Sparganosis is a parasitic infection caused by the plerocercoid larvae of diphyllobothroid tapeworms belonging to the genus Spirometra, as first described by Manson in 1882. The infection is transmitted by ingestion of contaminated water, frogs, and snakes, and contact between a second intermediate host and an open wound or mucus membranes. Humans are accidental hosts in the life cycle, but dogs, cats, and other mammals are definitive hosts. Once a human becomes infected, the plerocercoid larvae migrate to a subcutaneous location, where they typically develop into a painful nodule. We misdiagnosed vulva sparganosis as a Bartholin`s gland abscess. The patient was a green consumer, so she may have been infected by consuming health foods. Sparganosis should be considered as a cause of soft tissue masses especially among patients who have ingested health foods.

Background: Spitz nevus and malignant melanoma have common features clinically and histologically, and in some cases it is impossible to distinguish between the two. Heat shock proteins (HSPs) serve to protect cells, and are activated by cell injury. Some HSPs are shown to be elevated in many types of cancers. Previous studies have reported the expression of heat shock protein in association with melanoma; however, a similar relationship with Spitz nevi has never been investigated. Objective: This study was designed to measure the expression pattern of HSP 105 in both Spitz nevi and melanomas. Methods: The specimens of 4 of Spitz nevi and 10 of malignant melanomas were analyzed for heat shock protein 105 expression through immunohistochemical staining. Results: Immunohistochemical examination of HSP 105 showed strong expression in malignant melanoma specimens. On the other hand, weak expression was observed in Spitz nevus specimens. The degree of expression of HSP 105 showed a statistically significant difference (p<0.05). Conclusion: These findings provide the possibility of using HSP 105 as a effective marker for differentiating between Spitz nevi and malignant melanomas. In support of this, HSP 105 is considered to be a tumor-associated antigen of malignant melanoma. (Korean J Dermatol 2009;47(2):148∼153)

Environmental endotoxin exposure influences the severity of asthma in house dust mite allergen sensitized subjects. In the domestic environment high levels of endotoxin are present in reservoir floor and bedding dust. Clothing is known to be an important source of allergen exposure, but it is not yet known if this is so for endotoxin. We determined endotoxin levels from winter upper garment clothing with the Limulus amebocyte lysate assay. The geometric mean (95% CI) endotoxin level from 34 colothing items was 17.540 EU/gm. More recently washed clothing (<2 weeks) contained lower endotoxin levels than less frequently washed clothing (>2 weeks), geometric means (95% CI): 9.772 (3,500~28,440) and 20,420 (16,670~24,950) EU/gm dust restectively (p=.017). These levels are high and similar to endotoxin levels from carpets and bedding. This results suggest that clothing is potentially a major source of endotoxin exposure.

An immunoelectron microscopic observation of various antigenic sites of the lung fluke, adult Paragonimus iloktsuenensis, was performed by applying the indirect immunoperoxidase technique to localize the selective antigenic sites. The fixative was mixture of 1% paraformaldehyde and 0.2% glutaraldehyde in 0.1M phosphate buffer, pH 7.4. LR-white resin(EMS, hard grade), the embedding media, was polymerized at -20℃ under the ultraviolet light(365nm long-wave length) for 48 hours. Nickel grids(EMS, 200 mesh) were coated with 2% collodion(in isoamyl-acetate) and 0.5% polyvinyl formvar. The antiserum produced from the rat infected with Paragonimus iloktsuenensis(10 weeks after infection) was used as a primary antibody. The secondary antibody was gold conjugated goat anti-rat IgG(particle size=10nm, Sigma). The intestinal epithelial cells containing numerous antigenic granules which seem to be secreted into the intestinal lumen. The secretory granules and luminal contents were predominantly labeled by gold particles. The excretory bladder was also proved to be a highly antigenic organ, since its lamellated structures and contents were labeled by numerous gold particles. The antigenicities of secretory materials of the vitelline glands, testes, and ovaries were moderately recognizible, too. But the tegument was very weakly labeled by gold particles. The above results suggests that the secretory substances from the intestinal epithelial cells and excretory contents from excretory bladder in the adult Paragonimus iloktsuenensis are the most potent antigenic stimulants to the host immune system.

We were study for influence of proteinase in adult worm extract antigenic protein components of Paragonimus iloktsuenensis by use of proteinase inhibitor, E-64 ([L-3-transcar-boxyoxiran-2-carbonyl]-L-leucyl-agmatin). Examined antigens were no-added antigens with proteinase inhibitor(E-64) and added antigens with E-64, and these antigenic proteins were separated by SDS-PAGE. The results are summarized as follows : Added antigens were more made up 5 to 20 protein(polypeptide) bands in molecular weight from 40Kda to 200Kda than no-added antigens. Especially, stain density of protein bands were strong markedly on molecular weight 45Kda, 60Kda and 83Kda in added antigens compared with no-added antigens. On the other hand, in molecular weight level below 40Kda, there were similar numbers or stain density of protein bands between added antigens and no-added antigens, but the protein band of 32Kda was more strong stain density in no-added antigens than in added antigens. In conclusion, adult worm extract antigens of P. iloktsuenensis were affected by intrinsic proteinase of P. iloktsuenensis in SDS-PAGE. There are regarded as the protein component of molecular weight 45Kda was affected most by proteinase in SDS-PAGE, and the protein component of molecular weight 32Kda was affected partially by proteinase inhibitor.
