      • 한국인 위암에서 Caspase 2 단백 발현 양상

        Purpose: Caspase 2, a member of the family of ICE-like proteases, is activated by the Fas pathway and induces apoptosis by triggering the caspase cascade. The purpose of this study was to determine whether the expression pattern of caspase 2 might be associated with gastric cancer development and if so, to determine to which pathologic parameter it is linked. Materials and Methods: For the construction of the gastric cancer tissue microarray, 78 paraffin-embedded tissues containing gastric cancer areas were cored 3 times and transferred to the recipient master block. The expression pattern of caspase 2 was examined on tissue microarray slides by using immunohistochemistry and was compared with pathologic parameters, including histologic type, depth of invasion, lymph node metastasis, and peritoneal dissemination. Results: Caspase 2 was expressed on superficial and foveolar epithelial cells and lymphocytes in the gastric mucosa, mainly in cytoplasm. We found loss of caspase 2 expression in 41 ($52.6\%$) of the 78 gastric cancer tissues. Statistically, histologic type and other pathologic parameters were not related with loss of caspase 2 expression Conclusion: Our findings provide enough evidence that loss of caspase 2 expression may contribute to the development of Korean gastric cancer and that it might be one of the possible escape mechanisms from apoptosis in gastric cancer.

        정보 품질 성숙도 모델에 관한 연구

        정보는 효과적으로 활용되어 이윤 창출에 기여하고, 신속하고 올바른 경영 판단을 지원하며, 재사용이 가능해야 하는 중요한 자원이다. 최근의 정보 시스템들은 사용자들의 다양한 요구를 반영하여 기업의 경쟁력을 높이고, 급변하는 환경 변화에 적응하기 위해 점차 대형화, 복잡화 되고 있으며 정보 품질의 중요성도 점차 강조 되고 있는 추세이다. 사용자들이 원하는 정보 제공에 있어 가장 큰 문제점은 저 품질의 데이터를 기반으로 하는 낮은 품질의 정보 제공에 있다. 낮은 정보 품질에 의해 기업 경영이 이루어질 경우 기업의 전략 수립, 전략 수행, 고객과 경쟁 기업에 대한 경영 집중력 분산 등 기업의 경영에 있어 경쟁력이 떨어 질 수밖에 없다. 낮은 품질의 정보는 부정확한 데이터를 개선하거나 조정하기 위한 시간 및 비용을 증가 시키고, 특정 현황에 대한 정확한 정보를 제공 받기가 어려워진다. 이러한 문제점들을 해결하기 위해서는 데이터에 대한 명확한 이해, 데이터 관리 체계 확립, 그리고 체계적인 데이터 관리 수행 등을 통하여 고품질의 데이터를 획득 할 수 있게 해야 한다. 현재까지 정보 품질과 관련된 연구 및 방법론은 부분적으로 진전이 있으나, 정보 품질 관리 전반에 대한 체계적인 방법론은 존재하지 않는 실정이다. 따라서 본 논문에서는 정보 품질 관리를 위한 프로세스들을 도출하고 정보 품질과 관련된 평가 요소를 도출하여 이를 정보 품질 보증 프로세스 단계인 CMM(Capacity Maturity Mode]) 5단계를 참조하여 제시한다. 본 논문은 정보 품질 개선 관리 프로세스 정립을 통해 기업의 정보 품질 관련 전략을 수립 하고, 경쟁력 있는 기업 또는 조직 활동에 이바지 하며, 품질 좋은 정보 활용에 따른 경쟁력 있는 운영에 이바지 하고자한다. Information is used effectively and contributes in profit creation and not only support management judgment quick but important resource to be possible recycled. The recent information systems improve enterprise's competitive power by reflection of user's various requirements and becoming big and complex for adaptation of rapidly circumstance change. Also it is trend that importance of information quality is emphasized gradually. The biggest problem in user requirement that is based on low quality data support. In case of business management is achieved by low quality information, company can not help dropping their competitive power such as company's strategy establishment, strategy achievement and management concentration breakup against competitor. Information of low quality increase time and expense to improve inaccurate data or revise and it is hard to accept correct information from specific situation. To solve these problems, we have to gain high quality data through definite comprehension, data management system establishment, and systematic data management achievement etc. Up to now, information quality and connected study were developed partially, but systematic methodology of information quality management's whole condition was not existed. Therefore, in this paper can show you how to extract process for information quality management & related evaluate factor with CMM (Capacity Maturity Mode]) 5 steps that is information warranty of quality process step. This paper whishes to contributes in competitive company or organization activity through information quality improvement management process.

