        약물로 유도된 망막변성 흰 쥐에서 발생한 망막전위도의 초기 가역적 변화

        Purpose: To evaluate the early ERG (electroretinogram) changes in N-methyl-N-nitrosourea (MNU)-induced retinal degeneration in rats. Methods: Thirty-six 6-week-old male rats were injected intraperitoneally with 60mg/kg MNU and divided into 6 groups. Histology and ERG were recorded for the rats of each group before treatment and at 3, 6, 12, 18, and 24 hours after MNU injection. Promptly after the ERG recording, rats were sacrificed and the eyeballs prepared for histologic sectioning. The Tdt-mediated dUTP-digoxigenin nick end labeling (TUNEL) method was used to detect photoreceptor cell death. Results: The first decreases of ERG responses were noticed maximally at 3 hours after the treatment. Thereafter, the amplitude of the responses was partially recovered at 12 hours post-treatment. The second decrease of ERG amplitudes was observed in the 18-hour recordings, and those changes progressed to 24 hours after the treatment. In the histologic findings, TUNEL (+) cells in the Outer Nuclear Layer (ONL) were not detected at 3 hours after MNU injection, but were initially noticed at 6 hours post-injection. Conclusions: The first decreases of ERG amplitudes proceeded the appearance of TUNEL (+) cells in ONL, and these electrophysiological changes seemed to not be related to photoreceptor cell death. We propose that electrophysiological changes observed might be related to the MNU-induced activity enhancement of guanylate cyclase in the phototransduction pathway. We also show that photoreceptor cell death in the MNU-induced retinal degeneration model occurs at 6 hours after the treatment, which is earlier than the results of previous reports.

        당뇨황반부종에서 망막두께분석기로 측정한 망막두께와 다국소망막전위도의 반응의 연관성

        Purpose: To assess the correlation between retinal thickness determined with a retinal thickness analyzer (RTA) and the responses of multifocal electroretinogram (mfERG) in patients with diabetic macular edema. Methods: RTA and mfERG were performed in patients with diabetic macular edema (20eyes, 10 patients) and normal subjects (10eyes, 10 patients). The measured retina of RTA and mfERG overlapped based on the anatomical landmarks of the fundus. The center of macula was defined as “Area 1”, and the central 5 degree retina including the center of macula was defined as “Area 2”. The correlations between retinal thickness and the local responses in each area were assessed. Results: The mean retinal thickness of patients with diabetic macular edema was significantly greater than that of normal subject, and the results of mfERG typically represented decreased amplitudes and delayed latencies. In the matching of retinal thickness and local responses in Area 2, amplitudes of N1 & P1 decreased and latency of P1 delayed as the retinal thickness increased. The correlation was prominent in patients with diabetic macular edema. Conclusions: In the matching of RTA and mfERG of patients with diabetic macular edema, retinal thickness and local responses were significantly correlated.

        오구치병에서 관찰된 특징적 빛간섭단층촬영 소견 1예

        Purpose: To report specific spectral domain OCT findings of Oguchi disease diagnosed with fundoscopic examination and electrophysiological study. Case summary: A 14-year-old patient visited our clinic with a complaint of night blindness for ten years. Fundoscopic examination showed a golden-yellow fundus reflex. After three hours of dark adaptation, the fundus color returned to normal (Mizuo-Nakamura phenomenon). In full-field ERG, rod b-wave was not detectable. The a-wave amplitude in maximal combined response increased after three hours of dark adaptation, although the b-wave amplitude was similar to the amplitude before dark adaptation, demonstrating a negative waveform. In the spectral domain OCT images of the perifoveal area, no gap between the retinal pigment epithelium and the inner segment/outer segment (IS/OS) junction was detected before prolonged dark adaptation, and a highly reflective band was shown. However, the gap appeared after three hours of dark adaptation, and two highly reflective bands were detected in the OCT images. Conclusions: The characteristic OCT finding in addition to the specific fundoscopic finding and full-field ERG results may be useful to diagnose Oguchi disease.         산소로 유도된 망막증 모델에서 인간재조합 적혈구생성인자가 망막혈관에 미치는 영향

        Purpose: To evaluate the effect of recombinant human erythropoietin (rhEPO) on the retinal vessels in a rat model of oxygen-induced retinopathy (OIR). Methods: Twenty-six Sprague-Dawley rat siblings were exposed to seven cycles each of hyperoxia (80% O2, 23.5 hours) and room air recovery (0.5 hours), followed by exposure to room air for five days. After each of the test group and control group was divided into two groups of rhEPO injected group and normal saline injected group, 400 u/kg of rhEPO or normal saline were intraperitoneally injected, respectively on the third, fifth, and eighth days of the experiment. Retinal neovascularization and capillary densities were compared between the rats on the eighth and fourteenth day after injection. The extent of VEGF164, and IGF-1 mRNA expression was measured in the rats on the 14th day post-injection. Results: Neovascularization and capillary density was the most increased in the rhEPO injected test group, but the avascular area was widely distributed in the vicinity of the retina. The degree of VEGF164 mRNA expression was highest in the test group that had been injected with normal saline. We found no difference in IGF-1 mRNA levels between the groups. Conclusions: We can infer that rhEPO can be a potent factor for inducing retinal neovascularization.

