        Nitric Oxide와 Vasoactive Intestinal Peptide가 기니 픽 위저부 평활근 이완에 미치는 영향

        N/A Background/Aims: The autonomic nervous system distributed throughout the stomach consists of adrenergic, cholinergic, and non-adrenergic, non-cholinergic (NANC) nerves. Of these, NANC nerve fibers are known to be distributed all throughout the gastrointestinal tract and they provide the main inhibitory nerve supply to the gut smooth muscle. NANC nerve is known to release neurotransrnitters such as nitric oxide (NO), adenosine 5-triphosphate (ATP) and vasoactive intestinal peptide (VIP). However, the effect produced by each of the NANC neurotransmitter depends on an animal species and parts of gastrointestinal tract. Therefore, this experiment applied electrical field stirnulation at the circular musc]e of a guinea pigs gastric fundus to observe any relation of NANC nerve in muscu]ar relaxation. Methods: The effect of ATP, VIP, and NO on muscle re)axation was determined through the electrical field stimulation, and each neurotrans- mitters ability to regulate the muscle relaxation was compared. Results: Muscle strips were relaxed in response to electrical field stimulation that was applied. The relaxation was not inhibited by atropine or guanethidine, but was completely inhibited by tetrodotoxin. The muscle relaxation caused by NANC nerve stimulation was inhibited by L-NNA, VIP antagonist, and suramin. L-NNA was the most potent inhibitor among them. The muscle relaxation caused by electrical field stimulation was not further inhibited by VIP antagonist after the pretreatment of L-NNA, however, it was further inhibited by L-NNA following the treatment of VIP antagonist. The electrically stimulaed induced muscle relaxation was almost complete]y abolished by coadministration of L-NNA with VIP antagonist and suramin, The exogenous VIP-induced muscle relaxation was not affected by presence or absence of L-NNA. Oxyhemoglobin or methylene blue treatment inhibited the muscle relaxation induced by electrical field stimulation. Conclusions: Non-adrenergic and non-cholinergic nerve has an inhibitory neurotransmission in a guinea pigs gastric fundus circular muscle and NO is the principal inhibitory neurotransmitter of NANC nerve. NO mediated muscle relaxation is thought to be associated with increase of cGMP in muscle cells. (Korean J Gastroen- terol 1997;29:2S9 - 300)

        한영 문학 번역에서의 이름 및 지칭어 고찰

        본 연구는 노르트와 허만스의 고유명사에 관한 이론에 기반하여 한국 문학작품에서 작가의 의도가 담긴 `동기가 부여된 고유명사`가 영문으로 번역될 때, 그 의도와 의미가 제대로 전달되는지를 살펴보고 그 대안을 제시한다. 지금까지의 한영 문학번역의 실태를 보면 고유명사의 경우 음차번역이 당연시되고 있는 실정인데, 의미적 함축이 용이한 한자어로 된 한국 소설의 고유명사는 영어로 번역될 때 그 의미의 설명이 어떤 형식으로든 반드시 이뤄져야 할 것으로 본다. 따라서 본 연구는 대다수의 등장인물 이름이나 고유명사, 지칭어 등이 영어 번역문에서 의미가 손실되지 않고 제대로 전달되는 지의 여부를 몇몇 한국 현대 단편 소설 텍스트와 영어 번역 텍스트의 대조 분석을 통해 확인한다. 텍스트 분석을 통해 한자어 기반의 작명 문화가 반영된 한국 문학 작품 내 등장인물의 이름이나 별명, 지칭어가 영어 번역문에서 그 의미가 손실되거나 제대로 전달되지 않았다고 판단되는 경우, 이를 보완하면서 그 의미를 목표텍스트(Target Text: TT) 독자에게 전달할 수 있는 유용한 대안이 무엇인지 모색하는 것이 본 논문의 목적이다. Based on the proper noun-related theories of Nord (2003) and Hermans (1988), this study aims to examine whether the intended meanings of `loaded proper nouns` appearing in Korean literature are properly delivered through translation into English. Another goal of this research is proposing an alterative to adequately transfer those `loaded proper nouns` into English text. Given the translation practices of Korean literature into English so far, the transliteration strategy has been taken for granted. For Sino-Korean proper nouns facilitating meaningful implication shown in Korean literature, however; their intended meanings are considered to be necessarily explained in any way as translated into English. In this regard, this study firstly examines whether most characters` names and proper nouns of Korean literature are transferred into English version without any loss of meaning through the comparison analysis of several modern Korean short story text and their translated English text. And then in case the Korean loaded proper nouns reflecting the naming culture established on Sino-Korean words are not properly transferred or their own meanings are missed in the English version, a useful alternative enabling to deliver the intended meanings to the Target Text (TT) readers is suggested through this research.

        팔십여 회의 색소 레이저 치료는 멜라닌 세포 모반의 어느 깊이까지 영향을 미칠까?

