        유리체내 트리암시놀론 주사 후 발생한 비감염성 안내염 1예

        Purpose: We report a case of presumed noninfectious endophthalmitis following intravitreal triamcinolone acetonide injection. Methods: A 63-year-old female patient was treated with intravitreal triamcinolone acetonide for diabetic macular edema in her right eye. Two days after the injection, the patient complained of decreased visual acuity but didn`t visit the hospital until five days after the injection. Visual acuity had dropped to hand motion, and the slit lamp examination showed 3+ cells and hypopyon in the anterior chamber. In the vitreous there was a dense haze completely obscuring the view of the fundus. Results: The position of the hypopyon was gravity dependent and shifted with changes in the patient`s head position. Vitreous taps and culture were performed followed by the intravitreal injection of vancomycin and ceftazidime. The anterior chamber had cleared and hypopyon resolved completely eight days after the original injection, but the diffuse vitreous haze remained. The vitreous haze resolved slowly over four weeks and visual acuity recovered to baseline vision. Conclusions: In the presence of inflammation signs after intravitreal triamcinolone acetonide injection, the differentiation of infectious endophthalmitis from noninfectious endophthalmitis is mandatory. The pseudohypopyon was distinguishable from an infective or inflammatory hypopyon by its shifting position, which was dependent upon the patient`s head position. Close observation and differential diagnosis are needed to avoid unnecessary surgical intervention.

      • RPM System<sup>TM</sup>을 이용한 호흡 관찰의 유용성 평가

        목 적 : 정위적체부방사선치료(Stereotactic Body Radiation Therapy, SBRT)를 시행하는 폐암환자의 호흡 변화에 따른 종양 움직임의 재현성을 비교 분석하고, 치료 중 환자 호흡의 안정도를 확인하여 호흡 신호 확인 과정의 유용성을 평가하고자 한다. 대상 및 방법 : 호흡연동방사선치료(Respiratory Gated Radiation Therapy, RGRT)를 실시한 30명과 RGRT를 시행하지 않는 30명을 그룹1, 2로 정하고, 4-Dimensional Computed Tomography (4-DCT), Cone-Beam CT (CBCT), 투시 영상에서 측정한 종양의 움직임 차이를 비교하였다. 그룹2를 대상으로 치료 중 호흡의 안정도를 확인하기 위해서 Real-time Position Management (RPM) System (version 1.7.5, Varian, USA)을 이용하여 호흡 신호의 기준 구간을 설정하고 치료 중 환자의 호흡이 정해진 구간에서 벗어난 횟수를 측정하였다. 결 과 : 4DCT에서 측정한 평균 움직임과 CBCT에서 평균 움직임의 차이가 그룹1은 전 후 방향, 축 방향, 측 방향 각각 1.0 mm, 1.1 mm, 1.9 mm, 그룹2는 0.3 mm, 0.9 mm, 0.4 mm로 확인되었다. 4DCT와 투시 영상의 축 방향에 대한 평균 움직임 차이는 그룹1에서 0.6 mm, 그룹2에서는 2.3 mm였고, CBCT와 투시 영상의 축 방향에 대한 차이는 그룹1에서 1.7 mm, 그룹2에서 1.4 mm의 차이를 보였다. 그룹2에서 1회 치료(15Gy)를 시행하는 동안 호흡 기준을 벗어나지 않고 치료가 시행된 경우는 대상 환자 30명의 총 120회 중 32회였고, 가장 많이 벗어난 환자는 1회 치료 시 최대 108번 설정 기준을 벗어난 것으로 확인되었다. 결 론 : 전체 대상 환자의 종양 움직임은 각각의 영상에서 평균적으로 5 mm 미만의 차이를 보였지만 특정 환자의 경우 13 mm이상의 차이가 있었고, RGRT를 적용하지 않은 환자들의 치료 중 호흡이 불안정한 것을 확인할 수 있었다. 본 연구를 통하여 고 선량이 전달되는 폐암 환자의 SBRT 시 RGRT를 적용하지 않는 환자에 대해서도 종양 움직임을 확인하고 RPM system 을 이용한 치료 중 호흡 관찰을 실시한다면, 치료 중 계획된 선량 전달의 정확성을 높일 수 있을 것으로 사료된다. Purpose : The purpose of this study is to evaluate the variability of tumor motion and respiration pattern in lung cancer patients undergoing Stereotactic Body RadioTherapy(SBRT) by using On-Board imager (OBI) system and Real-time Position Management (RPM) System. Materials and Methods : This study population consisted of 60 lung cancer patient treated with stereotactic body radiotherapy (48 Gy / 4 fractions). Of these, 30 were treated with gating (group 1) and 30 without gating(group2): typically the patients whose tumors showed three-dimensional respiratory motion > 10 mm were selected for gating. 4-dimensional Computed Tomography (4DCT). Cone Beam CT (CBCT) and Fluoroscopy images were used to measure the tumor motion. RPM system was used to evaluate the variability of respiration pattern on SBRT for group1. Results : The mean difference of tumor motion among 4DCT, CBCT and Fluoroscopy images in the cranio-caudal direction was 2.3 mm in group 1, 2. The maximum difference was 12.5 mm in the group 1 and 8.5 mm in group 2. The number of treatment fractions that patient's respiration pattern was within Upper-Lower threshold on SBRT in group 2 was 31 fractions. A patient who exhibited the most unstable pattern exceeded 108 times in a fraction Conclusion : Although many patients in group 1 and 2 kept the reproducibility of tumor motion within 5 mm during their treatment, some patients exhibited variability of tumor motion in the CBCT and Fluoroscopy images. It was possible to improve the accuracy of dose delivery in SBRT without gating for lung cancer patient by using RPM system.

