        담훈(痰暈)환자의 임상적 고찰과 복부 적외선 체열촬영의 의의

        To make a objective diagnosis of Dam Hun(痰暈), we investigated the 63 patients with Dam Hun(痰暈) and 34 normal people as control group. we compared the thermal difference between Chon-jung(CV17 ) and Chung-wan(CV12 中脘), Chon-jung(CV17 ) and Gwan-won(CV4 關元) and we compared the thermal difference of Yang-mun(ST21 梁門), Chun_chu(ST25 天樞), Su-do(ST28 水道), too. All 63 patients had dizziness and GI trouble. They had the symptom - fatigue, nausea vomitting, head heaviness, indigestion, cold limbs, headache, heart burn, constipation, diarrhea, anorexia. And some had edema, numbness, insomnia, palpitation, facial flushing, dysuria, ringing. Considering the average temparature of Chon-jung(CV17 ), Chung-wan(CV12 中脘), Gwan-won(CV4 關元), Yang-mun(ST21 梁門), Chun-chu(ST25 天樞), Su-do(ST28 水道) between the patients group and the control group, the patients group were lower than the control group except for Chon-jung(CV17 ), Gwan-won(CV4 關元), but the statistical significance was not. Considering the thermal difference of Chon-jung(CV17 ) and Chung-wan(CV12 中脘), Gwan-won(CV4 關元), Yang-mun(ST21 梁門), Chun-chu(ST25 天樞), Su-do(ST28 氷道) between the patients group and the control group, the temparature deviation of Chon-jung(CV17 ) and Chung-wan(CV12 中脘), Chon-jung(CV17 ) and Yang-mun(ST21 梁門) was significant. We suggest that coldness of upper abdomen is significant in diagnosis of Dam Hun(痰暈).

        In oriental medicine, Byun-jo(煩躁) is including in insomnia, restless, uneasiness, chest discomfort, irritation et all. Byun-jo is one of symptom in Soyangin(少陽人)(one of human type in Sasang constitutional medicine(四象醫學) the lower chest-overheated disease(胸膈熱證). In the book 'dongyi soose bowon(東醫壽世保元)', Soyangin Yanggueksanwha-tang(凉膈散火湯) is used at Soyangin the lower chest-overheated disease. One patient usually who had suffered from chest discomfort, insomnia of Byun-jo, after cerebral infarction the symptom of Byun-jo had grown worse, the patient was classified as Soyangin by somatotype, personality, therefore the patient treated with Soyangin Yanggueksanwha-tang for 10days, and the patient improved in clinical symptoms. so we report it for the better treatment.

        Back pains include pain in one or both side costa portion and lateral abdomen. There are different kinds of flank-related disease such as intercostal nerve pain, chronic gastritis, duodenitis, herpes zoster, pepticulcer, cholecystitis, cholelithiasis and so on. Back pain arises from pathological abnormal activities. On the other hand psychogenic pains are caused by psychogenic factors without pathological abnormal activities. This is similar to KiWol back pain, which resulted from psychogenic factors like stress, anxiety, anger and etc. In oriental medicine, KiWol is the state of depression of Ki, and Shihosogansan used to treat the back pain diagnosed as stagnation of the liver Ki. So, we decided to apply Shihosogansan to a patient who suffered from back pain diagnosed as stagnation of the liver Ki. Therefore the patient treated with Shihosogansan and improved in consciousness symptoms, so we report it for the better treatment.

