We present the efficiency evaluation and development of pilot equipment for emulsification of waste vinyl. The system of pilot equipment were composed of the melting furnace, first pyrolysis furnace and second pyrolysis furnace. For the evaluation of manufactured pilot equipment, the temperature, yield and electric power were measured for various emulsification temperatures using the waste vinyl. The results demonstrated that an emulsification temperature in the range of 360 to 370℃ had a deep influence on the yields, and the melting times of waste vinyl had need of approximately two hours. 1kWh of electric power was spent, and the yields was measured approximately 68%.

혈관손상을 동반한 하지장관골의 골절

The authers report a clinical experience of 7 cases having fracture of the lower extremity associated with vascular injury from August 1989 to July 1990 in the Department of Orthopedic Surgery, In Je University Sang Gye Paik Hospital. We obtained the noticeable key point in the diagnosis and treatment of fracture of lower extremity associated with vascular injury. The result were as follows: 1. Orthopedic trauma rarely produces injuries requiring vascular surgical repair. This relative rarity is further complicated by the potentially subtle nature of vascular injury. 2. Displaced fractures of the distal femur and proximal tibia are associated with highest risk of concomitant vascular injury. 3. Doppler signals and even palpable pulses do not exclude vascular damage. 4. Amputation before life threatening sepsis develops is required for the extremities which were cadaveric foot at the initial examination, and had severe muscle crushing injury and bone loss.

경골 결절의 견열 골절에 합병된 구획 증후군

Avulsion of the tibial tubercle is an uncommon physeal injury. Complications from this fracture have rarely been reported and seldom affect the long-term outcome. Three adolescent boys who sustained avulsion fracture of the tibial tubercle, were complicated by compartment syndrome and treated at Inje University Sang-Gye Paik Hospital from September 1989 to February 1995. Injury to the soft tissue surrounding the tibial tubercle avulsion may be more extensive than is usually appreciated. The anatomy of the proximal tibia and the tibial tubercle with nearby branches of the anterior tibial recurrent artery suggest a predisposing factor for the development of compartment syndrome. So, compartment syndrome should be added to the list of possible complications of this type of fracture.

수근 관절 부위의 골절 탈구에서 소형 외고정기구와 내고정을 겸한 치료

Authors reviewed 8 cases of wrist fracture-dislocation treated with mini-external fixator and internal fixation form Septmeber 1989 to May 1992 with average 6 manths follow up. The results were as follows; 1. Mean ages were 47 years, most patients were young age. 2. We could achieve good results in intra-articular, communited, displaced fracture and open fracture of the wrist by using the mini-externall fixator and internal fixation. 3. Radial length and inclination was maintained mainly by the external fixator. Articular surface restoration and reconstruction was performed by bone graft and the limited internal fixation. We would like to recommend to use the mini-external fixator and limited internal fixation instead of plate and screws for the intraarticular fractue, displaced, communited farcture and open fracture of the wrist.

고관절 치환술중 또는 술후에 발생한 대퇴골 간부 골절에 Partridge Cerclene System의 이용 6례 보고

The treatment of femur shaft fracture during and after the hip replacement arthroplasty is difficult and troublesome. The methods of treatment include hip spica cast, open reduction and internal fixation with plate and screws, and revision arthroplasty with long stem prosthesis, etc. The authors report 6 cases of femur shaft fracture during and after the hip replacement arthroplasty which were treated with Partridge cerclene system with or without addition of cerclage wiring. plate and screws, long stem prosthesis, etc. The results were as follows; The procedure of Partridge cerclene system application was relatively simple and effective fixation could be obtained for the fracture at distal stem area, where screws could not be fixed. With rigid fixation, range of motion excercise and early ambulatian was possible, so hospital days could be reduced also. The periodic observation will be necessary for evaluation of late complications such as implant loosening etc.

소아 슬개골의 Sleeve 형 골절

The sleeve type fracture of the patellar in children which is an avulsion fracture of distal pole of patella occurs in immature skelaton and the incidence is extremely rare, about 1% of childrens patellar fracture$quot;. This fracture is a reason of acute patellar dislocation in children together with osteochondral fracture and marginal fracture. Clinical importance of this fracture is difficulty in diagnosis because the distal small fragment with large articular cartilage is not easily detected by radiograpy. Therefore early optimal Treatment is also difficult. This fact resulted in complication such as patella alta, continous knee joint pain, quadriceps atrophy. The authors report two cases of patedllar fracture with early diagnosis, early surgical treatment and good result.

척골 신경 마비가 동반된 원위 요골 골절

There are several complications of distal radius fracture such as median nerve injury. malunion, nonunion, rupture of EPL, and ischemic contracture. Lesion of ulnar nerve as a complication of fracture of the distal radius are very rare. T he authors report 1 case of the distal radius fracture with ulnar nerve palsy. The electromyography & nerve-conduction studies showed incomplete axomotmesis of ulnar nerve on 1 month following injury. A second electromyography & nerve-conduction study two months after injury showed complete recovery of nerve function.

족근-중족 관절의 골절 탈구

Tarsometatarsal injury has not been paid much attention due to its unusual frequency. But the cases of tarsometatarsal injuries are on the increase in proportion to the increasing $quot;high energy injury$quot; result from the traffic or industrial accidents. To prevent the disability after injuries of the Lisfranc joint(tarsometatarsal joint), the accurate diagnosis, anatomical reduction and its maintenance are essential. The authors analyzed 34 cases of Lisfranc joint injury treated at Sanggye Paik Hospital from August l989 to December 1995, which showed the following results. l. The most common cause of the injury was traffic accident(50%); the associated injury was common. 2. According to the Hardcastle's classification, the injuries were classified as follows: total incongruity in 9 cases(27%), partial incongruity in 20 cases(59%) and divergent congruity in 5 cases(14%). 3. Eleven cases treated by open reduction and multiple pinning had the results as follows: good in 5 cases. fair in 4 cases and poor in 2 cases. Twenty three cases treated by closed reduction and cast immobilization or percutaneous pinning showed the results as follows: good in 11 cases, fair in 10 cases and poor in 2 cases. In conclusion, the anatomical reduction and its maintenance, whether achieved by closed or open means, seems to lead to the good result in long-term follow-up.

경골 Pilon 골절에 대한 임상적 고찰

The pilon fracture result from axial compression and rotational forces causing variable degrees of metaphyseal disruption, articular damage, and malleolar displacement. It' s managements are closed reduction and plaster immobilization, skeletal traction, fibular stabilization alone, limited open reduction and external fixation, primary arthrodesis, and even immediate amputation. Recently limited open reduction and external fixation has been proved to provide good clinical results for the severely comminuted or open pilon fractures. Authors reviewed 20 cases of the pilon fractures. Among them 10 cases were treated with limited open reduction and external fixation from October l989 to January 1994. The results were as follows: l. Age distribution was from l4 years to 77 years(mean; 47 years). 2. Of the 20 cases, male were 14, and female were 6. 3. The cause of injury were tracffic accident in 10 cases, slip down in 6 and fall from a height in 4. 4. The most frequent type of fractures was type 0(12 cases: 60%) according to Ruedi and Allgower classification. 5. Limited open reduction and external fixation(10 cases) has shown better results than classic open recduction and internal fixation for severely comminuted or open pilon fractures.
