        중량조끼를 착용한 뇌졸중으로 인한 보행장애 환자의 즉각적인 보행 개선 효과 1례

        Objective: To confirm the immediate gait improvement in a post-stroke gait disturbance patient equipped with a weighted vest. Methods: We selected a patient who was able to walk without another’s help or with tools. The selected patient had an unstable gait because she had only started an independent gait within the past week, so we thought that a weighted vest could be very helpful for her. We first collected gait parameters using a treadmill gait analysis system while the patient walked on the treadmill without the weighted vest. After a 10-minute break, gait parameters were collected again while the patient walked on the treadmill while wearing the weighted vest. The gait parameters we collected included step length (cm), stance phase (%), swing phase (%), SW/ST, and gait line length (mm). For objective evaluation of gait improvement, we calculated the ratio of gait parameters of the right and left limbs. Results: The gait of the post-stroke patient was more symmetrical when wearing the weighted vest than without the weighted vest. Without the weighted vest, her step length ratio was 0.78, stance phase ratio was 0.88, swing phase ratio was 1.50, SW/ST ratio was 1.70, and gait line length ratio was 0.91. With the weighted vest, her step length ratio was 0.88, stance phase ratio was 0.90, swing phase ratio was 1.38, SW/ST ratio was 1.54, and gait line length ratio was 0.98. No side effects were observed due to the weighted vest.

        재활치료 중단 후 보행 양상이 다시 악화된 뇌졸중 환자 1례에 관한 고찰

        Objective: This study describes the case of a patient with a left basal ganglia (BG) infarct who had made almost a complete recovery upon discharge from the hospital in 2014 but who was re-admitted after 31 months with a worsened hemiplegic gait. Methods: The patient had undergone no rehabilitation treatment in the 31 months since his discharge. When the patient was re-admitted to our hospital in 2017, stance and gait data were collected on the patient using a treadmill gait analysis system. In addition, the patient underwent a manual muscle test (MMT) evaluation, and his Motricity index (MI) and modified Barthel index (MBI) scores were recorded. After rehabilitation for one month, the patient was reassessed, and the results were compared to those on the day of re-admission and those recorded in 2014. Results: Compared to the 2014 evaluation results, the patient's stance parameters and gait parameters had worsened at re-admission. However, there was no significant change in the patient's MMT grade or MI and MBI scores in comparison to the results of the 2014 evaluation. After one month of rehabilitation, the patient was re-evaluated again, but there was no significant change in comparison to the evaluation results at re-admission. Conclusion: Some of the stroke patients who have passed six months since the onset of their stroke may require ongoing rehabilitation although the functions of them is almost recovered. Because there is a possibility that recovered functions get worse again without any rehabilitation for a long time. And once the recovered functions get worse, re-recovery of them is not easy.

        트립탄 계열 약물에 반응하지 않는 만성 편두통 환자 치험 1례

        Introduction: This case report assessed the effects of Korean medicine treatments on a chronic migraine patient who did not respond to triptan. Case Presentation: The patient received Korean medicine treatments, such as acupuncture, moxibustion, cupping, and herbal medication, during a hospitalization period. The effects of treatments were assessed with the Numerical Rating Scale (NRS) and migraine attack frequency. The intensity and attack frequency of migraines were significantly reduced after the complete treatment. Conclusions: For migraines, Korean medicine such as acupuncture and herbal medication could be an alternative to conventional therapy.

        합성의약품으로 호전되지 않은 소화기 증상 환자(KCD K-코드)에 대한 반하사심탕 제제 병용의 효과 : 후향적 차트 분석

        Objectives: The aim of this study was to evaluate the effectiveness of additional administration of Banha-sasim-tang formulation to patients with gastrointestinal symptoms that did not improve with Western medicines. Methods: The patients who met the inclusion and exclusion criteria were analyzed retrospectively. A paired t-test was performed on the Nepean Dyspepsia Index-Korean version (NDI-K) scores before and after administering the Banha-sasim-tang formulation. Results: A total of 27 subjects were included. The mean duration of taking the Banha-sasim-tang formulation was 15.6±3.1 days. The most commonly used Western medicine before taking the Banha-sasim-tang formulation was rebamipide. The means of the NDI-K score were 57.4±21.8 before administering Banha-sasim-tang formulation and 34.9±16.5 after administration, and the difference was statistically significant (p<0.0001). No serious side effects were encountered while taking the Banha-sasim-tang formulation. Conclusions: For patients whose gastrointestinal symptoms do not improve with Western medicines, additional administration of Banha-sasim-tang formulation may be an option. However, as this study is a retrospective chart review, prospective, large-scale, double-blind studies are needed in the future.

