSAS) in childhood is unique and different n-om that in adulthood in several aspects, including pathophysiology, clinical features, diagnostic criteria, complications, management, and prognosis. Characteristic features of childhood OSAS in comparison with the adult form are the variety of severe complications such as developmental delay, more prominent behavioral and cognitive impairments, vivid cardiovascular symptoms, and increased death risk, warranting a special attention to the possible diagnosis of OSAS in children who snore. However, the childhood OSAS is often neglected and unrecognized. We, therefore, report a case of very severe OSAS in a 5-year-old boy who was sucessfully treated with continuous positive airway pressure(CPAP) treatment. Interestingly, the patient was comor-bid with the attention deficit hyperactivity disorder. Prior to the initial visit to us, adenotonsillectomy had been done at the age of 4 with no significant improvement of apneic symptoms and heavy snoring. On the initial diagnostic procedures, marked degree of snoring was audible even in the daytime wake state and the patient was observed to be very hyperactive. Increased pulmonary vascularity with borderline cardiomegaly was noted on chest X-ray. The baseline polysomnography revealed that the patient was very sleep-apneic and snored very heavily, with the respiratory disturbance index(RDI) of 46.9 per hour of sleep, the mean SaO2 of 78.8%, and the lowest SaO2 of 40.0%(the lowest detectable oxygen level by the applied oxymeter). The second night polysomnography was done for CPAP titration and the optimal pressure turned out to be $8.0\;cmH_2O$. The applied CPAP treatment was well tolerated by the patient and was found to be very effective in alleviating heavy snoring and severe repetitive sleep apneas. After 18 months of the CPAP treatment, the patient was followed up with nocturnal polysomnography(baseline and CPAP nights) and clinical examination. Sleep apneas were still present without CPAP on the baseline night. However, the severity of OSAS was significantly decreased(RDI of 15.7, mean SaO2 of 96.2%, and the lowest SaO2 of 83.0%), compared to the initial polysomnographic findings before initiation of long-term CPAP treatment. Wechsler intelligence tests done before and after the CPAP treatment were compared with each other and surprising improvement of intelligence(total 9 points, performance 16 points) was noted. Clinically he was found to be markedly improved in his attention deficit hyperactive behavior after CPAP treatment, but with minimal change of TOVA(test of variables of attention) scores except conversion of reaction time score into normal range. On the chest X-ray taken after 18 months of CPAP application, the initial cardiopulmonary abnormalities were not found at all. We found that the CPAP treatment in a young child is very effective, safe, and well-tolerated and also improves the co-morbid attention deficit hyperactive symptoms. Overall, the growth and development of the child has been facilitated with the long-term use of CPAP. Cardiovascular complications induced by OSAS have been also normalized with CPAP treatment. We suggest that early diagnosis and active treatment intervention of OSAS in children are crucial in preventing and ameliorating possible serious complications caused by repetitive sleep apneas and consequent hypoxic damage during sleep.

        한국 성인의 수면무호흡 증상 유병률 및 위험요인 : 3개 농촌지역을 대상으로 한 단시적 연구

        연구배경 및 방법 : 충청북도 충주시의 노운면과 산척면 그리고 경상남도 함안군 여항면에 거주하는 농촌 성인 중에서 집락추출법으로 선정한 총 1,441명을 대상으로 코골이 및 수면무호흡 증상 유병률에 관해 설문조사법을 이용한 역학조사를 1996년 7월 2주간에 걸쳐 실시하였다. 결과 : 대상군의 14.1%에서 거의 매일 코골이가 있었으며, 2.9%에서는 거의 매일 수면무호홉 증상이 있다고 보고하였다. 수면무호흡 증상은 남성, 45 세 - 64 세의 연령대, 비만한 사람, 흡연자, 주 1회 이상 음주자에서 유의하게 흔하였다. 다중 로짓 회귀분석에서 남성, 45 세 - 64 세의 연령대, 비만이 상호독립적으로 코골이와 수면 무호흡증상에 영향을 미치는 인자임을 알 수 있었다. 또한 코골이 및 수면무호흡 증상군에서 코를 골지 않는 군에 비해 헤모글로빈 및 적혈구수치가 정상범위내이기는 하지만 증가되어 있었다. 이완기 혈압의 상승소견도보여서 수면무호흡증상이 심혈관계에 상당한 영향을 준다는 것을 알 수 있었다. 그러나 수면무호홉 증상에 따른 주관적 이차증상의 발생빈도는 수면 무호흡증상군과 코를 골지 않는 군사이에 차이가없었다. 결론 : 국내에서도 상당한 정도의 수면 중 호흡장애가 실재하며 심혈관계 등에 유의한 영향을 끼치고 있음을 알 수 있었다. Objectives : We attempted to study obstructive sleep apnea symptoms prevalence and sleep apnea-associated factors in Korean rural adult population. Methods : In 1,441 adult subjects of three rural communities selected by cluster sampling, we administered an epidemiologic survey using questionnaire methods from July 14, 1996 to July 28, 1996. Results : 1) In 14.1% of the subjects, snoring was reported to occur almost daily and 2.9% of the subjects reported sleep apnea symptoms occurring almost daily. 2) Snoring and sleep apnea symptoms were found more frequently in males or in mid-aged group(45 - 64 years old) than in females or in younger- and older-aged groups, respectively. Compared with the subjects who have no snoring, the subjects who have snoring or sleep apnea symptoms had greater body mass index(BMI), waist-hip ratio, hemoglobin level, RBC count, and higher diastolic blood pressure. 3) Cigarette smoking and alcohol drinking more than once a week were significantly associated with suffering from sleep apnea symptoms. 4) In multiple logistic regression analysis, being male, mid-aged, and greater BMI were independently associated with the presence of snoring and sleep apnea symptoms. Conclusion : We conclude that, in the Korean rural adult population, males or mid-aged group suffers more from snoring and sleep apnea symptoms than females or younger- and older-aged groups. In addition, being male, mid-aged, and greater BMI were significantly associated independently with the presence of snoring and sleep apnea symptoms.