        증식당뇨망막병증에서 수정체초음파유화술 및 인공수정체삽입술을 병행한 유리체절제술의 임상분석

        Purpose: To describe the results and complications of combined pars plana vitrectomy and phacoemulsification with intraocular lens implantation in patients with proliferative diabetic retinopathy, and to analysis the factors to be associated with visual outcome. Methods: Retrospective study was performed on 70 eyes of 58 patients who had undergone combined pars plana vitrecotmy and phacoemulsification with intraocular lens implantation because of proliferative diabetic retinopathy. Preoperative, intraoperative, and postoperative complicated factors to be associated with visual outcome were analyzed. Results: The changes of visual acuity were classified into “improved” 57eyes (81%), “stationary” 7eyes (10%), and “decreased” 6eyes (9%). Posterior capsule rupture during operation was occurred in 2 eyes (2.9%). After cataract, macular edema and neovascular glaucoma were developed in order of incidence of postoperative complications. The factors to be associated with visual outcome were diabetic nephropathy, posterior capsule rupture, macular edema, neovascular glaucoma, and retinal detachment. (P<0.05) Conclusions: Combined pars plana vitrectomy and phacoemulsification with intraocular lens implantation may be relatively safe for the patients with proliferative diabetic retinopathy.

        선천비진행야맹증의 분류와 다국소망막전위도 소견

        Purpose: To report on the classification of congenital stationary night blindness (CSNB) and multifocal ERG findings. Methods: We studied three patients (case 1-3) with night blindness but normal fundus and one patient (case 4) with decreased vision after trauma with respect to the patients` medical history, familial history, refraction, slit lamp examination, fundus examination, FAG, ERG, and multifocal ERG. Results: The ERG in case 1 showed a decreased rod response, and this patient was diagnosed with Riggs-type CSNB. The typical “negative ERG” was found in all three cases of ERG (except case 1), and these cases were diagnosed with Schubert-Bornschein-type CSNB. Based on Miyake`s classification, case 2 was incomplete, and cases 3 and 4 were complete. The mfERG of the cases with complete Schubert-Bornschein type had delayed implicit times of the first-order kernel and decreased amplitudes of the second-order kernel. The delayed implicit times and the decreased amplitudes of the first- and second-order kernels were found in the cases of Riggs- type and incomplete Schubert-Bornschein-type.

        망막중심정맥폐쇄에서 유리체강내 베바시주맙 주사 전 후에 망막전위도를 통한 망막기능의 평가

        Purpose: To investigate the changes of electroretinogram (ERG) at baseline and during the first 3 injections of intravitreal bevacizumab in central retinal vein occlusion (CRVO) patients. Methods: Thirteen eyes of 13 CRVO patients who received 3 injections of intravitreal bevacizumab at 6-week intervals and who could be examined for visual acuity, optical coherence tomography and ERG at 4 weeks after each injection were included in the present study. In addition, the ERG results of the unaffected fellow eyes group at 4 weeks after each injection were compared with the CRVO eyes group. Results: Amplitudes of rod b wave, a and b waves of maximal combined response and cone response, oscillatory potentials and 30-Hz flicker significantly decreased in the CRVO eyes group than the fellow eyes group at 4 weeks after each injection (<em>p</em> < 0.001). After injection, amplitudes of all the above-mentioned ERG parameters gradually increased during the 1st and the 2nd injection (<em>p</em> < 0.001). However, there was no significant difference at 4 weeks after the 2nd and 3rd injections (<em>p</em> > 0.05). Conclusions: After intravitreal bevacizumab injection, macular functions significantly improved and amplitudes of all ERG parameters gradually increased in CRVO patients.         약물 유도 망막변성 생쥐 모델에서 안구내 면역세포에서의 F4/80와 nestin 발현 양상

        Purpose: To evaluate the expression patterns of F4/80 and nestin in the ciliary body and the optic nerve following N-methyl- N-nitrosourea (NMU)-induced retinal degeneration in adult mice. Methods: After intraperitoneal injection of MNU (60 mg/kg) in adult mice, the eyes were enucleated at 2, 4, 7 and 30 days. Hematoxylin and eosin (H&E) stain, terminal deoxyribonucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) stain and immunohistochemical stains of F/80 and nestin were performed. Results: After MNU treatment, the photoreceptors were destroyed by cell apoptosis. According to immunohistochemistry, F4/80 and nestin were not co-expressed in the control group, but F4/80 was expressed within the ciliary body and optic nerve in the MNU-treated group; the expression of nestin also increased. In the outer nuclear layer, F4/80 and nestin co-expressing cells were observed. Conclusions: In response to retinal damage, the F4/80 and nestin co-expressing cells migrated to the retina from the ciliary body and optic nerve and were activated.

        보크트-고야나기-하라다 증후군 환자 1예에서 치유 경과에 따른 다국소망막전위도 변화

        Purpose: To assess the changes of retinal function together with recovery progress in a patient with Vogt-Koyanaki-Harada syndrome using a multifocal electroretinogram (mfERG). Methods: The retinal changes of a 32-year-old woman diagnosed with Vogt-Koyanaki-Harada syndrome have been investigated and compared for different times such as before treatment and 3 and 6 months after treatment using a fluorescein angiogram, electroretinogram (ERG) and mfERG. Results: As time passed, the patient`s visual acuity was recovered, the serous retinal detachment in the posterior pole was restored, and the phenomenon of late subretinal leakage disappeared on fluorescein angiogram, while aggregation and atrophy of the retinal pigment epithelium were detected. The early-stage responses of the retina decreased on ERG; however, it has gradually recovered and was determined to be normal after 6 months. Responses measured on the mfERG have increased during the 6 months after surgery compared to the pre-treatment, but there was still a partial lack of response. Conclusions: In this case, the abnormal condition of retinal function existed continuously on the mfERG, even when the serous retinal detachment in the posterior pole was recovered. In this regard, a long-term systematic investigation and subsequent mfERG treatment have been thought to be necessary for patients with this disease.
