        코경유 눈물주머니코안연결술에 사용되는 드릴의 비교

        Purpose: To evaluate and compare two types of microdrills that are used in endonasal dacryocystorhinostomy. Methods: We retrospectively analyzed medical records and video recordings of 51 patients with 65 affected eyes who underwent endonasal dacryocystorhinostomy for treatment of chronic dacryocystitis or primary acquired nasolacrimal duct obstruction at our hospital between 2005 and 2007. All procedures were performed by the same surgeon. The patients were randomly divided into two groups. For patients in group 1 the surgeon used the Diego powered dissector (GyrusⓇ), while patients in group 2 were treated using the Ossepro (Bein AirⓇ) microdrill. Results: The operation success rate of group 1 was 96.6% and of group 2 was 94.2%. This difference was not statistically significant (p>0.05). The mean operation time was longer in group 1 (45.6 min) than in group 2 (65.8 min). These values, along with the mean drill usage times for each group, were significantly different (p=0.03). The mean revolution per minute(RPMs)or the two groups were also significantly different (p=0.05). Conclusions: Our results suggest that microdrills used in endonasal dacryocystorhinostomy should have protective shields to minimize mucosal damage, and employ rapid RPMs to efficiently produce bony openings in the thick anterior processes of the maxilla. The tips of microdrills should also be exposed, to better visualize and acquire good operating fields. J Korean Ophthalmol Soc 2010;51(4):479-484

        안와연조직염 환자에서 생긴 전두골경막외 농양 1예

        Purpose: To report a case of simultaneous orbital cellulitis and intracranial complications of acute sinusitis in a young patient. Case summary: The 11-year-old male presented with a one-day history of left periorbital erythematous swelling, fever and headache. He was diagnosed with acute sinusitis at the department of otorhinolaryngology, and endoscopic sinus drainage of pus was performed. However, the patient’s symptoms did not improve. The next day, a subdural abscess was found on brain computed tomography. He was treated by external drainage of the periosteal abscess via a subbrow incision and systemic antibiotics, as well as anticonvulsant medication. Conclusions: Twenty days after external drainage and beginning systemic antibiotics and anticonvulsant treatment, he was discharged with improved orbital cellulitis symptoms and a resolved subdural abscess

        상아모세포의 조건배지를 이용한 백악모세포의 분화와 석회화 조절

        For the regeneration of periodontal tissues, the microenvironment for new attachment of connective tissue fibers should be provided, At this point of view, cementum formation in root surface plays a key role for this new attachment. This study was performed to figure out which factor promotes differentiation of cementoblast Considering anatomical structure of tooth, we selected the cells which may affect the differentiation of cementoblast - Ameloblast, OD11&MDPC23 for odontoblasts, NIH3T3 for fibroblsts and MG63 for osteoblasts. And OCCM30 was selected for cementoblast cell line. Then, the cell lines were cultured respectively and transferred the conditioned media to OCCM30. To evaluate the result, Alizarin red S stain was proceeded for evaluation of mineralization. The subjected mRNA genes are bone sialoprotein(BSP), alkaline phosphate(ALP) , osteocalcin(OC), type I collagen(Col I), osteonectin(SPARC ; secreted protein acidic and rich in cysteine). Expression of the gene were analysed by RT-PCR, The results were as follows: 1. For alizarin red S staining, control OCCM30 didn't show any mineralized red nodules until 14 days. But red nodules started to appear from about 4 days in MDPC-OCCM30 & OD11-OCCM30. 2. For results of RT-PCR, ESP mRNAs of control-OCCM30 and others were expressed from 14 days, but in MDPC23-OCCM30 & OD11-OCCM30 from 4 days. Like this, the gene expression of MDPC23-OCCM30 & OD11-OCCM30 were detected much earlier than others. 3. For confirmation of odontoblast effect on cementoblast, conditioned media of osteoblasts(MG63) which is mineralized by producing matrix vesicles didn't affect on the mineralized nodule formation of cementoblasts(OCCM30). This suggest the possibility that cementoblast mineralization is regulated by specific factor in dentin matrix protein rather than matrix vesicles. Therefore, we proved that the dentin/odontoblast promotes differentiation/mineralization of cementoblasts. This new approach might hole promise as diverse possibilities for the regeneration of tissues after periodontal disease.