        지구성 운동 수행에 따른 obese Zucker rat 골격근의 GLUT-4, v-와 t-SNARE 발현의 변화

        인슐린 자극에 의한 골격근 GLUT-4의 세포막으로의 전좌(translocation)는 골격근 세포의 글루코스 섭취 능력을 향상시킨다. 특히, GLUT-4의 전좌는 특이적인 액포(vesicle)의 결합 단백질인 v-SNARE(vamp-2)가 표적 세포막에 위치한 상동성이 큰 t-SNARE(syntaxin-4)단백질을 특이적으로 인식하고 상호 작용하여 GLUT-4를 세포막 표면으로 가역적으로 전좌시켜 글루코스 섭취 능력을 증가시킨다. 따라서 이 연구에서는 지구성 운동수행에 따른 골격근의 GLUT-4 작용 기전과 글루코스 섭취 능력을 규명하는데 그 목적이 있다. 이를 위해 이 연구에서는 비만 실험모델 동물인 obese Zucker rat(fa/fa)를 활용하여 운동집단과 비교집단으로 구분한 후 8주간 실험을 실시하였다. 실험 결과, 지구성 운동수행에 따라 obese Zucker rat의 골격근(가자미근과 족저근) 내 인슐린 조절(insulin regulated)에 의한 글루코스 운반 단백질인 GLUT-4와 막 결합 단백질인 v-SNARE(vamp-2), t-SNARE(syntaxin-4)단백질의 발현이 증가한 것으로 나타났으며, In vitro 수준에서 2-deoxy-D-glucose(2-DG)수준을 측정하여 골격근 형태별(가자미근, 족저근) 글루코스 섭취능력을 분석한 결과, 운동집단이 비교집단에 비해 현저하게 높은 것으로 나타났다. 이러한 결과는 지구성 운동이 골격근의 v-SNARE(vamp-2)와 t-SNARE(syntaxin-4)의 상호작용을 향상시키고 GLUT-4 단백질의 전좌를 활성화시켜 글루코스 섭취 능력을 극대화시키는 것으로 나타났다. Insulin stimulation of skeletal muscle results in the translocation of GLUT4 from an intra cellular location to the plasma membrane; this translocation is defective in insulin resistance. Studies have suggested an important role for synaptobrevin and syntaxin homologues in this event, particularly the v-soluble N-ethylmaleimide attachment protein receptors (SNAREs) cellubrevin and vesicle-associated membrane protein-2 (V AMP-2) and the t-SNARE syntaxin-4, but the expression of these proteins has not been studied in insulin resistant tissues. Therefore, we examined SNARE protein content in skeletal muscle from obese Zucker rats and determined the effect of exercise on these proteins. GLUT4 levels in skeletal muscle from exercise were elevated compared with control group. V AMP-2, and syntaxin 4 protein levels were elevated in skeletal muscle from obese Zucker rats compared with controls. Restoration of normoglycemia and normo insulinemia in exercised obese Zucker rats improved V AMP-2, and syntaxin 4 protein to levels approaching normal in lean animals. These data show that elevated v- and t-SNARE protein levels are associated with insulin resistance in skeletal muscle and that these increase may be achieved by exercise concomitant with a restoration of glycemic control. In summary, we have demonstrated that v- and t- SNARE concentration as well as muscle GLUT4 protein and mRNA can increase significantly after prolonged exercise confirmed that this increase in GLUT4 protein is controlled at both the pre-translational and translational levels. The observation that improved glucose uptake suggests that the increased expression of GLUT4 protein expression can serve as a molecular yardstick for exercise-dependent glycogen deposition.

