        항암화학요법 유발 말초신경병증 변증평가도구 개발을 위한 기초연구

        Objectives: The aim of this study was to develop a standard instrument of pattern identification and evaluation for chemotherapy-induced peripheral neuropathy (CIPN).Methods: The advisory committee for this study was organized by 12 professors of traditional Korean medicine from the Korean Association of Traditional Oncology. The items and structure of the instrument were designed based on a review of previous publications. We revised the instrument in consultation with the advisory committee and received additional advice via email.Results: We divided the symptoms and signs of CIPN into four pattern identifications: wind arthralgia (風痺), cold arthralgia (寒痺), dampness arthralgia (濕痺), and arthralgia of the deficiency type (虛痺). We obtained the mean weights to reflect the standard deviations from each symptom of the four pattern identifications, which were scored on a 5-point scale by the advisor committee. After we obtained the answers to discrimination between variable symptoms (變症) and ordinary symptoms (素症) from the 12 experts, we gained the final weight from the combination of the ratio of pattern identification to the number of total answers of the advisory committee and the mean weight.Conclusions: The Instrument on Pattern Identification and Evaluation for CIPN was developed through a discussion between 12 experts. There was a limitation that the validity and reliability of this instrument have not been proven. However, the significance of this study was that it is the first Instrument on Pattern Identification and Evaluation aimed at assessing CIPN in traditional Korean medicine.

        연질현미의 이화학적 및 취반 특성

        Purpose: To improve the cooking quality of brown rice, newly inbred soft rice cultivars were investigated. Methods: The physicochemical properties of brown rice flour and water absorption patterns and cooking characteristics of brown rice grain were compared to Ilmi white and brown rice. Results: General composition and total dietary fiber contents of five rices were significantly different with higher ash, protein, and total dietary fiber contents in soft brown rice than white rice. The hardness of raw rice grain was higher in Ilmi brown rice than in soft brown rice. The water absorption increased rapidly in 30 min of white rice and in 4-6 h of brown rices. The apparent amylose content of soft brown rice was lower than that of Ilmi rice. The initial pasting temperature and all viscosities were significantly different, but the trend was not similar. The textural properties of hardness and roughness were higher, but adhesiveness, cohesiveness, and stickiness were lower in Ilmi brown rice than white and soft brown rices. In sensory preference test, not only textural properties, hardness, adhesiveness, cohesiveness, stickiness, and roughness, but also color, glossiness, and roasted flavor were higher in soft brown rices. Especially soft brown rice B showed the best cooking quality among all rices. Conclusion: The results of the study suggested that soft brown rice is developed for cooking with high nutritional and functional quality.

