        진동 자극 유무에 따른 호흡 저항 훈련 시 폐 기능과 호흡근의 즉각적인 차이가 있을까?

        Purpose: The purpose of this study was to investigate the effect of whole body vibration combined breathing resistance on lung capacity and respiratory muscle and to suggest a mediation method for improvement of respiratory function and lung function in the future. Methods: This study was a preliminary study design of two groups of 54 healthy young adults who were randomly assigned to an experimental group (n=27) with core exercise combined with respiratory resistance and whole body vibration and a control group with respiratory resistance and core exercise (n=27). All interventions consisted of 6 core exercises every 40 seconds and rest for 20 seconds. To compare the effects of intervention, we measured spirometry and respiratory muscle strength. Results: Both the experimental group and the control group showed a significant increase in Forced vital capacity (FVC) and Maximum voluntary ventilation (MVV) (p<.05). However, FEV1 and FEV1% were significantly increase only in the experimental group (p<.05). FVC, FEV1%, Maximum Inspiratory Pressure (MIP), Maximum Expiratory Pressure (MEP) showed more significant increase in the experimental group than the control group. Conclusion: These findings indicate that whole-body vibration combined breathing resistance is an effective intervention for people, with FVC, FEV1%, MIP, MEP increase.

        퉁퉁마디를 가해하는 퉁퉁마디뿔나방 Scrobipalpa salinella (Zeller) (나비목, 뿔나방과)의 재발견 및 피해

        전남 신안군의 퉁퉁마디 재배지에서 대 발생하여 피해를 초래한 해충이 뿔나방과의 Scrobipalpa salinella (Zeller)로 확인되었다. 본 종은 Park & Ponomarenko (2006)에 의해 국내에서 처음 보고된 바 있다. 그러나 동정이 어려운 암컷 한 개체를 대상으로 하였으며, 염생식물이 없는 산간지역에서 발견된 점 등으로 분포에 대한 의문점이 상존해 왔었으나 금번 조사결과로 국내 분포가 재 확인되었다. 종의 분류동정을 위한 성충과 생식기의 사진 제공과 함께 유충의 발생 및 피해에 대해 조사된 내용을 기술하였다. In a cultivated field of Salicornia europae in the Jeonam Province, Korea, Scrobipalpa salinella (Lepidoptera, Gelechiidae) which feeds and leads to a serious damage was found. The discovery of this species is to reconfirm its distribution in Korea because the first report by Park and Ponomarenko (2006) was based on a single female and it was collected in a montane area far from seaside where its host plants, halophytes, are planted. For identification, photos of imago and genitalia are given, and the damage by larvae on Salicornia europae are noted.

        Al고압주조공법에서 사출슬리브 온도 조절을 통한 Chill layer의 최적 제어

        In this study, the effect of chill layers occurred in shot sleeve on the molten metal filling was analyzed through computer simulation. The behavior of chill layers with temperature variation of shot sleeve set from 200 to $280^{\circ}C$ was also investigated. The simulation results showed the chill layers set in the in-gates during the injection process change the main filling direction and cause turbulent flow pattern, resulting in porosities inside the castings. The amount of chill layers with the increasing temperature of shot sleeve was considerably reduced. Particularly, at the setting temperature of $280^{\circ}C$ by heat control unit, the biggest reduction in chill layers, excellent trimmed surface and the highest density were achieved, suggesting that as the optimal sleeve condition in aluminum high pressure diecasting, especially for highly complex parts like valve body.

        국부가압 다이캐스팅 공정에서 3차원 유동 및 응고해석을 통한 자동차 변속기 Gear Housing의 주조방안 설계 최적화

        In the partial squeeze casting process, the filling behavior of liquid metal and solidification pattern in thick area have significant influence on the quality of casting products and die life. For the optimal casting design of automobile transmission gear housing, various analyses were performed in this study by using computer simulation code, MAGMAsoft and the simulation results were compared and analyzed with experimental results. By air pressure criteria, internal porosities caused by air entrap during the mold filling were predicted and reduced remarkably by modification of gating system. Also, optimal squeeze-time lag to apply partial squeeze pin in thick area was calculated and the castings was free from shrinkage defects with the result of solidification analysis. Consequently, casting design for automobile transmission gear housing was optimized and approved by Computer Tomography.