        VAR 모형을 이용한 유통단계별 갈치가격의 인과성 분석

        This study aims to analyze causalities among Hairtail prices by distribution channel using a vector autoregressive model. This study applies unit-root test for stability of data, uses Granger causality test to know interaction among Hairtail Prices by distribution channel, and employes the vector autoregressive model to estimate statistical impacts among t-2 period variables used in model. Analyzing results of this study are as follows. First, ADF, PP, and KPSS tests show that the change rate of Hairtail price by distribution channel differentiated by logarithm is stable. Second, a Granger causality test presents that the producer price of Hairtail leads the wholesale price and then the wholesale price leads the consumer price. Third, the vector autoregressive model suggests that the change rate of Hairtail producer price of t-2 period variables statistically, significantly impacts change rates of own, wholesale, and consumer prices at current period. Fourth, the impulse response analysis indicates that impulse responses of the structural shocks with a respectively distribution channel of the Hairtail prices are relatively more powerful in own distribution channel than in other distribution channels. Fifth, a forecast error variance decomposition of the Hairtail prices points out that the own price has relatively more powerful influence than other prices.

        본 연구를 통한 신규 가속 내구시험모드 개발로 검증 시간을 45% 수준으로 단축시키는 효과를 얻었으며, 국부적이지만 메인프레임의 안전율을 낮춤으로써 13.4%의 무게 경량화를 이루었고 메인프레임에 대한 CAE 동역학 및 구조해석 결과와도 비교하기 위한 기틀을 마련하였으며 향후 개발되는 모든 섀시 부품에 평가 프로세스의 수평 전개가 가능 할 것으로 판단한다.

        수분주입량에 따른 압출성형 옥수수 섬유질의 물리화학적 특성

        본 연구는 옥수수 섬유질을 수분주입량의 변화에 따라 압출성형 옥수수 섬유질과 탈전분 압출성형 옥수수 섬유질의 물리화학적 특성 변화를 측정하였다. 원료의 수분주입량은 30, 40, 50%로 조절하였으며 스크루 회전속도와 사출구 온도는 각각 200 rpm, 140℃로 고정시켜 압출성형 하였다. 일반성분 함량은 원료에 비해 감소하였으며, 수분주입량이 30%일 때 옥수수 섬유질과 탈전분 옥수수 섬유질의 불용성 식이섬유 함량은 감소하고 수용성 식이섬유 함량은 증가하였다. 압출성형 옥수수 섬유질의 비기계적 에너지, 직경팽화율, 밀도, 파괴력, 탄성계수는 수분함량이 증가할수록 감소하였고, 비길이는 증가하였다. 압출성형 옥수수 섬유질의 수분흡착지수와 환원당은 수분주입량이 증가할수록 변화가 없었으며 수분용해지수는 감소하였다. 반면에 탈전분 옥수수 섬유질의 수분흡착지수는 감소하고 수분용해지수와 환원당은 증가하였다. 수분주입량 30%일 때 유용성분의 추출이 미량 증가하는 것으로 보아 압출성형공정의 적용하여 섬유질에 포함된 유용성분의 추출이 용이할 것으로 생각된다. This study was designed to examine changes in the physicochemical properties of extruded corn fibers with different amounts of feed moisture (30, 40, and 50%). The screw speed and die temperature were fixed to 200 rpm and 140℃, respectively. The crude ash, fat, and protein in corn fiber decreased from the extrusion process. The insoluble dietary fiber in corn fibers decreased, while soluble dietary fiber increased at a feed moisture of 30%. The specific length of the extruded corn fiber increased while the specific mechanical energy input, density, breaking strength, and elastic modulus decreased. The water absorption index (WAI) and reducing sugar content of the corn fibers did not significantly change, but the water soluble index (WSI) decreased as the feed moisture content of the corn fiber increased. On the other hand, the WAI of de-starched corn fiber decreased while WSI and reducing sugars increased as the feed moisture content of the corn fiber increased.