        황반박리가 동반된 열공망막박리 수술 후 선택적초시력시야계를 이용한 변형시의 분석

        Purpose: To determine the efficacy of using the preferential hyperacuity perimeter (PHP) for the analysis of metamorphopsia following macular-off rhegmatogenous retinal detachment (RRD) surgical repair. Methods: PHP examination was performed in those who complained of metamorphopsia after successful surgical reattachment of RRD. The subjective symptoms were evaluated based on a scale, ranging from 0 to 10, and were compared with the results of PHP examination. Patients age, duration of retinal detachment, pre and post-operative visual acuity, postoperative presence of subretinal fluid (SRF), surgical methods, presence of epiretinal membrane, and time from operation to PHP test were analyzed to find the correlation with the PHP results. Results: Of the 19 eyes included in this study, nine eyes were found to have a hyperacuity defect upon PHP examination. In this group, the subjective level of metamorphopsia was significantly worse (p<0.01), the age was significantly higher (p=0.02), and SRF persisted in the macula more frequently at two months postoperatively (p=0.04), as compared to those without the defect. There were no significant differences in the other factors. Conclusions: Subjectively severe metamorphopsia after surgery for RRD could be confirmed objectively using PHP. In the case of older patients and persistent submacular fluid, metamorphopsia would develop more frequently due to displacement of the photoreceptors in the process of reattachment.         비배우자간 인공수정이 권유된 남성불임환자의 의식조사

        Objectives: In determining to perform non-spousal artificial insemination by donor (AID) to an infertile married couple, infertile couple requires not only the thorough understanding of the medical procedure but also scrutinizing the effect, which it will have on the relationship of the family including the baby to be born itself. Materials and methods: 148 cases with non-curable male infertility were enrolled in this inquiry survey. The donor insemination questionnaire consists of 18-items which are assessing subjects' clinical properties, the background for AID practice, psychological traits, and long term influence. Results: Of the survey, 49 cases were returned (33.1%) and 10 cases (20.4%) of these gave birth after AID practice. The mean age of husbands and wives of the 49 cases were $34.6{\pm}4.2$ and $32.1{\pm}3.0$ yers old, respectively and the duration of marriage was 5 years and 4 months. In about half of the cases, AID was first suggested by husband and the decision was made by only the couple. The major reason for the operation was to form a complete family. In the item of the psychological effects, two-third of the couples felt anxiety related to the procedure which are mostly about the possible congenital or acquired deformity of baby. The AID was positively suggested in overall by all of recipients. After giving birth to a child, most couples felt positive about their decision. As a child grows up, about half of the couples felt the child as their own and expected not to tell of the AID. In overall, about 50% of couples presented satisfaction with the procedure. Conclusions: As the above results, various psychological impacts including anxiety about a child-to-be-born were accompanied to those who were recommended of AID. To overcome these problems, sufficient medical information and consultation about the course of selecting the donor and the whole procedures of AID should be provided beforehand.

