        녹용약침으로 치료한 De Quervain's Disease 4례 증례보고

        De Quervain's disease is caused by stenosing tenosynovitis of the first dorsal compartment of the wrist. The first dorsal compartment at the wrist includes the tendons of the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). Patients with De Quervain's usually report pain at the dorsolateral aspect of the wrist with referral of pain toward the thumb and/or lateral forearm. This condition responds well to nonsurgical treatment. Objective : The aim of this study was to find out the therapeutic effect of the Cervus elaphus Herbal-acupuncture in the treatment of patients with De Quervain's Disease. Design : A prospective study of Cervus elaphus Herbal-acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients : The study comprised 4 patients with De Quervain's Disease. Methods : We investigated 4 cases patients the De Quervain's Disease. We evaluated wrist function that before and after Cervus elaphus Herbal-acupuncture treatment by VAS score, tenderness and Finkelstein test. the treatment is operated every other day. Results : Pain reduction(VAS) was found out in the case treated with Cervus elaphus Herbal-acupuncture. Conclusions : Clinical results show that Cervus elaphus Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.

        무작위 대조 시험을 통한 족관절 염좌의 자침 중 심자와 천자의 비교 연구-준비 조사

        Objective : The aim of the study was to compare the therapeutic effect of the superficial and in-depth insertion of acupuncture needles in the treatment of patients with ankle sprain. Design : A prospective randomized single-blind study of superficial and deep acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients: The study comprised 14 patients with ankle sprain who were divided into two groups (A and B). Intervention : In group A, the needle was introduced in the skin at a depth of 2 mm, whereas in group B the needle was placed deeply into muscular tissue or the articular capsule at a depth more than 1.5 Cm The treatment was planned for a duration of 1 week, 3 times. Outcome Measures : The intensity of pain was evaluated with the Ankle grade pain chart(AGPC) before and after treatment and at the 1-week follow-up examination. The AGPC includes Swelling, Tenderness, Exudation, Bruise, Medial/Lateral. stability test, Anterior drawer test, Squeeze test, Bearing weight, Pressure Algometer, Walking state and VAS. Results : Although at the end of the treatment there was no evidence of significant statistical differences between the two different groups, Pain reduction(VAS) was greater in the group treated with deep acupuncture. A statistical difference existed between the two groups at the 1 week follow up, with a better result in the deeply stimulated group in VAS. Conclusion : Clinical results show that deep stimulation has a better analgesic effect when compared with superficial stimulation in pain reduction(VAS, Visual analogic scale).

        전완 골절 후 도수 정복 시 골절 부위 간격에 따른 골절 인접 연부 조직의 생체역학적 특성 평가: 유한요소해석

        The purpose of this study is to evaluate the biomechanical properties of fractured adjacent soft tissue during closed reduction after forearm fracture using the finite element method. To accomplish this, a finite element (FE) model of the forearm including soft tissue was constructed, and the material properties reported in previous studies were implemented. Based on this, nine finite element models with different fracture types and fracture positions, which are the main parameters, were subjected to finite element analysis under the same load and boundary conditions. The load condition simulated the traction of increasing the fracture site spacing from 0.4 mm to 1.6 mm at intervals of 0.4 mm at the distal end of the radioulnar bone. Through the finite element analysis, the fracture type, fracture location, and displacement were compared and analyzed for the fracture site spacing of the fractured portion and the maximum equivalent stress of the soft tissues adjacent to the fracture(interosseous membrane, muscle, fat, and skin). The results of this study are as follows. The effect of the major parameters on the fracture site spacing of the fractured part is negligible. Also, from the displacement of 1.2 mm, the maximum equivalent stress of the interosseous membrane and muscle adjacent to the fractured bone exceeds the ultimate tensile strength of the material. In addition, it was confirmed that the maximum equivalent stresses of soft tissues(fat, skin) were different in size but similar in trend. As a result, this study was able to numerically confirm the damage to the adjacent soft tissue due to the fracture site spacing during closed reduction of forearm fracture.

        체육수업 재미거리 질문지(PCESQM) 타당성 및 신뢰성 검증

        This paper verified the validity of the structure and reliability whether the questionnaire on the source of enjoyment in physical education classes for middle school students developed by Chang-sub Lee and Sang-woo Nam(2003) can effectively measure the fun factors in the physical education classes for the male and female students(592) of middle schools in S city in 2008. As a result of the item analysis and the reliability analysis, since questions with a mean over 4.5 and questions with the skewness and kurtosis values over ±2.0 did not appear, the results of this paper showed to have an appropriate level of the mean, standard deviation, skewness, kurtosis, and reliability. Furthermore, as the results of the correlation matrix between questions, show that since more than 50% of the questions have a correlation over ±3.0 between each question, the question of this paper are judged to be adequate questions for the factor analysis. Moreover, in the exploratory factor analysis, the maintenance of physical and mental health factor was developed into questions number 2, 3, 5, 6, 7, sense of accomplishment factor to questions number 1, 4, 12, the easiness of the class factor to questions number 9, 10, 15, the freedom of the class factor to questions number 13, 14, 16, and the sociability cultivation factor to questions number 8, 11, 17 that are the sub-factor of the questionnaire on the source of fun in physical education classes for middle school students developed by Chang-sub Lee and Sang-woo Nam(2003). Also in the exploratory factor analysis of this paper, as the questions related to factor 1 showed to be questions number 6, 7, 5, 2, 3, factor 2(1, 4, 12), factor 3(10, 9, 15), factor 4(14, 13, 16), and factor 5(17, 8, 11), it indicates that is composed of the five sub-factors developed by Chang-sub Lee and Sang-woo Nam(2003). For the confirmatory factor analysis that assesses the fitness of the model, as the Q value was 2.004, GFI .910, AGFI .897, CFI .884, RMSEA .71, and RMR .51, it is judged that the model is appropriate. As a result of evaluating the fitness of the model through the item analysis, correlation analysis, exploratory factor analysis, and the confirmatory factor analysis as mentioned above, the questionnaire on the source of enjoyment in physical education classes for the middle school students developed by Chang-sub Lee and Sang-woo Nam(2003) showed to be an appropriate questionnaire in measuring the enjoyment factors for the male and female students of middle schools in S city.

