        사람 치은 섬유아세포에서의 Tannerella forsythia 전세균, 막단백질, 당지질에 의한 염증성 사이토카인 발현

        Purpose: The purpose of this study was to investigate induction of cytokine expression in human gingival fibroblasts (HGFs) by whole cell and the components of T. forsythia. Material and Methods: After HGFs were treated with lipopolysaccharide (LPS), membrane protein isolated from T. forsythia or culture media of T. forsythia, the induction of interleukin (IL)-1, IL-6 and IL-8 was examined with real-time PCR and ELISA. Their induction ability of cytokines was compared with whole bacteria. Result: The expression of IL-6 and IL-8 was significantly induced in HGFs by whole bacteria and membrane protein. The expression of IL-$1{\beta}$ was induced by membrane protein of T. forsythia, not by whole bacteria. LPS and condition media of T. forsythia slightly activated HGFs. Conclusion: The membrane protein of T. forsythia could be one of virulence factors.

        비교법적 해석을 통한 자기결정권의 재고

        May 21, 2009. The 'Case of grandma Kim' that was highlighted as an issue in Korea. It led to the result that recognized initially in Korea about death with dignity. Since the initial sentence that recognized the death with dignity by the Supreme Court, It was actively discussed by configure the related social council and activities or Taking the initiative by Shin sang jin. But it couldn't draw a conclusion by religious community, the medical profession and related legal system etc. However, recently, the try are showing again. Nowadays discussions and interests about death with dignity are increasing gradually to government and whole of society. Therefore, examine the each countries opposing discussion. And should be sought reasonable solutions for its admissibility and validity. And to prepare the related legislation, review all the matters to National Bioethics Committee and in charge of the advice and decision about family. And if they can control the process of treatment interruption through the Commission. Then, it will be able to achieve the social consensus about the basis of treatment's discontinuation and will be able to escape from present practice that made by agreement between the family and the individual physician under the unclear standard. The decision of the judgment between 'unrecoverable patients' and 'terminal patients' is the decision of the judgment between 'unrecoverable patients' and 'terminal patients' is including the clear procedures that be the basis of judgment for medical standards of judgment. Also, may be considered the ways that participation of at least two or more doctors. The unrecoverable patients, they do not have the opportunity to express their free opinions. So, substitute for specific others and need to consider the agent's consent. In this case, the legal controversy arises which is the agent's decision about life and the patients' own decision are same. In order to overcome this, the preparation of the legal basis is essential about agree on the consent of agents. 우리나라에 떠들썩한 이슈로 부각되었던 '김할머니'사건은 존엄사에 대하여 대한민국 최초로 인정되는 결과로 이어졌다. 대법원의 국내 첫 존엄사 인정 판결 이후 관련 사회협의체 구성 및 활동과 신상진 의원의 법안 발의로 활발히 논의된 존엄사는 종교계, 의료계, 관련 법제 등의 이유로 이견을 좁히지 못하여 결론을 도출해 내지 못하였으나 최근 들어 다시금 존엄사에 대하여 논의를 재계하려는 움직임이 보이고 있다. 존엄사에 대한 논의와 관심이 정부와 국민 사회 전반에 점차적으로 증가하고 있는 지금 존엄사를 인정하고 있는 각국의 입법례와 우리나라에서 서로 대립하여 논의 되었던 바를 살펴보고 그 허용성과 타당성에 대하여 합리적인 해결방안을 모색하고 관련 법제 마련을 위하여 국가생명윤리심의위원회에 제반 사항의 검토와 가족에 대한 권고, 결정을 담당하게 하고 위원회를 통해 치료중단의 과정을 절차적으로 통제할 수 있게 된다면, 치료중단의 기준에 대한 사회적인 합의를 이루어나갈 수 있을 것이며, 불분명한 기준 아래 개별 의사와 가족 간의 합의로 이루어지는 지금까지의 관행에서 벗어날 수 있을 것이다. 또한, 위원회는 치료중단 결정에 관여한 가족과 의료진의 윤리적 부담과 법률적 불안정성을 해소시켜줄 수도 있을 것이다. '회복불능상태의 환자' 또는 '말기상태의 환자'의 판단여부의 결정은 판단의 근거가 될 수 있는 의학적 판단의 기준이 되는 명확한 절차를 포함하면서 최소한 2인 이상의 의사가 참여하는 방안을 고려할 수 있을 것이다. 의식불능의 환자에게는 자유로운 의사를 표명할 기회가 없으므로 특정한 타인에게 의사를 대행하게 하고 그 자의 의사결정을 생명주체의 의사결정으로 대신하는 방법인 대리인의 동의를 고려할 필요가 있다. 이 경우 대리인의 생명에 대한 결정이 환자 자신의 결정권과 동일한 가치로 볼 수 있는가에 대하여 법적 논란이 제기되는데, 이를 극복하기 위하여 대리인의 동의의 경우에 그 동의에 대한 법적 근거의 마련이 필수적인 것이다.

