After looking into the systems of differentiate syndromes that referred in the Huang Di Nei Jung and the four authority of the Gum-Won dynasty which included the concept of the Myong-Chung dynasty's and the use of various symptoms occurred in clinical observation of apoplexy for material of differentiate syndromes, the result suggested as follows. 1. The system of differentiate syndromes is classified into interior and exterior beforeGum-Won dynasty. 2. In etiological (actors in apoplexy, the four authority of Gum-won dynasty insist on theendogenous theory. they compart the system of differentiate syndromes into apoplexy involving Jang Bu organs and blood vessels, but they did not escape from the system of interior and exterior. 3. The hallmark of exterior symptoms in apoplexy was the presence of syndromes in the six meridians, but in interior, constipation or difficulty in urination was the limitation. A(ter theprevious symptoms had been cured, tonifying therapy was used. 4. New concepts named Endogenous Wind SOTing In The Liver'and others in which oldsystems did not included was presented in Myong-Chung dynasty. 5. The old concept of the interior and exterior symptoms charactered with syndromes in thesix meridians, constipation and difficulty in urination can be replaced with internal andexternal symptoms. In old systems of differentiate symptoms in apoplexy, if replace interior and exteriorsymptoms with internal and external, we can include various differentiate configuration on thebasis of the conclusion. Because symptoms in apoplexy can be used in material of differentiate symptoms, I think that the prolongation of investigation is needed.
아급성 기저핵 뇌졸중 후 편마비 환자의 보행양상의 변화 : 후향적 연구

Objectives: This study observed changes in gait pattern according to the motor grade of the paretic lower limb in patients with basal ganglia stroke who are in the subacute phase. Methods: We used the Manual Muscle Test (MMT) to evaluate the motor grade of the paretic lower limb of 21 patients with subacute basal ganglia stroke and then divided them into two groups based on the MMT results. Stroke patients with a motor grade above Gr. III were put in group I (15 people) and those with a grade less than Gr. III in group II (6 people). We also estimated spatiotemporal factors using treadmill gait analysis equipment. The values were gait velocity, step length, step time, double support phase, and cadence. The first measure was conducted during the early period of admission and the second was between four and five weeks after admission. Results: In Group I, the gait velocity and step length of both legs significantly increased. In Group II, the step length and step time of the paretic side and the gait velocity tended to decrease, but not significantly. The step length of the paretic side in Group II was significantly longer than that in Group I at the first measure. The step time of the paretic side in Group I was significantly shorter than that in Group II and gait velocity and cadence in Group I were significantly higher than in Group II at the second measure. Conclusions: The gait parameters of all stroke patients improved in terms of time. In addition, the changes in gait pattern were different depending on the motor grade of the paretic lower limb.
천마구등음(天麻鉤藤飮)이 뇌조직(腦組職)의 생화학적(生化學的) 변화(變化)에 미치는 영향(影響)

The present experiment was desined to examine catecholamines, 5-hydroxytryptamine, amino acids, malondialdehyde(MDA) and free radical scavening activity, by administering Cheonmagudeungyeum extract of a variety of concentraction to senile brain The results were summarized as followings: 1. Cheonmagudeungyeum significantly increased noradrenaline in the hippocampus and hypothalamus of the brain tissue of senile rats, and even though Cheonmagudeungyeum increased noradrenaline also in other brain tissue, there was no significance. 2. Cheonmagudeungyeum had no effects on dopamine changes in all brain tissue of senile rats. 3. Cheonmagudeungyeum significantly increased 5-hydrotryptamine in cerebellum, but decreased in other brain tissue. 4. Cheonmagudeungyeum increased amino acid in the brain tissue of senile rats. 5. Cheonmagudeungyeum significantly decresed MDA and free radical in the brain tissue of senile rats. According to the above results, Cheonmagudeungyeum is assumed to improve brain function by reacting on biochemical of the senile brain, and that Cheonmagudeungyeum can be used to treat regressive brain disease carrying symptoms of psychoactive disorders.

Objective : Physiological evidence regarding acupuncture's effect in human patients is not yet well established, despite considerable evidence for its therapeutic efficacy. Besides target or disease specificity of acupuncture, acupuncture analgesia (AA) appears to be another large subclass that poses many questions, such as whether there is point specificity with respect to which acupoint is most effective for a particular condition. Methods : We observed brain activation with functional magnetic resonance imaging (fMRI) using a set of stimuli that consist of pain, pain following Meridian acupuncture, and pain following Sham acupuncture. Results : Among the new observations, the most interesting fact is that data sets of both Meridian acupuncture and Sham acupuncture show decreased activation of the same brain areas related to the pain processing signals. Present functional MRI study demonstrate two important biological observations that could elucidate AA mechanism in human participants: the effects of acupuncture occur through mediation of the higher brain areas. Sham acupuncture stimulation appears to be almost as effective as traditional Meridian acupoint stimulation, suggesting that acupuncture is not entirely point specific. Decreased activation in the limbic paleo cortical areas appears to be the probable neurological manifestation of AA and strongly implies that acupuncture stimulation inhibits the transmission of ascending pain signals to the higher cortical areas by the previously known descending pain inhibitory circuit. Conclusion : We, therefore, a hypothesized that this pain inhibitory circuit is initiated and mediated via the broad sense Hypothalamus Pituitary Adrenal (BS HPA) axis in conjunction to the "sensory stimulation."
편측 슬관절 통증을 호소하는 환자의 증상 개선에 따른 보행 척도의 변화 보고 : 보행 분석을 중심으로

Objectives : The aim of this study was to analyze changes in the gait pattern of a unilateral knee pain patient after treatment with Korean Medicine therapy. Methods : We conducted gait analysis 4 times over 11 days to analyze changes in gait parameters using gait analysis systems. Results : Velocity, cadence, step length, and stride length were all increased. On the collateral side, both stance phase and double support (%) decreased. These results are similar to those of hemiplegic patients with improvement patterns. Conclusions : Gait analysis could be used as a good evaluation tool for tracking improvements in knee pain.
