Parosteal osteosarcoma is characterized as a densely ossifying lesion, usually occurring on the surface near the metaphyses of a long bone. The histological pattern is a well- differentiated mature bone trabeculae with a hypocellular spindle-cell stroma. The cytological details are those of a low-grade malignant lesion. The natural history of this lesion is indolent local growth, late invasion of the underlying bone, and infrequently, distant metastasis. However, there is a significant risk of eventual dedifferentiation into a high-grade lesion. We report here-a case of parosteal osteosarcoma dedifferentiated into a high-grade lesion, which occurred in the left distal femur of a 40-years-old woman, and discuss the experience in detail.
석회와 유기물이 산성 산림토양의 Al과 Pb 불용화에 미치는 영향

The toxic effects of Al and Pb on trees, earthworms, and microorganisms in acidic forest soils are well known. As a remedial measure the surfaces of forest soils worldwide have been limed. Depending on the thickness of the organic layer on the forest soils the applied lime leaches with a delay of several years into deeper layers of the mineral soil. Furthermore, the lime applied is often neutralized in the organic layer even before it reaches mineral soil horizons. Therefore, this study was conducted to investigate the effect of liming on soil pH and immobilization of Al and Pbin acidic forest soils aftermixing the lime with the mineral soil. The dolomitelime together with the organic layeron the forest soils was strip wise mixed with the soil material to a depth of 35 cm. The investigation took place 8 months after the treatment. Soil treatment led to significant increases of the pH in the upper 35 cm of the soil. The soil of the untreated control strip was highly acidic with pH(CaCl2)of 3.4-3.7, whereas pH(CaCl2) in the treated strips were between 4.0 and 5.1. The total contents of Al and Pbin soils did not differ from the treated strip to the untreated strip. However, the mobile fractions of Al and Pb were significantly reduced from an average of 362 to 69.3 mg kg-1 for Al and 2.9 to 0.7 mg kg-1 for Pb. This improvedinitial soil conditions will probably lead to an advanced growingfor the planted trees.

8MHz 라디오파를 이용한 자입식 온열치료를 위한 기초실험의 일환으로 조직등가물을 이용하여 다양한 needle electrode의 배열을 통한 적정 온도분포를 규명하고자 하였고, 직접 개의 뇌실질에 자입 온열요법을 시도하여 이에 따른 조직 병리학적 소견을 관찰하고자 하였다. 조직등가물 실침에서 저자들은 needle electrode 1 cm 간격의 정방형 배치에서 횡단면상 $90\%$ relative SAR 분포가 약 1.25 cm 반경의 균일한 원형으로 관찰됨을 알 수 있었고 종단면상 needle electrode의 길이에 따라 균일한 온도분포가 이루어짐을 관찰할 수 있었다. 정상 개의 뇌실질에 자입하여 직접 정방형의 중심을 $43^{\circ}C$로 유지하며 50분간 온열요법을 시행한 후 관찰한 조직 병리학적 소견은 liquefactive necrosis, pyknosis of neuronal element 및 Polymorphonuclear leukocytes들이 회백질에서 급성기에 관찰 되었고 liquefactive necrosis 주위에 lipid-laden macrophage들이 관찰됨이 공통적인 특징 이었으며 후기 변화로 괴사조직 주위로 신경교세포의 증식이 관찰되었다. An interstitial radiofrequency needle electrode system was constructed for interstitial heating of brain tissue. Radiofrequency electrodes with Thermotron RF 8 were tested in an agar phantom and in a normal canine brain to determine how variations in physical factors affected temperature distributions. Temperature distributions were checked after heating with 1 mm diameter needle electrode implants on the corners of 1 and 2 cm squares in a phantom and plot isotherms for various electrodes arrangement. We observed that the 1 cm square array would heat a volume with a 1.25 cm radius circular field cross section to therapeutic temperatures ($90\%$ relative SAR using Tm) and the 2 cm square array with a 1.75 cm radius rectangular field with central inhomogeneity. With 2 cm long electrode implants, we observed that the 1 cm square array would heat a 3 cm long sagittal section to therapeutic temperature ($90\%$ relative SAR using Tm). We found that radiofrequency electrodes could be selected to match the length of the heating area without affecting its performance. The histopathological changes associated with RF heating of normal canine brains have been correlated with thermal distributions. RF needle electrode heating was applied for 50min to generate tissue temperatures of $43^{\circ}C$. We obtained a quarter of the heated tissue material immediately after heating and sacrificed at intervals from $7\sim30$days. The acute stage (immediately after heating) was demonstrated by liquefactive necrosis, pyknosis of neuronal element in the gray matter and by some polymer-phonuclear leukocytes infiltration. The appearance of lipid-laden macrophages surrounding the area of liquefaction necrosis was demonstrated in all three sacrificed dogs. Mild gliosis occurring around the necrosis was demonstrated in the last sacrificed (Days 30) canine brain.
Monocephalus Tripus Tribrachius 환아의 부속 하지 관절 이단술 및 골반 재건술 후 18년 추시 관찰 결과

결합 쌍둥이의 일종인 monocephalus tripus tribrachius는 하나의 머리와 세 개의 상지 및 하지를 가지고 태어나는 보기 드문 기형으 로 현재까지 국내 문헌으로는 분리 수술 후 장기 추시 관찰 결과가 보고되지 않았다. 저자들은 monocephalus tripus tribrachius 환아 에 대하여 생후 부속 하지 및 그와 관련한 고관절 및 골반골의 문제들의 해결을 위하여 부속 하지의 관절 이단술 및 골반 재건술을 시 행한 바 있다. 이후 추시 관찰상 문제가 되었던 교정되지 않는 편측의 족부 변형에 대해 Syme 절단술을 시행하였으며, 대퇴 골두의 골반내 피복 불량이 관찰되어 추가적으로 편측의 대퇴골 내반 감염 절골술을 시행하였다. 저자들은 이러한 술식들에 대한 18년 장기 추시 관찰 결과를 문헌 고찰과 함께 보고하는 바이다. Monocephalus tripus tribrachius, a type of conjoined twins with one head and three upper and lower extremities, is a rare congenital disorder. To date, no long-term follow-up results of surgical procedures for this condition have been reported in Korean literature. We experienced a case of monocephalus tripus tribrachius, which had been surgically managed with an accessory lower limb disarticulation and pelvic bone reconstruction to manage this accessory limb and accompanying comorbidities in hip joint and pelvis. Subsequently, ipsilateral Syme amputation was done for intractable deformity of foot, and later, ipsilateral femoral varus derotational osteotomy was done for inadequate coverage of femoral head observed in follow-up. We report 18-year follow-up results of the procedures with a review of literatures.
