        6개월간 산삼약침요법을 시행 받은 ⅢB기 편평세포폐암 환자에 대한 증례보고

        Objective To derive further studies evaluating the effectiveness of Cultured Wild Ginseng Pharmacopuncture (CWGP) Therapy on squamous cell carcinoma as a first line. Methods Three cycles (4 weeks/cycle) of CWGP were administered as a dosage of 10 ml per day. Patient was diagnosed with stage IIIB squamous cell carcinoma and refused all therapy of conventional medicine because of old age and cardiac invasion of tumor. Intensive treatment of CWGP for 3 cycles was done on the patient. Computed Topography (CT) was performed to evaluate the therapeutic efficacy. Results After the intravenous infusion of 2 cycles of CWGP, chest CT revealed the mass size and pleural invasion sustained stable disease. After the point injection of 1 cycle of CWGP, chest CT revealed progressive disease. The disease free survival rate was 1 month. Conclusion This case may provide us the possibility that CWGP offers potential benefits for patients with squamous cell lung carcinoma. But this is a single case study and further case-series research should be compensated.

      • 암반에 전달된 밀장전 발파입력의 획률론적 예측 II - 최대압력 도달시간 예측을 중심으로 -

        The supersonic shock wave generated by fully coupled explosion will change into subsonic shock wave, plastic wave, and elastic wave consecutively as the wave propagates through rock mass. While the estimation of the blast-induced peak pressure was the main aim of the companion paper, this paper will concentrate on the estimation of the rise time of blast-induced pressure. The rise time can be expressed as a function of explosive density, isentropic exponent, detonation velocity, exponential coefficient of the peak pressure attenuation, dynamic yield stress, plastic wave velocity, elastic wave velocity, rock density, Hugoniot parameters, etc. Parametric analysis was performed to pinpoint the most influential parameter that affects the rise time and it was found that rock properties are more sensitive than explosive properties. The probabilistic distribution of the rise time is evaluated by the Rosenblueth'S point estimate method from the probabilistic distributions of explosive properties and rock properties. Numerical analysis was performed to figure out the effect of rock properties and explosive properties on the uncertainty of blast-induced vibration. Uncertainty analysis showed that uncertainty of rock properties constitutes the main portion of blast-induced vibration uncertainty rather than that of explosive properties. Numerical analysis also showed that the loading rate, which is the ratio of the peak blasting pressure to the rise time, is the main influential factor on blast-induced vibration. The loading rate is again more influenced by rock properties than by explosive properties. 밀장전한 암반발파공에서 화약폭발로 전파되는 초음속 충격파는 암반중에 전파되면서 차자로 저음속 충격파, 소성파, 탄성파로 변화된다. 이 연구는 발파압력파의 최대압력 도달시간 산정에 중점을 두었고 연계된 논문 I (the companion paper)에서는 최대 발파업력 산정에 중점을 두었다. 이 연구에서 최대압력 도달시간을 화약밀도, 단열지수, 폭광파속도, 감쇠지수, 동적항복강도, 소성파속도, 암반밀도, 탄성파속도, Hugoniot 상수의 함수식으로 유도하였다 최대합력 도달시간에 대한 매개변수분석 결과 암반특성치가 화약특성치보다 더 크게 영향을 미쳤다. 최대압력 도달시간의 확률분포는 화약과 암반 특성치의 확률분포로부터 Rosenblueth 확률모델로 조합하여 산출되었다. 화약과 암반특성의 불확정성이 발파진동의 불확정성에 미치는 영향을 수치해석으로 분석하였다. 불확정성 분석결괴 화약특성보다 암반특성의 불확정성이 발파진동에 더 크게 영향을 미쳤다. 수치해석 분석결괴 최대 발파양력과 최대양력 도달시간의 바인 하중재하율이 발파진동에 큰 영향을 미쳤다. 또한 화약특성보다 암반특성이 하중재하율에 더 크게 영향을 미쳤다.