        가와사키병 환아에서 면역글로불린 투여 전후의 혈중 Nitric Oxide와 Tumor Necrosis Factor-α에 대한 연구

        목 적 : 가와사키병은 면역 조절 인자의 이상을 동반하는 전신적 혈관염으로 관상동맥질환을 초래한다. NO는 혈관내 평활근의 granulocyte cyclase의 기전에 영향을 미쳐 혈관의 이완을 유발하는 역할을 하며 과다하게 분비될 경우 혈관의 변성을 초래하는 것으로 알려져 있다. 본 저자들은 가와사키병에서 NO와 $TNF-{\alpha}$의 혈중 농도를 측정하여 관상동맥질환 발생과 연관이 있는지 알아보기 위해 본 연구를 실시하였다. 방 법 : 가와사키병 환아 24명을 관상동맥 확장이 없는 군(1군)과 관상동맥 확장이 있는 군(2군)으로 분류하여 각 군의 임상 양상과 면역글로불린 투여 전과 후, 회복기에서의 NO, $TNF-{\alpha}$의 혈중 농도를 면역효소법(ELISA)으로 측정하여 비교하였다. 대조군으로는 같은 시기에 내원한 열이 없는 정상 대조군(3군) 13명과 열이 있는 대조군(4군) 10명으로 하였다. 결 과 : 면역글로불린 투여 전의 혈중 NO는 1군($13.2{\pm}5.7{\mu}mol/L$), 2군($20.4{\pm}10.7{\mu}mol/L$)과 4군($12.4{\pm}8.9{\mu}mol/L$)이 3군($3.1{\pm}1.4{\mu}mol/L$)보다 높았고 2군이 1군과 4군에 비해 유의하게 높았다(P<0.05). $TNF-{\alpha}$는 2군($858.4{\pm}934.0pg/mL$)에서 3군($8.7{\pm}2.3pg/mL$)과 4군($226.7{\pm}647.2pg/mL$)에 비해 유의하게 높았으며 1군($522.4{\pm}859.6pg/mL$)도 3군에 비해 높았다(P<0.05). 면역글로불린 투여 후 NO는 1군, 2군과 4군이 3군에 비해 유의하게 높았으며 $TNF-{\alpha}$는 각 군별로 유의한 차이가 없었다. 가와사키병 관상동맥 확장군과 비확장군 모두에 있어 NO와 $TNF-{\alpha}$의 혈중 농도가 면역글로불린 투여 전에 가장 높았고 면역글로불린 투여 후와 회복기로 갈수록 감소하였다. 또한 관상동맥 확장군에서 백혈구 수치와 혈청 NO는 유의한 양의 상관관계가 있었다(r=0.430). 결 론 : 가와사키병 환자에 있어 NO, $TNF-{\alpha}$의 혈중 농도가 유의하게 높았으며 NO의 농도가 관상동맥이 확장된 환자에서 비 확장군보다 의미있게 높은 것으로 보아 NO가 관상동맥질환에 관여할 것으로 생각한다. Purpose : Kawasaki disease(KD) is a systemic panvasculitis that causes coronary artery lesions. KD is accompanied by immunoregulatory abnormalities. Nitric oxide(NO) can induce relaxation of blood vessels by activating guanylate cyclase in smooth muscle cells and high levels of NO may result in coronary artery lesions. We investigated tumor necrosis factor$(TNF)-{\alpha}$ and NO production before and after intravenous immunoglobulin(IVIG) therapy to study the roles of NO and $TNF-{\alpha}$ in KD with coronary artery lesions. Methods : Serum levels of NO and $TNF-{\alpha}$ were measured in 24 patients with KD(group I, eight patients with normal coronary artery; group II, 16 patients with coronary artery lesions) and 23 controls(group III, 13 afebrile controls; group IV, 10 febrile controls). Blood samples from each subject were drawn before and after IVIG therapy and in the convalescent stage. Serum concentrations of NO and $TNF-{\alpha}$ were measured by enzyme linked immuno sorbent assay. Results : The NO levels before IVIG therapy were significantly higher in group II than in group I, group III and group IV. After IVIG therapy the levels of NO were significantly higher in group I and group II than in group III. The $TNF-{\alpha}$ levels before IVIG therapy were significantly higher in group I and group II than in group III. The serum $TNF-{\alpha}$ and NO levels were higher before IVIG therapy and decreased through the convalescent stage in KD patients. In the acute stage of KD patients with coronary artery lesions, serum NO levels significantly correlated with white blood cells (r=0.43, P<0.05). Conclusion : The serum concentration levels of $TNF-{\alpha}$ and NO were abnormally high in KD patients and NO concentrations were statistically higher in the KD patients with coronary artery abnormalities than those without coronary abnormality during the early stage of the KD. These results suggest NO may be involved in the development of coronary artery lesions.

        전자침술로 호전된 유방암 환자의 항암화학요법 유발 말초신경병증 증례보고

        Objectives : The purpose of this study is to report the effect of electroacupuncture on chemotherapy induced peripheral neuropathy in breast cancer patient. Methods : The patient is a female with chemotherapy induced peripheral neuropathy (CIPN) who was diagnosed with breast cancer and suffering from paresthesia of extremities after chemotherapy of docetaxel. The patient was treated with electroacupuncture (15 minutes per one time) twice a day for 14 days. The clinical outcomes were measured by Patient Neurotoxicity Questionnaire (PNQ) grade and visual analogue scale (VAS). Results : In this case, PNQ Item 1 (numbness) grade was improved from D to C, Item 2 (weakness) grade was from C to B. The score of upper limbs VAS was decreased from 7 to 2, lower limbs was from 7 to 5 respectively. Edema of both extremities was also improved. Conclusions : This case study suggests that electroacupuncture therapy may have significant effects of CIPN in breast cancer patient.

