        일산 소재 초등학생의 굴절이상도와 비구면계수 관계

        Purpose: To determine the relationships among refractive error, corneal asphericity, and axial length in elementary school students. Methods: One hundred eleven eyes from 56 subjects were included in this study. All subjects underwent cycloplegic refraction corrected to the spherical equivalent. Axial length was measured, and corneal topography was performed. Corneal asphericity was assessed using eccentricity (e) calculated according to the formula Q=-e2. The relationship among spherical equivalent, asphericity, and axial length was determined using a linear regression model. Results: Subjects were between 8 and 12 years of age (mean, 9.99±1.33). The average spherical equivalent was -1.38±2.08D (-7.13~2.63D), the average axial length was 23.84±1.17 mm (20.10~26.37 mm), and the average corneal asphericity was -0.29±0.10 (-0.55~-0.07). An increase in myopia was positively correlated with an increase in axial length (p<0.0001). The degree of myopia was negatively associated with corneal asphericity (p=0.019). An increase in axial length was related to an increase of negativity in asphericity (p= 0.012). Conclusions: An increase in myopia was correlated with an increase in axial length. As the degree of myopia and axial length increased, corneal asphericity became more prolate. A longitudinal study with more subjects is required to validate these results.         In this study, the results were that the duty of optician was analysed with DACUM(Developing A Curriculum Method). Task/Skill Matrix was formed and selected the elements above 2.15 points with the job description. The job specification made out the job description and the preference survey for the optician with the job model. These elements made a draft of drawing up standard of the Optician National Licensing Examination by Job Analysis.

      • 콘택트렌즈 착용자에서 각막곡률과 TBUT의 변화

        RGP 및 Soft 콘택트렌즈 착용자 28명을 대상으로 콘택트렌즈 착용 후 1년 동안 각막곡률과 굴절이상도 및 눈물의 변화도를 알아보았다. 각막곡률과 굴절이상도에 대한 데이터분석은 디옵터굴절력을 디옵터굴절력매트릭스로 변환시키고 단변량 및 다변량 통계분석을 하고 3차원 h벡터 공간에 신뢰영역을 도시하였다. 각막곡률의 평균변화도는 RGP 착용군에서 주로 h_(3) 성분이 플랫하게 변화되며 1년후에는 수평성분도 플랫하게 변화되었고, Soft 착용군에서는 착용초기 플랫하게 변하다가 1년후에는 약간 스티프해지는 변화를 보였으나, 착용전후 각막곡률의 평균에 대한 3변량 가설검증 결과 두 그룹 모두 착용전과 비교하여 차이가 없는 것으로 나타났다(w<F_(0.05. 3. n-3)). 굴절이상도의 평균변화도는 RGP착용군에서 h_(1) 및 h_(3) 성분이 착용전과 비교하여 감소한 것으로 나타났으나 (p<0.05), Soft 착용군에서는 유의한 차이가 없었으며, 3변량 가설검증 결과 두 그룹 모두 착용전과 비교하여 차이가 없는 것으로 나타났다(w<F_(0.05. 3. n-3)). 각막곡률의 변화와 굴절이상도의 변화는 RGP 착용군에서 h_(2) 성분만 낮은 상관관계를 보였다(p<0.05). 비침입성 눈물막 파괴시간은 침입성 눈물막 파괴시간보다 평균값에서 길게 측정 되었으나 통계학적으로 유의한 차이는 나타나지 않았다. 눈물막 파괴시간을 측정하는 TBUT 방법과 NIBUT 방법은 서로 상관관계가 없는 것으로 나타났다. 콘택트렌즈 l년 착용군의 TBUT 검사에 의한 눈물막 파괴시간은 대조군에 비하여 감소하였으며 통계학적으로도 유의한 차이가 나타났다 (p<0.05). 따라서 RGP 렌즈를 장기간 착용하게 되면 각막굴절력 및 각막난시도의 감소와 굴절이상도의 구면성분 및 원주성분이 감소될 것이며, 콘택트렌즈 착용에 의하여 눈물이 불안정해진다고 생각된다. 28 patients wearing RGP and soft contact lenses were studied for one year. Data analysis of variation in corneal curvature and refractive error was carried out using matrix representations of conventional dioptric power. Confidence regions for mean dioptric power were represented by principal axes in a 3 - D space called h - space. Variations in the corneal curvature of RGP Group did show a significant decrease (h_(3) component). but those of Soft Group showed irregular change of intial flattening and minor steepening during experimental period. Variations in refractive error were significant decrease in both h1 component and h₃component of RGP Group. However, no significant changes were found in either corneal curvature and refractive error by the multi-variate statistics. NIBUT values were slightly higher than the TBUT values in average. but there was no difference statistically in control eyes. No significant correlation was found in either TBUT or NIBUT values. The stability of the precorneal tear film after 1year of wear was slightly lower than those of the precorneal tear film before lens insertion in the values of TBUT.

      • 한국인에서 표준모형안의 설계

        The purpose of our study was modelling of schematic eye in Korean. Subjects for the study were 105 students(average 23.5 years) in department of ophthalmic optics of Seoul Health College. Ocular components were measured by Orbscan topography system. ultrasonic pachymetry, and A-scan ultrasonography under cycloplegic condition. Crystalline lens radii were calculated by Bennett's method. Prior to obtain the data for modelling of schematic eye in Korean, we were assess the various methods for measurements of principal optical dimensions. Equivalent powers and principal points of schematic eye were calculated using the Gaussian paraxial equation. Refractive indices of intraocular media were used values in Gullstrand - Emseley schematic eye. Korean four-surface schematic eye modelled by authors was significant different in Gullstrand - Emsley three-surface schematic eye. That is emmetropic eyes of Korean were flatter anterior radius of cornea, steeper posterior radius of cornea, weaker equivalent corneal power, thinner lens thickness, weaker equivalent lens power, shorter axial length and stronger equivalent power of the eye than Gullstrand - Emsley schematic eye.