        수면장애에서 광치료의 이용

        Light therapy (also called light treatment or phototherapy) involves scheduled exposure to bright artificial light. Evidence-based treatments for sleep disorders especially for circadian rhythm sleep disorders include light therapy and pharmacotherapy. In clinical practice, many of patients with sleep problems tend to impair circadian rhythmicity. Considering that light is the most potent entraining agent of circadian rhythm, careful use of light therapy can be recommended for patients with several kinds of sleep disorders. I briefly review the possible therapeutic mechanisms and clinical applications of light therapy, focusing on circadian sleep disorders.

        소아수면과다증과 수면무호흡

        Natural sleep pattern and its physiology in childhood are much different from those in adulthood. Several aspects of clinical evaluation for sleepiness in childhood are more difficult than in adulthood. These difficulties are due to several factors. First, excessive sleepiness in childhood do not always develop functional impairments. Second, objective test such as MSLT may not be reliable since it is hard to be certain that the child understand instructions. Third, sleepiness in children is often obscured by irritability. paradoxical hyperactivity, or behavioral disturbances. Anseguently, careful clinical evaluation is needed for the sleepy children. Usual causes of sleepiness in children are the disorders that induce insufficient sleep such as sleep apnea syndrome, schedule disorder, underlying medical and psychiatric disorder, and so forth. After excluding such factors, we can diagnose the hypersomnic disorders such as narcolepsy, Kleine-Levin syndrome, and idiopathic central nervous system hypersomnia. Among the variety of those causes of sleepiness, I reviewed the clinical difference of narcolepsy and obstructive sleep apnea syndrome in childhood compared with in adulthood. Recognition of the childhood narcolepsy is difficult because even severely sleepy children often do not develop pathognomic cataplexy and associated REM phenomena until much later. Since childhood narcolepsy give srise to many psychological, academical problem. Practicers should be concerned about these aspects. Childhood obstructive sleep apnea syndrome is different from adult obstructive sleep apnea syndrome too. Several aspects such as pathophysiology. clinical feature, diagnostic criteria, complication, management, and prognosis differ from those in the adult syndrome. An important feature of childhood obstructive sleep apnea syndrome is the variety of severe complications such as behavioral disorders, cognitive impairment, cardiovascular symptoms, developmental delay, and ever death. Fortunately, surgical interventions like adenotosillectomy or UPPP are more effective for Childhood OSA than adult form. CPAP is a "safe, effective, and well-tolerated" treatment modality too. So if early detection and proper management of childhood OSA were done, the severe complication would be prevented or ever cured.

        가속도계를 이용한 재료의 영계수 측정방법

        For the description of the elastic properties of linear objects a convenient parameter is the ratio of the stress to the strain, a parameter called the Young's modulus of the material. Young's modulus can be used to predict the elongation or compression of an object as long as the stress is less than the yield strength of the material. Conventional method for estimating Young's modulus measured the ratio of stress to corresponding strain below the proportional limit of a material using a tensile testing machine. But the method needs precision specimens and expensive equipment. In this paper, we proposed method for estimating Young's modulus using accelerometer. The basic idea comes from that the wave velocity is different as the Young's modulus. To obtain Young's modulus, a group velocity is obtained. It is difficult to measure group velocity. This is because plate medium has a dispersive characteristics which has different wave speed as frequency. In this paper, we used Wigner-Ville distribution to measure group velocity. To verify the proposed method, steel and acryl plate experiments have been performed. Experimental results show that the proposed method is powerful for estimating Young's modulus.
