        전이성 골암에 의한 해면골의 미세구조와 골화 분포 변화

        Purpose: The aim of present study is to detect longitudinal alterations of mechanical characteristic determined by bone quality (microarchitecture and degree of mineralization) on femur trabecular bone due to metastatic bone tumor Materials and Methods: Each 6 female SD rats (12 weeks old, approximate 250g) were allocated in SHAM and TUMOR Group. W256 (Walker carcinosarcoma 256 malignant breast cancer cell) was injected into the right femur (intraosseous injection) in TUMOR Group, whereas 0.9% NaCl (saline solution) was injected in SHAM Group. The right hind limbs of all rats were scanned by in-vivo micro-CT to acquire structural parameters, bone mineral density, X-ray attenuation and bone mineralization distribution at 0 week and 4 weeks after surgery. Results: BMD, BV/TV and Tb.N of trabecular bone in TUMOR group were markedly decreased (26%, 11% and 23%) while those in SHAM group were significantly increased (34%, 48% and 11%) (p<0.05). BS/BV, Tb.Sp and SMI in TUMOR group were significantly increased (-16%, 38% and 2%) compared with those in SHAM group (-33%, 12% and -16%) (p<0.05). Additionally, bone mineralization in TUMOR group significantly decreased while those in SHAM group was significantly increased (p<0.05). Conclusion: It is identified that how much bone microarchitecture and mineralization are diminished due to the metastatic bone tumor. The results may be helpful to prediction of fracture risk by metastatic bone tumor.

        전이성골암 연구를 위한 동물모델 개발 및 검증

        Bone is one of the most favored sites of tumor metastasis. However, the existent animal models developed to understand mechanism of occurrence and progress of metastatic bone tumor generally showed difficulties of reproducibility and performance of longitudinal study, and inaccuracy of validation. The aim of this study was, therefore, to newly develop and accurately validate an animal model for study of metastatic bone tumor with overcome of the limitations shown in the existent animal models. Eighteen female Sprague-Dawley rats (12 weeks old, 250+7 g) were randomly allocated in Sham and Tumor groups. W256 breast cancer cell was inoculated in the right femur of the rat for Tumor group, while 0.9% NaCl was injected for Sham group. Urine was collected by metabolic cages for DPD (deoxypyridinoline) test in order to evaluate bone resorption at 0, 4, 8, 12 weeks after surgery. At the same time, the right hind limbs of all rats were scanned by in-vivo micro-computered tomography (CT) to identify tumor-mediated bone destruction driven from metastatic bone tumor. Finally, positron emission tomography was examined to directly identify existence of tumor cells inoculated in the bone. DPD test showed that bone resorption markedly increased in the bone of Tumor group compared to that of Sham group (p<0.05). In-vivo micro-computed tomography showed that there were significant bone losses and X-ray attenuation values in the bone of Tumor group compared to that of Sham group after surgery. In addition, tumor cells were directly identified in the bone of Tumor group by positron emission tomography, not in the bone of Sham group. The results indicated that the developed animal model might be confidential and reasonable to performances of studies related to metastatic bone tumor, with easy reproducibility, accuracy of validation, and suitability to performance of longitudinal study. To our knowledge, this study may prove valuable as the first development of the animal model overcoming the limitations shown in the existent animal models. The animal model developed in the present study may be useful for further metastatic bone tumor studies as mentioned.

        이 연구는 다양한 가치가 공존 하는 사회에서 패션의 변화와 젠더리스 스타일의 패션이 의미하는 사회적 단면에 주목하였다. 본 논문에서는 젠더리스 스타일 동향에서 패션의 관념적인 의미와 시각적인 기호와의 연관성 및 상징성의 의미가 주는 내포적 의미를 기호학적 의미로 분석하였으며, 분석 방법은 퍼스의 기호학을 중심으로 젠더리스 스타일이 의미하는 상징과 표현특성을 관련된 저술, 선행 연구 및 사례분석을 통해 연구하였다. 본 연구의 목적은 현대사회의 패션에 나타난 젠더리스 스타일의 표현 특성을 파악하여 그들의 변화하는 패션에 대한 인식을 기호학적으로 분석 정리하는 것이다. 젠더리스 스타일이 보여주는 패션의 기호적 의미는 각자의 개성과 자기중심적 사고로 사회적 관념과 전통 등을 배제한 자기만의 스타일 즉 인간중심적 디자인의 욕구를 나타내는 것이다 본 논문에서는 적극적인 사고방식과 보다 자유로운 감성을 추구하게 하는 패션 트렌드의 한축으로써 젠더리스 스타일이 보여주는패션의 기호적 의미에 대한 시각을 제공하였으며, 향후 더욱 다양화되는 패션 디자인을 위한 시사점 모색 및 방향 에 도움이 되고자 하였다. This research is valuable in a society where a variety of fashion change and genderleese-style fashion, this means that the social section and the ideological meaning of fashion trends in genderleese style and visual symbols and the meaning of symbolism, rather than giving a sense of continuity and of the connotation meaning symbol meaning analysis and forensic analysis approach is centered on the semiotics of gender in Peirce leasing means symbols and epresentation of the attributes that the style of the associated writings and research on leading research and case analysis. The purpose of this study on modern society appeared on the agenda is an expression of style and fashion by identifying the characteristics of leasing their changing fashion as a symbol for awareness of rock showing the fashion style of gender analysis by organizing symbolic meaning of leasing their own individuality and self-centered in an accident, such as the traditional notion of the social excluded-style that is indicated by the demands of design and anthropocentric to seek aggressive mindset and to fashion a free sensibility than a trend showing the style of the axis of the symbolic meaning of gender by leasing a comprehensive perspective on the fashion to provide and , Further implications sought for fashion design, which is diversified and want to be helpful in the direction.