        Total excision is the treatment of choice of congenital melanocytic nevus. However, it is often infeasible, laser therapy is being explored as a useful therapeutic approach. A 26-year-old woman presented with a solitary, skin-colored plaque surrounded by whitish patches on her right calf. She was diagnosed with a congenital melanocytic nevus and underwent 84 laser treatment sessions using a 1,064 nm Nd:YAG laser over 7 years. Histopathological examination of the whitish plaque revealed small foci of nevus cells that penetrated the tissue to a depth that was 12-fold higher than the epidermal depth (1,260 μm). No report in the literature describes histopathological findings of a congenital melanocytic nevus treated with 84 sessions of laser therapy. This case report highlights the depth of penetration of pigment laser to treat a congenital melanocytic nevus. Our findings will serve as useful clinical guidelines to perform laser treatment for congenital melanocytic nevi. (Korean J Dermatol 2021;59(6):474∼476)

        만성 특발성 변비증의 임상적 고찰

        N/A Objectives: As nonorganic, or functional factors play an important etiopathogenetic role in chronic constipation, we have reviewed functional tests on patients with chronic idiopathic constipation. Results of investigation and subsequent proper treatment are suggested. Methods: Clinical assessment, colonic transit, defecogram, anorectal manometry and pelvic floor electrophysiologic tests were performed in 30 patients with chronic idiopathic constipation, Results: 1) The ratio of male to female was 1:4, and the mean age was 48.2 years. Primary complaints were incomplete evacuation (83.3%), abdominal fullness (70.0%), abdominal pain (70.0%), rectal bleeding (43.3%), nausea (33.3%), tenesmus (33.3%) and dysuria (20.0%). Hemorrhoids and melanosis coli were present in 70.4% and 11.1% of the patients respectively at colonoscopy. The types of personality using the Minnesota Multiphasic Personality Inventory showed somatization 50.0% and psycho- physiological reaction 41.7%. 2) Colonic transit studies identified three patterns: normal transit 38.9%, colonic inertia 22.2%, pelvic outlet obstruction 16.7%, and mixed type 22.2%. The mean transit time of total colon was 56.4±5.0 hr. in colonic inertia and mixed type. 3) Defecogram revealed rectocele 77.8%, incomplete evacuation 48.1%, paradoxical contraction of puborectalis 40.7%, rectal intussusception 25.9%, parietal rectal rigidity 11.1%, and excessive pelvic floor descent 11.1%. 4) The mean minimal sensory volume, anal sphincter length, maximum resting and squeeze anal pressure was 47.9±17.5 ml, 3.9±0.9 cm, 104.3±17.9mmHg and 156.4±15.0mmHg respectively. The anorectal inhibitory reflex was intact in all patients. 5) Electrophysiologic studies suggested pudendal neuropathy in 5 patients (27.8%). 6) The patients were treated by drug therapy 83.3%, biofeedback therapy 10.0% and operation 6.7%. Conclusion: From the above results, systematic diagnostic evaluation and proper treatment for the patients with chronic idiopathic constipation in view of functional motility disorder are required and treatment can be helpful particularly when directed by a multidisciplinary team approach.

        경련성 골반상 증후군의 임상적 연구

        N/A The spastic pelvic floor syndrome is a functional disorder due to abnormal contraction in- stead of relaxation of the pelvic floor muscle during straining. This inhibits defecation and gives rise to constipation. We carried out the clinical and psychologic analysis, defecogram, electrophysiologic tests, and treatment in 17 patients with spastic pelvic floor syndrome. The mean age of patients was 48.2+10.1 years. The male to female ratio was 1:7.5. Prirnary com plaints were incomplete evacuation 82.3%, constipation 52.9%, tenesmus 41.2%, straining 41.2 %, and dysuria 29.4%. The patterns of personality using the MMPI showed somatization type in 7 cases and psychophysiologic reaction type in 4 cases. Defecogram revealed paradoxical contraction of puborectalis in 100.0% of cases,incomplete evacuation 76.5%, rectocele 58.8%, incontinence 17.6%, intussusception 5.9%, and excessive pelvic floor descent 5.9%. The ano- rectal angle in patients did not increase during straining and rernained about 90. The mean maximum resting and squeezing anal canal pressures were 94.4 + 48.4mmHg, and 136.4 + 28.8 mmHg. Seven patients(70.0%) were unable to expel a balloon. The pudendal neuropathy re- vealed in 4 patients(40.0%). Seven patients underwent a EMCr-based biofeedback therapy. Four among them(57.1%) had increased bowel frequency and improved the symptom of in- complete evacuation. In conclusion, careful history taking, physical examination and colorectal motility tests perrnit diagnosis of spastic pelvic floor syndrome. This disease is related with incoordination of pelvic floor muscle, so treatment should be aimed at restoration of normal muscle function through biofeedback.(Korean J Gastroenterol 1994; 26: 482 490)