        Natural ecosystem is an essential part to connect with the plan for biodiversity conservation in response strategy against climate change. For connecting biodiversity conservation with climate change strategy, Europe, America, Japan, and China are making an effort to discuss protection necessity through national biodiversity valuation but precedent studies lack in Korea. In this study, we made biodiversity maps representing biodiversity distribution range using species richness in National Forest Inventory (NFI) and Forest Description data. Using regression tree algorithm, we divided various classes by decision rule and constructed biodiversity maps, which has accuracy level of over 70%. Therefore, the biodiversity maps produced in this study can be used as base information for decision makers and plan for conservation of biodiversity & continuous management. Furthermore, this study can suggest a strategy for increasing efficiency of forest information in national level.

        변형된 후두경 날이 삽관의 난이도에 따른 삽관 후 혈역학적 변화에 미치는 영향

        Background: Laryngoscopy and tracheal intubation are known to have profound cardiovascular effects. The Callander modification of Macintosh blade is associated with greater field of laryngoscopic view and decreased risk of dental contact. The purpose of this study was to compare the hemodynamic responses to laryngoscopy and tracheal intubation according to the degree of difficult airway, and to evaluate the usefulness of Callander modification of Macintosh blade for attenuating the hemodynamic responses. Methods: One hundred, forty-eight patients scheduled for elective surgery were divided into Easy group and Difficult group by Wilson`s risk sum score. Laryngoscopy was performed using either an ordinary Macintosh No. 3 blade or the modified Macintosh blade. The modification consisted of reducing the height of the flange by partial removal, as described by Callander et al. Hemodynamic variables (systolic, diastolic, mean blood pressure, heart rate and rate pressure product) were noted before induction (baseline) and immediately after intubation. Results: The hemodynamic changes after tracheal intubation in Difficult group were significantly greater than those in Easy group (P<0.05). When using the modified blade, systolic, diastolic and mean blood pressure after tracheal intubation were lower than those using the conventional blade regardless of Wilson`s risk sum score, but no statistical significances could be found. Conclusions: The hemodynamic changes after tracheal intubation increased as the degree of airway difficulty increased. Laryngoscopy with the Callander`s modified blade did not reduce the degree of hemodynamic stimulation compared with the conventional Macintosh blade. (Korean J Anesthesiol 2009;56:11~7)

        흰쥐에서 척수 손상후 반응성 별아교세포에서의 CNTF 발현 증가

        N/A Background: Ciliary neurotrophic factor (CNTF), identified as a survival factor for developing peri- pheral neurons is upregulated by reactive astmcytes in the traumatized tissue and in areas of terminal degeneratian after a brain lesion. But in the spinal cord, CNTF is expressed in the non-astrocytic pheno- typic, maybe oligodendrocytes. The present study was undertaken to determine the upregulation of CNTF expression in reactive astrocytes following spinal cord lesion in the rat. Methods: Unilateral incision of the dorsal funiculus at the thoracic level was performed and rats were sacrificed on days 3, 7, 14 and 28 postlesion. Western blot analysis, immunocytochemical analysis and double immunofluorescence for CNTF and glial fibrillary acidic protein (GFAP) were performed after spinal cord lesion. Results: A major band with 24 kDa and additianal band of higher molecular weight form were detect- able, and the intensity of the 24 kDa immunoreactive band increased up to 14 days postlesion and de- creased toward huninectomized control values. CNTF immunoreactivity was markedly upregulated in the injured dorsal funiculus and adjacent gray matter. The time course of CNTF expression is coincident with the appearance of reactive astrocytes in the injured spinal cord. Moreover, double immunofluo- amcence for CNTF and glial fibrillary acidic protein (GFAP) revealed that CNTF immunoreactivity was in GFAP immunoreactive astrocytes. Conclusions: These results show that CNTF upregulation occurred in reactive astrocytes following spinal cord lesion, and suggest a role for CNTF in the regulation of astrocytic responses after spinal cord injury.