        위장관 ; Lipopolysaccharide로 유도된 HT-29 세포주의 염증에서 Lactobacillus rhamnosus GG의 항염증 작용과 기전

        Background/Aims: Probiotics are live non-pathogenic organisms that belong to the resident microflora, and confer health benefits by multiple mechanisms. Lactobacillus rhamnosus GG (LGG) is one of the probiotic bacteria that ameliorates intestinal injury and inflammation caused by various stimuli. We aimed to evaluate the anti-inflammatory effect and mechanism of LGG in lipopolysaccharide (LPS)-stimulated HT-29 cells. Methods: HT-29 cells were stimulated with interleukin (IL)-1β (2 ng/mL), tumor necrosis factor (TNF)-α (20 ng/mL), and LPS (20 μg/mL) in the presence or absence of LGG (107-109 colony forming units/mL). Production of the pro-inflammatory chemokine IL-8 was measured by ELISA and semi-quantitative PCR. Transcriptional activity of NF-κB-responsive gene was evaluated by luciferase assay with reporter gene. Toll-like receptor 4 (TLR4) mRNA expression was assessed by semi-quantitative PCR. The IκBα degradation was evaluated by western blot and intranuclear translocation of NF-κB was determined by western blot and immunofluorescence. Results: LGG did not affect the viability of HT-29 cells. Pretreatment of HT-29 cells with LGG significantly blocked TNF-α, and LPS induced IL-8 activation at both mRNA and protein level (p<0.05). Pretreatment of HT-29 cells with LGG attenuated LPS-induced NF-κB nuclear translocation and also blocked LPS-induced IκBα degradation. LGG also down-regulated TLR4 mRNA activated by LPS. Conclusions: LGG attenuates LPS induced inflammation, and this may be associated with TLR4/NF-κB down-regulation. (Korean J Gastroenterol 2012;60:86-93)

        후박추출물과 옥수수 불검화 추출물 혼합 경구용 제제가 비글견에서 실험적으로 유발된 치주염에 미치는 영향

        It has been reported that Magnoliae cortex extract has antibacterial and antimicrobial activity against pathogenic microbes and Zea Mays L. extract is effective for improving gingival tissue health. The purpose of this study was to examine the anti-inflammatory and antimicrobial effects of Zea Mays L. and Magnoliac cortex extract mixtures through experimental peridontitis induced beagle dog model. Nine beagle dogs with experimentally induced periodontitis were selected. Baseline clinical indices which includes plaque index, gingival index, probing pocket depth, clinical attachment level, gingival fluid flow rate were recorded and microbial assays were done. Magnoliac cortex and Zea Mays L., mixed at 2:l ratio in 105mg capsular dosage, were taken by 3 capsule (Group I) or 6 capsule dosages (Group II) three times a day. After 4,8,12 weeks, clinical indices were recorded. All data of clinical indices were compared through one-way ANOVA with 95% confidence level. Clinical indices of group I and II showed significantly better results than those of control group. There were no significant differences between group I and II. In conclusion, it was confirmed that mixture of Magnoliae cortex and Zea May L. (mix ratio 2:1) possessed clinical improving effects to periodontitis.

        Zea Mays L. has been known to be effective for improving tissue health and Magnoliae cortex to have effective antibacterial and antimicrobial activity against pathogenic microbes. The purpose of this study was to examine the antimicrobial effects of Zea Mays L. and Magnoliae cortex extract mixtures on periodontal pathogens(Prevotella intermedia, Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis, Streptococcus mutans )and to examine the effects on human gingival fibroblast cellular activity. Zea Mays L. and Magnoliae cortex extracts and their mixtures were prepared with various mixing ratios (0.5:1, 1:1, 1.5:1, 2:1). These extracts were loaded to periodontal pathogen cultured petri dish for antimicrobial test and also loaded to cultured human gingival fibroblast for cellular activity test. Each test was repeated 3 times and data were analyzed by one-way ANOVA with 95% confidence level. Mixture of these two extracts showed greater amount of inhibition area on periodontal pathogen and more improved gingival fibroblast activity as Zea Mays L. ratio reduced. So, mixture ratio 0.5:1 (Zea Mays L. : Magnoliae cortex) group showed statistical significance in antimicrobial activity and cellular activity among various mixtures(p < 0.05). In conclusion, 0.5:1 (Zea Mays L. : Magnoliae cortex) mixture possessed best gingival fibroblast cellular activity and antimicrobial activity toward periodontal pathogens.

        I. Purpose of Study Zea Mays L. has been known to be effective for improving periodontal health and Magnoliae cortex to have effective antibacterial and antimicrobial activity against periodontal pathogens. The purpose of this study was to examine the biologic effects of Zea Mays L. and Magnoliae cortex extract mixtures on healing of rat calvarial bone defects. II. Materials & Methods 8mm circular defects were prepared on rat calvaria during surgical procedures of 180 Sprague-Dawley rats. The ethanolic extracts of Magnoliae cortex and Zea Mays L. and these two natural extract 1:1 and 2:1 (Magnoliae: Zea Mays L.) ratio mixtures were oral administrated by oral zondes once a day at two different dose of 94.5mg/kg, 189mg/kg body weight. There are nine groups of rats in this study: control group (no sample loading), Magnoliae cortex extract loading groups (I,II)(94.5mg/kg,189mg/kg respectively), Zea Mays L. extract loading groups (I,II), M:Z(1:1) loading groups (I,II), M:Z(2:1) loading groups(I,II). Rats were sacrificed at 4 weeks and 6 weeks after surgery. New bone formations around calvarial defects were radiographically and histologically measured by computerassisted histomorphometry. Each data was statistically analyzed by One-way ANOVA test. III. Results There were statistical significances between negative control group and the other test groups on radiographical and histological quantitative assessments. Among test groups, mixture groups showed statistical significances, especially, M:Z (2:1) groups (I and II) were highly significant.(p<0.05) These results implicated that the mixture of Magnoliae and Zea Mays L. (2:1 mixing ratio) with 94.5mg/kg concentration might be highly effective on the wound healing of bony defected site and have potential possibilities as a useful drug to promote bone tissue regeneration.