        한국 영아의 Acid Steatocrit에 대한 연구

        목 적 : Acid steatocrit 방법을 이용하여 12개월까지의 정상 영아에서 생리적 지방변 배출의 감소시기와 수유방법에 따른 지방변 배출의 차이를 알아보고자 한다. 방 법: 1998년 5월부터 2001년 4월까지 건국대학교 민중병원에 내원한 환아 중 특별한 소화기질환이 없는 12개월 미만의 정상 영아 128명을 대상으로 하였다. 생후 1개월까지(79명), 1-6개월(28명), 7-12개월(21명)으로 구분하였다. 생후 1개월까지의 신생아에서는 인공영양아(46명), 모유영양아(33명)이었고 1-6개월 영아에서는 인공영양아(18명), 모유영양아(10명), 7-12개월 영아에서는 인공영양아(11명), 모유영양아(10명)으로 분류하여 각 월령별 acid steatocrit 치를 측정하였다. 결 과 : 1) 생후 1개월까지 신생아의 평균 acid steatocrit치는 $94.40{\pm}1.36%$이었고, 1-6개월 영아는 $85.02{\pm}5.98%$이었고, 7-12개월 영아는 $58.35{\pm}26.99%$로 유의하게 acid steatocrit 치가 떨어지는 것을 알 수 있었다. 2) 생후 1개월까지의 신생아에서 인공영양아의 acid steatocrit 치는 $94.98{\pm}1.36%$, 모유영양아는 $93.61{\pm}0.89%$로 유의한 차이(P=0.0066)를 보였고, 1-6개월 인공영양아의 acid steatocrit 치는 86.59${\pm}6.07%$, 모유영양아의 경우는 $82.19{\pm}4.88%$로 유의한 차이(P=0.0240)를 보였으며, 7-12개월 인공영양아는 $75.79{\pm}8.77%$, 모유영양아는 $24.25{\pm}27.44%$로 유의한 차이 (P=0.0010)를 보였다. 결 론: Acid steatocrit 방법을 이용한 영아기 지방변 측정은 생리적 지방변 배출을 검사하는 쉽고 정확한 방법으로 생각되며 향후 지방흡수장애를 보이는 환아를 진단하고 경과 및 치료의 평가시 acid steatocrit 방법이 널리 이용될 수 있을 것으로 사료된다. Purpose : Steatorrhea tests have been developed using various methods. Acid steatocrit is a simple method to detect steatorrhea and has very high sensitivity and specificity. This present study was designed to establish the normal values of acid steatocrit in Korean infants and to find the difference according to the various feeding methods. Methods : Acid steatocrit tests were conducted on 128 infants who were under 12 months of age and who had non-specific gastrointestinal diseases between May 1998 and April 2001. The results were classified into neonatal ages(79 neonates), 1-6 months(28 infants), 7-12 months(21 infants). This included formula-fed(46 neonates) and human milk-fed(33 neonates), 1-6 months formula-fed (18 infants) and human milk-fed(10 infants), 7-12 months formula-fed(11 infants) and human milk-fed(10 infants). Results : The acid steatocrit values decreased by infant age in months. Acid steatocrit values decreased much more after 7 months of ages. The acid steatocrit values of human milk-fed infants were significantly lower than those of formula-fed infants. Conclusion : Our study confirms that a physiologic steatorrhea was found in the infant period and decreases by infant age of months. The acid steatocrit test might be useful for the evaluation of gastrointestinal milk fat malabsorption disorders and therapeutic effects.

      • 설문조사를 바탕으로 부서 간 만족도 향상을 위한 개선 안

        Purpose Recently, there has been increasing demands for clinical departments at Asan Medical center in Seoul. They want to see the results of lab tests about blood samples as soon as possible and to be reported to them on the day. As the main contents of the survey, we explained the goodness of Asan medical center's in vitro and the points to be improved. Based on this, it became an opportunity to create a positive image of In vitro laboratory. In addition, we could identify their specific requirements through the surveys. Materials and Methods In September 2016, a total of 14 questionnaire surveys were conducted for 49 clinical departments and outpatient nurses at Asan Medical center in Seoul. The survey consists of description questions to be able to express the intention of the individual and the questions made on the Likert 4 point scale. The main contents of 14 questions are composed of goodness of In Vitro laboratory and points to be improved. Results 62% answered that the best service in In Vitro laboratory was "good accuracy and reproducibility". On the other hand, as an inconvenience when requesting blood tests, 73% pointed out that "the result report time was long", which was recognized as a part to be improved. There are many contents that "The result of all tests is reported within 2 hours" on the day of the examination. In the question - 'Are there some examination results which do not coincide well with clinical observations?', 55 of 56 people answered "no". Above all, the majority answered that waiting for re-examination results is too long. This problem must be causing discomfort to the patients. Conclusion In order to improve these problems, the first thing is to increase the number of blood tests by using the current personnel, equipments and reagents to the fullest by item in Asan Medical Center's Nuclear medicine in vitro part. Secondly, in case of re-examinations, we use the "AMIS message" to show other clinical departments the reporting time. This methode improves the efficiency of work with nurses and increases satisfaction of custom service. Thirdly, the correlation was evaluated by selecting the test species that can be carried out by the shortening method. Currently, C-peptide and insulin are implemented in the reaction process to shorten 2 hours into 1 hour. Finally, we are considering purchasing new equipments for quick test results.