        Backpropagation을 이용한 악보인식

        본 논문에서는 신경회로망 알고리즘 중 하나인 backpropagation network을 이용한 악보인식 기법과 그에 필요한 악보 영상에 대한 전처리 기법을 제안한다. 전처리과정으로 이진화, 기울기 보정, 오선제거 등의 과정을 수행하여 인식에 필요한 음악 기호와 음표를 분리한다. 분리된 음악 기호와 음표들은 backpropagation 알고리즘을 사용하여 구성된 음표 인식 신경망과 비음표 인식 신경망을 통해 각각 음표와 비음표 인식과정을 거친다. 다양한 복잡도를 가진 악보를 대상으로 한 실험 및 분석 결과를 통해 제안한 악보 인식 기법의 정확도를 기술하였다. This paper presents techniques to recognize music using back propagation network one of the neural network algorithms, and to preprocess technique for music mage. Music symbols and music notes are segmented by preprocessing such as binarization, slope correction, staff line removing, etc. Segmented music symbols and music notes are recognized by music note recognizing network and non-music note recognizing network. We proved correctness of proposed music recognition algorithm though experiments and analysis with various kind of musics.

        전문조작원 유무에 따른 사상체질 음성진단의 신뢰성 분석

        Objectives This study was aimed to analyze the reliability of vocal features and probabilities for being in Sasang constitutional types calculated from Sasang constitutional voice diagnosis system according to operator presence. Methods We acquired 96 voice recordings from one male and one female for 4 days. For the first 2 days, the subjects recorded their voice by themselves. For the last 2 days, they recorded according to the instruction of an operator following the standard operating procedure. We analyze the standard deviations of vocal features, probabilities for being three constitutional types, Tae-Eum (TE), So-Yang (SY), and So-Eum (SE) Results In the case of the female, coefficients of variations of the voice variables and the probabilities for being each constitutional type were all within 20%. In the case of the male, coefficients of variations were all within 20% except one variable. Even if there was no instruction from the operator, standard deviations of the probability did not increase for both genders. When recorded without the operator, for male, the probability for being SE decreased by 3.2%. For female, the probability for being TE increased by 5.438%, and that of SE decreased by 3.057%, and that of SY decreased by 2.394%. Conclusions When recorded without operators, for men, there was a significant difference in the probability for being SE. And for women, there were significant differences in the probabilities for all constitutional types.

        Multiple Active Appearance Model을 이용한 얼굴 특징 추출 기법

        영상에서 얼굴 및 얼굴 특징을 추출하기 위한 기법으로 active appearance model(AAM)이 있다. 본 논문에서는 두 개의 AAM을 이용하여 얼굴 특징을 추출하는 multiple active appearance model(MAAM) 기법을 제안한다. 두 개의 AAM은 학습 데이터에 대한 파라미터를 조절하여 상반되는 장단점을 가지도록 생성하고, 서로의 단점을 보완할 수 있도록 한다. 제안된 방법의 성능을 평가하기 위해 100장의 영상에 대해서 얼굴 특징추출 실험을 하였다. 실험 결과 기존의 AAM 하나만을 사용하는 기법에 비해 적은 횟수의 피팅만으로도 정확도 높은 결과를 얻을 수 있었다. Active Appearance Model(AAM) is one of the facial feature extraction techniques. In this paper, we propose the Multiple Active Appearance Model(MAAM). Proposed method uses two AAMs. Each AAM trains using different training parameters. It causes that each AAM has different strong points. One AAM complements the weak points in the other AAM. We performed the facial feature extraction on the 100 images to verify the performance of MAAM. Experiment results show that MAAM gives more accurate results than AAM with less fitting iteration.

