        대퇴골두 무혈성 괴사의 뼈스캔상의 병기

        N/A We studied 90 patients(179 femoral heads) with avascular necrosis of femoral head, who had been performed X-ray, bone scan and MRI to copare of the findings of AVN on bone scan between each other, retrospectively. The patients were 82 males and 9 females, their mean age was 45 years. Radiographic stages were classifted by Steinberg modification, radionuclide stages were classified as followed; stage o(or type 0): noraml, stage 1: faint ring like uptake around the femoral head, stage 2: intense ring like uptake, stage 3: irregular increased uptake with central photon defect, stage 4: intense diffuse increased uptake at fem- oral head and stage 5: hip joint deformity with relatively mild increased uptake. The findings of MRI were classified according to extent, location, early or advanced lesion, signal int,ensity of the lesion and joint effusion. 156(87%) of 179 femoral heads had avascular necrosis, 68(75.5%) of 90 patients had bilateral AVN, 36 femoral heads had early stage and 120 had advanced stage. The detection rate of AVN by X-ray and bone scan were 85%(134), 91.6%(143), respectively. Early AVN with atypical types of bone scan showed larger extent, moderate to large amount of joint effusion, soft tissue hypertrophy within joint, and secondary degenerative changes. Bone scan had relatively high detection rate in the diagnosis of AVN of femoral head, and demonstrated various t.ypes depending on the disease stage.

        서울 지하철 객차 내 PM<sub>10</sub>과 CO<sub>2</sub>의 농도 변화

        The subway is the major public transportation system in Seoul with 2.2 million people using it everyday. Indoor air pollution in the subway can be a significant part of population exposure because of the number of people using the subway, time spent in transit and potentially high exposure for certain pollutants. The Korea Ministry of Environment has established the level 2 of recommended standards of $PM_{10}$ and $CO_2$ in subway trains. The aims of this study were to determine the airborne levels of $PM_{10}$, $CO_2$ and any correlation between pollutant levels and number of passenger in a subway train. The airborne $PM_{10}$ and $CO_2$ were measured on the inside of trains on line #4 for 4 different days from October to November in 2008. Average $PM_{10}$ and $CO_2$ levels were $113{\pm}25{\mu}g/m^3$ and $1402{\pm}442$ ppm, respectively. These levels did not exceed the level 2 of recommended standards of $250{\mu}g/m^3$ for $PM_{10}$ and 3500 ppm for $CO_2$. $PM_{10}$ level was not correlated with the number of passengers, while $CO_2$ levels were positively correlated with the number of passengers. The findings suggested that $PM_{10}$ in subway trains may have sources other than those directly associated with the number of passengers.

        급성 전방십자인대 파열:자기공명영상을 이용한 파열위치와 정도의 분석

        Purpose : To evaluate the sensitivity of MR imaging in determining tear sites and degrees in acute anteriorcruciate ligament tear. Materials and Methods : MR imagings were undertaken in 19 patients who had trauma on theirknee joints. All imaging studies were performed within 2 weeks after trauma and compared with operative findings.The degrees of ligament tear were divided into complete and incomplete, and sites of tears were divided intosuperior, middle and inferior portions. MR findings were compared with operative findings. Results : There were 14cases of complete ligament tear and 5 cases of partial ligament tear. We could diagnose correctly in all 14 caseswith complete tear and in 3 of 5 cases with partial tear. The tear sites were correctly predicted in 10 of 14cases with complete tear(71%) and 1 of 5 cases with partial tear(20%). In complete tears, MR findings weretransversely or obliquely coursed band-like high signal intensity within the ACL or abrupt switch over to asindistinct signal intensity. In partial tears, the tear sites could not be evaluated mostly and the tears appearedas linear low signal intensity lesions in posterolateral bundles of ACL. Conclusions : MR revealed highersensitivity in determining the degree and sites of ACL tear in complete tear as compared with partial tear.

        만성 탈구 또는 아탈구를 초래한 고관절의 변화 : 전산화 단층촬영과 단순 필름의 비 교 분석

        Secondary osteoarthritis of the hip jjoints is a common disease and is frequently followed by chronic sublexation and dislocation. Twenty four cases of the secondary osteoarthritis associated with chronic subluxation and dislocation of the hip joints were evaluated with plain radiography and computed tomotraphy. We retrospectively analyzed 1) the ossification and calcification of the acetabular labrum, 2) the thickeness of the quadrilateral plate of the ilium, and 3) anteroposterior diameter of the acetabulum. The changes of the hip joints in subluxation (n=14) revealed ossification of the acetabular labrum in 12 cases (86%), thickening of the quadriateral plate of the ilium in 11 cases (78%) but anteroposterior diameter of the acetabulum was not changed The changes of the hip joints in dislocation (n=10) revealed no evidence of the ossification of the acetabular labrum, thickening of the quadrilateral plate of the ilium in 10 cases (100%) and decreased anteroposterior diameter of the acetabulum. We conclude that CT findings of subluxation and dislocation of the hip joints can be helpful in the evaluation of the secondary osteoarthritis of the hip joints.