      • 근시에서의 라섹과 TransPRK라섹 수술 후 고위수차 비교

        Purpose: The aim of this study is to compare total high order aberrations between surgery which uses TransPRK and general laser assisted sub-epithelial keratomileusis. Methods: The patients who had general laser assisted sub-epithelial and TransPRK laser assisted sub-epithelial by visiting ophthalmic clinic in Ulsan from January 2014 to August 2014 was researched. Results: When comparing total high order aberrations before and after operation, it was found that total high order aberration in general laser assisted sub-epithelial group increased to $0.222{\pm}0.078{\mu}m$ from $0.074{\pm}0.019{\mu}m$ while that in TransPRK assisted sub-epithelial group increased to $0.179{\pm}0.045{\mu}m$ from $0.076{\pm}0.032{\mu}m$. When comparing the increment in total high order aberrations between two groups after surgery, it was found that the increments of TransPRK assisted sub-epithelial group were statistically smaller than that of general laser assisted sub-epithelial group (p<0.05). Conclusions: This study made a comparative analysis of total high order aberrations between general laser assisted sub-epithelial and TransPRK assisted sub-epithelial which was introduced recently and the result showed a significant difference. The findings of this study suggest that TransPRK assisted sub-epithelial can be used widely in simply improving eye sight and quality of vision by reducing an increase in high order aberration caused by orthokeratology surgery. It is expected that TransPRK assisted sub-epithelial will be helpful for improving the understanding quality of eye sight which occurs by several methods of vision correction surgery.

        칼만필터를 이용한 3상 PFC AC/DC 컨버터의 센서리스 제어

        3상 PWM 컨버터는 입력 전류를 정현적으로 제어할 수 있어 입력 전류의 고조파 성분을 감소시킬 수 있고 입력 전압에 대하여 입력 전류의 위상을 제어할 수 있다. 본 논문은 3상 PWM 컨버터의 역률개선을 위하여 전원전압센서를 사용하지 않고 가상자속기반 벡터제어를 적용한 새로운 센서리스 제어방식을 연구하였다. 직류출력전압은 칼만 필터를 적용하여 추정한 가상자속에 의해 제어되며, 동기위상은 추정된 가상자속을 이용하여 위상을 얻는다. 제시한 PFC 알고리즘은 역률개선 및 가변영역에서 직류출력전압, 부하변동에서 직류출력전압을 정밀하게 제어할 수 있는 우수성을 가진다. 본 논문에서 제안한 가상자속추정 알고리즘을 적용한 칼만 필터 성능은 실험과 시뮬레이션을 통해서 검증하였다. This paper investigates a new sensorless control appling a virtual flux oriented vector control without the line voltage sensor for the power factor correction of 3 phase PWM converter. The DC output voltage is controlled by applying the kalman filter algorithm for the virtual flux estimation and the synchronous phase is obtained by using the estimated virtual flux equation based on kalman filter. This method is used to reduce the calculation time of the system and obtain a stable control that the input current including the harmonics and the noise is improved. The proposed system implement PFC algorithm in the variable region of DC output voltage. It can obtain the unity power factor, and can precisely control the DC output voltage in the load variation and in the variable voltage range. The performance of the proposed algorithm is verified through simulation and experiment.

        족관절 외측 인대 봉합 후 실패의 위험 인자

        A modified $Brostr{\ddot{o}}m$ procedure has been widely accepted as a treatment of choice for patients with chronic lateral ankle instability. The procedure is highly reliable and provides successful outcomes in approximately 90% of cases. Severe degree of instability, absence or poor quality of the ligamentous remnant, obesity, and generalized joint hypermobility have been regarded as poor prognostic factors related to the modified $Brostr{\ddot{o}}m$ procedure. However, these perceptions are based on a low level of evidence studies or expert opinions. Therefore, the aim of this article was to search for evidences regarding the poor prognostic factors of the modified $Brostr{\ddot{o}}m$ procedure.

        카멜리나 (Camelina sativa Crtz.) 발아 적온 및 발아초기 뿌리생육 특성

        A genus Camelina has been attracted as a promising oil crop, especially available in drought and marginal conditions. Due to more demands on arable land for bioenergy crops, price of agricultural products has been a challengeable issue. In that respect, development of Camelina crop with higher germination rate and germination energy can be a strategy to secure seedling establishment, nutrient uptake and long vegetative period. In order to be easily available in the field and laboratory conditions, Camelina seed needs to be optimized for its germination temperature. Germination temperature regime was in a range of 8 to 32℃ initially, and consecutively narrowed down to 8 to 20℃. Based on the temperature range, Camelina germinated greater than 96% at 8-16℃ in two weeks after sowing, but germination rate started to decrease at the higher than 24℃ and was significantly low at higher than 32℃. In terms of rapid time to reach the maximum germination rate and greater germination energy, temperature ranged from 12 to 16℃ was found to be desirable for Camelina germination. Although germinationa rate was greater at 16℃, lower temperature close to 12℃ would be favored for the field conditions where greater root growth leading to healthier seedlings and better nutrient or water availability is considerably demanded.