        디지털 영상 객체의 불투명도 추정을 위한 SOM Matting

        본 논문은 인공신경망을 이용한 새로운 매팅 기법을 제안한다. 매팅이란 영상에서 객체의 불투명도를 추정하는 기술이다. 매팅 기법을 이용하면 객체를 자연스럽게 추출할 수 있다. 먼저 trimap을 이용하여 영상을 배경 영역, 전경 영역, 미지 영역으로 구분한다. 배경 영역과 전경 영역의 정보를 이용하여 미지 영역 화소의 불투명도를 추정한다. 제안하는 알고리즘은 배경 영역과 전경 영역의 정보를 SOM을 이용하여 학습하고 그 결과를 이용하여 미지 영역의 각 화소의 불투명도를 추정한다. 본 논문에서는 배경 영역과 전경 영역의 정보를 학습하는 방법에 따라서 전역적 SOM matting과 지역적 SOM matting으로 구별한다. 제안하는 알고리즘의 성능을 평가하기 위하여 영상에 적용해보았다. 이를 통해 제안하는 알고리즘이 객체를 영상에서 분리 가능함을 확인 할 수 있다. This paper presents new matting techniques. The matting is an alpha estimation technique of object in an image. We can extract the object in an image naturally using the matting technique. The proposed algorithms begin by segmenting an image into three regions: definitely foreground, definitely background, and unknown. Then we estimate foreground, background, and alpha for all pixels in the unknown region. The proposed algorithms learn the definitely foreground and definitely background using self-organizing map(SOM), and estimate an alpha value of each pixel in the unknown region using SOM learning result. SOM matting is distinguished between global SOM matting and local SOM matting by learning method. Experiment results show the proposed algorithms can extract the object in an image.

        도로 시공 자동화를 위한 스마트 모터 그레이더의 구성 기술 소개 및 적용에 관한 연구

        Some problems, such as aging workers, a decreased population due to a low birth rate, and shortage of skilled workers, are rising in construction sites. Therefore research for smart construction technology that can be improved for productivity, safety, and quality has been recently developed with government support by replacing traditional construction technology with advanced digital technology. In particular, the motor grader that mainly performs road surface flattening is a construction machine that requires the application of automation technology for repetitive construction. It is predicted that the construction period will be shortened if the construction automation technology such as trajectory tracking, automation work, and remote control technology is applied. In this study, we introduce the hardware and software architecture of the smart motor grader to apply unmanned and automation technology and then analyze the traditional earthwork method of the motor grader. We suggested the application plans for the path pattern and blade control method of the smart motor grader based on this. In addition, we verified the performance of waypoint-based path-following depending on scenarios and the blade control's performance through tests.

        급속가열용 플라스틱 사출금형을 위한 고기능성 표면처리

        Plastic injection molds used for rapid heating and cooling must minimize surface damage due to friction and maintain excellent thermal and low electrical conductivity. Accordingly, various surface treatments are being applied. The properties of Al<sub>2</sub>O<sub>3</sub> coating and DLC coating were compared to find the optimal surface treatment method. Al<sub>2</sub>O<sub>3</sub> coating was deposited by thermal spray method. DLC films were deposited by sputtering process in room temperature and high temperature PECVD (Plasma enhanced chemical vapor deposition) process in 723 K temperature. For the evaluation of physical properties, the electrical and thermal conductivity including surface hardness, adhesion and wear resistance were analyzed. The electrical resistance of the all coated samples was showed insulation properties of 24 MΩ/sq or more. Especially, the friction coefficient of high temp. DLC coating was the lowest at 0.134.
