N/A Background:Full-thickness skin graft(FTSG) for deep skin defects often results in poor contour cosmetically due to insufficient thickness and does not take on defects with exposed bone or cartilage. These lesions may require more difficult methods such as skin flaps. Objective:To describe the usefulness of free skin graft(FSG) with some fat and vascular network on the underside, and therefore thicker than FTSG, for repairing deep cutaneous defects on the nose. Methods:Five patients, four with skin cancers and one with a combined nevus, were treated with FSG harvested from the postauricular area as the donor site. Results:These grafts provided excellent cosmetic and functional results in all patients without complications. Conclusion:FSG with subcutaneous vascular network is useful in reconstruction of deep skin defects instead of complex, multistaged skin flaps. Moreover, its cosmetic outcome is superior to FTSG. A study was performed to investigate a relationship between the status of T lymphocyte subsets in the peripheral blood and the pathogenesis of erythema nodosum leprosum (ENL) reactions. T lymphocyte subsets, B lymphocytes and serum immnnoglobulin levels were explored in 40 leprosy patients (10 tuberculoid (TT), 17 lepromatous (LL), and 13 lepromatous with ENL). In patients with LL, suppressor cells increased and helper cell diminished, resulting in a decrease in the helper/suppressor (Th/Ts) ratio to 0.86 + 0.20, as compared with controls (1. 77 + 0.41). But in ENL patients, there was a significant diminution of suppressor cells and an increase of helper cells, resulting in an increase in Th/Ts ratio to 2.08 + 0.30. Four weeks after treatment of ENL, Th/Ts ratio decreased to l.18+ 0.31 again. The mean percentages of B cells and serum immunoglobulin levels in with LL. and ENL increased significantly compared with controls. Only IgG increased in patients with ENL compared with patients with LL. The results in patiients with TT did not differ significantly from those of controls. The findings suggest that cell-mediated immune responses, imbalance in T lymphocyte subsets, may play a role in the pathogenesis of ENL, either directly or by inducing an antibody critical to the formation of the immune complex

Symposium 2-3-1 (SYP 2-3-1) : Management of nasal defects: Single-stage reconstruction

Critical analysis of a nasal defect should precede any repair of a wound. The first step in nasal reconstruction is thoughtful evaluation of the defect. Primary goal is to determine viable reconstructive options and to choose the optimal modality among them. Nasal tissue is comprised of cover(skin, subcutaneous tissue, muscle), structure(lateral cartilages, septum, nasal bones, tip-supporting system), and inner lining(vestibular skin, nasal mucosa). Although most defects involve the skin and subcutaneous tissue, the reconstruction may also handle cartilage, mucosa, or muscle. The most obvious aspects of a nasal defect are its location and size. The nose is divided into nine aesthetic units and has many junctions. The principle of aesthetic unit repair is that, whenever possible, incision lines should be placed at the boundaries of units or within the nasal sulci and grooves. If a defect extends into more than one unit, separate repair of each unit may be appropriate. There are no simple guidelines to nasal reconstruction, however, one reasonable approach to the repair is to consider more conservative options first. The reconstructive ladder is a helpful paradigm for planning nasal reconstruction. This concept organizes reconstructive options from simple to more complex and invasive. The bottom rung of the ladder is second intention healing. The middle rungs represent skin grafting and local skin flaps. The highest rung is free-tissue transfer , which is beyond the scope of the current issue. Herein, management of nasal defects, especially single-stage reconstruction will be discussed in terms of reconstructive ladder and regional approach. Background: The use of autologous fat with microlipoinjection is one of fat transfer and has recently been popular with the advent of tumescent anesthesia. Objective: This study was performed to evaluate the effect of microlipoinjection on the improvement of atrophic, depressive hand changes present as sequelae in forty-one patients with Hansen`s disease. Methods: Abdominal fat was obtained under tumescent anesthesia. After washing with saline, the fat was transferred to 1 cc syringes with an 18 G needle for injection. Microlipoinjection was repeated at an interval of 3 or 4 months for 1 year. Any unused fat was frozen for the following session. Results: Thirty-six of the 41 patients (90%) showed a clinical improvement at the 3-month follow-up after the last session, and no serious complications were observed. Conclusion: Microlipoinjection, using autologous fat, has proven to be a safe and effective procedure for the correction of tissue defects and has shown no significant complications. Etretinate ( Ro 10 - 9359 ) 로 치유된 소아에서 발생한 범발성 농포성 건선

The retinoid Ro 1Q-9359 (Etretinate) is a synthetic derivative of vitamin A acid. It is potent and specific dermatotherapeutic drug used in various keratinizing disorders. Etretinate is today one of the best single oral treatment of severe psoriasis, particuhrly pustular psoriasis and psoriatic erythroderma. We present a case of generalized pustular psoriasis in 12-year-old male which was treated successfully with etretinate. The laboratory examinations revealed no aberrations which could be attributed to the therapy.

나병의 미모소실증에 대한 단모 미세이식술

N/A Background:Madarosis or loss of eyebrows is one of the complications in leprosy, which takes expressions out of the face and is considered as a stigma of leprosy in society. So the patients with leprosy are usually eager to have the situation corrected through various methods such as tatoo or surgery including scalp flap or hair transplantation. Objective:To describe the use of single hair transplantation for the correction of madarosis in patients with leprosy. Methods:An elliptical donor strip was removed with a No.10 blade from the occiput. It was sectioned into small slices and subsequently into single hair micrografts. Then these micrografts were inserted into preformed slits of eyebrows using hair implantation device. Results:Excellent cosmetic results and high patient satisfaction were seen in almost all patients. Reoperations were done in 3 patients due to partial sparsity or unnatural shape of the eyebrows. There were no complications except for pruritic papules which developed from the transplanted areas in 2 patients with eyebrow tatoos. Conclusions:Single hair transplantation proved to be useful in the reconstruction of eyebrows in patients with leprosy and provided superior cosmesis to other grafts despite a technically difficult and time-consuming procedure. Eruptive Vellus Hair Cysts 2예

We present two oases of Eruptive Vellus Hair Cysts(EVHC) in 30- year-old and 27- year old women. They had 6 to 10 discrete, asymptomatic, brown or reddish brown papules on the neck, axilla, chest and extremities. The clinical impressions were epidermal cyst and steatocystoma multiplex espectively, but histopathological exarnination revealed smal1 cysts in middermis containing numerous vellus hairs cut transversely or obliquely and surrounding granulomatous foreign body reaction typical to EVHC. Transepithelial elimination was not found.

An immunohistochemical and electron microscopic study of an atypical fibroxanthoma of the skin (AFX) was reported. The lesion developed on the left scapulararea in a 70-year-old man. The result using antibody to S-100 protein was negative and the tumor cells observed with electron microscope were undifferentiated, fibroblast-like, histiocyte-like and multinucleated giant cells. But Langerhans-like cells or Langerhans were not found.