        We investigated the optical characteristics of a bi-convex lens using interferometric method. A Twyman-Green interferometer has been constructed for assessment of the lens. As a coherent light, a spatiallyfiltered and collimated He-Ne laser beam was employed. The fringe, formed by interference, was detected by a CCD camera with spatial resolution, 10m and displayed by a multi-synchronizing CRT monitor. The intensity distribution of the interferogram was digitized by a image card, and processed and analyzed by a personal computer. The focal point and power of a lens were determined with the accuracy comparable to the light wavelength. The astigmatic aberration and a fine mechanical defect on the surface of the lens under test were also observed.

        屈折異常矯正眼鏡의 프리즘效果에 관한 調査

        A survey of dispensing error was conducted among 159 students who wearing the ametropic corrective spectacles in Seoul health junior college. The results were summarized as follows; 1. Among the 159 spectacles wearers, the cases of dispensing by the hospital prescrip tions were 35 students and the cases of dispensing by the optical shop prescriptions were 124 students. 2. The cases of coincidence between visual axis and optical center in regard to monocular were 46 eyes (14.6%), and the cases of coincidence which the height of optical centers in right and left lenses were 56 students. 3. In 188 eyes (59.1%), the horizontal prismatic effect in monocular were less than 0.50 prism diopter, and showed 70 significant difference between the hospital prescription group and the optical prescription group (p>0.01). 4. In respect to horizontal induced phoria, the cases of induced exophoria were 74 students whereas the cases of induced esophoria were 77 students. Both induced esophoria group and induced exophoria group showed no significant difference between the hospital prescription group and the optical shop prescription group, respectively(p>0.01). But the groups between induced exophoria and induced esophoria showed significant difference (p<0.01). 5. In 143 students (89.9%), vertical induced phoria less than 0.50 prism diopter, and the group between hospital prescription and optical shop prescription showed no significant difference (p>0.01). 6. On the basis of centration which of RAL-RG 915 in Germany Standards, improper spectacles wearers were 88 students (55.3%). Among the improper spectacles wearers, the cases of the cause by the horizontal prismatic effect were 39 students, its by the vertical imbalanced prismatic effect were 27 students and its by the both as above were 22 students. From the above results, establishment of the Korean Standards for ametropic corrective spectacles' dispensing errors is requested in order to supply the ametropes with proper spectacles.

      • 한국인 근시안의 각막형태에 관한 연구

        The analysis and description of corneal topography is receiving more attention in contact lens fitting procedures and eximer laser photokeratectomy. It is known that there are inter-racial differences in corneal topography. In this study, we measured the refractive error and the corneal contour of 475 myopic eyes. The corneal topography was analyzed by using the EyeSys Corneal Analysis System V 2.1V. There is no significant correlation between the refractive error and the K-reading. We found that the K-readings were significantly different between males and females. The K-reading of Korean was similar to that of Chinese, but it was different from that of Caucasians. The rate of peripheral flattening in the nasal and temporal region is not symmetric, and curvature of the nasal peripheral region is the flattest in horizontal and vertical meridians of the cornea.

      • 연속착용 콘택트렌즈에 의한 각막부종

        각막질환이 없고 콘택트렌즈를 착용하지 않고 있는 근시안을 대상으로 국내 제품의 3가지 소프트 콘택트렌즈를 착용시키고 9시간 동안 취침을 시킨 후 유발된 각막부종과 회복되는 시간을 측정하였다. 각막두께는 optical pachometer 를 사용하여 매번 10회씩 측정하여 평균값을 얻었다. 대조군의 overnight corneal swelling은 2.34%였으며, 각 렌즈에 대한 overnight corneal swelling은 A 렌즈 8.49%, B 렌즈 12.34% 및 C렌즈 11.19%로, A 렌즈와 나머지 렌즈 사이에 유의한 차이가 있었다(p<0.05). 또한 deswelling에 대한분석 결과대조군에서 기상 후 1시간이 지났을 때 잠자기 전의 각막두께로 회복되었으며, 콘택트렌즈 착용군에서는 A 렌즈를 착용했을 때에만 기상 후 3시간이 지난 후 잠자기 전의 수준으로 회복되었다. 콘택트렌즈의 Dk/L값과 overnight corneal swelling은 상관관계가 높은 것으로 나타났다. 이상의 결과로부터 본 연구에 사용된 국내 제품의 렌즈는 매일착용이 안전하고, 연속착용하는 경우 각막부종으로 인한 부작용을 예방하기 위한 정기검진이 필요할 것으로 생각된다. An overnight study was performed to measure the corneal swelling and deswelling induced by three soft contact lenses manufactured in Korea. All subjects were no lens wear or inexperienced to contact lens wear. The overnight corneal swelling of 5 subjects(l0 eyes) was measured with an optical pachometer. Corneal thichness measurements were taken just before eye closure and after nine hours of sleep. The overnight corneal swelling in control group was 2.34 %, and those in contact lens wearing groups were 8.49% for A lens, 12.34% for B lens, 11.19% for C lens, respectively, Overnight swelling for A lens was significantly less than that for the other lenses(p<0.05). Cornea] deswelling time to the level of a base line(pre-sleep) with no lens wear occurred within 1 hour in control group. Among the contact lens wearing groups, corneal deswelling occurred in 3 hours for A lens only, The average corneal swelling response for A and B lens was inversely proportional to the oxygen transmissibility(Dk/L) of the lenses. It was considered that the contact lenses in this study used were appropriate for daily wear mode, and careful evaluation should be need to prevent from adverse effects during extended wear.