        정상안 및 황반 질환에서 스펙트럼 영역 빛간섭단층촬영(3D-OCT 1000)의 황반 두께 및 부피 반복성

        Purpose: To evaluate the repeatability of macular thickness and total macular volume measurements made using spectral domain optical coherence tomography (OCT) in normal subjects and subjects with macular disease. Methods: Among a total of 108 subjects, there were 50 normal subjucts, 20 patients with diabetic macular edema, 10 patients with retinal vein occlusion, 15 patients with age-related macular degeneration, and 13 subjects with other conditions. Two serial macular measurements were obtained from each subject by a single experienced examiner using spectral domain OCT. The repeatability of the measurements was evaluated by comparing two consecutive foveal and perifoveal thickness measurements and total macular volume measurements. The intraclass correlation coefficient was also calculated to evaluate the repeatability of measurements made in normal and macular disease subjects. Result: Spectral domain OCT measurements of macular thickness and macular volume were found to be consistent. Measurements of normal subjects were the most consistent, followed by measurements of patients with age-related macular degeneration, retinal vein occlusion, and diabetic macular edema. Conclusions: Although measurements made using spectral domain OCT were repeatable across all subjects, they were more consistent in normal subjects than in patients with macular disease. The differences in repeatability should be considered in the context of diseased pathologic anatomy. Physicians should remain cautious when using these measurements for clinical evaluation.       • 강원도 18개 시·군의 도시형태 특성 - 시가지 지역의 도시조직을 중심으로 -

        This study surveyed and analyzed the characteristics of urban form by studying eighteen cities and counties of the Gangwon area that can be categorized as small and medium-sized cities, particularly focusing on urbanized areas. In particular, to conduct this study, numerical map analysis of relevant areas, Figure & Ground work, and analysis using the Arc-Gis program were implemented and field surveys were also carried out. The deduced conclusions are as follows. First, physical characteristics that follow the generative processes of urbanized areas of cities and counties could be verified. With the organization of urbanized areas after liberation, most urbanized areas showed characteristics categorized by fixed norms and the differentiated environmental characteristics of some areas have become fixed as spatial environmental characteristics of entire regions. Second, city unit administrative districts have relatively high concentrations of function centered on urbanized areas and while the density of facilities that show urbanism are being maintained, spatial ranges of urbanized areas are limited in cases of county unit administrative regions and the functions and densities of central streets and facilities appear limitedly. Third, city unit administrative regions and county unit administrative regions showed respectively high urbanized area density but analysis showed that there were great differences of mutual degrees. Such characteristics are grounded upon perspectives that observe cities in periods of high growth and verification was also made of the need for urban management that is essential in periods of low growth.

        선진 기업의 생산

        우리기업들은 물류합리화의 중요성은 인식하면서도 대개의 경우 이를 위한 노력은 단면적 수준에 그치고 있는 실정이다. 따라서 기업들이 국제경쟁력을 유지하기 위하여 필요한 전체적이고 일관적 시각에서의 물류시스템혁신 방안에 대한 연구가 절실히 요망되고 있다. 본 논문에서는 물류의사결정지원을 위한 실용적 모형의 개발노력을 부분적 합리화 모형, 일관적 합리화 모형, 그리고 국제물류 합리화 모형의 순으로 체계적으로 정리해 보았다. 다음으로, 물류모형이 갖추어야 할 바람직한 특성들에 비추어 기존 모형들의 장단점을 분석하고, 이에 근거하여 향후의 모형개발 노력방향을 제시하였다.

        일감 몰아주기 증여세 과세제도에 대한 위헌성과 입법미비에 대한 검토

        최근 입법된 상속세 및 증여세법 상 일감몰아주기 세제가 논란이 되고 있다. 동세제는 아직 과세가 이루어지기도 전부터 여러 가지 문제점과 비판을 받고 있는데, 그 가운데 아직까지 심도 있는 논의가 이루어지고 있지 않은 위헌성에 대하여 검토해보고자 한다. 일감몰아주기 세제가 위헌이라고 주장하는 근거는 미실현 이익에 대한 과세, 이중과세, 계약자유의 원칙 침해 등 다양한 이유이다. 이에 미실현 이익에 대한 과세, 이중과세, 계약자유의 원칙 침해 등에 대하여 위헌성이 존재하는지 여부를 검토할 필요가 있으며, 과잉금지원칙 위배에 대하여 세부적으로 논의해 볼 필요가 있다. 일감몰아주기 세제는 현재까지의 헌법재판소 입장에 따를 때 반드시 위헌이라고 보기는 어려운 부분도 있으나 그 가능성은 일정부분 존재하므로 추후 명확한 헌법재판소의 입장을 기다려 볼 필요성이 있다. 그러나 일감몰아주기 세제가 입법상 미흡하다고 생각되는 부분도 상당히 존재한다. 일감몰아주기 증여세는 30%라는 매출 비중으로 그 적용을 판단하고 있으나 업종을 고려하여 대통령령으로 정하라는 비율을 30%라는 일률적인 비중으로 정하고 있다는 점은 입법에 있어서 아쉬운 부분이다. 근본적으로도 그 과세대상이 주식가치 증가분이 아닌 세후영업이익과 관련하여 계산한 가액이라는 점은 객관적인 세액 산정이 어렵다는 문제점도 존재한다. 일감몰아주기와 관련하여 기업집단 외의 기업과 거래를 하는 경우에는 그 적용을 제외시키는 규정을 둔 것은 기업집단에 해당할 수 없는 기업들에게 입법취지와 반대로 역차별을 하는 것으로서 이에 대한 보완적인 입법이 있어야 할 것으로 보인다. 또한, 거래에 부득이한 사정이나 정당한 사유가 존재하는 경우까지 아무런 제한이 없이 과세를 하는 것도 입법적으로 보완되어야 할 부분이라고 본다. Funneling of Business (‘Il-Gam-Mol-Ah-Joo-Gi’) in the Inheritance Tax and Gift Tax Act is becoming an issue lately. This particular taxation system is being criticized even before it is enforced, and the not-yet discussed unconstitutionality of it shall be reviewed in depth. The reasoning behind asserting unconstitutionality of Funneling of Business arises from the imposition of tax on unrealized profit, double taxation, and violation on the liberty of contract, among many others. We need to review the potential unconstitutionality of imposition of tax on unrealized profit, double taxation and violation on the liberty of contract, etc, while discussing in detail on violation of the Less Restrictive Alternative (“LRA”). It is premature to define the Funneling of Business as unconstitutional according to the perspective of the Constitutional Court, but it is worth to wait for the ruling on this issue, because possibilities of unconstitutionality exist. However, Funneling of Business may be considered as incomplete from a legislative perspective. 30% of sales is applied in the gift tax of Funneling of Business, but the application of uniformed 30% could have been more flexible when industry difference is to be considered according to the Presidential decree. Difficulty in calculating the objective tax amount also exists when the taxable object is not the increased amount of stock value, but the operating profit after tax. It also requires for a complementary legislation to avoid the reverse discrimination of companies that are not in the conglomerate, when the regulation shall not be applied to the companies outside of conglomerate. Furthermore, the regulation needs to be revised setting the exceptions for application in cases of unexpected circumstances and/or cases with fair and just reasons.