        Epidural block is widely used for the diagnosis and treatment of the acute and chronic pain in pain clinic. But, it has several complications like as headache, spinal injury, epidural hematoma and infection including epidural abscess, meningitis and osteomyelitis. Among them, the simultaneous occurrence of psoas and epidural abscesses is extremely rare. We report a patient diagnosed as epidural and psoas absesses one month after lumbar epidural block and treated successfully with surgical decompression and drainage.         석탄층 메탄가스 저류층의 생산방법 선정을 위한 지능형 시스템 개발

        석탄층 메탄가스 저류층의 개발을 위해서는 석탄층 특성에 부합하는 생산방법을 적용하는 것이 중요하지만 대부분의 개발현장에서는 경험적인 판단에 의존하여 생산방법을 선정하고 있어 결과에 대한 신뢰도가 떨어질 뿐만 아니라 복잡한 시뮬레이션 해석을 통해 최적 생산방법을 규명해야 하는 등의 많은 시간과 노력이 필요하다는 문제점이 있다. 이에 본 연구에서는 인공신경망을 활용하여 석탄층 메탄가스 최적 생산방법을 선정하기 위한 지능형 시스템을 개발하였다. 시스템 개발에 앞서 문헌 조사를 통해 석탄층 메탄가스 생산방법 선정 가이드라인을 분석하였고, 이를 활용하여 시추 방법과 유정완결법 선정 시스템, 수압파쇄공법 수행시 파쇄액 선정 시스템을 개발하였다. 개발된 생산방법 선정 시스템의 타당성 평가 결과, 높은 정확도를 나타냄으로써 본 연구에서 개발된 생산방법 선정 시스템은 향후 석탄층 메탄가스의 개발에 앞서 석탄층 특성에 부합하는 생산방법을 선정할 수 있는 효율적인 도구로 활용될 것으로 판단된다. To develop a coalbed methane(CBM) reservoir, it is important to apply production methods such as drilling, completion, and stimulation which coincide with coal properties. However, the reliability of the selected resulted in most of CBM field is not enough to accept because the selection of production method has been done by empirical decision. As the result, the empirical decision show inaccurate results and need to prove using simulation whether it was true exactly. In this study, the intelligent system has been developed to assist the selection of CBM production method using artificial neural network(ANN). Before the development of the system, technical screening guideline was analyzed by literature survey and the system to select drilling and completion method, and hydraulic fracture fluid was developed by utilizing the guideline. The result as a validation of the developed system showed a high accuracy. In conclusion, it has been confirmed that the developed system can be utilized as a effective tool to select production method in CBM reservoir.

        임상연구 : 정형외과적 수술을 받는 고령의 환자에서 주술기 혈전탄성묘사도의 변화

        Background: There is a high incidence of thromboembolism after major orthopedic surgery. However, more perioperative thromboembolic complications are expected after orthopedic surgery in geriatric patients due to the more offensive therapeutic measures and the increasing number of such patients with multimorbidity. Therefore it is important to investigate the perioperative blood coagulation status in detail. Methods: Forty-five patients who were over 65 years old and scheduled for major orthopedic surgery were enrolled in this study. Patients with preoperative coagulation abnormalities, or receiving anticoagulants or antiplatelet medications were excluded. Preoperative thromboelastography (TEG), intraoperative TEG after blood loss equaling approximately 10% of the estimated blood volume, and postoperative TEG at the recovery room were measured and compared. Results: During the operation, the R time and coagulation time (r + k) showed significant decreases, whereas the alpha angle, maximum amplitude (MA) and TEG index increased significantly (P < 0.05), indicating increased coagulability. The A60, CL30 and CL60 also increased, indicating decreased fibrinolysis (P < 0.05). These hypercoagulable findings were relieved after surgery to levels similar to those observed preoperatively. Conclusions: The intraoperative coagulability increased compared with the preoperative and postoperative period according to the TEG. This means that the intraoperative period is the period of most susceptibility to thromboembolic complications. (Korean J Anesthesiol 2006; 50: 422~7)