      • 터발파압력에 기인한 이방성 암반손상의 수치해석적 분석

        Blast-induced anisotropic rock damage around a blast-hole was analyzed by a using numerical method with user-defined subroutine based on continuum damage mechanics. Anisotropic blasting pressure was evaluated by applying anisotropic ruck characteristics to analytical solution which is a function of explosive and rock properties. Anisotropic rock damage was evaluated by applying the proposed anisotropic blasting pressure. Blast-induced isotropic rock damage was also analyzed. User-defined subroutines to solve anisotropic and isotropic damage model were coded. Initial rock damages in natural ruck were considered in anisotropic and isotropic damage models. Blasting pressure and elastic modulus of rock were major influential parameters from parametric analysis results of isotropic rock damage. From the results of anisotropic rock damage analysis, blasting pressure was the most influential parameter. Anisotropic rock damage area in horizontal direction was approximately 34% larger and about 12% smaller in vertical direction comparing with isotropic rock damage area. Isotropic rock damage area under fully coupled charge condition was around 30 times larger than that under decoupled charge condition. Blasting pressure under fully coupled charge condition was estimated to be more than 10 times larger than that of decoupled charge condition. 발파압력이 발파공 주위 암반에 발생시키는 암반손상을 연속체 손상역학과 사용자 부프로그램을 이용한 수치해석을 통하여 분석하였다. 암반의 이방성 특성을 적용한 이방성 발파압력을 산출하였으며, 이를 이용하여 이방성 암반손상을 분석하였다. 또한 등방성 암반손상에 대해서도 분석하였다. 자연 암반이 가지고 있는 초기손상을 함께 고려하여 암반손상을 분석하였다. 등방성 암반손상의 매개변수 분석결과, 발파압력과 탄성계수가 암반손상에 가장 크게 영향을 미쳤다. 또한 이방성 암반손상의 매개변수 분석결과, 발파압력이 이방성 암반손상에 가장 크게 영향을 미쳤으나 탄생계수는 이방성 암반손상에 크게 영향을 미치지 않음을 알 수 있었다. 등방성과 이방성 암반손상을 등방성 암반손상과 비교한 결과, 이방성 암반의 수평방향 암반손상이 약 34% 증가 하였고 수직방향은 12% 감소하는 결과를 얻을 수 있었다. 등방성 암반에 대한 밀장전 조건과 디커플링장전 조건하에 발파로 인한 암반손상을 비교한 결과, 밀장전 조건의 암반손상이 디커플링장전 조건보다 약30배 정도 크게 나타났으며, 밀장전 조건의 발파압력은 디커플링장전 조건보다 10배 이상 큼을 알 수 있었다.

        밀장전한 암반 발파공에서 화약 폭발시 발생하는 고압의 폭굉압력 전파메카니즘을 충격파 이론을 적용하여 규명하고 전달된 발파압력 산정식을 유도하였다. 유도된 발파압력 산정식은 폭굉파속도, 단열지수, 화약밀도, Hugoniot 상수, 암반밀도의 함수였다. 에멀젼 화약과 서울 화강암의 특성시험을 시행하여 각 특성치의 확률분포를 정의하고 발파압력 산정식에 적용하여 발파압력의 확률분포를 산출하였다. 화약 특성치와 암반 특성치의 확률분포는 정규분포를 나타냈으며 따라서 발파압력의 확률분포도 정규분포로 추정되었다. 발파압력에 대한 매개변수분석을 시행한 결과 폭굉파속도가 발파압력에 가장 크게 영향을 미쳤다. 또한 이런 특성치의 불확실성이 발파압력의 불확실성에 미치는 영향을 분석하였다. 분석결과 암반특성치의 불확실성이 화약특성치보다 더 크게 영향을 미쳤다. 비록 매개변수분석에서 폭굉파속도가 발파압력에 가장 크게 영향을 미치는 요소이지만 암반특성치의 불확실성이 폭굉파속도의 불확실성보다 더 크기 때문에 발파압력은 후자보다 전자에 의해서 더 크게 영향을 받는다. The propagation mechanism of a detonation pressure with fully coupled charge is clarified and the blasting pressure propagated in rock mass is derived from the application of shock wave theory. The blasting pressure was a function of detonation velocity, isentropic exponent, explosive density, Hugoniot parameters, and rock density. Probabilistic distribution is obtained by using explosion tests on emulsion and rock property tests on granite in Seoul and then the probabilistic distribution of the blasting pressure is derived from the above mentioned properties. The probabilistic distributions of explosive properties and rock properties show a normal distribution so that the blasting pressure propagated in rock can be also regarded as a normal distribution. Parametric analysis was performed to pinpoint the most influential parameter that affects the blasting pressure and it was found that the detonation velocity is the most sensitive parameter. Moreover, uncertainty analysis was performed to figure out the effect of each parameter uncertainty on the uncertainty of blasting pressure. Its result showed that uncertainty of natural rock properties constitutes the main portion of blasting pressure uncertainty rather than that of explosive properties. In other words, since rock property uncertainty is much larger than detonation velocity uncertainty the blasting pressure uncertainty is more influenced by the former than by the latter even though the detonation velocity is found to be the most influencing parameter on the blasting pressure.