      • 결합조직형성 섬유종 - 2례 보고 -

        결합조직형성 섬유종은 골에 발생하는 종양중 매우 드문 종양으로, 조직학적으로나 생물학적 성상이 연부조직에 발생하는 복강외 섬유종과 유사하다. 저자들은 22세 남자에 발생한 견갑골 병변과, 34세 남자에 발생한 치골병변을 보고하는 바이다. 방사선학적으로 종양은 골융해성 병변으로, 팽창성 성장을 하였다. 조직학적으로, 다량의 교원질 섬유를 함유하고 있는 방추형 세포로 구성되어 있다. 광범위 절제술을 시행하였으며, 재발은 없었다. 골에 원발성으로 발생하는 결합조직형성 섬유종은 매우 드문 골 종양으로 참고문헌과 함께 보고하는 바이다. Desmoplastic fibroma is a rare primary tumor of bone that histologically and biologically mimics the extra-abdominal desmoid tumor of soft tissue. This report is dealt with two cases of desmoplastic fibroma occurring in a 22-year-old male, scapular lesion and 34-year-old male, pubic lesion. Radiologically, the tumors were lucent and expansile lesions. Histologically, they contained slender spindle cells and various amounts of collagen fibers. Radical excision was done on both cases and no recurrence was reported. Because of its rarity, we report two cases of desmoplastic fibroma.

        뇌하수체 선종의 자기공명영상 소견 : 조영증강 형태의 분석

        The magnetic resonance images (MRI) of 30 patients with surgically or biochemically confirmed pituitary adenomas (20 macroadenomas, 10 microadenomas) were retrospectively evaluated. Ten patients had hyperprolactinemia, another eight had acromegaly, another eight had nonfunctioning adenomal and four had Cushing disease. The examinations were performed at a 1.5-T superconducting MR system using a multisection spin-echo technique with 3mm-thick sections and a 256x224 matrix. T1 weighted sagittal and coronal images were obtained before and within 30 minutes after the administration of Gd-DTPA (0.1mmol/kg) Analysis of the MRI was focused on the signal intensity and enhamcement patterns of the pituitary adenoma before and after Gd-DTPA administration. Compared with endocrinologic diagnosis, macroadenoma showed heterogeneous enhacement in 55%, rim enhancement in 35% and homogeneous enhancement in 10%, Conclusively, the enhancement patterns of the pituitary adenoma did not correlate with the subtypes made according to hormone production.

      • 국내 일부 축산 농업에서의 미생물 노출 실태 파악

        In this study, we evaluated the concentration levels of bioaerosol which could be encountered in swinery workplace and the level of personal exposure concentrations. The target microorganisms were bacteria, fungi, gram-negative bacteria(GNB), and endotoxin. The level of concentrations of bacteria and fungi in pigsty were 7,161 CFU/m3 and 2,741CFU/m3, respectively. In the point of view of use of pigsty, the concentrations of bioaerosol in pigstries of delivery were higher than those of other pigstries. And when there were activities such as separation of pigs, the personal exposures to bioaerosol and dust were much higher than those of cleaning using water. To estimate the probabilities of scattering of contaminants in respective pigsty, we categorized each pigsty according to pigs` activity and dryness of floor. As a result, the probabilities of scattering of contaminants in respective pigsty were strongly correlated with concentration of bioaerosol, especially with GNB concentration. In conclusion, the level of concentration of bioaerosol in pigsty and personal exposure to them were very high and the severity of exposure were correlated with workers` and pigs` activities.

        Diagnostic display monitor QA according to AAPM TG18 is usually performed by PACS administrator, product manager and reading doctor, and for acceptance testing and periodic quality control evaluation, a combination of visual and quantitative tests can be used, as outlined in sections 5 and 6 of 'assessment of display performance for medical imaging systems'. Although many display tests can be performed visually, a more objective and quantitative evaluation of display performance requires special test tools. The required instruments vary in their complexity and cost, depending on the context of the evaluation(research, acceptance testing, or quality control) and how thorough the evaluation needs to be. Objective and reliable assessment of many display characteristics can be performed with relatively inexpensive equipment, So, we made 'AAPM TG18 guiding instrument' to ues variable purpose of the evaluation of 'geometrical distortions(quantitative"', 'veiling glare(visual)' and 'sensor calibration'. The spatial measurements for the quantitative evaluation of geometric distortions, and the measurement of the veling-glare ring response function which provides information regarding the spatial extent of the luminance spread, can be performed using the TG18 guiding instrument can be used to sensor calibration to standardize the basic rate of 0% luminance when periodic calibration.