        아동기에 주의력결핍 과잉행동증상을 경험한 성인의 정신병리와 기질 성격 요인

        Objectives:The objective of this study is to find out the differences in psychopathology, temperament, and character factors between young adults with and without childhood attention-deficit hyperactivity disorder (ADHD) symptoms. Methods:The subjects consisted of 314 university students (male=72, female=242) in Gongju. They were divided into childhood ADHD group (N=54) and normal group (N=260) to the Wender-Utah Rating Scale. Participants were assessed using Korean Adult ADHD Scale, Beck Depression Inventory-II, Beck Anxiety Inventory, Kimberly Young’s Internet Addiction Test, Barratt Impulsiveness Scale, Paranoia Scale, Korean-Schizotypal Ambivalence Scale, Lubben Social Network Scale, and Temperament and Character Inventory-Revised. Descriptive statistics, t-test, chi-square, spearman correlation, and stepwise multiple regression were applied to analyze the data. Results:Participants with childhood ADHD symptoms had high level of adult ADHD symptoms (p<.001), impulsiveness(p=.001), depression (p<.001), anxiety (p<.001), internet addiction (p<.001), paranoia tendency (p<.001) and low level of selfesteem (p<.001) compared to normal group. Self-directedness (p<.001) was lower, Harm avoidance (p=.001) and Self-transcendence (p=.029) were higher in the childhood ADHD group. In correlation and stepwise multiple regression, childhood ADHD symptoms were significantly associated with cooperativeness (r=-0.515 and β=-0.547, p<.001 respectively). Conclusion:This result showed that young adults with childhood ADHD symptoms had various forms of psychopathology and childhood ADHD symptoms were influenced by character such as cooperativeness. Therefore, more thorough evaluation regarding childhood ADHD symptoms is needed.

      • 견관절 불안정성의 견갑하근 및 관절 낭 수평 분할을 이용한 하방 관절 낭 이동술

        목적 : 견관절 불안정성의 관절경적 수술로 재발의 가능성이 높은 환자에 대해견갑하근 및 관절낭 수평 분할을 이용한 하방관절낭이동술을시행하고이에대한결과를분석하고자하였다. 대상및방법 : 1994년부터 1999년까지수술을시행한환자중1년이상추시(평균: 2년, 범위: 1$\~$3년)가가능한 15례를대상으로하였다. 남자가13례여자가2례이었으며평균연령은27 이었다. 질환은다방향성불안정성4례, 수의성불안정성3례, 골성Bankart 병변이있는전방불안정성2례, 접촉성운동선수의전방불안정성6례였다. 13례는관혈적봉합술과하방관절낭이동술을 동시에 시행하였고, Bankart 병변이없던2례는하방관절낭이동술만 시행하였다. 관절낭중첩 정도는평균 19 mm (범위: 10$\~$5 mm)였다. 결과 : 14례에서우수혹은양호의결과를보였으며술후1례에서아탈구되었고, 1례에서전방염려검사에양성소견을보였다. 술후전방거상과팔을체간에붙인외회전, 90$^{\circ}$ 외전에서의외회전의운동범위는술전과통계학적인차이가없었다. 결론 : 견갑하근및관절낭수평분할을이용한하방관절낭이동술은관절운동범위의감소없이과잉관절낭과Bankart 병변을치료할수있어관절경적치료로재발의가능성높은환자에서권장될치료법으로사료되었다. Purpose: The aim of this retrospective study were to report the short-term results of inferior capsular shift procedure using splitting subscapularis and capsule for the patients who had shoulder instability and were apt to recur after arthroscopic stabilization procedure. Materials & Methods : Fifteen cases of instability of the shoulder were included with an average follow-up of 2 years (range: 1$\~$3 years). There were 13 men and 2 women with an average age of 27years. Multidirectional instability was found in 4 cases, voluntary instability in 3 cases, bony Bankarte lesion in 2 cases and 6 cases were contact sportmen. Thirteen shoulders underwent the inferior capsular shifts and Bankart repairs and 2 shoulders without Bankart lesion underwent the inferior capsular shift only. Average 19 mm of shift (range: 10$\~$25 mm) was done. Results : Fourteen patients showed good and excellent results with one subluxation and one positive apprehension test. Postoperative ranges of motions did not change in forward elevation, external rotation at side and external rotation at 90$^{\circ}$ abduction (p>0.05). Conclusion : Inferior capsular shift procedure using splitting subscapularis and capsule can be helpful in shoulder instability patients who were high-risk group of recurrence with arthroscopic procedure.