        국내에 보고된 한국 한의학의 암치료 임상연구에 대한 계통적 고찰

        Objectives : Recently, the number of cancer patients who use complementary and alternative medicine (CAM), including oriental medicine, are increasing. Also, evidence for efficacy and safety of cancer CAM is reported continuously. This study was designed to evaluate the quality of the published papers for clinical studies about oriental medical cancer treatment in Korea. Methods : We searched for clinical research papers on the websites of the Korean oriental medical society and subsidiary academies, Korean traditional knowledge portal. Koreanstudies information service system (KISS), etc. We used the reference words 'cancer', 'malignant tumor', 'leukemia', 'lymphoma', 'multiple myeloma', 'melanoma', etc, We then chose studies that had been presented between 1991 and 2008 and analyzed them by publication year, design, cancer type, intervention, subject and journal. Results : There were 86 clinical research papers that were published between 1991 and 2008. The number of clinical studies has increased in a time-dependent manner. Case reports and case series studies account for 78% of total clinical research. Studies of lung cancer account for 17%. Studies of cancer-related symptoms of cancer patients account for 38%. Studies of Hang-Am-Dan account for 33%. Studies published in the journal of the Korean Society of Oriental Internal Medicine account for 35%. Conclusion : The clinical research papers about oriental medicine are increasing over time, but most of the studies are retrospective case reports or case series. Well-designed and prospective clinical trials would be recommended to provide more reliable evidence.

        경상북도 동해안지역의 B 형간염 표면항원 (表面抗原) 보유율에 관한 연구

        N/A To investigate the distribution of hepatitis B surface antigen (HBsAg) in the Pohang city and its neighbourhood area, surveyed 1334 persons who visited hospital general health screening center for check up. The study was performed by RPHA (693 cases) and EIA method (641 cases) with sera obtained. 1) EIBsAg was detected in 108 persons with the positive rate of 8.1% 2) The positive rate among females (10.55%) was greater than that of males (6.22% ) (p<0.005). 3) The age distribution of occurrence rate was found between 20 and 79 years of age ancl the highest rate was 70 to 79 age group(p<0.01).

        한방치료로 호전된 섬망을 동반한 좌측 중대뇌동맥경색 환자의 치험 1례

        Objectives: The purpose of this case study is to describe the effect of Korean medicine on a patient with delirium caused by a left middle cerebral artery infarct. Methods: The patient was treated with Sopungbosimdodam-tang, acupuncture, and moxibustion. The Korean version of the Delirium Rating Scale (K-DRS), Neelon and Champagne (NEECHAM) Confusion Scale, and Korean Version of the Mini-Mental State Examination (MMSE-K) were used to evaluate the effect of the treatment. Results: After 50 days of treatment with Sopungbosimdodam-tang, the patient's K-DRS score decreased from 16 to 8. The NEECHAM Confusion Scale also improved from 17 to 23, and the MMSE-K score improved from 1 to 7. Conclusion: This clinical study suggested that Korean medicine could contribute greatly to the treatment of delirium caused by a left middle cerebral artery infarct.

        본 논문은 l㎾급 수직축 다극형 풍력발전용 인버터 설계에 관한 연구이다. Class-D Type의 인버터 사용하여 기존의 인버터보다 비교하여 인버터의 효율 및 고조파 특성이 좋은 동작하는 전압형 모드의 인버터를 설계하였다. 삼각파 제어신호를 통해 Opamp를 사용하여 Sine Wav e와 50㎑의 Triangle Wave를 비교하여 IR2110을 이용 부스트 스트랩핑 하여 H-Bridge에 입력하여, LC필터를 통해 220V정현파의 출력을 만들었다.

        위 아니사키스증 12 예에 대한 임상적 고찰

        N/A We studied 12 cases of gastric anisakiasis who contacted the Dept. of Internal Medicine of St. Mary's Hospital and Sun Rin Hospital, Pohang, from March 1986 to Feb. 1989 and were confirmed by gastrofiberscopic findings and removal of larva of Anisakis. Our analysis gave the following results: 1) Of 12 cases, the sex ratio was 1 to 1,4 (male to female) and the 40s age group was most common, followed by the 30s, the 20s and 60s, in order of frequency. 2) In terms of seasonal incidence, May to July showed 6 cases (50%). Among causative fish, Astroconcer myriaster caused 6 cases (50%) and showed highest freguency. 3) The most common symptoms when the patients contacted the hospital were epigastric pain in 9 cases (75%) followed by nausea, epigastric fullness, and vomiting in order of frequency. 4) The onset of symptoms in 8 cases (66.7%) was within 6 hours since ingestion of those fish. 5) Six cases (50%) underwent gastrofiberscopic exam within 24 hours of onset of symptoms. 6) The most commonly involved site of the stomach was the anterior wall of the mid body (4 cases). The gastrofiberscopic findings were, in frequency of incidence, normal mucosa with larva (5 cases), erosion, hyperemia, mucosal thickening, petechia and ulceration. 7) We performed a biopsy in 4 among 12 cases, and the microscopic findings of the gastric mucosa were congestion and edema, broadening of the foveolar ridge and inflammatory cell infiltration, especially eosinophils and lymphocytes. The antral mucosa revealed findings of chronic superficial gastritis and small fragmental necrotic debris was seen along the surface of the mucosa. Early gastroscopic examination may be very useful in the diagnosis and treatment of anisakiasis if anisakiasis is clinically suspected, In addition, the general public should be enlightened that ocean fish are not necessarily safe when they are eaten a raw and a more detailed epidemiologic study should be conducted for anisakiasis.