        마황부자세신탕(麻黃附子細辛湯)의 마우스 단회 경구투여 독성 및 골수세포를 이용한 유전독성 평가

        The object of this study was to obtain acute information single oral dose toxicity of Mahwangbujaseshin-tang extracts, with mouse bone marrow cell micronucleus test for detecting possible genotoxicity. In order to observe the 50% lethal dose, approximate lethal dosage, maximum tolerance dosage and target organs, test articles were once orally administered to ICR mice at dose levels of 2000, 1000, 50 mg/kg according to the recommendation of KFDA Guidelines. The mortality and changes on body weight, clinical signs and gross observation were monitored during 14 days after dosing according to KFDA Guidelines with organ weights of 12 types of principle organs. In addition, after twice oral treatment of Mahwangbujaseshin-tang extracts 2000, 1000 and 500 mg/kg, we checked the changes on the number of MNPCE. We could not find any mortality, clinical signs, changes in the body weight and gross findings upto 2000 mg/kg treated group. The limited dosages in rodents except for increases of lymphoid organ weights and hypertrophy encounted as results from pharmacological effects of Mahwangbujaseshin-tang extracts, immune modulator effects with some sporadic accidental findings not toxicological signs. No evidence of increases of MNPCE numbers were also detected in all three different dosages of Mahwangbujaseshin-tang extracts treated mice. The results obtained in this study suggest that the LD50 and ALD of Mahwangbujaseshin-tang extracts in mice were considered as over 2000 mg/kg because no mortalities were detected upto 2000 mg/kg that was the highest dose recommended by KFDA and OECD. And the results of mouse bone marrow micronucleus test of Mahwangbujaseshin-tang extracts is negative results.

        마황윤폐탕(麻黃潤肺湯) 추출물의 수컷 SD Rats에서 경구 단회투여 독성 평가

        Objectives & Methods : To obtain the 50% lethal dose(LD50), approximated lethal dose(ALD) and approximated target organs of 'Mahwangyounpae-tang' for further study into such things as repeated dose toxicity, genotoxicity and reproductive toxicity, single dose toxicity was tested in male SD rats according to KFDA Guideline 1999-61[KFDA, 1999] at dosage levels of 2,000, 1,000, 500, 250 and 125 mg/kg/$10m{\ell}$. In this study, mortalities, clinical signs, body weight changes and body weight gains, gross findings and weight of principal organs were detected during and/or after 14 days of single dosing. Results & Conclusions : After 2 or 3 days of dosing, 1 or 2 animals in 2,000 mg/kg-dosing groups died. Excitation and leaping response were observed as test article-treatment related clinical signs. These abnormal signs were restricted to 2,000 and 1,000 mg/kg-dosing groups and survivors recovered to normal within 3 or 4 days after dosing. Significant decrease in body weight were observed in some periods of observation in 2,000 and 1,000 mg/kg-dosing group, from 1 days after dosing compared to those of vehicle control group. Significantly diminished body weight gains were observed in observation periods in 2,000 and 1,000 mg/kg-dosing group compared to those of vehicle control group. Hypertrophy and hemorrhage of heart and decoloration of kidney were observed as test article-treatment related gross findings. These abnormal findings were restricted to 2,000 and 1,000 mg/kg-dosing groups. A significant increase of absolute and relative heart and kidney weight were demonstrated in 2,000 mg/kg-dosing groups. The value for LD50 found in this study was 2,218.57 mg/kg. ALD in this study was 2,000 mg/kg, and the target organs are considered to be the heart and the kidney.