        음성구간 검출기의 실시간 적응화를 위한 음성 특징벡터의 차원 축소 방법

        본 논문에서는 다양한 잡음환경에서의 실시간 적응화 기법을 적용하기 위한 선결 과제로 다차원 음성 특정 벡터를 저차원으로 축소하는 방법을 제안한다. 제안된 방법은 특징 벡터를 확률 우도 값으로 매핑시켜 비선형적으로 축소하는 방법으로 음성 / 비음성의 분류는 우도비 검증 (Likelihood Ratio Test; LRT) 을 이용하여 분류하였다. 실험 결과 고차원 특징 벡터를 이용하여 분류한 결과와 대등하게 분류됨을 확인할 수 있었다. 그리고, 제안된 방법에 의해 검출된 음성 데이터를 이용한 음성인식 실험에서도 10차 MFCC(Mel-Frequency Cepstral Coefficient)를 사용하여 분류한 경우와 대등한 인식률을 보여주었다. In this paper, we propose the dimension reduction method of multi-dimension speech feature vector for real-time adaptation procedure in various noisy environments. This method which reduces dimensions non-linearly to map the likelihood of speech feature vector and noise feature vector. The LRT(Likelihood Ratio Test) is used for classifying speech and non-speech. The results of implementation are similar to multi-dimensional speech feature vector. The results of speech recognition implementation of detected speech data are also similar to multi-dimensional(10-order dimensional MFCC(Mel-Frequency Cepstral Coefficient)) speech feature vector.