        원전 방사성 콘크리트 기계적 절단의 방사성 에어로졸에 대한 작업자 내부피폭선량 평가

        원전 해체 공정 중 다량의 콘크리트 방사성 폐기물의 절단 과정에서 불가피하게 방사성 에어로졸이 생성된다. 방사성 에어로졸은 인체 호흡기 흡착에 의한 내부피폭을 유발하기 때문에 작업자의 방사선 방호를 위한 내부피폭평가가 필수적으로 시행되어야 한다. 그러나 실제 작업환경의 에어로졸 특성값을 사용하기에는 선행 연구가 미비하며 콘크리트에 포함된 방사성 핵종의 수가 많기 때문에 정확한 작업자 내부피폭평가를 위해서는 상당한 시간과 인력이 필요하다. 따라서, 본 연구에서는 사전 연구된 콘크리트 에어로졸 특성값을 활용하여 원전 해체 전 절단 작업자의 내부 피폭량을 빠르게 예측할 수 있는 새로운 방법론을 제시하고자 한다. 본 연구팀은 콘크리트 절단 시 발생하는 사전 연구에서 발표된 에어로졸의 수농도 크기 분포데이터를 뉴턴-랩슨법을 이용하여 피폭평가 계산에 필요한 방사능중앙 공기중역학직경(Activity Median Aerodynamic Diameter)값으로 변환하였다. 또한 원전 정지 10년 후 비방사능 값을 ORIGEN code로 계산하였으며, 최종적으로 핵종별 예탁유효선량을 IMBA 프로그램을 이용하여 계산하였다. 핵종별 예탁유효선량값을 비교한 결과 152Eu에 의한 최대 예탁유효선량은 전체 선량값의 83.09%를 차지하고, <sup>152</sup>Eu를 포함한 상위 5개 원소(<sup>152</sup>Eu, <sup>154</sup>Eu, <sup>60</sup>Co, <sup>239</sup>Pu, <sup>55</sup>Fe)의 경우 최대 99.63%를 차지함을 확인하였다. 따라서 원전 해체 전 콘크리트의 구성 원소 중 상위 5개 주요 원소 측정을 먼저 시행한다면 더 빠르고 원활한 방사능 피폭관리 및 해체 작업 안전성 평가가 가능할 것으로 판단된다.

        이러닝학습에서 실재감, 학습만족도, 학업성취도의 관계에서 학습몰입의 매개효과

        The purpose of this study is to investigate the mediating effect of learners" learning flow in the effect of presence on academic achievement in web-based e-learning. For this purpose, this study analyzed the influencing relationship between the each factor based on the structural model with the learning flow as a mediator variable. Based on existing theoretical studies, learning satisfaction and academic achievement, which represent learning outcomes, are set as dependent variables, and teaching presence, cognitive presence, and social presence are set as independent variables. Data collected from a total of 256 e-learning learners were used in the analysis of this study. According to the results of the analysis, teaching presence, cognitive presence, and social presence were found to have a significant effect on academic achievement when a learning flow is a mediator variable. Concretely, teaching presence, cognitive presence, and social presence have a positive effect on the learning flow, while learning flow has a positive effect on learning satisfaction. On the other hand, learning flow has a negative effect on academic achievement. As a result of verifying the mediating effect of learning flow on the relationship between presence, learning satisfaction, and academic achievement, there was meditating effect in the aggregate. This study implies that in order to increase the level of learning satisfaction and academic achievement, it is necessary to make the teaching-learning design in the provision of contents and materials for e-learning so that the learner can feel the presence. The results of this study can be used as a basic data for seeking support and promotion strategies for enhancement of future learning flow and presence.

        중심시야 결손 여부에 따른 녹내장시야지표의 유용성

        Purpose: To evaluate the glaucoma discrimination ability of visual field index (VFI), a new perimetric index of Humphrey field analyzer II, in glaucoma patients with central and peripheral visual field defects (VFD). Methods: Humphrey visual field test and OCT were performed in 204 glaucomatous eyes and 70 healthy eyes. The associations of VFI with mean deviation (MD), pattern standard deviation (PSD), and average retinal nerve fiber layer thickness (RNFLT) were analyzed using Pearson’s correlation. The diagnostic abilities of the parameters were analyzed using the areas under the receiver operating characteristic curves (AUROC). The AUROC were compared between MD-matched patients with central VFD (at least one point with p < 1% within the central most 16 points of 30-2 SITA standard automated visual field) and peripheral VFD (VFD beyond the central most 16 points of 30-2 SITA standard automated visual field). Results: The associations between analyzed parameters and VFI were statistically significant. The MD, RNFLT, age, intraocular pressure, and central cornea thickness were not different between the two groups (p > 0.05). The AUROC value of VFI was greater than those of the MD and average RNFLT but was not different from that of PSD (p = 0.332) in the central VFD group. However, there were no significant differences between AUROC value of VFI and those of other parameters in the peripheral VFD group (all, p > 0.05). Conclusions: The results from the present study suggest that VFI may be more useful than MD in diagnosing glaucoma, especially in patients with central VFD.