        금은화(金銀花)추출물의 마우스 단회 경구투여 독성실험

        Objectives : The object of this study was to obtain accurate information (single oral dose toxicity) of Lonicerae Flos (LF; Dried flower bud parts of Lonicera japonica Thunb (Caprifoliaceae)), which has traditionally been used in Korean medicine for treating various inflammatory diseases. Methods : In order to observe the 50% lethal dose (LD 50), approximate lethal dosage (ALD) and target organs, test articles were once orally administered to female and male ICR mice at dose levels of 2,000, 1,000, 500 and 0 (control) mg/kg (body weight.). The mortality and changes on body weight, clinical signs and gross observation were monitored for 14 days after single oral treatment of LF aqueous extracts with organ weights and histopathological observations of 12 types of principle organs. Results : 1. After single oral treatment of LF aqueous extracts, we could not find any mortality and toxicological evidences up to 2,000 mg/kg treated group, the limited dosages in rodents at body and organ weights, clinical signs, gross and histopathological observations. 2. Slight diarrhea was detected in most mice treated with 2,000 mg/kg of LF aqueous extracts and male mice of LF aqueous extracts 1,000 mg/kg within 2 days after end of treatment, respectively. Conclusion : The results obtained in this study suggest that the LD 50 and ALD of LF aqueous extracts in both female and male mice after single oral treatment were considered as over 2,000 mg/kg because no mortalities were detected up to 2000 mg/kg, the highest dose recommended by KFDA and OECD. However, we also observed the possibility of digestive disorders like diarrhea when over 1,000 mg/kg of LF aqueous extracts were administered in the present study.

        금은화가 LPS로 유발된 급성 폐 손상에 미치는 영향

        Objective : The object of this study was to observe the effects of Lonicerae Flos (LF) aqueous extracts on lipopolysaccharide (LPS)-induced rat acute lung injury. Method : Five different dosages of LF extracts were orally administered once a day for 28 days before LPS treatments, and then all rats were sacrificed after 5 hour-treatment of LPS. Eight groups of 16 rats each were used in the present study. The following parameters caused by LPS treatment were observed ; body weights, lung weights, pulmonary transcapillary albumin transit, arterial gas parameters (pH, $PaO_2$ and $PaCO_2$) bronchoalveolar lavage fluid (BALF) protein lactate dehydrogenase (LDH), and proinflammatory cytokines tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukin-$1{\beta}$ (IL-$1{\beta}$) contents, total cell numbers, neutrophil and alveolar macrophage ratios, lung malondialdehyde (MDA), myeloperoxidase (MPO), proinflammatory cytokines TNF-${\alpha}$ and IL-$1{\beta}$ contents. In addition, the histopathologic changes were observed in the lung in terms of luminal surface of alveolus, thickness of alveolar septum, number of polymorphonuclear neutrophils. Result : As results of LPS-injection, dramatical increases in lung weights, pulmonary transcapillary albumin transit increases, increases in $PaCO_2$, decreases in pH of arterial blood and $PaO_2$, increases of BALF protein, LDH, TNF-${\alpha}$ and IL-$1{\beta}$ contents, total cells, neutrophil and alveolar macrophage ratios, TNF-${\alpha}$ and IL-$1{\beta}$ contents increases were detected with decreases in LSA and increases of alveolar septum and PMNs numbers, respectively as compared with intact control. These are means that acute lung injuries (resembling acute respiratory distress syndrome) are induced by treatment of LPS mediated by inflammatory responses, oxidative stress and related lipid peroxidation in the present study. However, these LPS-induced acute lung injuries were inhibited by 28 days continuous pretreatment of 250 and 500mg/kg of LF extracts. Because of lower three dosages of LF treated groups, 31.25 and 62.5 and 125mg/kg did not showed any favorable effects as compared with LPS control, the effective dosages of LF in LPS-induced acute lung injuries in the present study, is considered as about 125mg/kg. The effects of 250mg/kg of LF extracts showed almost similar effects with ${\alpha}$-lipoic acid 60mg/kg in preventing LPS-induced acute lung injuries. Conclusion : It seems that LF play a role in protecting the acute respiratory distress syndrome caused by LPS.

        한양방협진 및 황기별갑탕으로 호전된 폐염(肺炎)환자 1례 보고

        This is clinical report of one patient with pneumonia in stroke who improved with the administration of an herb drug. During treatment for stroke, he broke out with pneumonia, a complication of stroke, which was confirmed by chest radiography. He was administrated Hwangkibyulgap-tang with antibiotics because of old age middle stage pneumonia. After administration, the symptoms improved and chest x-ray film showed no pneumonic finding. This result suggests that oriental medicine is an effective treatment for pneumonia but more clinical case reports are needed.
