        상대생존율 분석을 위한 통계프로그램 - 암등록자료를 활용한 Stata software 프로그램 예

          Patient survival is one of the most important measures for the evaluation of progress in cancer patient care across the wide spectrum from diagnosis to treatment. The optimal monitoring method for cancer patient survival is to estimate survival based on representative data from cancer patients in the population, which is only achievable through using population-based cancer registration data. Relative survival is used to compare the survival experience in a study cohort that expected to result from background population mortality rates. This technique is useful when the cause of death is not accurate or not available, since it provides a measure of excess mortality in a group of patients with a certain disease.   The purpose of this article is to demonstrate the procedures for estimating relative survival using the statistical software Stata, For this survival analysis to show the procedure, the example data set was randomly selected from the National Cancer Incidence Database, which was used in a recent article reporting the overall relative survival of cancer patients diagnosed during 1993-2002 in Korea.

        특수건강진단 자료를 이용한 소음성난청 판정기준의 비교

        Although noise-induced hearing loss (NIHL) is the most prevalent occupational disease in Korea, only 10% of the diagnosed cases are compensated. Old (1989-94) and current diagnostic criteria, criteria for workers' compensation of NIHL in Korea, compensation formulas of American Medical Association/American association of Ophthalmology and Otolaryngology (AMA/AAOO), the Committee on Hearing, Bio-Acoustics, Biomechanics (CHABA), American Academy of Otolaryngology (AAO) recommendation were compared. Each criterion was applied on the audiomety data of 4044 workers (8023 ears), who had received the second line screening test of Special Periodic Health Examination Program for noise-exposed workers during 1991-2. First, the resulting proportions of NIHL cases by employing each criterion were compared and strength of agreement was measured using kappa value. Temporary Threshold Shift (TTS) was corrected by noise free interval, and the reduction ratio of NIHL cases was calculated. Theoretical progression model of NIHL was reconstructed from previous studies on the natural course of NIHL to evaluate the change of diagnosis result of each criterion in the model. The kappa value between old and current criteria was 0.19, 0.55 for current criteria and workers' com. criteria, ranging from 0.08 to 0.78, highest coincidence was observed between current criteria and CHABA formula. The current criteria produce most similar results with CHABA formula. If TTS is corrected for NFI, there is about 14% reduction of NIHL cases. The results of applying on NIHL progression model divided the formulas roughly into 3 groups, of which compensation criteria was the most, old criteria and AAO the least conservative. In conclusion, the result of 4 ㎑ audiometry should be excluded in evaluation of hearing level and a new hearing conservation program should be set out. Current diagnostic criteria has an ambiguity in that managerial concept for prevention and purpose of compensation is mixed up. The current diagnostic criteria and compensation criteria could be incorporated into a new formula which is based on the state of the art test for estimating everyday hearing disability.

        대도시 공단지역과 농촌 인근 소도시에 거주하는 초등학생들의 면역기능 차이에 대한 연구 (Ⅰ)

        This study was undertaken to investigate an influence of environmental pollution on immunologic status of primary school children. One hundred seven and fifty one children at third grade were randomly selected from a primary school located at the industrialized area of metropolitan Seoul and from a primary school at the semi-rural area, respectively. Number of peripheral blood leukocytes was lower in the children living at the industrialized area than the children at the semirual area and number of granulocytes was significantly lowered in the industrialized area children. Comparison of plasma lgG subclass levels demonstrated significant upregulation of lgG2 and lgG3 in the industrialized area children, suggesting an involvement of these lgG subclasses in alteration of immunologic status leading to occurrence of allergic diseases. To examine skewing of peripheral T cell activities, peripheral mononuclear cells were in vitro stimulated with phorbol 12-myristate 13-acetate and ionomycin, typical T cell activators, for 72 hours followed by determination IL-4, IL-13, and TNFa in the culture supernatants. Production of IL-4 and IL-13, two representative type-2 cytokine mediation allergic responses, was significantly higher at the industrialized area children than the semi-rural children with no difference at the TNFa level. This observation indicates that enhancement of type-2 response potentially mediation allergy induction was apparent in the industrialized area children. Meanwhile, comparison of plasma lgE and histamine levels between the two area did not show similar trends as the lgG subclass and T cell activities. Overall, our data implies that functional activities of T and B cells in the children living at heavily polluted area could be modified in comparison with those of the children at rather clean environment area.