        포스트 팬데믹 시대 드론 물류배송을 위한 지하철 역사의 활용방안

        COVID-19로 인해 사람들은 외출과 모임 자제로 온라인 쇼핑, 온라인 여행, 화상회의 등 새로운 라이프 스타일을 구축하고 있다. 이러한 급격한 사회의 변화는 새로운 문제들을 야기시킴과 동시에 기존의 문제들을 심화시키고 있다. 특히 온라인 소비가 크게 증가함에 따라 생활물류산업에서는 교통체증, 대기오염, 배송기사의 과중한 업무 등이 심화되고 있으며 대면배송이 새로운 문제점으로 나타나고 있다. 4차 산업혁명의 등장으로 드론에 인공지능, 자율주행 등을 활용한 무인 배달이 기존 생활물류산업의 대체산업으로 떠오르고 있으나 드론의 비행에 따른 물류시설의 공간과 추가적인 물류부지 확보가 해결되어야 하는 새로운 문제점으로 나타난다. 따라서, 기존의 문제들과 새롭게 생겨난 문제들을 해소할 수 있는 드론을 위한 물류시설의 공간으로 기존 운수 사업인 지하철을 활용하여 물류 이동과 보관, 배송 등과 같은 부가적인 서비스를 연계하고자 한다. Due to COVID-19, people are building new lifestyles such as online shopping, online travel, and video conferencing by limiting going out and gatherings. Such rapid social change is causing new problems and deepening existing problems at the same time. In particular, as online consumption increases significantly, traffic congestion, air pollution, and the heavy workload of delivery drivers are deepening in the daily logistics industry, and face-to-face delivery is emerging as a new problem. With the advent of the 4th industrial revolution, unmanned delivery using drones, artificial intelligence, and autonomous driving is emerging as an alternative to the existing logistics industry. However, space for logistics facilities and securing additional logistics sites due to drone flight are emerging as new problems to be solved. Therefore, it is intended to link additional services such as logistics movement, storage, and delivery by utilizing the existing transportation business, the subway, as a space for a logistics facility for drones that can solve existing problems and new problems.

        신장(腎臟) 조직(組織)에서 제조의 항산화(抗酸化) 효과(效果)의 기전(機轉) 연구(硏究)

        This study was carried out to examine mechanisms by which Holotrichia (HTC) produces protective effect against renal cell injury. HTC extraction (5%) prevented $H_2O_2(50mM)$-induced LDH release and lipid peroxidation in renal cortical slices. When slices were treated with 5% HTC extraction, cellular glutathione content and the superoxide dismutase activity were not altered in control and $H_2O_2$-treated tissues. When slices were treated with 50 mM $H_2O_2$, the catalase activity was significantly inhibited, which was completely restored by addition of 5% HTC. Treatment of slices with 5% HTC extraction increased the glutation peroxidase activity in $H_2O_2$-treated tissues. These results suggest that HTC prevents oxidant-induced cell injury and lipid peroxidation by increasing the activities of catalase and glutathione peroxidase in renal cortical slices.