      • 상완골 근위부 골절의 수술적 요법

        근위 상완골 골절은 골절의 형태에 따라 크게 관절편 또는 해부학적 경부, 대 결절, 소 결절, 상완골 간부 또는 외과적 경부의 4개의 골절편으로 나눌 수 있다. 현재 널리 사용되고 있는 Neer의 근위 상완골 골절의 분류는 골절선에 의해 골절편을 나누는 분류가 아니며, 1 cm이상전이나 45$^{\circ}$ 이상의 각형성 이 있는 경 우를 전 이 골절편으로 생각하였다. 골절에 대한 관혈적 정 복 및 내고정술의 일차적 적응증은 골다공증이 없는 젊은 환자에서 발생된 튼튼한 내고정물을 시행할 수 있는 삼분 골절로 만족할 만한 결과를 위해 술후 오랜 기간동안 시행할 재활치료에 잘견딜수 있는 활동력 이 좋은 환자여야 한다. 수술적 치료의 절대적 적응증은 개방성 골절 ,혈관이 나 신경 손상이 동반될 때 , 정복이 불가능한 골절 탈구등이다. 반대로 환자가 골다공증이 심하거나, 근위 상완 골절편의 분쇄정도가 심하며 , 튼튼한 내고정을 시행할 가능성 이 희박한 나이가 많은 경 우에는 관혈적 정복 및 내고정술보다 일차적 인공삽입물을 이 용한 관절성형 술을 시행한 후 조기 재활 치료를 시행하는 것을 고려할 수 있다. 상완골 근위부 골절의 수술적 요법에는 다양한 수술 기법과 이에 따른 여러 합병증이 발생할 수 있으며 지속적 이고 체계적 인 재활 치 료가 꼭 필요하다. 여러 수술 기 법 중 관혈적 정복 및 장력 대 강선 기법을 시행할 때 만족할 만한 결과를 얻 을 수 있다. 이 방법 은 수술적 기법이 어렵지 않고, 골에 대한 고정과 함께 회전근 개의 건부착 부위에 대한 봉합을 추가할 수 있으나 역시 여러 가지 합병증이 있으므로 골절의 양상이 나 환자의 상태 ,환자의 활동력 등을 고려하여 치료 방법을 결정하여야 할것으로 사료된다.EX>46N으로, 슬개건-티타늄 간섭나사군이평균 1067.4$\pm$145N에서평균 601.8$\pm$134N으로, 슬개건-생체흡수성간섭나사군이평균 987.1$\pm$168N에서588.7$\pm$124N으로각각40$\%$, 39$\%$, 50$\%$, 24$\%$, 44$\%$, 40$\%$가감소하였다. 결론: 수술후초기고정력은슬괵건을LA나사(R) 또는Semifix(R)로고정하는방법과슬개건을티타늄및생체흡수성간섭나사로고정하는방법등이우수하였으며, 슬괵건을생체흡수성간섭나사나Endobutton(R)으로 고정하는 방법 등은고정력이 상대적으로매우약함을 알수있었다. 최대인장력은단순인장검사로는이상의대퇴골측고정방법의고정력이초기부하를견뎌내는데충분하다고생각되었으나주기성부하실험후현저히감소되어충분한초기안정성을제공하지못함을알수있었다.를 나타내었다. 또한 3m깊이에서의 측방 선량분포에서 Spoiler의 거리변화(6, 10cm)는 심부선량의 변화에 영향을 주지 않는 것으로 확인할 수 있었다. 그리고 위의 실험측정치를 치료계획 시스템에 입력하여 선량분포를 확인한 결과 Spoiler를 사용하는 경우 OPEN에 비해 선량분포 영역을 표면으로 끌어 올릴 수 있으며 Bolus 보다 피부 보호효과는 어느 정도 유지가 되는 것을 보여주었다. 4.결론 이와 같이 Spoiler는 Bolus와 비교하여 6MV 광자선의 build up 영역을 표면으로 증가시키는 동시에 Skin Sparing(피부보호)효과를 유지할 수 있으며 두경부암의 치료에서 Spoiler의 사용이 가능한 조건으로는 조사면이 Fracture about proximal humerus may be classified as the articular segment or the anatomical neck, the greater tuberosity, the lesser tuberosity, and the shaft or surgical neck. Now, usually used, Neer's classification is based on the number of segments displaced, over 1cm of displaced or more than 45 degrees of angulation , rather than the number of fracture line . Absolute indication of a operative treatment a open fracture, the fracture with vascular injury or nerve injury , and unreductable fracture-dislocation . Inversely, the case that are severe osteoporosis, and eldly patient who can't be operated by strong internal fixation is better than arthroplasty used by primary prosthetic replacement and early rehabilitation program than open reduction and internal fixation. The operator make a decision for the patient who should be taken the open reduction and internal fixation, because it's different that anatomical morphology, bone density, condition of patient. The operator decide operation procedure. For example, percutaneous pinning, open reduction, plate & screws, wire tension bands combined with some intramedullary device are operation procedure that operator can decide . The poor health condition for other health problem, fracture with unstable vital sign and severe osteoporosis , are the relative contraindication. The stable fracture without dislocation is not the operative indication . The radiologic film of the prokimal humerus before the operation can not predict for fracture evaluation. It's necessary to good radiologic film for evaluation of fracture form. The trauma serise is better than the other radiologic film for evaluation. The accessary radiologic exam is able to help for evaluation of bone fragment and anatomy. The CT can be helpful in evaluating these injury, especially if the extract fracture type cannot be determined from plain roenterogram of the proximal humerus, bone of humerus head. If the dislocation is severe anatomically , we could consider to do three dimentional remodelling. The MRI doing for observing of bony morphology before the operation is not better than CT If we were suspicious of vascular injury, we could consider the angiography.

        관절경적 회전근 개 봉합술 : 1 - 4년 추시 결과

        Purpose : To compare and analyze the results of arthroscopic subacromial decompression and rotator cuff repair between partial rotator cuff tear and complete rotator cuff tear. Material and Methods : The authors studied 42 patients of rotator cuff tear with operation and followed over one year at Dankook university hospital from September, 1998 to March, 2001 The patient average age is 53 years and follow up period is 23 months (12-42mon1hs). We obtained 22 cases in the partial rotator cuff tear group and 20 cases in the complete rotator cuff tear group. In all cases, we used ASES methods to evaluated pain scale and function. Result : In the last follow up patients, the pain scale is decreased from 7.2 to 0.9 (ASES method : 34 to 91) in the partial rotator cuff tear group and from 7.6 to 1.2 (ASES method . 29 to 88) in the complete rotator cuff tear group, but there was no evidence of statistical difference between two groups (P>0.05). The range of motion after operation were increased in two groups. Excellent to good results were obtained 93% and 95% patients had pain relief and satisfied function. Conclusion : Arthroscopic subacromial decompression and arthroscopic rotator cuff repair showed good results for both of (ult-thickness and partial thickness rotator cuff tear patients in pain relief and improving the function.