        엉덩관절 수술 환자에서 스마트폰을 이용한 관절가동범위 측정의 신뢰도 및 타당도 연구

        PURPOSE: The aims of this study were to assess the intra-tester and inter-tester reliability and validity of hip range of motion using a smart phone in hip operative patients. METHODS: Twenty-five patients (eight total hip arthroplasty and seventeen femur neck fracture) participated in this study. The range of motions in active and passive hip flexion, abduction and external rotation were measured with a goniometer and smart phone over two times by two observers. The intra-tester and inter-tester reliability were evaluated using the intraclass correlation coefficient (ICC2,1). The validity was measured by Pearson’s correlation coefficient RESULTS: The intra-observer reliability was good in all measured items (ICC>0.78). The inter-observer reliability was high with ICC (>0.90). All correlation coefficients of smart phone and goniometer was greater than 0.85 and showed a significant positive correlation (p<0.01). CONCLUSION: The range of motion measurement with a smart phone showed acceptable reliability. Therefore, it could be convenient and have economical benefits to measure the range of motion of the hip joint using a smart phone.

        임산부에서 부신절제술 후 잔여 선종인한 일차성 고알도스테론증

        Primary aldosteronism is present in approximately 1 percent of hypertensive patients, and is rarely associated with pregnancy. We report a case of unilateral aldosterone producing adenoma in a pregnant woman. A 29-year-old woman was referred to with hypertension and proteinuria at 14 weeks` gestation. She had known that she had unilateral aldosterone producing adenoma for 18 months and refused medical or surgical treatment. She was prescribed antihypertensive medication and preformed laparoscopic adrenalectomy in left adrenal gland at 22 weeks` gestation. After adrenalectomy, her blood pressure was normal. At 29 weeks` gestation, she complained headache, and her blood pressure was 200/100 mmHg with pitting edema. We thought preeclampsia and performed cesarean section. After the baby was delivered, she had uncontrolled hypertension, hypokalemia with persistent proteinuria. We checked plasma renin activitity, aldoterone and abdminopelvic computed tomography. We found remnant left adrenal adenoma and perfomed laparoscopic operation. After reoperation, her blood pressure was normal and no proteinuria.

        비디오 컨텐츠를 사용한 홈 트레이닝이 만성 허리 통증이 있는 대학생의 배근육 두께, 허리기능 및 통증에 미치는 영향

        PURPOSE: This study examined the effect of training using video content on abdominal muscle thickness, Oswestry disability index (ODI), and pain in college students with chronic back pain. METHODS: Twenty-nine college students with chronic back pain participated in this study. The subjects were assigned randomly to 15 experimental groups who trained using video content and 14 control groups who exercised voluntarily using back exercise leaflets. The video used for the intervention was obtained from YouTube, and the difficulty level of the video was classified into six levels. Both groups participated in the intervention for 40 minutes/day, three times a week for six weeks, and the variables of abdominal muscle thickness, ODI, and pain were compared before and after the intervention. RESULTS: In the experimental group, there were statistically significant changes in the thickness of the internal oblique and transverse abdominis muscles, ODI, and pain after the intervention, except for the external oblique muscle (p < .05). In the control group, there was no statistically significant difference in all variables after the intervention (p > .05). A statistically significant difference in all variables was observed between the experimental group and the control group after the intervention except for the external oblique muscle (p < .05). CONCLUSION: Treatment of chronic low back pain using video content is a possible alternative treatment if quality images are selected and the difficulty levels are adjusted.
