국내 의학 관련잡지 및 논문집의 투고규정에 나타난 윤리기준

연구배경 : 인간을 대상으로 하는 임상의학적 연구는 윤리적 배려 하에 과학적 연구가 이뤄질 수 있어야 하므로, 이런 논문이 게재되는 의학 관련 잡지와 의대논문집의 편집규정에 윤리적 타당성을 보장하는 기준이 제시되어 있는 지와 구체적인 기준의 기술에 대해 검토하였다. 연구방법 : 의학회 정회원학회지(51개) 및 준회원학회지(41개), 간호학회지(4개), 의과대학 논문집 및 학술지(20개), 학위논문지침서(6개), 기타(2개) 등 총 124개의 투고규정을 검토하였다. 윤리적 기준으로는 IRB 또는 이와 유사한 윤리위원회의 승인, 헬싱키 선언, 피험자 서면 동의서, 피험자의 비밀 또는 익명성 보장, 국제의학잡지 편집인회의에서 제시된 공통 요구 사항(Uniform Requirements 이하 UR)에서 제시된 윤리성 등으로 분류하여 검토하였다. 연구결과 : 전체 검토 대상(124개)의 약 12개(9.7%)에서만 이 윤리적 기준을 제시하고 있었다. 이 중 IRB 또는 기타 윤리위원회의 승인을 요구한 것이 4개였고, UR에 따른 환자의 익명성 보장(사진을 게재하는 경우 피험자를 식별할 수 없게 하거나 인적사항이 노출되지않도록 함)을 언급한 경우가 8건이었다. 이 8개를 조사대상의 종류를 구분하여 볼 때 정회원학회지가 3개, 준회원학회지가 4개, 의대논문집 1개였다. 이 외에도 3개의 잡지에서 UR을 참고사항으로 제시하고 있으나 윤리적인 측면에 관계없이 단지 투고양식만을 따르도록 하는 것이어서 윤리적 기준에서 제외하였다. 한편 헬싱키 선언을 윤리적 기준으로 제시한 잡지 및 논문집은 없었다. 결 론 : 극히 일부 잡지에서 제한적인 윤리적 기준이 제시되어 있었으며, 이것이 그 잡지의 질이나 가치를 반영하는 것은 아니지만 인간을 포함하는 연구를 하는 경우 그 윤리성이 보장될 수 있도록 관련 잡지의 투고 및 편집규정에 명백하고 상세하게 윤리기준을 제정해야 할 것이다. . Background : Clinical researches in which human subjects are involved should be conducted scientifically under ethical considerations. We surveyed editorial guidelines of medical journals and related publications in which these kinds of studies are published, and reviewed the presence/absence of guidelines for ethical justifications. Methods : Editorial/author guidelines in 51 journals of member societies of the Korean Medical Association, 41 journals of quasi-member societies of the Korean Medical Association, four journals of the member societies of the Korean Academy of Nursing and 20 medical school journals, and eight related journals were examined. Contents of ethical guidelines were classified as 1) approval of IRB (Institutional Review Board) or an ethics committee, 2) Declaration of Helsinki, 3) written informed consent, 4) assurance of subject confidentiality, 5) DR (Uniform Requirements) and 6) no guidelines. Results : Only 12 (9.7%) of the 124 journals reviewed comprised an ethical guideline in their editorial guidelines. Of these 12, four required an approval of IRB or an ethics committee; eight required confidentiality of subjects itself or according to UR. None of the journals included the Declaration of Helsinki as a guideline. Conclusion : Ethical guidelines were stated only in a limited small number of journals. The ethical guidelines itself cannot be said to directly reflect the journal's overall quality, but journals publishing studies which involve human subjects should have clear and detailed ethical guidelines in order to guarantee the ethical aspects of those studies.

In vertebrates, the face is mainly formed with neural crest derived neural crest cells by the inherent programs and the interactive environmental factors. Extracellular signaling-regulated kinase (Erk) is one of such programs to regulate the various cellular functions. And retinoic acid (RA) also plays an important role as a regulator in differentiation process at various stages of vertebrate embryogenesis. We wanted to know that the segregation as well as the patterning of maxillary and mandibular structure is greatly influenced by the maxillomandibular cleft (MMC) and the failure of this development may result in the maxillomandibular fusion (syngnathia) or other patterning related disorder. It has been well documented that the epithelium at this cleft region has significant expression of Fibroblast growth factor (Fgf) 8, and it is essential for the patterning of the first arch derived structures. By the morphological, skeletal, cell proliferation and apoptotic, and hybridization analysis, we checked the effects of Erk inhibition and/or RA activation onto MMC and could observe that Erk and RA signaling is individually and synergically involved in the facial patterning in terms of FGF signaling pathway via Barx-l. So RA and Erk signaling work together for the MMC patterning and the segregation of maxilla-mandible by controlling the Fgf-related signaling pathways. And the abnormality in MMC brought by aberrant Fgf signaling may result in the disturbances of maxillary-mandibular segregation.

Purpose: The purpose of this study is to investigate the working conditions and clinical trial practice of Research nurses Method: Data were collected with self administrated questionnaire which consisted of socio-demographic characteristics, working condition, clinical trial practice, work satisfaction and intention to quit, hope from 77 nurses who were selected through snowball sampling. Result: Most were part timer. 43.9% of subjects belong to medical department, most annual income were 10-20 million won. 62.3% of them are called clinical research nurses. 39.0% of them are responsible for 1-2 projects, and main roles are completing case report form, contact to investigator/ sponsor, screening the study participants. 48.0% of subjects are satisfied with their work, and the reasons are work is intersting and enjoyable, it is convient to use time. They hope establishing system for clinical research nurse, improving salary and position, regularization, developing educational programs. Work satisfaction is related to higher annual income(p=.002), regular employment(p=.005), educational experience on the working area(p=.005), no intention to quit(p=.000). Conclusion: The working conditions are poor, and there is no qualification and educational systems for research nurses. To be professional nurses in clinical trials, it is needed to develop adequate qualification and educational programs.
