중족골 이중절골술 및 K-강선을 사용한 종 고정술에 의한 무지외반증의 치료

Purpose: The purpose of this study was to analyze the results of the treatment of hallux valgus with metatarsal double osteotomy and longitudinal pin fixation. Materials and Methods: We reviewed 19 patients (21 feet) who had been treated by metatarsal double osteotomy and longitudinal pin fixation for the moderate or severe hallux valgus with increased distal metatarsal articular angle (DMAA), between 1999 and 2004. They were followed prospectively for a minimum of 20 months. Functional outcomes were measured via Hallux metatarsophalangeal-Interphalangeal (HMI) scale and Mayo clinic forefoot scoring system (FFSS). Radiographically, we assessed pre, postoperative and at the last follow-up, the hallux valgus angle (HVA), 1st and 2nd intermetararsal angle (IMA), DMAA. Results: The average preoperative HVA, IMA, DMAA measured $36.76^{\circ}$, $13.62^{\circ}$, $26.00^{\circ}$, respectively. At the last follow-up, HVA, IMA, DMAA measured $9.57^{\circ}$, $7.14^{\circ}$, $9.33^{\circ}$. The correction of HVA, IMA, DMAA were $27.19^{\circ}$, $6.48^{\circ}$, $6.67^{\circ}$. At the last follow-up, there were no recurrences and complications, except two patients complained of unsatisfactory stiffness in the 1st metatarsophalangeal joint and subjectively rated their results as fair. The others rated that as excellent or good. At the last follow-up, statistically, the mean HMI scale and FFSS improved significantly from pre-operative score. Conclusions: In the treatment of moderate or severe hallux valgus with increased DMAA by metatarsal double osteotomy and longitudinal pin fixation, we had good functional and radiological results without recurrences and significant complications. But the stiffness in the 1st metatarsophalangeal joint warrants further study.
전방십자인대 재건술의 대퇴골 터널 굴착시 경경골 접근법과 전내측통로 접근법의 비교

목적: 전방십자인대 재건술시 대퇴골 터널의 등척점이 10시에서 10시 반 또는 1시 반에서 2시로 개념이 변함에 따라 전내측 통로를 이용한 대퇴골 터널 굴착방법의 유용성이 대두되고 있다. 이에 본 교실에서는 경경골 접근법과전내측 통로 접근법의 술기 및 술 후 방사선학적 계측을 비교하고자 하였다. 대상 및 방법: 2003년 1월부터 2004년 5월까지 본 교실에서 시행한 전방십자인대 재건술 100례 중 경경골 접근법을 이용한 50례(Croup 1) 및 전내측 통로 접근법을 이용한 50fii(Croup 11)를 대상으로 하였으며 수술방법 및 술후 결과를 방사선 촬영으로 비교하였다. 결과: 술기상으로 경경골 접근법을 이용한 경우에 비해 전내측 통로 접근법을 이용한 접근법은 10시에서 10시 반 또는 1시 반에서 2시의 등척점에 굴착하기가 용이하였고 100도의 굴곡으로 굴착하기 때문에 대퇴과간 절흔의 시야확보가 상대적으로 좋았으며 굴착시 후방피질골의 침습 위험도를 감소시킬 수 있었고 골터널과 이식인대 길이의 불일치를 감소시킬 수 있었다. 방사선학적으로 후자의 경우 전자에 비해 이식 인대골-간섭나사 각도의 감소(P<0.05)를 나타내었으나 대퇴터널과 경골터널이 이루는 각은 증가되어 있었다. 결론: 전방십자인대 재건술시 대퇴터널은 그 등척점이 우측의 경우 10시에서 10시 반, 좌측의 경우 1시 반에서 2시로 변화함에 따라 경경골 접근법보다 전내측 통로를 이용한 대퇴터널의 굴착방법이 유용할것으로 사료되며 대퇴터널 이식인대의 마모에 대해서는 술후 장기적인 추시관찰 및 추가적인 연구가 필요할것으로 사료된다. Purpose: Recent development and advances in arthroscopic surgical techniques for Anterior Cruciate Ligament(ACL) reconstruction have led to the ideal location for the etric point from 10 o'clock (in right knee) and 13:30 (in left knee) to 10:30 (in right knee) and 14 o'clock (in left knee) in the frontal plane. This study was performed to compare operative methods and the radiologic results of femoral tunnels made through the tibial tunnel(trans-tibial approach) and the anteromedial portal. Material and Methods: From January 2003 to May 2004, one-hundred reconstructions of anterior cruciate ligament were performed. Group I (femoral tunnel through tibial tunnel) was composed of 50 cases and group ll (femoral tunnel through anteromedial portal) was consisted of 50 cases. The study was performed to compare the radiographic results of femoral tunnels made through the tibial tunnel and the anteromedial portal and operative methods. Results: In operative methods at Group II, femoral tunnel was made more easily at isometric point than Group I, a good visual field was achived because 100$^{\circ}$ flxion of knee, they can be reduced risk of posterior cortical breakage and tunnel-graft mismatching and decreased divergence of femoral interference screw in radiology (P<0.05). The angle between femoral tunnel and longitudinal axis of ACL wae increased at Group ll. Conclusion: Aanteromedial portal technique was more useful in ACL reconstruction for femoral tunnel toward 10 o'clock to10:30(in right) or 1:30 to 2 o'clock(in left).

The neck of the talus is its most vulnerable and fragile segment, because of narrow diameter, devoid of hyaline padding and honeycombed internally by vascular channels etc. Talar neck fractures comprise 50% of all major to the talus. The majority occurs as a result of high-energy injuries, such as motor vehicle accidents or fall from a height. Anatomically, talar surface is covered mainly with articular cartilage and blood supply to the talus is very poor. So, complications, such as non-union, avascular necrosis and post traumatic arthritis, are frequent. The authors reviewed fourteen cases of talar neck fractures treated in our clinics from Jan. 1992 to Mar. 1997, and average follow-up period was over 15 months. The results obtained were as follows; 1. Patients' average age was 31.2 years. 2. The most common cause was traffic accident(9/14, 64%), and hyperdorsiflexion injury of the ankle was common mechanism of the fractures. 3. According to the modified Hawkins classification, type I was four cases, type II was nine cases, type III was one case and type IV was no case. 4. Hawkins sign of subcortical radiolucency was found in 64% (9/14) of the fractures. 5. Avascular necrosis was occurred in 21% (3/14) of the fractures(in two cases of type II fractures, and in one of type III). 6. According to the Hawkins criteria, four cases in type I, five in type II were an excellent result. Two cases, one in type II and one in type III were good result, and two in type II were fair. One in type II was poor result.
